Selection method of public transportation vulnerable area using GIS buffering analysis (GIS Buffering기법을 이용한 대중교통취약지구 선정방법)

  • Public transportation network in our country is concentrated and advanced focusing on urban area in order to secure economic feasibility. As a result, as dependence on private vehicles is relatively getting higher in public transportation vulnerable area, traffic problem occurs since the average running speed in urban area is 22.5km/h. This paper has an objective to suggest an improvement plan by selecting public transportation vulnerable area, and defining according to urban structure, formation and function, and understand traffic characteristics and draw problems. As a method selecting public transportation vulnerable area, an area with high division rate of vehicle was selected as a primary proposed site by calculating division rate of means of public transportation according to area. Final proposed site was selected by using GIS Buffering technique aiming at selected proposed site, and selecting non-benefit area 500m outside, which is the road limit distance from each subway and bus station. Lastly, the degree of improvement effect was studied by constructing imaginary public transportation network aiming at final proposed site and comparing to the amount of change in division rate of means of transportation.

Cerebral Aneurysms Arising from Unbranched Site of Intracranial Arteries (혈관가지가 없는 두개강내 혈관에서 발생한 뇌 동맥류)

  • Objective : In general, most of saccular aneurysms arise at arterial divisions, but those arising at unbranched site are rare. These aneurysms might impose neurosurgeons a formidable surgical challenge, due to uncommon features and a difficult surgical technique. Methods : Between the period of Jan. 1996 and Dec. 1998, a total of 110 cases of aneurysms were operated. Among them, five cases of unbranched site aneurysms were retrospectively analyzed through medical records, angiographic and operative findings. Results : The incidence of aneurysms unrelated to arterial branches was 4.5%. All cases presented with subarachnoid hemorrhage(SAH) ; three(60%) were at internal carotid artery(ICA) and two(40%) at middle cerebral artery (MCA). Two had a history of hypertension and three showed atherosclerotic changes in the arterial wall. One ICA and one MCA aneurysms proved to be a blood blister-like aneurysm(BBA) in their shape. Strikingly, a rapid neurological deterioration was shown in two ruptured ICA variants, which resulted in death and another two aneurysms left with morbidity. Conclusion : The preoperative neurological status in most cases was relatively poor compared to that of aneurysms arising at a branched site. Regarding its pathogenesis, atherosclerosis as well as hemodynamic factors may play an important role in formation of these variant aneurysms among various etiological factors. The overall prognosis of unbranched site aneurysms was worse than that of branched site aneurysms. In cases of BBAs, special attention was requisite to handle them during clipping and/or wrapping due to an easy fragile, thin aneurysmal wall.

EXPERIMENTAL STUDIES ABOUT HEALING PROCESS OF BLOOD VESSELS FOLLOWING MICROVASCULAR ANNASTOMOSES (미세혈관문합 후 혈관내벽의 치유과정에 관한 실험적 연구)

  • Microvascular surgery has been widely used in the clinical field of replantation and reconstructive surgery. Since the last 20 years, microsurgical techniques and instruments have been rapidly developed and the success rate is remarkably increased. But thrombotic occlusion of vessels remains the major reason for clinical failure. The change of vessel wall is the most important factor in thrombus formation. If we can reduce the traumatic changes in the vessel walls during surgery, the success rate can be markedly increased. For this study, femoral arteries and veins of 36 Sprague-Dawley rats with average weights of 300gm were used. The author observed the histological changes and healing process in the anastomostic site after 1 hour, 24 hours, 1, 2, 3 and 4 weeks under light microscopy and scanning electron microscopy. The results were as follows : 1. The patency rate was 100% in femoral arteries and 85% in femoral vein. 2. At the early stages after microvascular anastomosis, the loss of endothelial cell in the vessel walls was observed in the wide area including anastomotic site. In scanning electron microscopic finding the anastomotic site was covered with much fibrin, many red blood cells and some platelets. 3. At 1st week, new endothelial cells were formed toward anastomotic site and at 3rd week, the anastomotic site was completely covered by new endothelial cells. At 4th week, the complete endothelialization over the threads was observed. 4. The media extended from the anastomotic site toward the end of the specimen. At later stages, the extent of media necrosis was markedly decreased. But the media necrosis of anastomotic site was not regenerated till 4th week. 5. Intimal hyperplasia appeared at 1st week and increased till 4th week. The layer consisted of endothelialization the most luminal layers and smooth muscle in the deeper layers. But in veins, the response was less pronounced than in arteries. 6. Foreign body granuloma remained during 4 weeks and aneurysm was observed at 3rd week in artery. In aneurismal wall, media necrosis, loss of elastic lamina and intimal hyperplasia were seen.

Full thickness skin grafts from the groin: donor site morbidity and graft survival rate from 50 cases

  • Objectives: Full thickness skin grafts (FTSG) offer several advantages; they are esthetically superb, have less postoperative shrinkage, and offer minimal postoperative pain and scar formation at the donor site. As a donor site of FTSG, the groin offers a relatively large area of skin with high elasticity. The aim of this study was to evaluate FTSG from the groin for reconstruction in oral and maxillofacial surgery. Materials and Methods: In a retrospective study, 50 patients (27 males, 23 females) who received FTSG from the groin were evaluated for their operation records, clinical photography, and medical records. Results: The width of skin from the groin was distributed from 2-8 cm (mean: 5.1 cm) at the donor site, while the long axis length was distributed from 3-13 cm (mean: 7.4 cm). A high number of patients, 47 patients (94%) out of 50, showed good healing at the donor site. Wound impairment was seen in 3 patients (6%), minor wound dehiscence in 2 patients, and severe wound dehiscence in 1 patient. In the recipient site, delayed healing was observed in 2 patients (4%). Conclusion: FTSG from the groin to repair soft tissue defects in reconstruction surgery is a good method due to the relatively big size of the graft, decreasing morbidity at the donor site, and higher graft survival rates.

Autogenous transplantation of tooth with complete root formation (치근단 완성된 치아의 자가이식)

  • Purpose: Autogenous transplantation of teeth can be defined as transplantation of teeth from one site to another in the same individual, involving transfer of impacted or erupted teeth into extraction sites or surgically prepared sockets". Successful autogenous transplantation of teeth depends upon a complex variety of factors. Such factors include damage to the periodontal ligament of the donor tooth, residual bone height of the recipient site, extra-oral time of tooth during surgery. Schwartz and Andreasen previously reported that autogenous transplantation of teeth with incomplete root formation demonstrated higher success rate than that of teeth with complete root formation. Gault and Mejare yielded similar rate of successful autogenous transplantation both in teeth with complete root formation and in teeth with incomplete root formation when appropriate cases were selected. This case report was aimed at the clinical and radiographic view in autogenous transplantation of teeth with complete root formation. Materials and Methods: Patients who presented to the department of periodontics, Chonnam National University Hospital underwent autogenous transplantation of teeth. One patient had vertical root fracture in a upper right second molar and upper left third molar was transplanted. And another patient who needed orthodontic treatment had residual root due to caries on upper right first premolar. Upper right premolar was extracted and lower right second premolar was transplanted. Six months later, orthodontic force was applied. Results: 7 months or 11/2 year later, each patient had clinically shallow pocket depth and normal tooth mobility. Root resorption and bone loss were not observed in radiograph and function was maintained successfully. Conclusion: Autogenous transplantation is considered as a predictive procedure when it is performed for the appropriate indication and when maintenance is achieved through regular radiographic taking and follow-up.

Structural and Biochemical Studies Reveal a Putative FtsZ Recognition Site on the Z-ring Stabilizer ZapD

  • FtsZ, a tubulin homologue, is an essential protein of the Z-ring assembly in bacterial cell division. It consists of two domains, the N-terminal and C-terminal core domains, and has a conserved C-terminal tail region. Lateral interactions between FtsZ protofilaments and several Z-ring associated proteins (Zaps) are necessary for modulating Z-ring formation. ZapD, one of the positive regulators of Z-ring assembly, directly binds to the C-terminal tail of FtsZ and promotes stable Z-ring formation during cytokinesis. To gain structural and functional insights into how ZapD interacts with the C-terminal tail of FtsZ, we solved two crystal structures of ZapD proteins from Salmonella typhimurium (StZapD) and Escherichia coli (EcZapD) at a 2.6 and $3.1{\AA}$ resolution, respectively. Several conserved residues are clustered on the concave sides of the StZapD and EcZapD dimers, the suggested FtsZ binding site. Modeled structures of EcZapD-EcFtsZ and subsequent binding studies using bio-layer interferometry also identified the EcFtsZ binding site on EcZapD. The structural insights and the results of bio-layer interferometry assays suggest that the two FtsZ binding sites of ZapD dimer might be responsible for the binding of ZapD dimer to two protofilaments to hold them together.

The effect on the formation of keratinized attached gingiva using free gingival graft with strip technique (Strip 형 유리치은 이식술이 부착지은 형성에 미치는 영향)

  • This study was performed to evaluate the effect on the formation of keratinized attached gingiva when free gingival graft with strip technique was used. After the partial thickness flap was prepared, it was positioned apically more than 7mm using mucoperiosteal suture. At the donor site, strip was achieved with 2mm width, less than 1mm thickness, (mesiodistal teeth length + 6)$^{\ast}$2 mm length. Then, it was divided into 2 same pieces and located at the recipient bed apico-coronally with more or less 3mm interval. Follow-up check was made at 1,2,4,8,12 weeks after the operation. The results of this study were as follows : 1. The inter-strip space was mostly filled with keratinized attached gingiva. 2. The contraction of the graft was nigligible, and 92% of planned area was filled with keratinized attached gingiva. 3. The inconvenience of the patients at donor site was minimal. 4. The width of the strip was widened. In conclusion, free gingival graft using strip technique is useful to achieve the keratinized attached gingiva maximally with free gingival graft minimally and to minimize the inconvenience of donor palatal site of the patients.

A Study of the Construction Method for Soft Ground Reinforcement of Baekje Dynasty Based on the Low Swamp Remains in the Buyeo Area (백제시대 부여지역 저습지에 조성된 유적에서 나타나는 연약지반 개량공법 연구)

  • King Seong carried out the large-scale construction transferring the capital from Wungjin (Gongju) to Sabi (Buyeo) in 538. But because most of the Buyeo area was the low swamp in the time, it needed above all to form a site before transferring the capital. Until recently, in addition to the scientific excavation, the relief excavations for the construction of new building or the formation of road were conducted on many sites in the Buyeo area. As a result, many remains which were formed on the low swamp including the temple site of Neungsan-ri, Dongna Castle, the remains of Ssangbuk-ri (280-5 Bukpo, Hyeonnaedeul) and the remains of Gua-ri were identified. Also in these remains, the various engineering construction methods irrespective of the nature of remains were used for the purpose of the soft ground reinforcement as follows: mattress method of construction, pile designation, stone alignment, filing of decomposed granite soil, culverts and storage tanks. Especially, the mattress method of construction and the pile designation are thought to be the traditional engineering construction method at least in that they are appeared since the Three Han Sates era. And these soft ground construction methods had an effect on the construction of reservoir in Japan at the time. In the future, the construction method for soft ground reinforcement shall be concerned and studied further in the architecture and the civil engineering as well as the archeology.

Development of a Subsurface Exploration Analysis System Using a Clustering Technique on Bore-Hole Information (시추공 정보의 클러스터링 기법을 이용한 지반분석시스템의 개발)

  • Every, year, a great amount of site investigation data is collected on site to obtain sufficient conditions. Investigation of subsurface conditions is prerequisite to the design and construction of structures and also provides information on ground properties such as geologic formation and types of soil. This data set, which portrays real representation of ground conditions over the existing geologic and soil maps, could be further utilized for analyzing the subsurface conditions. It is therefore necessary to develope a subsurface exploration analysis system which is able to extract the valuable information from the heterogeneous, non-normalized subsurface investigation data. This paper presents the overall design scheme and implementation on a subsurface exploration analysis system. The analysis system employs one of data set such as bore-hole data. The clustering technique employed in the developed system makes a large volume of bore-hole data into several groups in terms of ground formation and geographical vicinity. As a result of clustering, each group or cluster consists of bore-hole data with similar characteristics of subsurface and geographical vicinity. In addition, each clustered data is displayed on digital topographical map with different color so that the analysis of site investigation data could be performed in more sensible ways.

CONTROL OF SCARRING IN ADULT WOUNDS USING ANTISENSE TRANSFORMING GROWTH FACTOR-$\beta$ OLIGODEOXYNUCLEOTIDES

  • Although synthetic antisense oligodeoxynucleotides (ODNs) have been used to dissect gene function in vitro, technical difficulties of targeted delivery prevented the use of this approach for investigating the effect of gene products in vivo. Here we report the use of local delivery of antisense transforming growth factor-${\beta}$l (TGF-${\beta}$1) oligonucleotides to decrease the fibrosis in the skin wound. Adult wounds heal with scar-tissue formation, whereas fetal wounds heal without scarring and with a lesser inflammatory and cytokine response. We reasoned that strategy emptying antisense TGF-${\beta}$1 ODNs complementary to TGF-${\beta}$1 mRNA might decrease the scarring in dermal wound of mouse. To evaluate this concept, we tested the effects of antisense ODNs targeted to TGF-${\beta}$1 mRNA by topical application of the chemical on the skin wound. Phosphorothioate antisense ODNs was employed to retard their degradation. When antisense TGF-${\beta}$1 ODNs were applied into the wound site, there was a maked reduction of scar compared with control wound site, These effects of antisense TGF-${\beta}$1 ODNs on the scar formation were associated with decreased expression of TGF-${\beta}$1 gene. However sense TGF-${\beta}$l ODNs had no effect on expression of TGF-${\beta}$1 gene. Also, control wounds healed with excessive fibrosis, whereas the antisense treated wounds healed with less fibrosis. In conclusion, our results indicate that antisense TGF-${\beta}$1 ODNs could be used for amelioating scar formation during wound healing.
