Risk factors predicting development of complications in 72 dogs with esophageal foreign bodies

  • The aim of this study was to determine predictive risk factors implicated in complications in dogs with esophageal foreign bodies. Medical records of 72 dogs diagnosed with esophageal foreign bodies by endoscopy were reviewed retrospectively. Factors analyzed included age; breed; gender; body weight, location, dimension, and type of foreign body; and duration of impaction. To identify risk factors associated with complications after foreign body ingestion, categorical variables were analyzed using the chisquare or Fisher's exact tests and multivariate analysis, as appropriate. Complications secondary to esophageal foreign body ingestion included megaesophagus, esophagitis, perforation, laceration, diverticulum, and pleuritis. Univariate analysis revealed that the location and duration of impaction after foreign body ingestion were associated with an increased risk of esophageal laceration and perforation. Multivariate analysis showed that age, duration of impaction, and foreign body dimension were significant independent risk factors associated with the development of complications in dogs with esophageal foreign bodies. In conclusion, these results showed that longer duration of impaction and larger foreign body dimensions may increase the risks of esophageal laceration, perforation, and plueritis in dogs.

강직형식도경술을 이용하여 치료한 이물에 대한 임상적 고찰 (Treatments of esophageal foreign body treated using Rigid esophagoscopy)

  • OBJECTIVE : To study the management (diagnostic and therapeutic) of esophageal foreign bodies with rigid esophagoscopy. MATERIAL AND METHODS : All 100 patients admitted to the Dankook University Hospital for ingestion of foreign bodies between May 1994 and July 1999. The outcome for each patient was determined by examining hospital records of demographic information, identification of the foreign body and the removal procedure used. RESULTS : Rigid esophagoscopy was performed for suspected foreign bodies in 100 cases an impacted. The most frequent location was the upper third of the esophagus (68%). The most common type of foreign body was fish bones in adult(61%) and coins in children(70%). In 12 patients. flexible endoscopy had failed previously to remove the foreign body and 4 cases were migrated esophageal metal stent in esophageal stenosis. CONCLUSIONS : The rigid esophagoscopy is appropriate techniques for managing esophageal foreign bodies.

체내 소형 이물질 진단에서 초음파 검사의 유용성 (Usefulness of Ultrasonography in Diagnosis of Small Foreign Bodies)

  • 목적: 체내 소형 이물질이 있는 것으로 의심되나 이학적 검사 및 단순 방사선 검사로 확인이 되지 않을 때, 이물질의 유무, 위치, 크기와 성상을 확인하고 제거하는데 있어 초음파 검사의 유용성을 알아보고자 하였다. 대상 및 방법: 2007년 3월부터 2012년 2월까지 수부 및 족부의 소형 이물질을 수술적으로 제거 받은 환자 중 수술 전 초음파 검사를 시행한 9명을 대상으로 하였다. 수상일로부터 수술일까지 이환 기간은 평균 32개월이었으며 부위는 수지 5예, 수장부 2예, 수배부 1예 및 족저부 1예였다. 2예에서 수술 전 단순 방사선 검사상 이물질을 확인할 수 있었으나 7예에서는 확인되지 않았으며 초음파 검사로 진단할 수 있었다. 고해상 선형 탐색자를 이용하여 초음파 검사를 시행하였으며 이물질의 유무, 위치 및 크기에 대해서 수술 소견과 비교하였다. 결과: 수술 전 초음파 검사에서 확인된 위치와 수술 소견이 잘 부합하였다. 이물질은 유리조각이 4예, 가시가 4예, 연필심이 1예였으며 평균 크기는 3.9 mm (2~7 mm) 였다. 결론: 체내에 소형 이물질이 잔류하는 것으로 의심되는 환자에서 초음파 검사는 진단 및 치료에 있어 유용한 검사로 사료된다.

흥미있는 식도이물 1례 (A Double-coin in esophagus)

  • 식도 및 기도이물은 이비인후과 영역에서 흔히 접하는 질환으로써 특히 식도이물의 보고 예는 국내문헌상에서도 수 많이 찾아 볼 수 있으며 이의 진단에는 X-선 촬영이 필요불가결하며 중요한 수단으로 되어 있음은 주지의 사실이다. 이중 특히 경부측면 X-선 검사를 간과하는 경우가 있으나 최근 저자들은 4세의 여아에서 2개의 동전이 중첩 계류된 것을 경부측면 X-선상에서 확인하여 식도경술하에 제거한 예를 경험하였기에 문헌고찰과 더불어 보고하면서 식도 및 기도이물, 특히 동전의 식도이물이 의심되는 환자에서는 경부 전후경 X-검사는 물론 특히 경부 측면엑스선 검사를 반드시 실시하여야 한다는 것을 강조하는 바이다.

성인 식도이물의 내시경적 치료 (Endoscopie Removal of Adult Esophageal Foreign Bodies)

  • The majority of esophageal Foreign body ingestions occur in the pediatric population. In adults, true foreign object ingestion occurs more commonly among those with psychiatric disorders, mental impairment. The management of esophageal foreign bodies is influenced by the age, clinical condition of ingested material, anatomic location and technical abilities of the endoscopist. Recently the therapeutic endoscopy is becoming wider and more rational in application. We evaluated the role of endoscopy for removal of esophageal foreign bodies during the period of 4 years from January 2000 to December 2003 at the Department of Otolaryngology and Gastroenterology, Seonam University Hospital. The results were as follow, 1) The age ranged from 21 to 74 years old (mean 50.5), most frequent age group was between 61-70 years old and male to female ratio was 1:1.4. 2) Fish bone was the most frequent foreign body in the esophagus ($47.1\%$), food material ($23.5\%$) and meats ($17.6\%$) were next frequent foreign bodies. The most frequent site of lodgement was the first ($78.4\%$), second ($17.6\%$) and third narrowing ($3.9\%$) in order. 3) The most common symptom was foreign body sensation (28.6%). the next common symptoms were chest discomfort($23.8\%$) and dysphagia($19\%$). 4) In duration of lodgement, 49cases ($96.1\%$) were lodged for less than one day. 5) The foreign bodies of esophagus were removed successfully by flexible endoscope with basket, snare, forceps, overtube and endoscopic variceal ligation cap. There were only 3 cases of minimal complications, esophageal mucosal tearing. In conclusion, endoscopic esophageal foreign body removal is useful and safe with minimal or no complications.

  • 구강악안면 영역에 발생한 foreign body impaction은 흔하지 않지만, 일단 발생하면 즉시 제거되어야 하며 술후 후유증을 충분히 방지하기위해 주의깊은 병력청취, 임상 검사, 방사선 사진을 이용한 localization이 꼭 필요하다. 그리고 수술시에는 조심스럽게 조직을 박리하고 전체 손상부위를 철저히 탐진해서 foreign body를 제거해야 한다. 저자등은 침구용 바늘, nasolacrimal ductoplasty용 silicone tube, 산탄 총알등 다양한 구강 악안면영역에서 발견된 이물질을 제거한 경험후 문헌고찰과 함께 보고하는 바이다.

안면부 농양 및 골염으로 오인된 나무 이물 1례 (A Case of Wooden Foreign Body Misinterpreted as Facial Abscess and Osteitis)

  • It is difficult to find the penetrating foreign bodies in the head and neck area only with history taking and physical examinations. One of the most important things is to detect the precise location of foreign bodies or possibly remained materials. The detection of wooden foreign bodies is important because they can cause phlegmon formation and because wood is often contaminated by Clostridium tetani bacteria. CT has proved to be an expedient method for detecting foreign bodies of various materials in soft tissues, but the wooden foreign body is often misinterpreted as a gas bubble in soft tissue. We have experienced a case of wooden foreign body which has penetrated through nasal dorsum and remained for 4 months. It had been initially misinterpreted as longstanding inflammatory lesion and osteitis of maxilla and nasal bone.

경부를 관통한 후두 내 금속이물 1예 (A Case of Intralaryngeal Metallic Foreign Body which Penetrated by Transcutaneous Route)

  • Laryngeal foreign bodies are not common among the foreign bodies of aerodigestive tract. It is relatively easy to diagnose in acute phase of entry because of a readily\ulcorner available history of intake, and signs or symptoms referable to the foreign body in the highly sensitive air passage. However, on occasion, sudden death by respiratory failure occurs due to complete obstruction of airway. Therefore, it is common and safe to remove the laryngeal foreign bodies by suspension laryngoscope under general anesthesia after tracheostomy. Recently, the authors experienced a case of metallic foreign body in larynx penetrating neck, which was removed by suspension laryngoscope under general anesthesia without any life threatening complication.

An unusual cause of duodenal perforation due to a lollipop stick

  • Children have a natural tendency to explore objects with their mouths; this can result in the swallowing of foreign objects. Most ingested foreign bodies pass uneventfully through the gastrointestinal tract. However, some foreign bodies cause obstruction or perforation of the gastrointestinal tract, requiring surgical intervention. Perforation of the gastrointestinal tract may be associated with considerable morbidity and mortality. The most common sites of intestinal foreign body perforation are the ileocecal and rectosigmoid regions. Foreign body perforation of the duodenum is relatively uncommon. We report the first Korean case of duodenal perforation by an ingested 8-cm lollipop stick. Lollipops are popular with the children and fairly accessible to them, as most parents are not aware of their potential harm. Pediatric clinicians should be aware of the risks associated with lollipop stick ingestion. Our report also describes the feasibility and safety of laparoscopic diagnosis and management of pediatric patients with peritonitis induced by the ingestion of foreign bodies.

A Study of Performance and Internal Flow in a New Type of Sewage Pump

  • Sewage pumps are designed with a wide flow channel by, for example, sacrificing some efficiency and reducing the number of blades, in order to prevent plugging with foreign bodies. However, the behavior of foreign bodies which actually flow into a pump is extremely complex, and there are questions about whether the presumed foreign bodies will actually pass through. This paper proposes a new type of sewage pump impeller designed to further improve pump efficiency and performance in passing foreign bodies. This sewage pump impeller has a structure in which the suction flow channel of a closed type non-clog pump is wound in a helical spiral. The focus of this research was to investigate pump performance and internal flow in this single blade sewage pump impeller. The results clearly indicated the following facts: The developed sewage pump impeller exhibits high efficiency over a wide range of flow rates; internal flow of the pump is very complicated; and the internal flow state varies greatly when the flow rate changes.
