Risk factors predicting development of complications in 72 dogs with esophageal foreign bodies

  • The aim of this study was to determine predictive risk factors implicated in complications in dogs with esophageal foreign bodies. Medical records of 72 dogs diagnosed with esophageal foreign bodies by endoscopy were reviewed retrospectively. Factors analyzed included age; breed; gender; body weight, location, dimension, and type of foreign body; and duration of impaction. To identify risk factors associated with complications after foreign body ingestion, categorical variables were analyzed using the chisquare or Fisher's exact tests and multivariate analysis, as appropriate. Complications secondary to esophageal foreign body ingestion included megaesophagus, esophagitis, perforation, laceration, diverticulum, and pleuritis. Univariate analysis revealed that the location and duration of impaction after foreign body ingestion were associated with an increased risk of esophageal laceration and perforation. Multivariate analysis showed that age, duration of impaction, and foreign body dimension were significant independent risk factors associated with the development of complications in dogs with esophageal foreign bodies. In conclusion, these results showed that longer duration of impaction and larger foreign body dimensions may increase the risks of esophageal laceration, perforation, and plueritis in dogs.

Treatments of esophageal foreign body treated using Rigid esophagoscopy (강직형식도경술을 이용하여 치료한 이물에 대한 임상적 고찰)

  • OBJECTIVE : To study the management (diagnostic and therapeutic) of esophageal foreign bodies with rigid esophagoscopy. MATERIAL AND METHODS : All 100 patients admitted to the Dankook University Hospital for ingestion of foreign bodies between May 1994 and July 1999. The outcome for each patient was determined by examining hospital records of demographic information, identification of the foreign body and the removal procedure used. RESULTS : Rigid esophagoscopy was performed for suspected foreign bodies in 100 cases an impacted. The most frequent location was the upper third of the esophagus (68%). The most common type of foreign body was fish bones in adult(61%) and coins in children(70%). In 12 patients. flexible endoscopy had failed previously to remove the foreign body and 4 cases were migrated esophageal metal stent in esophageal stenosis. CONCLUSIONS : The rigid esophagoscopy is appropriate techniques for managing esophageal foreign bodies.

Usefulness of Ultrasonography in Diagnosis of Small Foreign Bodies (체내 소형 이물질 진단에서 초음파 검사의 유용성)

  • Purpose: The purpose of this study is to evaluate the usefulness of ultrasonographic examination for diagnosis and removal of small sized foreign bodies, which invaded extremity but difficulty to find on physical examination and/or plane radiographs. Materials and Methods: Since March, 2009 to February, 2012, we performed preoperatively ultrasonography and operation in 9 cases of foreign bodies of hand or foot. Mean symptom duration periods was 32 months. The location were fingers in 5, palms in 2, hand dorsum in 1, heel in 1 case. In 2 cases, foreign bodies were seen on plane radiographs. In the remaining 7 cases, foreign bodies could be diagnosed by ultrasonography only. Using high resolution stick probe, we performed evaluation on size, location, character of foreign bodies and compared preoperative ultrasonographic findings to intraoperative ones. Results: The site of foreign bodies in preoperative ultrasonography corresponded well with intraoperative findings. Foreign bodies were glass particles in 5, plant thorns in 4, pencil lead in 1. Mean size was 3.9 mm (2~7 mm). Conclusion: Ultrasonography is usefull evaluation tool for diagnosis and treatment in whom residual foreign bodies were suspected.

  • Foreign bodies in air and food passages are not uncommon conditions in otolaryngological field and we can find out numerous causes of esophageal foreign bodies in literatures. Radiological exammination is essential and important method in diagnosis of foreign bodies in esophagus. The authors experienced a case of foreign body double coins, in esophagus in a 4-year - old girl and the authors emphasizing that in cases of esophageal foreign bodies radiological examination should always be included lateral view as well as anteroposterior view of neck especially when coins in the esophagus is suspected.

Endoscopie Removal of Adult Esophageal Foreign Bodies (성인 식도이물의 내시경적 치료)

  • The majority of esophageal Foreign body ingestions occur in the pediatric population. In adults, true foreign object ingestion occurs more commonly among those with psychiatric disorders, mental impairment. The management of esophageal foreign bodies is influenced by the age, clinical condition of ingested material, anatomic location and technical abilities of the endoscopist. Recently the therapeutic endoscopy is becoming wider and more rational in application. We evaluated the role of endoscopy for removal of esophageal foreign bodies during the period of 4 years from January 2000 to December 2003 at the Department of Otolaryngology and Gastroenterology, Seonam University Hospital. The results were as follow, 1) The age ranged from 21 to 74 years old (mean 50.5), most frequent age group was between 61-70 years old and male to female ratio was 1:1.4. 2) Fish bone was the most frequent foreign body in the esophagus ($47.1\%$), food material ($23.5\%$) and meats ($17.6\%$) were next frequent foreign bodies. The most frequent site of lodgement was the first ($78.4\%$), second ($17.6\%$) and third narrowing ($3.9\%$) in order. 3) The most common symptom was foreign body sensation (28.6%). the next common symptoms were chest discomfort($23.8\%$) and dysphagia($19\%$). 4) In duration of lodgement, 49cases ($96.1\%$) were lodged for less than one day. 5) The foreign bodies of esophagus were removed successfully by flexible endoscope with basket, snare, forceps, overtube and endoscopic variceal ligation cap. There were only 3 cases of minimal complications, esophageal mucosal tearing. In conclusion, endoscopic esophageal foreign body removal is useful and safe with minimal or no complications.

  • Foreign bodies in the maxillofacial area are not commonly seen. If occurred, the immediate removal of foreign bodies is recommended to avoid further complications. The most important thing is a exact localization of foreign bodies using X-rays such as plain radiography, computed tomography, magnetic resonance imaging and ultrasound before treatment. During removal of foreign bodies, tissue should be dissected carefully and complete exploration of the whole wound required.

  • It is difficult to find the penetrating foreign bodies in the head and neck area only with history taking and physical examinations. One of the most important things is to detect the precise location of foreign bodies or possibly remained materials. The detection of wooden foreign bodies is important because they can cause phlegmon formation and because wood is often contaminated by Clostridium tetani bacteria. CT has proved to be an expedient method for detecting foreign bodies of various materials in soft tissues, but the wooden foreign body is often misinterpreted as a gas bubble in soft tissue. We have experienced a case of wooden foreign body which has penetrated through nasal dorsum and remained for 4 months. It had been initially misinterpreted as longstanding inflammatory lesion and osteitis of maxilla and nasal bone.

  • Laryngeal foreign bodies are not common among the foreign bodies of aerodigestive tract. It is relatively easy to diagnose in acute phase of entry because of a readily\ulcorner available history of intake, and signs or symptoms referable to the foreign body in the highly sensitive air passage. However, on occasion, sudden death by respiratory failure occurs due to complete obstruction of airway. Therefore, it is common and safe to remove the laryngeal foreign bodies by suspension laryngoscope under general anesthesia after tracheostomy. Recently, the authors experienced a case of metallic foreign body in larynx penetrating neck, which was removed by suspension laryngoscope under general anesthesia without any life threatening complication.

  • Children have a natural tendency to explore objects with their mouths; this can result in the swallowing of foreign objects. Most ingested foreign bodies pass uneventfully through the gastrointestinal tract. However, some foreign bodies cause obstruction or perforation of the gastrointestinal tract, requiring surgical intervention. Perforation of the gastrointestinal tract may be associated with considerable morbidity and mortality. The most common sites of intestinal foreign body perforation are the ileocecal and rectosigmoid regions. Foreign body perforation of the duodenum is relatively uncommon. We report the first Korean case of duodenal perforation by an ingested 8-cm lollipop stick. Lollipops are popular with the children and fairly accessible to them, as most parents are not aware of their potential harm. Pediatric clinicians should be aware of the risks associated with lollipop stick ingestion. Our report also describes the feasibility and safety of laparoscopic diagnosis and management of pediatric patients with peritonitis induced by the ingestion of foreign bodies.

A Study of Performance and Internal Flow in a New Type of Sewage Pump

  • Sewage pumps are designed with a wide flow channel by, for example, sacrificing some efficiency and reducing the number of blades, in order to prevent plugging with foreign bodies. However, the behavior of foreign bodies which actually flow into a pump is extremely complex, and there are questions about whether the presumed foreign bodies will actually pass through. This paper proposes a new type of sewage pump impeller designed to further improve pump efficiency and performance in passing foreign bodies. This sewage pump impeller has a structure in which the suction flow channel of a closed type non-clog pump is wound in a helical spiral. The focus of this research was to investigate pump performance and internal flow in this single blade sewage pump impeller. The results clearly indicated the following facts: The developed sewage pump impeller exhibits high efficiency over a wide range of flow rates; internal flow of the pump is very complicated; and the internal flow state varies greatly when the flow rate changes.
