30대 여성의 이마근피에 미치는 영향 연구(SUKI®중재에 의한) (The Study on the Effect of the 30's Females Forehead muscular-cutaneous (by SUKI® intervention))

  • 본 연구의 목적은 30대여성을 대상으로 4주간 이마근피에 미치는 영향에 대해 SUKI 중재가 어떠한 영향을 미치는 가에 대해 살펴보고자 실행되었다. 연구대상자는 총 18명이었고, 4주간 주 3회 씩 얼굴의 이마근피에 대한 SUKI process C1, SUKI process C2, SUKI process C3, SUKI process C4의 SUKI 중재프로그램을 실시하였고, 그 결과는 아래와 같다. 연구 평가의 일부분에서 이마근피의 실험군이 유의한 차이를 보였다. 따라서, 이마근피의 탄력의 유지에 SUKI 중재가 일부 효과가 있다는 것을 보여주었다(p<.000). 본 연구에서는 제한적이지만, 30대여성의 얼굴의 상단 면에 위치하고 있는 이마근피의 탄력 감소로 인한 주름등이 여성의 외적 용모에 대한 심대한 영향을 줄 수 있다고 판단되어 SUKI 중재를 적용하였다. 더불어 이마의 탄력 있는 근피의 역할이 30대 여성의 생애주기에 맞는 이마근피를 관리함으로서 외적 용모에 대한 관리를 효과적으로 다스릴 수 있는 대체의학 기법을 제시하고자 하였다. 추후 다른 연령대의 여성들에게 본 실험에 의한 방법들을 적용하여 여성의 가장 중요한 외적용모의 하나인 얼굴 이마근피의 주름을 효과적으로 관리하여 탄력적인 상태를 유지하는데 활용이 될 수 있는 대체의학 기법에 대한 실험들이 창의적인 새로운 연구로 참고 되고 이용되기를 바란다.

Frontal augmentation as an adjunct to orthognathic or facial contouring surgery

  • Background: The dimensions and shape of the forehead determine the esthetics of the upper third of the face. Korean young people consider a broad and smooth, rounded forehead more attractive. As a result, frontal augmentation becomes more popular in patients with dentofacial deformities. Various surgical procedures and materials have been used in frontal augmentation surgery, with associated advantages and disadvantages. Silicone is a good candidate for frontal augmentation. The author presents two cases of esthetic frontal augmentation with a prefabricated silicone implant in female patients with dentofacial deformities. Case presentation: In case 1, a 24-year-old female patient underwent frontal augmentation surgery with simultaneous maxillomandibular and zygomatic osteotomies to correct facial asymmetry. A silicone implant was fabricated preoperatively using a positive template stone mold of her forehead. In case 2, a 23-year-old female patient underwent total facial contouring surgery including frontal augmentation for improved facial esthetics. A computed tomography (CT)-guided rapid prototype (RP) model was used to make the silicone implants. The operative procedure was safe and simple, and the silicone implants were reliable for a larger degree of frontal augmentation. Six months later, both patients had recovered from the surgery and were satisfied with their frontal shape and projection. Conclusions: Frontal augmentation with silicone implants can be an effective adjuvant strategy to improve facial esthetics in patients with a flat and narrow forehead who undergo orthognathic reconstruction or total facial contouring surgery.

체형특성(體型特性)이 체온조절반응(體溫調節反應) 및 온열쾌적감(溫熱快適感)에 미치는 영향(影響) (Effects of Somatotype Characteristics on Body Temperature Control Reaction & Thermal Sensation)

  • This study is the first part of the research to reveal the effects of somatotype characteristics on body temperature control reaction as well as thermal sensation. Nine healthy female collegians (classified into 3 body types of thin, normal, and obese according to Rohrer index) living in Busan were chosen as the subjects. The following are the results: Significant differences of skin temperature appeared in the parts of epigastrium (thin/normal>obese), anterior forearm (normal>thin/obese), and anterior leg (obese > thin/normal) as well as mean skin temperature. Mean skin temperature temporarily dropped owing to the exercise but tended to recover as time went by. Skin temperature of normal/thin shows higher than obese type. The change of skin temperature was noticed in the order of forehead > epigastrium > anterior forearm > anterior leg > anterior thigh (obese type) ; epigastrium > forehead > anterior forearm > anterior thigh > anterior leg (normal type) ; epigastrium > forehead > anterior forearm > anterior thigh > anterior leg (thin type, before and after exercise); epigastrium > forehead > anterior forearm > anterior leg > anterior thigh (thin type, during exercise). Significant differences were shown in the temperature change inside clothes according to somatotypes. No significant differences were revealed in thermal sensation, moisture sensation, and comfortable sensation according to body types and time.

얼굴의 조형적 특성에 따른 유형 분석 (An Analysis According to the Shape on Formative Attributes of a Face)

  • The objective of this study is to analyze the formative attributes of face by measuring the shape and features of face. The faces of women in 20's were taken by digital camera and measured, then it has conducted a statistical analysis using a SPSS for factor analysis, correlation and cluster analysis. The findings are that it is consisted of six(6) different factors and it is responsible for 73.93%. In Factor 1 and Factor 2, it has explained the most significant factor to determine the shape of face. The result on cluster analysis is that it is classified into 5 groups and it is as follows. Attributes of each group is that Group 1 has a wide and long forehead, small and longish chin-line and chubby cheeks that represent polished and modern images, while Group 2 has small and longish forehead and chin-line that represent classical and mature images. On the other hand, Group 3 has a narrow forehead, small and longish chin-line and upward-style eyebrows that represents provocative images, whereas Group 4 has a shaped style that represent intellectual images and Group 5 has small and longish forehead and chin-line and cheekbones that represent polished and cute images.

신생아에 있어 TcB[transcutaneous bilirubinometry]를 이용한 고빌리루빈혈증의 사정 (Assessment of Neonatal Hyperbilirubinemia Using a Transcutas Bilirubinometry)

  • Purpose: The purpose of the study is to investigate the relationship between total serum bilirubin(TSB) and transcutaneous bilirubinometry(TcB) in neonates with jaundice. Method: TcB from various sites(forehead, sternum, abdomen, buttock, hand, dorsalis-pedia) was measured using a JM-102 in a total of 102 neonate, 42 female and 60 male, with the mean 37.5 gestational week and the mean 2,903 gram of birth weight, as well as TSB from capillary punctures. Result: The mean bilirubin was 11.73 in serum, 20.55 on the forehead, 17.23 on the sternum, 16.19 on the abdomen, 18.22 on the buttock, 15.83 on the hand and 15.49 on the dorsalis-pedia. The relationship between TSB and TcBs were formulated by simple regression with 0.406 < r < 0.668(p < .000). A higher relationship was revealed between TSB and TCB at the forehead in infants of full-term, ABO incompatibility, and Hb greater than 16 mg/dl(r =0.725, 0.790, and 0.717, retrospectively). Phototherapy altered the measurement of TcB per site. Conclusion: TcB on the forehead is a reliable, noninvasive and convenient measurement of TSB in normal infants(Institutions need to establish quantitative equations representing the specific relationship between TSB and TCB according to the hemodynamic problems of infants such as ABO incompatibility, or low Hb).

THE STUDY ON THE TOPOGRAPHIC VARIATIONS OF FACIAL SKIN IN KOREAN ADULT FEMALES

  • This study was observed the variations in skin color, hydration, sebum content according to age groups at the forehead, both cheeks, and chin in 154 healthy Korean women subjects with aged 30 to 59. We selected four face sites (forehead, right cheek, left cheek, and chin) and measured with chromameter, corneometer, and sebumeter. All statistical analysis was performed on the computer software package SPSS 8.0. There was no significant difference of lightness between facial four regions in age groups but lightness showed to tend to age-related decrease on each regions. Redness and yellowness showed the highest values on chin and forehead respectively but there was no tendency to steady changes according to age variation. Both cheeks showed the highest hydration level and the lowest sebum content compared with forehead and chin in all age groups and there was tendency to age-related changes of sebum content on both cheeks. So we concluded there was decreasing tendency in lightness and sebum content according to the increase of age and there was no age-related change of skin hydration level at least in facial skin.(omitted)

Human Bites Over Nose: Management and Reconstruction

  • Background Human bite wounds in emergency department need evaluation in regard of reconstruction. These are due to occlusive bite injuries over face. Most commonly, human bites over face involve ear and nose, and may lead to avulsion injury. Defects over nose can be reconstructed immediately after debridement or delayed till the wound heals and scar becomes supple. Thorough wash and lavage with broadspectrum antibiotic cover has utmost importance in preventing cartilage infection. Methods We report 20 cases of human bite injuries over nose who presented to us in emergency department between 2018 and 2020. At the time of presentation the wound was assessed for closure. If not possible, patient was planned for delayed reconstruction after 3 months. In case delayed reconstruction was planned, the skin and nasal mucosa were approximated at first presentation. The patients underwent paramedian forehead flap after recreation of defect with conchal cartilage graft. Second stage of flap detachment and insetting was done after 3 weeks. After three weeks of second stage, third stage of flap thinning was done. Patients were followed for 3-6 months and subjective satisfaction was noted. Results Nineteen patients underwent delayed staged reconstruction with paramedian forehead flap and one underwent primary wound closure. The flap survival was 100%. The patient satisfaction was excellent in most cases. Conclusion We recommend delayed reconstruction for human bite nasal injuries. For reconstruction, paramedian forehead flap with conchal cartilage graft, if required, provides excellent reconstructive option with good contour and color match and minimal donor site scar.

Accuracy and Safety of Bedside External Ventricular Drain Placement at Two Different Cranial Sites : Kocher's Point versus Forehead

  • Objective : External ventricular drain (EVD) is commonly performed with a freehand technique using surface anatomical landmarks at two different cranial sites, Kocher's point and the forehead. The aim of this study was to evaluate and compare the accuracy and safety of these percutaneous ventriculostomies. Methods : A retrospectively review of medical records and head computed tomography scans were examined in 227 patients who underwent 250 freehand pass ventriculostomy catheter placements using two different methods at two institutions, between 2003 and 2009. Eighty-one patients underwent 101 ventriculostomies using Kocher's point (group 1), whereas 146 patients underwent 149 forehead ventriculostomies (group 2). Results : In group 1, the catheter tip was optimally placed in either the ipsilateral frontal horn or the third ventricle, through the foramen of Monro (grade 1) in 82 (81.1%) procedures, in the contralateral lateral ventricle (grade 2) in 4 (3.9%), and into eloquent structures or non-target cerebrospinal space (grade 3) in 15 (14.8%). Intracerebral hemorrhage (ICH) >1 mL developed in 5 (5.0%) procedures. Significantly higher incidences of optimal catheter placements were observed in group 2. ICH>1 mL developed in 11 (7.4 %) procedures in group 2, showing no significant difference between groups. In addition, the mean interval from the EVD to ventriculoperitoneal shunt was shorter in group 2 than in group 1, and the incidence of EVD-related infection was decreased in group 2. Conclusion : Accurate and safe ventriculostomies were achieved using both cranial sites, Kocher's point and the forehead. However, the forehead ventriculostomies provided more accurate ventricular punctures.

이마에 발생한 석회화된 지방종 (Calcified Lipoma of the Forehead)

  • Purpose: Although lipoma is known as one of the most common soft tissue tumors, calcification in lipoma is very rare in its occurrence. This calcified lipoma has been reported by some as a result of regression of lipoma, but its genesis is not clearly known yet with various opinions being discussed regarding its possible metabolic relation to hypercalcemia or hyperphosphatemia to be considered as a regression phase of lipoma. The authors would like to present this unusual case of calcified lipoma. Methods: A 50-year-old male patient visited our hospital with complaint of an enlarging mass on his right forehead which has been acknowledged for 5 years' period. On physical examination, a mass was observed on his forehead as palpable, non-tender, mobile and firm in its consistency. Ultra sonogram examination revealed a well-demarcated mass (1.92 cm) with central echoic zone at deep layer of forehead. Mass excision and biopsy were performed subsequently. Results: According to the pathological report, the diagnosis confirmed the lipoma consisting of grown-up adipocyte and calcification. Neither growing lipoma nor relapse was observed for postoperative three years' follow-up of the patient. Conclusion: Now that the calcified lipoma was successfully removed and cured by a simple mass excision, authors hereby report the case of calcified lipoma on forehead.

  • Patients with thick skin mainly exhibit the aging processes of sagging, whereas patients with thin skin develop wrinkles or volume loss. Asian skin is usually thicker than that of Westerners; and thus, the sagging of skin due to aging, rather than wrinkling, is the chief problem to be addressed in Asians. Asian skin is also relatively large in area and thick, implying that the weight of tissue to be lifted is considerably heavier. These factors account for the difficulties in performing a facelift in Asians. Facelifts can be divided into forehead lift, midface lift, and lower face lift. These can be performed individually or with 2-3 procedures combined.
