Effect of Forceps Jars with Holes on the Drying Time, Rate of Wet Pack, Cost of Rewashing Linen and Maintenance of Sterilization (타공작업이 이동겸자의 건조시간, Wet Pack율, 린넨 세탁비용 및 오염여부에 미치는 영향)

  • Objectives : This study aims to design a method for drying forceps jars. As part of the process, holes were made in the forceps jars. Drying time, rate of wet pack, and cost of rewashing linen were then determined. Moreover, the study looked into the bacteria incidence rate in the forceps jars. This study suggests a practical way for preventing infection in a hospital through a surgical device. Methods : This study investigated the effect of forceps jars with holes on reducing drying time, rate of wet pack, and cost of rewashing linen. It also looked into the bacteria incidence rate in the forceps jars through a parallel design. Data was collected and analyzed using SPSS version 21. Results : The drying time of a forceps jars was remarkably reduced from $18.44{\pm}0.2$ minutes(non-hole forceps jars) to $0.58{\pm}0.02$ minutes(4-hole forceps jars). The rate of wet pack of non-hole forceps jars was 51.4%. However, the rate of 4-hole forceps jars reached 4.6%. The cost of rewashing linen saw a decline from 38,073 won to 3,381 won. In addition, no bacteria and virus were detected from the 24-hours usage of forceps jars even though the jars had holes. Conclusions : The 4-hole forceps jars greatly increased work efficiency as a result of the reduced drying time, rate of wet-pack and cost of rewashing linen. The 4-hole forceps jars did not influence their sterilizing effect.

Comparative evaluation of efficacy of Physics Forceps versus conventional forceps in pediatric dental extractions: a prospective randomized study

  • Background: This study aimed to determine the efficacy of Physics Forceps in pediatric dental extractions. Methods: This was a double-blind, randomized controlled trial with a parallel-arm design and identical allocation ratio (1:1). Children (n=104) were randomly divided into two groups for extraction of mandibular primary teeth (group I: Physics Forceps; group II: conventional forceps). The outcome variables assessed in the study were the time taken for extraction, pre- and postoperative anxiety (using RMS pictorial scale), incidence of fractured teeth, and postoperative pain on the first and third days (using the Wong-Baker faces pain scale). Results: A significant reduction (P < 0.001) in intraoperative time, anxiety, and incidence of tooth fracture was confined to group I. The pain significantly reduced from the first to the third postoperative day in both groups, but the mean reduction in RMS scores in the physics forceps group was far better than that in the conventional forceps group. Conclusion: Physics Forceps aid in extraction of primary teeth with minimal trauma to supporting structures, as well as reducing anxiety in the pediatric population.

Evaluation of the safety and efficacy for the technique of removing VFB from the bronchial tree in infants and early childhood using Fogarty balloon catheter. (Fogarty balloon catheter를 이용한 영유아 기관지 식물성 이물 제거술의 의의)

  • Background and Objectives: Vegetable Foreign bodies (VFB) in the bronchial tree may be complicated by fragmentation, slippage and impaction during the removal with forceps. This study is to evaluate the safety and efficacy for the technique of removing VFB from the bronchial tree in infants and early childhood using Fogarty balloon catheter. Materials and methods : The subjects consisted of 18 infants and early childhood (7-22 months old) with VFB in the bronchial tree from January 1991 through October 1998. The authors first attempted removal of VFB with forceps and if that failed, removed VFB with Fogarty arterial embolectomy catheter under the ventilating bronchoscopy and general anesthesia. Results: We removed 6 VFB with forceps. could not remove anymore, and so removed 12 VFB with Fogarty catheter. In 8 VFB of less than 24 hours, we could remove 6 VFB with forceps and 2 VFB which could not be removed with forceps were removed with Fogarty catheter. In 10 VFB of more than 24 hours, we could not remove with forceps and removed with Fogarty catheter. Conclusions : VFB in the bronchial tree of infants and early childhood can usually be removed with forceps. But we think that Fogarty balloon catheter technique is a easy, safe method for the removal of bronchial VFB of more than 24 hours, fragmentation, impaction, lower bronchus and too round or slippery to remove with forceps in infants and early childhood.

  • Object : To report the case of the improved patient after the extraction with hemostst forcept. Methods : We treated one patient who visited Gabsan Korean medical clinic.First two times, We used acupuncture, fumigation, exsanguination, external medincine. He had some agonistic response. But It was not satisfactory. So We operated extraction with hemostat forceps. Result and Conclusion : 1. We extracted nasal polyp, the length of polyp was 5.5 cm2. After extraction, the patient had not headache, nasal congestion and so on.3. On intranasal endoscopy, left nose did not have polyp any more.4. Hemostat forceps extraction is effective to nasal polyp.

A Microbiological Study of Sterilized Materials on Dressing Carts (병원"드레싱"상중 일부물품의 오염도에 관한 조사연구)

  • Bacterial cultures of the transfer forceps, 4"$\times$4"mesh gauze and polluted air of nursing units of general surgery, internal medicine, gynecology and pediatrics of S.N.U.H. were carried on for S consecutive days (from 17th to 21s1 August, 1972) to investigate the degree of contamination of such instruments which were used in dressing the patients. 1. The average hospitalized patients for each nursing unit were 24 persons. 2. The frequency of dressing, and using forceps and gauze in surgical nursing unit were 25,316 and 66 times respectively. Actually the forceps were used most frequently. There was no dressing in the nursing unit of internal median. 3. Most of dressing were carried on from 11 : 00 to 13 : 59 o′clock. 4. Averagely 121.5 persons passed through each nursing unit. 549 visitors dropped in the nursing unit of gynecology in maximum, and 219 visitors in the nursing of internal medicine in minimum. The visitors rushed in from 11 : 00 to 13 : 59 o′clock to he 142.5 persons for each nursing unit in average. 5. Colony counts after the bacterial cultures of the forceps and gauze disclosed 1098.44 and 704.51 per plate respectively. 6. The degree of contamination was parallel to the duration and frequency of dressings. 7. The degree of contamination of the instruments was severest in the nursing unit of internal medicine, and !east in the nursing unit of gynecology. On the matter of gauze, the contamination was most extreme in the pediatric nursing unit. 8. There was no significant daily differences in terms of contamination throughout the week. 9. It was disclosed that empty forceps jars were less contaminated than the jars with solution of any kind. 10. Bacterial cultures of the polluted air in corridor showed 73,88 colony counts per plate.

Effect of Endoscopic Forceps on the Quality of Duodenal Mucosal Biopsy in Healthy Cats (고양이 십이지장 점막 생검 시 내시경 생검 겸자가 조직의 질에 미치는 영향에 관한 연구)

  • Based on the results of previous studies, endoscopic biopsy sample's quality has a major impact on its adequacy for histopathology, and that the nature of the biopsy forceps can influence the specimen quality. The present study compared the effects of three different types of endoscopic biopsy forceps and two different operators on sample quality and adequacy for histopathology in three healthy cats. Every biopsy was performed between the major papilla and caudal duodenal flexure, and each operator performed five biopsies with each type of forceps on each cat, for a total of 90 biopsies. One pathologist evaluated the quality and adequacy of the obtained samples. Biopsies performed with large-cup forceps provided heavier and longer samples than the standard round forceps. With the same size forceps, the presence of alligator teeth had no effect on sample quality or adequacy for histopathological examination and assessment. Based on the results of the present study, although the standard round forceps could be used to obtain adequate samples for histopathology, large-cup forceps such as the standard oval and alligator jaw type have the advantage of obtaining high quality endoscopic samples.

Early Clinical Outcomes of Thoracoscopic Major Pulmonary Resection and Thymectomy Using Novel Articulating Endoscopic Forceps

  • Background: Video-assisted thoracoscopic surgery (VATS) is recognized as a safe and effective treatment modality for early-stage lung cancer and anterior mediastinal masses. Recently, novel articulating instruments have been developed and introduced to endoscopic surgery. Here, we share our early experiences with VATS major pulmonary resection and thymectomy performed using ArtiSential articulating instruments. Methods: At the Seoul Metropolitan Government-Seoul National University Boramae Medical Center, 500 patients underwent VATS pulmonary resection between July 2020 and April 2023, while 43 patients underwent VATS thymectomy between January 2020 and April 2023. After exclusion, 224 patients were enrolled for VATS major pulmonary resection, and 38 were enrolled for VATS thymectomy. ArtiSential forceps were utilized in 35 of the 224 patients undergoing pulmonary resection and in 12 of the 38 individuals undergoing thymectomy. Early clinical outcomes were retrospectively analyzed. Results: No significant differences were observed in sex, age, surgical approach, operation time, histological diagnosis, or additional procedures between the patients who underwent surgery using novel articulating instruments and the group treated with conventional endoscopic instruments for both VATS major pulmonary resection and thymectomy. However, the use of the novel articulating endoscopic forceps was associated with a significantly larger number of dissected lymph nodes (p=0.028) and lower estimated blood loss (p=0.009) in VATS major pulmonary resection. Conclusion: Major pulmonary resection and thymectomy via VATS using ArtiSential forceps were found to be safe and effective, with early clinical outcomes comparable to established methods. Further research into long-term clinical outcomes and cost-effectiveness is warranted.

Bronchial Brushing and Bronchial Washing for Diagnosis of Central Lung Cancer (중심형 폐암 진단을 위한 기관지찰과술과 기관지세척술)

  • Background : Forceps biopsy, bronchial brushing, and bronchial washing are used in conjunction with bronchoscopy to provide specimens for histologic and cytologic analysis in patients with suspected lung cancer. This study was performed to evaluate how many times brushing should be done and how much fluid should be used during bronchial washing for increasing diagnostic yield, and to evaluate which combination of these procedures gives the highest diagnostic yield. Methods : Forty patients, with suspected lung cancer, who had bronchoscopically visible lesions were enrolled in this prospective study. During one bronchoscopic examination four forceps biopsies, four bronchial brushings, and bronchial washing were done in all patients. The patients were divided into four groups by the amount of normal saline used for bronchial washing; group I, 10 ml ; group II, 20ml ; group III 30ml, and group IV, 40ml. We analyzed the results in 36 patients confirmed as lung cancer. Results : The diagnostic sensitivity of bronchial washing before and after forceps biopsy and bronchial brushing were 36% and 28%, respectively. The cumulative diagnostic sensitivity of bronchial washing was 47% and significantly higher than that of bronchial washing before or after forceps biopsy and bronchial brushing (p<0.05). The diagnostic sensitivity of bronchial washing with saline of 30ml was significantly higher than that of bronchial washing with saline of 10ml or 20ml (p<0.05). The diagnostic sensitivity of the first brushing was 75%, the second brushing 78%, the third brushing 83%, and the fourth brushing 67%. With repeated brushing up to three times, the diagnostic sensitivity increased to 92% (p<0.05). However, inclusion of the fourth brushing did not give a further increase of the diagnostic sensitivity. The diagnostic sensitivity of forceps biopsy was 86%. The diagnostic sensitivities of forceps biopsy by the type of bronchial lesion were as follows: tumor, 88%; infiltration, 67%; infiltration with nodularity, 80%; and collapse, 100%. The combination of forceps biopsy and bronchial washing gave a diagnostic sensitivity of 89%. The diagnostic sensitivity of combining forceps biopsy with bronchial brushing was 97%. Addition of bronchial washing did not increase the diagnostic yield over forceps biopsy and bronchial brushing. Conclusion : In patients with central lung cancer, forceps biopsies and repeated brushings up to three times should be done for maximal diagnostic yield.

Development of Efficient Screening Method for Resistance of Cabbage Cultivars to Black Rot Disease Caused by Xanthomonas campestris pv. campestris (양배추 검은썩음병에 대한 효율적인 저항성 검정법)

  • Black rot caused by Xanthomonas campestris pv. campestris (Xcc) is one of the most serious diseases of crucifers world-wide. To establish the efficient screening method for resistant cabbage to Xcc, different inoculation methods, inoculation positions, growth stages of seedlings, and incubation temperatures after inoculation were investigated with the seven cabbage cultivars showing different resistance degrees to the pathogen. Clipping with mouse-tooth forceps was better inoculation method than piercing with 18 pins or cutting with scissors to distinguish the level of resistance and susceptibility. In inoculation using mouth-tooth forceps, clipping the edges of the leaves near veins is more effective than injuring the veins of the leaves directly. In addition, the inoculated plants kept at $22^{\circ}C$ showed more clear resistant and susceptible responses than those kept at 26 or $30^{\circ}C$. On the basis of the results, we suggest that an efficient screening method for resistance of cabbage cultivars to black rot is to clip the edges of the leaves near veins of the four-week-old seedlings with mouth-tooth forceps dipped in a suspension of Xcc at a concentration of $7{\times}10^7$ cfu/ml and incubate the inoculated plants in a growth room at $22^{\circ}C$ with 12-hr light a day.
