Comparison of Pulmonary Function, Respiratory Muscle Strength, and Diaphragm Thickness between Underweight and Normal Adults

  • PURPOSE: This study aimed to investigate the relationship between being underweight and respiratory function indicators such as pulmonary function, respiratory muscle strength, and diaphragm thickness in normal adults without lung disease. METHODS: The participants in this experiment were thirty young adults. To compare the respiratory function between the underweight and normal weight individuals, 15 participants were selected from each of the underweight and normal weight groups based on body mass index. Respiratory function tests were conducted through pulmonary function tests and respiratory muscle strength tests. Diaphragm thickness was measure with ultrasonography, and physical characteristics were obtained from grip strength and waist circumference. An independent t-test was used to compare the averages of the parameters measured in the two groups. RESULTS: In the respiratory function tests between the two groups, statistically significant differences (p < .05) emerged in the ratio of the predicted forced vital capacity (%FVC), the ratio of the predicted forced expiratory volume in one second (%FEV1), maximal expiratory pressure (MEP), and diaphragm thickness at the functional residual capacity (FRC). There was no statistically significant difference in the forced vital capacity, forced expiratory volume in one second, maximal inspiratory pressure, diaphragm thickness at the total lung capacity, and thickening ratio (p > .05). CONCLUSION: Decreases in some variables of respiratory function, such as the %FVC, %FEV1, MEP, and diaphragm thickness at the FRC were observed in underweight subjects. However, it is difficult to determine whether it affected the overall respiratory function. Future studies are needed to clearly identify the relationship between being underweight and respiratory function.

  • Background: The pulmonary function test is the most basic test method to diagnosis lung disease. The purpose of this study was to research the correlation of the body mass index (BMI), the fat percentage of the body mass (fat%), the muscle mass, the fat-free mass (FFM) and the fat-free mass index (FFMI), waist-hip ratio (WHR), on the forced expiratory volume curve. Methods: Between March and April 2009, a total of 291 subjects were enrolled. There were 152 men and 139 female (mean age, $46.3{\pm}9.92$ years), and they were measured for the following: forced vital capacity (FVC), forced expiratory volume at 1 second ($FEV_1$), and forced expiratory flow during the middle half of the FVC ($FEF_{25-75}$) from the forced expiratory volume curve by the spirometry, and the body composition by the bioelectrical impedance method. Correlation and a multiple linear regression, between the body composition and pulmonary function, were used. Results: BMI and fat% had no correlation with FVC, $FEV_1$ in male, but FFMI showed a positive correlation. In contrast, BMI and fat% had correlation with FVC, $FEV_1$ in female, but FFMI showed no correlation. Both male and female, FVC and $FEV_1$ had a negative correlation with WHR (male, FVC r=-0.327, $FEV_1$ r=-0.36; p<0.05; female, FVC r=-0.175, $FEV_1$ r=-0.213; p<0.05). In a multiple linear regression of considering the body composition of the total group, FVC explained FFM, BMI, and FFMI in order ($r^2$=0.579, 0.657, 0.663). $FEV_1$ was explained only fat% ($r^2$=0.011), and $FEF_{25-75}$ was explained muscle mass, FFMI, FFM ($r^2$=0.126, 0.138, 0.148). Conclusion: The BMI, fat%, muscle mass, FFM, FFMI, WHR have significant association with pulmonary function but $r^2$ (adjusted coefficient of determination) were not high enough for explaining lung function.

뇌성마비 환자의 자세 차이가 호흡 기능에 미치는 영향 (Effects of Posture Difference on the Respiratory Function of Cerebral Palsy Patients)

  • Purpose : The purpose of this study was to investigate the effect of posture difference on respiratory function in cerebral palsy patients. Methods : Twenty-two cerebral palsy childrens were recruited this study. Respiratory Function test was measured with Cardio Touch 3000 and Micro Respiratory Pressure Meter. Cardio Touch 3000 was used to assess cerebral palsy childrens' forced vital capacity and forced expiratory volume at one second. Micro Respiratory Pressure Meter was to assess Maximum inspiratory pressure and Maximum expiratory pressure. Subjects had four respiratory functions measured in supine, slouched sitting, and elected sitting postures. Statistical analysis was used Paired t-test for within-group comparisons and Independent t-test for between-group comparisons. SPSS statistics Ver 20.0 was used for statistical anlysis and statistical significance was defined as a p-value less than 0.05. Result : The subjects' respiratory function according to posture showed significant difference in Forced Vital Capacity(FVC), Maximum Expiratory Pressure(MEP) and Maximum inspiratory pressure(MIP)(p<.05). Elected sitting posture had a positive effect on respiratory function than slouched sitting, supine. Conclusion : In conclusion, We could see that change of posture in children with cerebral palsy affects respiratory function and Elected sitting can be a positive help for the respiratory function of children with cerebral palsy.

수술 후 보행지지 프로그램이 노인 상복부 수술환자의 폐 환기능 회복에 미치는 효과 (The Effects of Walking-Support Program on the Pulmonary Ventilatory Functions of Elders Following Upper-abdominal Surgery)

  • Purpose: The purpose of this study was to evaluate the effects of a walking-support program on the pulmonary ventilatory functions of elderly people who had undergone upper-abdominal surgery. Method: The study was a quasi-experimental research design. There were 26 partcipants who were admitted for upper-abdominal surgery to P University Hospital in B city. Walking exercise education was provided individually to the experimental group the day before their operation and 20 minutes a day for five days after the surgery using the 'Walking Exercise Guide Document'. Pulmonary ventilatory function was with FVC (Forced Vital Capacity), PEF (Peak Expiratory Flow), FEVI (Forced Expiratory Volume in 1 Second), FER (Forced Expiratory Ratio), Oxygen Saturation, and VAS (Visual Analog Scale). Results: The objective indexes of pulmonary ventilatory function were not significantly different between the two groups, but the subjective index was significantly different. Conclusion: With the above results, the walking support program could be an effective nursing intervention for improving pulmonary ventilatory function of surgical patients.

흉곽저항운동이 만성폐쇄성폐질환 환자의 1초간 노력성 호기량과 피로도에 미치는 영향 (Effects of Chest Resistance Exercise on Forced Expiratory Volume in One Second and Fatigue in Patients with COPD)

  • Purpose: The purpose of this study was to determine the effects of chest resistance exercise on Forced Expiratory Volume per second and on fatigue in patients with chronic obstructive pulmonary disease (COPD). Methods: In all, 62 male patients with COPD were included in this study. The experimental group included 32 patients who were treated with chest resistance exercise using the PNF technique with medication. The control group included 30 patients who were treated only with medication. Subjects were stratified into the 2 groups by randomized clinical sampling. Before the start of the experiment, forced expiratory volume at the first second (FEV1) and lactic acid were tested in both experimental and control groups. The experimental group did chest resistance exercise for 6 weeks, 4 times per week, 30 min per day, and the effects of this exercise in patients with COPD was determined by comparing the results of FEV1 and lactic acid tests before and after the experiment between and within the experimental and control groups. Results: There was a statistically significant within group difference for FEV1 MEAS and FEV1 %PRED. There was statistically significant control group of FEV1 MEAS and FEV1 %PRED There was a statistically significant difference in the experimental group for fatigue, comparing scores before and after the test. There was a statistically significant control group of fatigue, in comparison of between the groups of FEV1 MEAS, FEV1 %PRED, fatigue(p<0.01)(p<0.05). Conclusion: More research on COPD will be necessary for improving pulmonary function and reducing fatigue. Further studies on COPD will be required for improving pulmonary function and reducing of fatigue.

Influence of Evjenth-Hamberg Stretching on the Lung Function of Adults with Forward Head Posture

  • This study was conducted to examine the effects of Evjenth-Hamberg stretching of the sternocleidomastoid, upper trapezius, and pectoralis major on the lung function of adults with forward neck posture. The subjects were 20 adult students in P university located in Pohang, Korea, whose degree of head forward displacement measured according to NEW YORK state posture test was mild. The subjects were randomly and equally assigned to the Evjenth-Hamberg Stretching group (EHSG, n=10) and the control group (CG, n=10). Their forced vital capacity (FVC), slow vital capacity (SVC), and maximal voluntary ventilation (MVV) were measured before and after the experiment. In within-group comparison, only the EHSG experienced statistically significant improvement in FVC, forced expiratory volume in the first second (FEV1), and peak expiratory flow (PEE) after the experiment, compared to before the experiment (.05

  • Purpose: This study examined whether breathing exercises might increase the chest expansion and pulmonary function of stroke patients. Methods: Twenty four patients with stroke were assigned randomly into two groups: a combination of diaphragmatic resistive breathing and pursed-lip breathing exercise (CB) group (n=10) and control group (n=14). The CB group completed a 4-week program of diaphragmatic resistive breathing and pursed-lip breathing exercise. The subjects were assessed using the pre-test and post-test measurements of the chest expansion (length for resting, deep inspiration, deep expiration, deep expiration-inspiration) and pulmonary function (forced vital capacity (FVC), forced expiratory volume at one second (FEV1), FEV1/FVC, peak expiratory flow (PEF), vital capacity (VC), tidal volume (TV), expiratory reserve volume (ERV), inspiratory reserve volume (IRV)). Results: A comparison of the chest expansion between the pre and post tests revealed similar rest, deep inspiration, deep expiration, and deep expiration-inspiration lengths in the CB and control groups (p>0.05). A comparison of the pulmonary function between pre and post tests, revealed significant improvements in the FVC, FEV, PEF, VC, IRV, and ERV in the CB group (p<0.05). There was a significant difference in the FVC, FEV1, PEF, VC and IRV between the 2 groups (p<0.05). Conclusion: These findings suggest that breathing exercise should help improve the pulmonary function, such as the volume and capacity. This suggests that the pulmonary functions of stroke patients might be improved further by a continued respiratory exercise program.

소아 천식에서 최대호기중간유량의 기관지 과민성 예측인자로서의 의의 (Utility of a forced expiratory flow of 25 to 75 percent as a predictor in children with asthma)

  • 목 적 : 천식은 하부기도의 만성 염증으로 정의될 수 있으며, 기관지과민성은 천식의 병태생리적인 특징이다. 하부기도를 직접적으로 평가할 수는 없지만 최대호기중간유량(forced expiratory flow between 25 and 75 percent, $FEF_{25-75}$)이 하부기도의 직경을 비교적 잘 반영하는 것으로 알려져 있다. 본 연구에서는 1초간호기량(forced expiratory volume in 1 second, $FEV_1$)과 $FEF_{25-75}$를 이용하여 얻어진 이들의 차이(difference between $FEV_1$ and $FEF_{25-75}$, DFF)와 비(ratio between $FEV_1$ and $FEF_{25-75}$, RFF)를 분석하여 기관지과민성과의 연관성을 알아보고자 하였다. 방 법 : 만 6세에서 15세 사이의 583명을 대상으로 하였다. 전체 대상자에서 폐기능 검사, 메타콜린 흡입 유발시험을 시행하였고, 혈액 내 호산구수, 혈청 총 IgE 농도, 혈청 ECP 농도를 측정하였다. 메타콜린 흡입 유발시험으로 얻어진 $PC_{20}$을 기준으로 기관지과민성 양성군($PC_{20}$>16 mg/mL)과 음성군($PC_{20}=16mg/mL$)을 정의하였으며, 그 중증도에 따라 4군으로(Group 1: <1 mg/mL; Group 2: 1-4 mg/mL; Group 3: 4-16 mg/mL; Group 4: >16 mg/mL, by American Thoracic Society, 1999) 분류하여 분석하였다. 결 과 : DFF는 기관지과민성 양성군에서 4.0 (-6.0-13.0), 음성군에서 -5 (-18.0-6.0)로 양성군에서 음성군에 비해 유의하게 높게 나타났다 (P<.001). RFF도 기관지과민성 양성군에서 1.07 (0.93-1.21), 음성군에서 0.95 (0.84-1.07)로 양성군에시 음성군과 비교하여 의미있게 높았다(P<.001). 또한 기관지과민성 중증도에 따라 나눈 4군 사이에서도 DFF (P<.001)와 RFF (P<.001) 모두 유의한 차이를 나타내었다. $PC_{20}$은 DFF ($\gamma$=0.337, P< .001) 및 RFF ($\gamma$=0.337, P<.001)와 의미있는 음의 상관 관계를 보였다. 결 론 : $FEF_{25-75}$를 이용하여 얻어진 DFF와 RFF는 기관지과민성과 밀접한 연관성을 보였고 이를 반영하는 지표가 될 수 있는 가능성을 보여주었다.

촛불끄기 호흡운동 프로그램의 시행이 성인의 노력성 호흡량의 기능향상에 미치는 영향 (Effect of Implementing Candle-Blowing Respiratory Exercise Program on Functional Improvement of Forced Breathing Volume among Adults)

  • 본 연구는 노력성 호흡 운동 중심의 촛불끄기 호흡훈련이 성인의 몸통 근력 강화를 의미하는 호흡 능력 향상에 어떤 유의한 효과가 있는지를 알아보기 위하여 실시하였다. 4주간의 촛불끄기 훈련을 통해 실험 전-후의 노력성폐활량(FVC)은 통계학적으로 유의하게 증가하였다(p<.05). 4주간의 촛불 끄기 훈련을 통해 실험 전-후의 1초간 노력성 날숨량(FEV1)의 변화는 통계학적으로 유의한 차이를 보이지 않았다(p>.05). 4주간의 촛불 끄기 훈련을 통해 실험 전-후의 최대날숨유속(PEF)은 유의하게 증가하였다(p<.05). 본 연구는 건강한 성인을 대상으로 노력성 호흡 운동 중심의 촛불끄기 호흡운동을 실시하여 성인의 몸통 근력 강화를 의미하는 노력성 폐활량(FVC), 1초간 노력성 날숨량(FEV1), 최대날숨유속(PEF) 등의 노력성 호흡훈련과 허파기능의 관계에 대해 조사하여 유의한 결과를 얻었다. 앞으로 물리치료가 꼭 필요한 중증 호흡기계 질환자를 대상으로 하는 연구가 진행되어야 할 것으로 사료된다.

일부 농촌 지역 성인에서 고혈압과 폐기능의 관련성 (Association between Hypertension and Pulmonary Function in Rural Adults in Korea)

  • Objectives : Whilst hypertension exerts a negative effect on several organs there have been few studies regarding its effect on pulmonary function. The objective of this study was to examine the relationship between hypertension and pulmonary function in rural Korean adults Methods : In 2006, 2534 people were recruited, aged 40 to 70, in Kangwha County. We selected 1454(male: 624, female: 830) participants whose pulmonary function results were repeatable. Blood pressure(BP) was measured twice and the average calculated. Participants were divided into two groups(hypertensive group and non-hypertensive group) in accordance with The Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure. Pulmonary function was measured by dry rolling seal spirometry. Forced expiratory volume in the one second and forced vital capacity were converted into percent-predicted values based on average pulmonary function amongst Koreans. Results : The number of hypertensive participants in the present study was 460(male: 205, female: 255) and the number of non-hypertensive participants was 994(male: 419, female: 575). Our findings have shown that the mean values for expiratory volume in the one second and forced vital capacity were significantly lower for hypertensive people than for non-hypertensive people, among women(P=0.002 for forced expiratory volume in the one second, p<0.001 for forced vital capacity volume). Odds ratio analysis revealed that hypertensive participants were more likely to have lower pulmonary function than non-hypertensive participants, again significantly among women. Conclusions : The pulmonary function of hypertensive women was significantly lower than that of non-hypertensive women aged 40-70.
