식중독에 의한 사회경제적 손실을 최소화하기 위해서는 식중독과 관련된 경제적 비용 손실을 추정하는 것이 중요하다. 하지만 자료의 부족과 다양한 비용 항목의 존재로 식중독과 관련된 사회경제적 손실비용을 정확하게 추정하기에 어려운 점 있다. 본 연구에서는 국내 식중독 발생에 의한 손실비용을 추정하기에 앞서 국외에서 사용되는 사회경제적 비용항목의 기반 자료 확보를 목적으로 수행되었다. 2009-2019년 국외 식중독과 관련된 사회경제적 손실비용 측정 연구의 사례 조사를 위해 문헌 조사를 실시하였다. 이 연구에서는 사회경제적 손실비용을 크게 의료기간에 방문한 외래/입원환자 및 의료기관에 미방문한 경험환자로 구분하였다. 또한 이들의 직접비용 및 간접비용을 고려하였고, 더 나아가 기업비용 및 행정비용 항목으로 구분하여 조사하였다. 조사 결과 문헌별, 나라별 상이한 비용항목을 사용하여 손실비용을 추정하는 것으로 관찰되었다. 직접의료비의 경우 모든 연구에서 외래 또는 입원진료비를 선정하여 비용을 산출한 것으로 관찰되었기 때문에 나라별 의료서비스의 체제 및 비용에 따라 항목을 선정해야 한다. 직접비의료비의 경우 몇 몇의 연구에서 외래 방문에 소요된 교통비만을 고려하였기 때문에 간병비 고려 여부에 대해서는 더 논의가 필요할 것으로 사료된다. 간접비용 중 조기사망비용, 작업손실비용, 여가손실비용 및 삶의 질 저하/고통비용은 고려하였으나, 병문안 기회비용은 모든 연구에서 고려하지 않았다. 직접의료비와 마찬가지로 행정비용의 경우에도 국가별 정부 예산이 상이하기 때문에 각 나라에 맞는 항목을 고려해야할 것이다. 따라서 이러한 조사 결과를 바탕으로 국내 식중독 발생에 의한 사회경제적 손실비용 분석을 위해 어떠한 비용 항목을 고려해야 할 것인지에 대한 검증절차가 필요할 것이다. 본 연구는 국내 식중독 발생에 의한 사회경제적 손실비용 분석을 위한 기초적인 정보를 제공할 것이다.
Norovirus is a major cause of viral gastroenteritis and a common cause of foodborne and waterborne outbreaks. Norovirus outbreaks are responsible for economic losses, most notably to the public health and food industry field. Norovirus has characteristics such as low infectious dose, prolonged shedding period, strong stability, great diversity, and frequent genome mutations. Besides these characteristics, they are known for rapid and extensive spread in closed settings such as hospitals, hotels, and schools. Norovirus is well known as a major agent of food-poisoning in diverse settings in South Korea. For these reasons, nationwide surveillance for norovirus is active in both clinical and environmental settings in South Korea. Recent studies have reported the emergence of variants and novel recombinants of norovirus. In this review, we summarized studies on the molecular epidemiology and nationwide surveillance of norovirus in South Korea. This review will provide information for vaccine development and prediction of new emerging variants of norovirus in South Korea.

이 논문에서는 1999년 1월 1일부터 2010년 6월 30일까지 전국 72개 관측소에서 측정된 우리나라 기상자료를 평균연결법에 의한 계층적 병합방법을 통해 군집분석을 실시하고 각 기상자료에서 유도된 군집의 특성을 파악해 본다. 이 분석에서 유도된 군집과 2010년 기후변화에 따른 식중독 발생연구에서 사용되었던 산맥을 경계로 구분한 군집을 비교해 본다.
현재 우리나라의 급식산업체에서는 급식활동에 대한 위생관리를 철저히 하기 위하여 새로운 위생관리시스템을 모색하고 있으며, 일부 대기업이 운영하고 있는 급식산업체에서는 식품제조업체에서 이미 적용하고 있는 HACCP 시스템을 적용하기 위하여 노력하고 있다. 국내 중소기업이 운영하고 있는 급식장에서는 정보 및 전문가의 부족으로 HACCP Plan을 체계적으로 개발하는데 상당한 부담을 느끼고 있어 정부나 관련기관의 지원이 필요한 실정이며, 정부에서는 현재 급식장에 HACCP을 적용하기 위하여 시범사업을 실시하고 있는 등 이에 따른 노력을 기울이고 있다. 학계에서도 HACCP Plan에 대한 연구보고가 간혹 있어 왔으나 이러한 연구의 대부분이 몇 가지 메뉴에 치우쳐 있어 매일 다양한 수십가지의 메뉴가 제공되고 있는 급식장 전체에 대하여 HACCP을 적용하기 어려워 급식장에서 제공되는 전체 메뉴의 HACCP적용을 위한 통합적인 HACCP Plan이 절실히 필요하였다. 본 연구에서는 단체급식시설에서 HACCP 시스템을 적용하기 위하여 위해분석, 중요관리점의 설정, 관리기준 설정, 중요관리점을 관리하기 위한 모니터링 방법의 설정, 수정조치의 확립, 검증절차의 확립 및 기록유지시스템의 확립과 같은 HACCP 기본적인 7가지 원칙에 의거하여 HACCP Pln이 개발되었으며, 검수에서부터 배식까지 모든 작업단계에서 확인된 많은 위해요소가운데 12개의 위해요소가 중요관리점(CCP)로 설정이 되었고 검수단계에서 운송차량의 배송온도가 유일한 관리점 (CP)로 설정되었다. 급식장에서 일어날 수 있는 위해는 수없이 많으며 본 연구에서 제시한 Generic HACCP Model은 1, 2명의 영양사와 몇 명의 조리사가 작업을 하는 일반적인 급식장의 현실에 맞도록 급식장에서 실제적으로 사용할 수 있도록 비교적 간단하게 계획되어 급식장에서 일어날 수 있는 모든 위해를 통제할 수는 없으므로 각 급식장의 현실에 맞도록 재구성하여 적용하는 것이 필요하다. 무엇보다도 급식장의 조리종사자를 대상으로 작업 위생을 위한 교육ㆍ훈련 프로그램이 선행 또는 병행되어야 HACCP의 효과를 기대할 수 있으며, HACCP 시스템을 성공적으로 급식부문에 도입하기 위해서는 급식장 현실에 맞는 전문적인 교육ㆍ훈련 프로그램의 개발이 필요하다. 또한 급식 경영자의 식품에 대한 안전성 의식 및 HACCP 시스템 실행 의지, 작업장 시설의 개선과 조리기계·기구의 위생 규격 강화와 철저한 유지ㆍ관리, 식재료 입고에서부터 배식까지 절차, 방법, 기준 등의 표준화, 특히 메뉴 레시피의 표준화 등은 HACCP 시스템을 급식장에 성공적으로 도입하기 위하여 필요한 요건들이라 할 수 있다.
본 연구는 국내에서의 가정내 냉장고 온도에 대한 조사를 수행하여, 현 시점에서의 냉장고에서의 식품보관 온도분포를 추정하였고, 이를 MRA(미생물 위해평가: Microbial risk assessment)의 입력변수로 활용할 수 있도록 적정 확률분포 모델을 제시하였다. 일반적으로 가정내 냉장고에서의 식품 보관온도는 식중독 발생 등에서 있어 중요한 위해인자로 작용하는 것으로 알려져 있다. 조사대상 가구는 총 139가구이었으며, 조사기간은 2009년 5월에서부터 9월까지 data logger를 이용하여 측정하였다. 조사된 냉장고 온도의 평균은 $3.53{\pm}2.96^{\circ}C$로, $5^{\circ}C$ 이상은 23.6%로 나타났다. 수집된 온도자료는 @RISK를 이용, 적합성 검정(GOF: K-S와 AD test)을 수행하여 적정 확률분포모델에 대해 추정하였고, 이중 LogLogistic(-10.407, 13.616, 8.6107)분포 모델이 가장 적절한 국내에서의 가정내 냉장고 식품보관 온도분포 모델로 나타났다. 이 확률분포 모델은 MRA적용에 있어 노출평가에서 입력변수로서 직접적 활용이 가능하다고 할 수 있겠다.
Prion diseases, often called transmissible spongiform encephalopathies (TSEs), are infectious diseases that accompany neurological dysfunctions in many mammalian hosts. Prion diseases include Creutzfeldt-Jakob disease (CJD) in humans, bovine spongiform encephalopathy (BSE, "mad cow disease") in cattle, scrapie in sheep, and chronic wasting disease (CWD) in deer and elks. The cause of these fatal diseases is a proteinaceous pathogen termed prion that lacks functional nucleic acids. As demonstrated in the BSE outbreak and its transmission to humans, the onset of disease is not limited to a certain species but can be transmissible from one host species to another. Such a striking nature of prions has generated huge concerns in public health and attracted serious attention in the scientific communities. To date, the potential transmission of prions to humans via foodborne infection and iatrogenic routes has not been alleviated. Rather, the possible transmission of human to human or cervids to human aggravates the terrifying situation across the globe. In this review, basic features about prion diseases including clinical and pathological characteristics, etiology, and transmission of diseases are described. Based on recently accumulated evidences, the molecular and biochemical aspects of prions, with an emphasis on the molecular interactions involved in prion conversion that is critical during prion replication and pathogenesis, are also addressed.
The strains of Salmonella othmarschen and Norovirus isolated from outbreak A, the funeral ceremonies incidents in Ouri-shi of Oyeonggido in, 2007, were subjected to genotyping by using PFGE and extraction of RNA by using RT-PCR. 24 Salmonella othmarschen were isolated from 37 cases (patients 12 cases, cookers 4 cases, foods 7cases, knife, kitchen board, dish towel 4 cases, etc) and PFGE patterns showed that 22 strains isolated from the same incident were almost of the same pattern and 2 strains exhibited 50% of the pattern. Antimicrobial susceptibility patterns were such that there were 22 case susceptibility patterns and 2 case resistance patterns (strain 4: AM, CF, CIP, NA, TE TIC and strain 13: CF, CZ). Norovirus G1 (1case), Norovirus G2 (7cases), Norovirus G1,G2 (1case) were detected by using RT-PCR. The fine typing ability and reliability of PFGE and BioNumerics are helpful for establishing out the foodborne pathogens of outbreaks.
Korea market of food service industry has been increased greatly since early 90's with the emergence of professional catering business and the expansion of school food service. Because of the characteristics of food service establishment, feeding many people at a time, there is always a high potential of foodborne outbreak which requires special sanitary management to prevent. Currently, many catering businesses are interested in HACCP(Hazard Analysis Critical Control Points), a new sanitary management system, and are trying to develop HACCP Plans for their food service establishment. Microbiological hazard analysis, a most important step for developing HACCP Plan, provides also basic information for evaluating sanitary aspect of prepared foods and their materials. There are not extensive data of microbiological hazard analysis for food service operations, especially for prepared foods and raw materials, which are helpful for developing a HACCP Plan. The extensive microbiological hazard analyses for 65 prepared foods and 51 raw materials selected from a couple of food service establishment were performed in this research. These results will provide useful information to determine hazards for developing a HACCP plan, and also to understand sanitary status for prepared foods and raw materials in food service establishment.
Scombroid fish poisoning (SFP) is a form of histamine food poisoning caused by the ingestion of improperly stored fish. The term "scombroid" derives from the family name of the fish family first implicated, such as tuna and mackerel. On the other hand, non-scombroid fish species, such as sardine and herring, can also cause histamine poisoning. The histamine is converted from histidine by a bacterial enzyme in the causative fish. Because the symptoms of SFP can easily be confused with food allergies, it is believed to have been significantly under-reported. In 2016, an outbreak of SFP occurred among primary school students who had eaten yellowtail steak in Korea. The most common findings consisted of a rapid onset of flushing of the face and trunk, erythematous and urticarial rash, diarrhea, and headache occurring soon after consuming the spoiled fish. Usually, the course is self-limiting and antihistamines can be used successfully to relieve symptoms, but several life-threatening SFP cases have been reported. Clinical toxicologists should be familiar with SFP and have competency to make a differential diagnosis between fish allergy and histamine poisoning. SFP is a histamine-induced reaction caused by the ingestion of histamine-contaminated fish, whereas a fish allergy is an IgE-mediated reaction. This review discusses the epidemiology, pathophysiology, diagnosis, treatment, and preventive measures of SFP.
본 연구는 2020년 10월 중 충청남도내 단체 급식소에서 발생한 대규모 집단 식중독 원인에 대하여 분석하였다. 전체 급식원 135명 중 21명(15.6%)에서 음식을 섭취한 후 1시간 이내에 주로 매스꺼움과 구토 증상을 보였다. 유증상자 21명 중 11명과, 조리종사자 1명, 조리기구 2건과 보존식 8건에서 B. cereus가 검출됨에 따라 B. cereus에 의한 집단 식중독으로 판단하였다. 분리된 21개의 균주를 PFGE 분석한 결과, 19개의 균주가 동일한 것으로 판단되었고, 이들 균주가 가지고 있는 독소 유전자는 CER, nheA, entFM이었다. 실험결과, CER을 포함하고, 증상 발현 시간이 1시간 이내로 매우 짧아 B. cereus의 구토형 식중독으로 판단하였다. 집단식중독 원인으로 안전하지 않은 급식환경과 제대로 관리되지 않은 음식에 의한 것이라 조사되었다. 이러한 결과는 단체급식에서의 급식환경과 제공되는 음식이 철저하게 관리되어야 한다는 것을 보여준다. 이와 더불어 보존식에서 원인 병원체를 찾아내는 것은 식중독의 원인을 추정하는데 매우 중요하므로, 집단급식소에서 규정에 맞는 보존식 용기를 이용하여 이를 적정온도에 잘 보관해야한다. 또한 정밀한 식중독 역학조사를 기반으로 사례를 분석하고 결과를 전파함으로써 유사한 식중독이 재발하지 않도록 해야 한다.
