A Study on the Use of Bihoon (鼻熏) Therapy, which focuses on Korean traditional medicine (한국 한의서를 중심으로 살펴본 '비훈(鼻熏)요법'의 활용에 대한 연구)

  • This paper is a study to find the philological basis of Bihoon (鼻熏) therapy. There is no separate philological study of Bihoon therapy to date, and for this reason, there is no clear definition or specific treatment manual. In this study, a related database was created and analyzed by examining literature data related to Bihoon therapy, focusing on Korean traditional medical books. There were about 1,000 data points related to Bihoon therapy in 45 kinds of medical books. They were largely classified into 1. Acute diseases such as insensitivity, 2. Diseases that occur in the upper human body such as nose, head, eyes, and throat, 3. Women's diseases related to childbirth, 4. Treatment of skin diseases and prevention of infectious diseases. In the case of insensitivity treatment, the focus was on awakening the patient's mind, and the treatment of diseases such as the nose, head, eyes, etc. was focused on resolving each symptom. Symptoms related to childbirth were mainly treated for uterine escapism or fainting after childbirth, while skin diseases were mainly treated for diseases that did not heal well, such as amniotic fluid. If a multifaceted approach to non-discipline therapy is added in the future, it is expected that clinical utilization will also be increased.

  • The purpose of this article was to provide basic knowledge and treatment principles of patient with lymphedema, which was usually not treated at all, or the treatment given didn't work efficiently. Lmphedema is defined as an abnormal accumulation of protein-rich fluid, edema, and chronic inflammation within an extremity. Lmphedema may be classified as either primary results from defects with aplasia, hypoplasia, and hyperplasia in the lymphatic system at birth or secondary is caused by known precipitating factors such as cancer, infection, inflammation, radiation, surgery, or trauma etc. There are essentially several conservative treatment methods which has been utilized successfully to treat lymphedema in Samsung Medical Center. We used following procedures: CPT (Complex Physical therapy) or CDP (Complex Decongesitive Physical therapy) such as skin care, MLD (Manual Lymph Drainage), compression with short-stretch bandage, exercise, elevation, elastic stocking, and pneumatic compression. Our experiences shows that conservative treatments can significantly reduce lymphedema and prevent different complications.

The effects of low dose doxycycline regimen on gingival crevicular fluid enzyme actmty of diabetic patients with periodontitis and adult periodontitis patients (저용량 독시싸이클린 투여가 당뇨환자 마치 성인성 치주염 환자의 치은열구액내 효소 활성도에 미치는 영향에 관한 비교연구)

  • It was reported that low dose doxycycline(LDD) regimen could inhibit pathologically elevated collagenase activity in the gingival crevicular fluid of petients with adult periodontitis without producing typical antibiotic side effects. The purpose of this study was to evaluate the effects of LDD regimen(20mg) administered during non-surgical therapy on clinical index and gingival crevicular fluid enzyme activity in diabetics who are at high risk for periodontal disease. Forty-nine subjects having at least two sites with probing pocket depths greater than 4mm were selected. In this double-blind, placebo-controlled study, the patients were administered 20 mg doxycycline capsule or placebo capsule b.i.d. for 2 weeks. Clinical parameters of dental plaque, gingival inflammation, probing pocket depth and probing attachment level were assessed at week 0, 2, 4, and 8. Gingival crevicular fluid samples were collected at the same time to evaluate the activities of collagenase and elastase. Clinical parameters and elastase activity were significantly reduced in all groups compared to the baseline value after treatment. Significantly greater reduction in pocket depth and gain in attachment level was shown in the LDD-administered group compared to placebo group in both adult periodontitis and diabetic patients. Total collagenase activity was also reduced significantly in all groups after treatment, but the greater reduction was seen in the LDD-administered diabetics group compared to relevant placebo group(at 4, 8week). Significantly greater reduction in active collagenase activity was also seen in the LDD-administered group compared to placebo group in diabetic patients(at 2, 4, 8week). These results indicated that use of low dose doxycycline could be aueseful adjunct to instrumentation therapy in the management of diabetic patient with periodontitis as well as adult periodontitis patient.

Chronic Obstructive Pulmonary Disease in a Dressage Horse (마장마술마에서 발생한 만성폐쇄성폐질환 증례)

  • A 14-year-old gelded dressage horse weighing 500 kg was presented to the Equine Medical Center of the Seoul Racecourse of Korea Racing Authority (KRA) due to coughing and mucopurulent nasal discharge. The horse was initiated with empirical antibiotic in the first place. However, the clinical signs did not improve but were rather exacerbated even after 3 weeks of therapy. Extensive diagnostic procedures including transtracheal wash (TTW) fluid cytology were undertaken. The localized wheezes and crackles were auscultated and an increase in the amount of mucopurulent exudate in trachea was observed at endoscopy. Infiltration of neutrophils was observed in the TTW fluid cytology implying chronic obstructive pulmonary disease (COPD). Therefore, the systemic glucocorticoid therapy was to be given for 3 weeks with improved ventilation provided at the same time. The respiratory symptoms started to improve in 7 days of therapy and were fully resolved by when the therapy was terminated. The horse is clinically normal now and being monitored for development of any signs of chronic obstructive pulmonary disease.

The Effect of Microcurrent Stimulation on Histological Structure of Wound in Rat (미세전류전기자극이 흰쥐 창상의 조직학적 구조에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effect of the intensity of microcurrent stimulation (with currents of 50 ${\mu}A$, 100 ${\mu}A$ and 300 ${\mu}A$ using a pulse frequency of 5 pps) on wound healing in rats. Methods: Sixty male Korean rats were randomly divided into four groups of 15 rats that were subjected to four different treatment protocols (control group, no treatment; experimental groups, treated with currents of 50 ${\mu}A$, 100 ${\mu}A$ or 300 ${\mu}A$). An experimental 20 mm linear wound was made in each animal and all animals in the experimental groups received microcurrent stimulation once a day for 20 minutes until the day of sacrifice on day 1, day 3 and day 6. An optical microscope was used to determine any histological changes. Results: The experimental results were as follows. 1. In an examination with the naked eye, all groups showed similar changes until the first day. However, from the third day, a little intercellular fluid soaked through wound region in the control group rats. In the experimental group animals, little intercellular fluid soaked through wound region, and swelling and redness did not appear, from the third day. 2. In an examination with histological evaluation, more significant changes were observed in all of the experimental group rats than the control group animals. Especially, a stimulation intensity of 50 ${\mu}A$ caused a more significant effect than the use of the other intensities by day 6 of wound healing. In addition, rapid recovery was observed. Conclusion: It was determined that microcurrent stimulation had a positive effect on wound healing. A stimulation intensity of 50 ${\mu}A$ was more effective than the other intensities (100 ${\mu}A$ and 300 ${\mu}A$) utilized for wound healing. Furthermore, low-intensity microcurrent stimulation was more effective for the purpose of wound healing.

Multiple Spinal Cord Recurrences of an Intracranial Ependymoma after 14 Years

  • Ependymoma can spread via cerebrospinal fluid, but late spinal recurrences of intracranial tumor are very rare. We describe a case of a 33-year-old male who presented with multiple, delayed, recurrent lesions in the spinal cord from an intracranial ependymoma. The patient underwent gross total resection and postoperative radiation therapy 14 years prior to visit for a low grade ependymoma in the 4th ventricle. The large thoraco-lumbar intradural-extramedullary spinal cord tumor was surgically removed and the pathologic diagnosis was an anaplastic ependymoma. An adjuvant whole-spine radiation therapy for residual spine lesions was performed. After completion of radiation therapy, a MRI showed a near complete response and the disease was stable for three years.

Combination therapy of cyclosporine and prednisolone in a dog with systemic lupus erythematosus

  • An 11-year-old, spayed female poodle presented with fever and shifting lameness. Physical examination revealed hyperthermia ($40.6^{\circ}C$), and proteinuria was detected upon urinalysis. Increased neutrophils (83%) and decreased viscosity were revealed upon synovial fluid analysis. Serum antinuclear antibody was positive at 1 : 80. Based on these findings, the dog was diagnosed with systemic lupus erythematosus. Immunosuppressive therapy was initiated with prednisolone and cyclosporine, and the condition was markedly improved after the treatments. This case report describes the clinical and laboratory findings, imaging characteristics and successful outcomes after prednisolone plus cyclosporine therapy in a canine systemic lupus erythematosus case.

Clinical Applications of Photobiomodulation Therapy in the Management of Breast Cancer-related Lymphedema

  • Breast cancer-related lymphedema (BCRL) is characterized by the persistent accumulation of interstitial fluid in the peripheral tissues after treatment for breast cancer. Photobiomodulation (PBM) therapy is widely used as supportive care for patients with BCRL. A search was performed in the PubMed database to find relevant articles published over the last 20 years. Randomized controlled trials that evaluated the efficacy of PBM therapy on BCRL were included. A total of 24 studies were identified through the PubMed database. Seven studies were used for the final analysis, after excluding items that did not meet the duplication and inclusion criteria. Although PBM showed some improvement in reducing arm circumference and the symptoms related to BCRL, the results of the meta-analysis did not show any significant benefit in alleviating lymphedema. Further studies are needed with the recruitment of more participants to evaluate the long-term efficacy and safety of PBM in the management of BCRL.

Negative-Pressure Wound Therapy for Septic Ankle Arthritis Following Intractable Lateral Malleolar Bursitis: A Case Report (족관절 화농성 관절염이 동반된 난치성 외과 점액낭염의 음압 창상치료: 증례 보고)

  • A bursa is an obstructive sac filled with synovial fluid and usually occurs in any area of the body exposed to friction. The bursa of the ankle is not a normal anatomical structure and is caused by repetitive trauma, constant friction, or inflammatory disease of the ankle. Bursitis can occur in any bursa in the human body; however it rarely progresses to septic arthritis. We report a rare case of septic ankle arthritis following intractable lateral malleolar bursitis successfully treated with negative-pressure wound therapy.

  • To find the cause, pathology, oriental presciptions for SLE(systemic lupus erythematous) in the field of oriental medicine, we studied the clinical reports. The results were obtained as follows: 1. SLE can' be thought to be a category of chronic fatigue, numbness, yang poison' & erythema. 2. The chief cause of SLE could be a febrile poison and the lack of physiological fluid and the mechanism was that febrile poison attacted area of circulation and blood and injuried the physiological fluid to arise edema and clotted blood. 3. The therapy of SLE was chiefly clarifying heat and toxiciding in acute stage while reinforcing kidney and liver and supplementing Yin & clarifying in chronic stage, sometimes supplementing Qi, eliminating clotted blood, excuding wind and draining water could be applied. 4. The therapy for SLE nephritis was shown to be increasing yin and clarifying heat with toxiciding, reinforcing spleen and kidney, draining water, supplementing yin & Qi etc. The chief prescriptions were Seogakjihwangtang(犀角地黃湯), Wookakjihwangtang(牛角地黃湯), yeechihwan(二至丸), Daepowonjeon(大補元煎), Daepoeumhwan(大補陰煎), Kikukjihwangtang(杞菊地黃湯), Yookmijihwangtang(六味地黃湯) which indicates Rhemanniae radix added prescrition could be used chiefly for the treatment of SLE.
