소아 개심술에 있어서 변형 초여과법(Modified Ultrafiltration)이 술후 상태에 미치는 영향 (Effect of Modified Ultrafiltration on the Postoperative State after Pediatric Open Heart Surgery.)

  • 개심술후에 영아나 작은 소아에서는 수분의 체내 저류현상이 더욱 심하게 나타난다. 이에 대한 치료법의 하나로 개심술시 변형 초여과법을 적용함으로써 술후 상태에 미치는 영향을 알아 보고자 하였다. 개심술을 받았던 5세이하의 소아를 대상으로 개심술후 변형 초여과법을 적용하였던 환아(초여과군) 10명과 초여과법을 실시하지 않았던 대조군 10명의 2군으로 나누어서 개심술에 따른 조기 성적을 비교 분석하였다. 환아의 연령, 체중, 심폐기 충진액의 총량과 이에 첨가한 혈액의 양, 그리고 체외순환시간과 대동맥차단시간 등의 값들은 모두 양군간에 유의한 차이가 없었다. 변형 초여과법을 적용함에 있어서 관류속도는 평균 171.4ml/min, 관류시간은 평균 18.9분이 소요되었고, 이 동안에 걸러진 수액의 총량은 평균 38.4ml/kg, 심폐기의 혈액을 환자에게 재수혈한 양은 평균 32.1ml/kg였다. 술후 24시간동안의 출혈량은 초여과군에서 평균 14.5ml/kg/24hr, 대조군에서 13.7ml/kg/24hr였고, 같은 기간에 수혈한 양은 초여과군에서 전혈 6.6ml/kg/24hr, 혈장 65.7ml/kg/24hr, 대조군에서는 전혈 15.2ml/kg/24hr, 혈장 59.6ml/kg/24hr이었다. 이는 임상적으로는 초여과군에서 전혈의 수혈량이 대조군에 비하여 훨씬 적어 보였지만 통계학적으로 유의한 차이는 없었다. 술전후 적혈구용적률의 변화는, 초여과군에서는 체외순환 시작후 10분에 평균값이 26.5%로 떨어졌다가 초여과법 종료 5분후에는 37.1%로 급격히 상승하였고, 대조군에서는 체외순환중의 24.7%에서 체외순환이 끝나고 20분이 경과한 후에도 30.5% 수준으로밖에 상승하지 못하였다. 그러므로 적혈구용적률의 변화를 체외순환중의 수치에 대한 상승률(%)로 나타냈을 때, 초여과군은 40%, 대조군은 23%로써 양군간에 통계학적으로 유의한 차이를 보였다(p=0.002). 초여과법 시행에 따른 수축기 및 이완기동맥압의 변화에 있어서는 양군간에 유의한 차이가 없었으며, 평균동맥압의 변화는 양군간에 통계학적으로 유의한 차이는 없었으나(p=0.086), 임상적으로는 상당한 차이를 보였다. 결론적으로, 소아환자에서 개심술후에 변형 초여과법을 적용했을 때, 거의 일률적으로 적혈구용적률과 동맥혈압이 상승함을 볼 수 있었고, 이는 환아를 중환자실로 옮긴 후에도 지속됨으로써 개심술직후 환아관리에 긍정적인 효과를 발휘하는 것으로 평가된다. 따라서 향후 영아나 작은 소아의 개심술에 있어서 변형 초여과법은 보다 많은 빈도로 이용될 것으로 전망된다.

산소-탄소 안정동위원소특성을 이용한 대기층 고품위 석회석의 생성기작 해석 (Verification of Genetic Process for the High-purity Limestone in Daegi Formation by Oxygen-carbon Stable Isotope Characteristics)

  • 태백산분지 북익부 고품위 석회석의 생성은 크게 동생적(syngenetic)과 후생적(epigenetic) 관점으로 나뉘어 있으며, 산소-탄소 안정동위원소 비 특성을 이용하여 이들의 생성환경을 고찰하였다. 북익부의 4개 광산(GMI, 정선, 백운, 백광)과 남익부의 상동(오미아)광산, 그리고, 삼척지역의 쌍용동해광산을 대상으로 하였으며, 이들과의 비교를 위해 정선지역 대기층 노두시료를 포함하였으며, 이들을 색상에 따라 암회색, 회색, 담회색, 백색으로 구분하였다. 산소 안정동위원소 비는 4.5 ~ 21.6 ‰의 넓은 범위의 변화를 보이지만, 탄소 안정동위원소 비는 -1.1 ~ 0.8 ‰ (방해석 맥 제외)로 매우 좁은 범위를 보여, 열수의 작용이 크지 않았음을 지시한다. 또한, 광산별, 색상별 산소-탄소 안정동위원소 비의 분포범위의 차이는 없으며, 주변 노두에서 채취된 탄산염암과 유사한 범위를 보이는 것으로 보인다. 이와 같은 결과는 고품위 석회석의 생성에 있어서 열수의 영향은 거의 없었다는 것을 지시한다. 산소 안정동위원소 비 값은 암석의 색상변화(백색화)보다는 조직의 변화와 관련되는데, 산소 안정동위원소 비 15 ‰ 전후를 기준으로 탄산염암의 산상을 구분할 수 있다. 이보다 높은 경우 괴상 또는 층리가 잘 보존된 특징을 보이는 반면, 낮은 경우 하나의 시료 내에서 두 가지 이상의 색상이 나타나고 있으며, 층리는 교란되거나 거의 소멸하여 흔적만 확인된다. 두 가지 이상의 색상이 나타나는 경우 밝은 부분이 어두운 부분에 비해 현저히 낮은 산소 안정동위원소 비를 보이는 경우가 다수 관찰되며, 이는 열수와의 반응에 의한 현상으로 해석된다. 다만, 이와 같은 현상은 소규모로 나타나고 있으며, 광상 전체적인 고품위 석회석의 생성에 대한 해석으로는 적당하지 않다. 특히, 재결정화한 백색의 탄산염암에서 높은 산소 안정동위원소 비 값을 보이는 것은 이 지역 탄산염암의 재결정화에 열수의 작용이 매우 적었음을 지시하며, 주변 마그마에 의한 열적 영향만을 받은 결과로 해석된다. 또한, 고품위 석회석광상 내 광체의 분포가 수평으로 연속적이며, 고품위 석회석 상하 주변에 백운석이 층상으로 협재되는 등의 산상을 고려할 때, 연구지역 고품위 석회석의 생성은 동생적(syngenetic) 해석이 보다 더 타당할 것으로 판단된다. 따라서, 태백산분지 전반적으로 순차층서적 관점에 의한 퇴적상의 해석이 필요할 것으로 사료되며, 이는 추가적인 고품위 석회석 광체의 확보에 매우 중요한 요소가 될 것이다.

CPFD를 이용한 태양열 유동층 흡열기의 수력학적 특성 해석 (Analysis of Hydrodynamics in a Directly-Irradiated Fluidized Bed Solar Receiver Using CPFD Simulation)

  • 실리콘 카바이드 입자(평균 입도 123 ㎛)의 유동층 태양열 흡열기의 성능 및 효율에 영향을 미치는 입자 거동 해석을 위해 MP-PIC 모델을 이용하여 전산모사를 수행하였고, 기존 실험결과와의 비교를 통해 검증하였다. 특히, 본 연구에서는 실험적으로 접근하기 어려운 유동층 표면 부근에서의 거동을 모사함으로써 흡열 성능과 입자 거동과의 상호 영향을 분석하였다. CPFD 모사결과는 입자층 및 프리보드에서의 평균 고체체류량과 압력요동 등 수력학적 특성 실험결과를 잘 예측하였다. 입자 흡열기에서 1차적으로 태양열 에너지를 흡수하여 층 내부로 전달하는 층 표면 부근에서의 국부 고체체류량은 입자층 내 기포거동에 따라 중심부에서 상대적으로 낮은 값을 나타내는 불균일 분포를 나타내었다. 프리보드 영역에서 국부 고체체류량은 기체속도가 증가할수록 축방향과 각 높이에서의 횡방향에서 불균일성이 증가하였고, 이는 입자 흡열기의 프리보드 영역 내 비산된 입자에 의해 반사된 태양광 에너지 손실과 연관된 압력강하 상대표준편차 증가의 원인임을 나타내었다. 입자 흡열기 내 기체속도 증가에 따른 국부적인 기체 및 입자 속도의 변화에 대한 고찰을 통해, 유동층 내 국부적인 입자거동 특성은 Geldart B 입자 물성과 관련된 입자층 내 기포 거동과 밀접하게 연관됨을 확인하였다. 유동층 입자 흡열기의 성능 척도인 일사량 당 유동기체의 출입구 온도차(∆T/IDNI)는 입자 층 표면 및 표면 상부 프리보드 영역 내 압력요동 RSD와 상관관계가 매우 높음을 확인하였고, 이 결과는 흡열기 성능 개선에 활용할 수 있을 것으로 판단되었다.

본류와 지류의 수온 차에 의한 합류부 혼합 양상 분석 (Analysis of the mixing effect of the confluence by the difference in water temperature between the main stream and the tributary)

  • 하천 합류부는 서로 다른 지형학적 특성과 수리학적 특성을 가지는 두 개의 하천이 하나로 합쳐지는 구간으로 급격한 흐름의 변화 및 퇴적물의 유입과 수리학적 지형변화가 발생하는 구간이다. 합류부 구간에서는 물질의 종류 또는 온도 차로 유체의 흐름이 밀도 차이로 발생하게 되는데 이것을 밀도류라고 한다. 밀도 차이에 의해 성층이 생긴 수체혼합거동을 파악하기 위해서는 본류 및 지류의 일정 구간을 포함하는 합류부 구간에 대한 정밀한 계측 및 관찰이 필요하다. 이러한 수체 혼합에 대한 종합적인 분석은 유속장 및 유량 정보를 취득하여 파악할 수 있지만, 성층류가 흐르는 하천의 서로 상이한 물리적 특성과 수질특성을 가지는 수체의 혼합양상 및 그에 따른 물질혼합양상을 파악하는데 한계가 있다. 따라서 본 연구에서는 합류부 구간에서의 수온 분포를 통하여 밀도류를 파악하고자 한다. 하천의 광범위한 데이터 중 연직 자료와 수표면 자료를 취득하였고, 이를 통해 합류부의 성층현상을 확인하고자 하였다. ADCP를 보트 측면에 설치하여 저속운행으로 수리량을 측정하는 방식과 YSI를 이용해 측선설치 없이 측선 선정 후 보트를 이용하여 흐름에 직각인 방향으로 이동하며 실시간 농도를 측정하는 방식으로 얻은 연직자료 중 수온, 전도도(Conductivity) 등의 직독식 센서 데이터 값을 사용하여 수온 차에 따른 수체혼합 패턴을 분석하여 합류부의 혼합양상을 분석하고자 하였다. 본 연구는 기존 수질 측정의 한계였던 1차원적인 측정결과가 나타내는 분석결과를 2차원적으로 보완이 가능하며, 비교 분석한 결과를 토대로 밀도류에 따른 혼합양상 결과가 지니는 혼합패턴을 분석한다면 향후 하천 하류구간의 취수 시스템에 많은 도움을 줄 뿐만 아니라 합류부 구간의 혼합패턴에 따라 수층 내 성층구간의 현황조사와 하천 합류부의 혼합특성 파악하여 합류구간의 수질 관리방안이나 수체 흐름특성을 제시할 수 있을 것으로 사료된다.

원거리 지진 주시 토모그래피를 이용한 멜라네시아 지역의 맨틀 속도 구조 연구 (Teleseismic Travel Time Tomography for the Mantle Velocity Structure Beneath the Melanesian Region)

  • 서태평양 멜라네시아 지역에는 복잡한 판들의 지구조 운동이 발생하고 있고, 가장 거대한 해양 해대인 온통-자바 해대와 열점인 캐롤라인 제도가 위치해 있다. 이 지역의 복잡한 지구동역학에 대한 이해를 높이기 위해 해저 지진계와 육상 지진계에 기록된 원거리 지진으로부터 상대 주시를 획득하여 P파 및 S파 속도 모델 및 𝛿 (VP/VS) 모델을 계산했다. 그 결과 멜라네시아 지역의 섭입대에서 약 400km 깊이까지 강한 고속도 이상이 관찰됐고, 이는 판경계를 따라 섭입하는 솔로몬해판, 비스마르크판, 그리고 호주판의 모습으로 생각된다. 섭입대를 따라 양의 𝛿 (VP/VS) 이상값이 나타나는데, 이는 탈수 작용에 의한 부분 용융의 결과로 생각된다. 온통-자바 해대 하부 600km 깊이 아래에서 넓은 고속도 이상체가 관찰되며, 음의 𝛿 (VP/VS) 이상값을 보인다. 이는 25-45 Ma 시기에 섭입한 태평양판이 분리된 잔재로 판단되며, 오랜 기간 맨틀 전이대에 머물면서 잔류판의 최상부에 포함되어 있던 유체가 빠져나가면서 주변 맨틀 물질에 비해 상대적으로 점성이 높고 건조해짐으로 인해 강한 고속도 이상과 강한 음의 𝛿 (VP/VS) 이상값이 나타난 것으로 생각된다. 캐롤라인 제도 하부에서는 강한 저속도 이상이 800km 깊이까지 관찰되며 맨틀 전이대에 위치한 태평양판 잔재 하부까지 연결되어 보인다. 이는 하부 맨틀에서 기인한 맨틀 플룸이 태평양판의 잔재와의 상호작용으로 인해 상승방향이 바뀌어 현재 위치에 도달한 것으로 보인다. 또한 맨틀플룸은 양의 𝛿 (VP/VS) 이상값을 가지는데 내포된 유체나 부분 용융에 의한 영향으로 생각된다. 온통-자바 해대 하부의 두꺼운 암석권의 영향으로 해석되는 고속도 이상체가 300km 깊이까지 관찰되었으며 음의 𝛿 (VP/VS) 이상값을 보여주는데, 이는 암석권에 쌓인 용융 잔류물에 유체가 거의 남아있지 않음을 나타내는 것으로 보인다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury.
