The Effects of Various Directions of Handle Grip on the Upper Limb Muscle Activity of Wheelchair Attendants during Ramp Climbing

  • Purpose: This study aimed to determine the effects of various wheelchair handling directions on the upper limb muscle activities of wheelchair attendants while climbing a ramp. Methods: For the study participants, healthy males over 20 years of age were chosen, and the order of the direction of wheelchair handle grip was determined using a randomized method. The handling directions for pushing the wheelchair up a ramp included a general grip with ulnar deviation, a medial grip with wrist pronation, and a neutral grip with a neutral wrist. The muscle activities in the participants' upper limbs were measured using surface electromyography. For statistical data processing, SPSS 18.0 was used to perform repeated measures ANOVA in order to compare the muscle activity among the intervention groups. A contrast test was also conducted among the participants. The significance level (${\alpha}$) was set to 0.05. Results: There was a significant difference between groups using a general grip and a medial grip in the biceps brachii, triceps brachii, and flexor carpi radialis muscles (p<0.05). There was also a significant difference between using a general grip and a neutral grip in the biceps brachii and flexor carpi radialis muscles (p<0.05), and there was a significant difference between using a medial grip and a neutral grip in the biceps brachii and extensor carpi radialis brevis muscles (p<0.05). Conclusion: In this study, the wheelchair assistants' wrist muscle activity was the lowest with a neutral grip while ascending a ramp. Accordingly, this study proposes that wheelchair assistants push wheelchairs up ramps with a neutral grip.

  • Purpose: This study was conducted to know the evidence case to assessment about literatures of extensor carpi radialis therapeutic strategy of tennis elbow about dysfunction wrist extension with anatomy, biomechanic and function. Method: Patient was received the physical therapy program with medication for two weeks. Physical therapy program consists of strengthening exercise, stretching exercise and extracopereal shock wave therapy. Results: Patients could do computer and house works at three weeks significant with improved pain. The amounts used repetitive muscle was reduced slowly over a period of a day or two days for wrist moving. Wrist flexor strengthening exercise and could reduced the wrist extensor injury. Conclusion: Clinicians certainly realized biomechanic effects and anatomy of extensor carpiradialis and elbow joint.

The Effects of Elastic Resistance and Pilates Exercise on EMG in Baseball Pitcher (탄성저항 및 필라테스 운동이 야구투수의 근전도에 미치는 영향)

  • The purpose of this study was to examine the pitching ability of baseball player by analysing the effect of elastic resistance exercise and pilates exercise on EMG. Five out of currently active highschool baseball player performed 5 times a week of elastic resistance exercise and pilates for 8 weeks, each for 100 minutes. They demonstrated significant increase in EMG after 8 weeks of exercise. Especially, in extensor carpi radialis brevis and flexor carpi radialis of fore arm, they demonstrated significant increase of inning figure after exercise. These results suggested that elastic resistance exercise and pilates exercise can improve pitching ability of baseball player according to inning.

Effects of a Notebook Computer Supporter on Biomechanical Characteristics in Wrist Joint Muscles of Healthy Young Adults (노트북 거치대가 건강한 젊은 성인 손목관절근육의 생체역학적 특징에 미치는 영향)

  • Purpose: As laptop use increases throughout the COVID-19 pandemic and its use outside of traditional workstations proliferates, it is imperative to expand the limited research available regarding ergonomic exposure. This study aimed to identify the effects of a laptop supporter on biomechanical characteristics in the wrist joint muscles of healthy young adults. Methods: This was a cross-sectional observational study design with thirty-four healthy young adults as participants. They conducted a typing exercise performed under two different conditions, which were with and without a notebook computer supporter. This study measured the biomechanical characteristics of the muscles of the wrist joints including the flexor carpi ulnaris (FCU), the flexor carpi radialis (FCR), the extensor carpi radialis longus (ECRL), and the extensor carpi ulnaris (ECU). Measurements were taken three times: before typing, immediately after typing for 30 minutes with a supporter, and immediately after typing for 30 minutes without a supporter. The statistical method to compare the three different measurement conditions was the repeated measures ANOVA. Results: The participants showed significantly different levels of dynamic stiffness in both the FCU before typing and immediately after 30 minutes of typing with a supporter, and showed significantly different levels of dynamic stiffness in the FCR before typing and immediately after 30 minutes of typing with a supporter. The dynamic stiffness level immediately after 30 minutes of typing without a supporter was significantly different than that immediately after 30 minutes of typing with a supporter. However, the muscle tone was not significantly different among the three different conditions. Conclusion: The results of this study revealed that a laptop supporter used to correct the eye level of the electronic screen increases the dynamic stiffness of the wrist joint flexors, so it is necessary to consider the neutral position of the wrist joint during typing.

  • Purpose: The reconstructive surgery of the anterior oblique ligament has been recommended for patients with unstable carpometacarpal joints. We reviewed retrospectively the functional results of two patients who had been treated by6 using a harvested portion of the tendon of flexor carpi radialis as a substitute for the anterior oblique ligament. Materials and Methods: In two patients with a instability of the trapeziometacarpal joint because of a rupture of the anterior oblique ligament, reconstruction was carried out using a slip of the tendon of flexor carpi radialis and the patients had been followed up for two years. Results: The results after a follow-up of two years that both of them had significant relief from pain and symptoms. The mean grip strength recovered to 90$\%$ of the controlateral side. patients felt that they had subjective improvement and would have undergone the operation again. Conclusion: Increased awareness of this lesion can lead to an early and clear diagnosis, so that the patient may be advised adequately.

Muscle activity in relation to the changes in peripheral nerve conduction velocity in stroke patients: Focus on the dynamic neural mobilization technique

  • The objective of this study was to investigate the dynamic neural mobilization program on the changes in muscle activity and nerve conduction velocity (NVC) in stroke patients. The participants were sampled and randomly divided into experimental group I (n=12) who underwent arm neural mobilization and experimental group II (n=13) who underwent arm dynamic neural mobilization. As the pretest, peripheral NVC of the radial, median, and ulnar nerves were measured using the Viking Quest; the biceps brachii, brachioradialis, flexor carpi radialis, and extensor carpi radialis activities were measured with sEMG. Each intervention program consisted of 10 trials per set and three sets per session. The intervention programs were performed once daily for four weeks (four days/week). Posttest measurements were taken equally as the pretest measurements. Significant differences in peripheral NVC in all sections of the radial and median nerves and wristbelow elbow and below elbow-above elbow areas of the ulnar nerve, as well as in muscle activity of all muscles except the biceps brachii. These findings indicate that dynamic neural mobilization was effective in increasing peripheral NVC and altering the muscle activity.

Comparative Analysis of Upper Extremities Muscle Activity during Baseball Pitching in Middle and High School Baseball Players (중, 고등학교 투수의 투구 시 상지의 근활성도 비교분석)

  • Background: The position of pitcher requires a lot of repetitive motion, and because of this, it is known that not only professional baseball players, but also middle and high school players are frequently exposed to injuries in baseball. The purpose of this study is to examine the differences in upper extremity muscle activity during repeated pitching and the activity of each muscle during repeated pitching by analyzing middle and high school pitchers, divided into groups by age. Methods: Twenty participants (10 middle school male students and 10 high school male students) were recruited for this research. The outcome measures included neuromuscular motor control, including the upper trapezius (UT), triceps brachii (TB), deltoid (DT), latissimus dorsi (LD), biceps brachii (BB), pectoralis major(PM), extensor carpi radialis(ER), and flexor carpi radialis (FR). Results: The two-way analysis of varaince (ANOVA) was used to compare the muscle activity variables between the middle school and high school students. The one-way ANOVA was used to compare the muscle activity variables within time differences each groups. Conclusion: Our results provided promising clinical evidence that guide for upper extremity muscles to increase pitching efficiency in middle and high school base ball players.

Effects of a Wrist Extension Splint on Muscle Power and Activities of the Forearm Muscles: Comparison of Day Versus Nighttime Wear Instructions

  • The objective of this study was to compare the differences on the activity and power of the wrist flexors and extensors in subjects before the use of a wrist extension splint, after nighttime wearing of the splint, and after daytime wearing of the splint. Ten healthy male and ten healthy female students (mean: $22.4{\pm}1.2$ years old) volunteered to wear custom-made wrist splints either during the night or during the day, The hand force of the wrist flexor and extensor, and grip force were measured by PowerTrack II and Dynatron, respectively. At the same time, the activities of the wrist flexor and extensor were recorded by' surface electromyography. The maximal hand force and motor unit recruitment of the flexor carpi ulnatis (FCU) increased significantly (p<.05) when tile subjects wore the wrist splints during the daytime, but the maximal hand power of the FCU decreased with nighttime use of the splints. The maximal hand power and motor unit recruitment of the extensor carpi radialis (ECR) and the ECR/FCU ratio decreased both during nighttime and daytime use. The decrement of the ECR/FCU ratio was significant (p<.05). Wearing a wrist extension splint during nighttime led to the maintenance of a lengthened position of the wrist flexor, resulting in the wrist flexor becoming weak. Wearing a wrist extension splint during the day induced the wrist flexors to be greater. In healthy people, the imbalance between the wrist flexors and extensors may be caused by the use of a wrist extension splint. This study indicates that therapists have to consider whether a splint will be effective, as well as the wearing time, when prescribing splints to people with problems of the musculoskeletal system.

Analysis of EMG Activities and Driving Performance for Operating Four Types of Left Hand Control Devices (4가지 종류의 좌측 핸드 컨트롤 장치에 대한 사용자의 EMG 분석 및 운전 성능 평가)

  • The main purpose of this research was to examine the EMG characteristics of driver's upper limb and driving performance for operating accelerator and brake pedal by using four types of left hand control devices(Push/Pull, Push/Right angle, Push/Rock, Push/Twist) during simulated driving. The persons with disabilities in the lower extremity have problems in operation of the vehicle because of functional impairments for controlling accelerator and brake pedal. Therefore, if hand control device is used for adaptive driving controls in persons with lower extremity loss, the disabled people could improve their quality of mobility life by driving a car. Twenty subjects were involved in this research to assess driving performance and EMG activities for operating accelerator and brake pedal by using four types of left hand controls in driving simulator. We measured EMG responses of six muscles(posterior deltoid, middle deltoid, biceps, triceps, extensor carpi radialis, and flexor carpi radialis) during pulling and pushing movement with four types of left hand controls for acceleration and braking. STISim Drive 3 program was used for evaluation test of four types of left hand control devices in straight lane course for time to reach target speed and brake reaction time. While operating the four types of left hand controls for acceleration, EMG activities of posterior deltoid in normal subjects were significantly increased(p < 0.05) compared to the disabled subjects. It was also found that EMG responses of triceps and posterior deltoid were significantly increased(p < 0.05) when using the Push/Right angle type than Push/Pull type. While operating the four types of left hand controls for braking, EMG activities of flexor carpi radialis and triceps in subjects with disability were significantly increased(p < 0.05) compared to the normal subjects. It was shown that muscle responses of posterior deltoid, middle deltoid and triceps were significantly increased when using the Push/Right angle type than Push/Rock type. Time to reach target speed and brake reaction time in subjects with disability was increased by 2.5% and 4.6% on average compared to normal subjects. The person with disabilities showed a tendency to relatively slow performance in acceleration at the straight lane course.
