Increased interest in alternative approach to thoractomy has developed because of the considerable morbidity associated with the standard posterolateral thoracotomy[ST]. Muscle sparing thoracotomy is appeared as excellent alternative because of less postoperative pain and morbidity than standard posterolateral one. Vertical axillary muscle sparing thoracotomy[VM] is the newly revised modified muscle sparing thoracotomy that overcomes the disadvantages of previous lateral muscle sparing thoracotomy such as seroma, cosmetic problems, and need of subcutaneous drains. We conducted a prospective study of 45 consecutive patients to compare postoperative pain, muscle strength of serratus anterior and latissimus dorsi, and range of motion of the shoulder girdle between ST and VM group. There were no difference in preoperative status, surgical procedure, morbidity, mortality and hospital stay between two groups. But there were significant less postoperative narcotics requirements, more preserved latissimus dorsi and serratus anterior muscle strength, nd larger range of motion of shoulder girdle [ especially flexion and internal rotation in VM group. The opening time was prolonged[p<0.01] but closing time was less in VM group [p<0.01]. The sum of opening and closing time was not different in two group. The length of incision line was shorter in VM group. The vertical skin incision was concealed by the upper arm.In conclusion vertical axillary muscle sparing thoracotomy is good alternative for various intrathoracic procedures with less postoperative pain, well preserved muscle strength, increased range of motion of the shoulder girdle and impressive cosmetic outcome.
Purpose: In this study, we provide a way to assess even a slight effect of exercise on trunk-muscle activity. Materials and Methods: Seven healthy male participants (mean age, 24.7 ± 3.2 years; height, 171.2 ± 9.8 cm; and weight, 63.8 ± 11.9 kg) performed 15 sets of an exercise with 20 repetitions of 90° hip and right-knee flexion while lying supine. The exercise intensity was measured using the 10-point Rating of Perceived Exertion Scale after the first and 15th sets of exercises. Although cross-sectional areas and functional T2 mapping using ultrafast imaging (fast-acquired muscle functional magnetic resonance imaging, fast-mfMRI) have been proposed for imaging to evaluate exercise-induced muscle activity in real time, no previous studies have reported on the evaluation of trunk-muscle activity using functional T2 mapping. As a method for assessing trunk-muscle activity, we compared functional T2 mapping using ultrafast imaging (fast-mfMRI) with cross-sectional areas. Results: Although the muscle cross-sectional areas were increased by the exercise, there was no significant difference at rest. On the other hand, for all sets, the changes in T2 were significant compared with those at rest (P < 0.01). These results demonstrate that T2, calculated from fast-mfMRI images can be used to detect even a small amount of muscle activity induced by acute exercise, which was impossible to do with cross-sectional areas. Conclusion: Fast-mfMRI, which can also display functional information with detailed forms, enabled non-invasive real-time imaging for identifying and evaluating the degree of deep trunk-muscle activity induced by exercise.
The purpose of this study was to investigate the effects of various cool-down exercises on muscular strength and endurance. After receiving a treadmill training for main exercise, the subjects conducted isotonic and isometric cool-down exercises four times for three weeks. Isotonic exercise with leg press of 10kg was repeated by 20 times and isometric exercise was conducted at flexion of hip joint and knee joint with leg press of 10kg by maintaining it for 6 sec and resting for 2 sec by 20 times. Muscular strength after exercise was measured with 1 RM by times and muscular endurance with maximum repetition frequency using time to keep for loading the weight of 10 RM and 65% of maximum muscular strength. As a result of comparing and analyzing measured values, exercise recovery shape of isotonic and isometric cool-down group were more effective than rest recovery shape of the control group. The isometric cool-down group was more effective than isotonic cool-down group. In conclusion, isometric exercise was more effective than isotonic exercise or simple rest on muscular strength and endurance.
The aim of the current study was to assess the effectiveness of backward gait training on the treadmill in patients with spastic diplegic cerebral palsy (CP). Twelve patients with spastic diplegic CP participated in the study. An 8-week course of backward gait training was administered to the subjects for 3 days per week. Pre-intervention and post-intervention assessments of temporal-spatial gait parameters, the symmetry of the bilateral lower extremity weight bearing, and gross motor function were analyzed using motion analysis system, force plate, and Gross Motor Function Measurement (GMFM). There were significant improvements (p<.05) in the measures of both step length and right stance phase time. Joint kinematics showed increase in right hip abduction in initial contact and terminal swing, right hip external rotation and knee flexion in mid-swing, left ankle dorsiflexion in initial contact and terminal swing (p<.05). The symmetry of the bilateral lower extremity weight bearing and GMFM also significantly increased (p<.05). These findings indicate that backward gait training using a treadmill is beneficial for patients with spastic diplegic CP.

Background: The purpose of this study was to determine if there is a difference in activation of the rectus femoris, vastus medialis oblique, vastus lateralis, semitendinosus, and biceps femoris when performing normal free squat with standinding position and free squat with $30^{\circ}$ flexed hip joint. Methods: Electromyograph surface electrodes were placed on the rectus femoris, vastus medialis oblique, vastus lateralis, semitendinosus, and biceps femoris of 19 healthy college students. The participants performed standing bilateral squats and standing bilateral squats with $30^{\circ}$ flexed hip joint with EMG measures taken upon initiation of muscle activity as confirmed by an electronic goniometer. Participants completed one trial with the EMG time measurements on each type. Results: There was a significant difference between normal squats(standing squats) and normal squats with $30^{\circ}$ flexed hip joint. The normal squat exercise was statistically higer than normal squat exercise with $30^{\circ}$ flexed trunk except for semitendinosus and biceps femoris that shown slightly high. Conclusions: As a result of this study, there were increases of muscle activity in both ways. In particular, it may be more beneficial for knee joint stabilization to perform normal squat exercise with standing position relatively.

Anterior cruciate ligament(ACL) injury of the knee is common and a serious ACL injury leads to ligament reconstruction surgery. Gait analysis is used to identify the result of surgery. The purpose of this study is to numerically evaluate and classify knee condition of patients through the chaos analysis. Experiments were carried out for 13 subjects (8 healthy subjects, 5 ACL deficient patients) walking on a treadmill. Sagittal kinematic data of the right lower extremity were collected by using a 3D motion analysis system. The recorded gait patterns were digitized and then coordinated by KWON3D. The largest Lyapunov exponent from the measured knee angular displacement time series was calculated to quantify local stability. It was found that the Lyapunov exponent becomes larger as the knee condition becomes worse. This study suggested a method of the severity of injury and the level of recovery. The proposed method discerns difference between healthy subjects and patients.
The purpose of this study is to provide training materials for practical use by investigating the kinematical variables of the successful landing by the type of the salto backward such as Tuck, Pike. For this study, the subjects are 4 male national gymnasts using 3-dimensional cinematographic method. Based on the results of this study, the conclusions are drawn as follows. 1. In flight phase, Tuck and Pike show fast extension after completing minimum angle of hip joint passing through the peak. It is very important factor to control body with gaining time before landing while decreasing the velocity of flight rotaion. 2. In Landing phase, the angles of each joint for successful landing are shown as $92deg{\sim}100deg$ for knee angle, $52deg{\sim}57deg$ for hip angle, and $56deg{\sim}70deg$ for shoulder angle. 3. Tuck and Pike dramatically decrease the height of COG, and horizontal/vertical velocity of COG from TD to LD. Also, it is shown that the knee angle, the hip angle and the shoulder angle decrease drastically. On the other hand, the angular velocity of trunk rotation shows negative direction and due to this, the angle of trunk rotation is shown as re-flexion.
The purpose of study was to investigate the kinematic variables of Kenmotsu motion in Parallel bars. To this study, by 3 dimensional kinematical analysis of 4 male national gymnasts participants in the 28th Athens Olympic Game in 2004, kinematic data collected using video camera. Coordinate data were smoothed by using a fourth-order Butterworth low pass digital filter with cutoff frequency of 6Hz. The conclusions were as follows. 1. In P2, because the constrained swing movement made the movement of a rising back difficult7, the movements of Reg. were performed at low position after Air phase. 2. In E5 event, for the shake of a stable handstand and applied techniques like a Belle(E-value), a Belle Piked(super E-value), a vertical velocity in E2, a horizontal velocity in E3 and a vertical velocity in E4 were necessary for high velocities. 3. In E4 event, it was appeared that for a flexible body's movement of a vertical up-flight, the large angle of the shoulder joint and the flexion & extension of the hip joint were necessary in Air phase and a long flight time and vertical displacement made Reg. movements stable at the high position.
The purpose of this study was to analyze the effect that various loads have on the lower limb biomechanics. The following variables were measured and analyzed; performance time for each phase, lower limb moments and joint angles, and ground reaction forces. The kinematic and kinetic data was recorded by 2 force platforms and a motion capture system while 12 healthy adults in their twenties stepped down three steps under loads of 0%, 10%, 20% BW. Results are as follows. First, the different loading conditions did not seem to significantly affect the performance times and the joint angles. Second, the largest ground reaction forces were observed at the 1 step at the 10% BW condition. Finally, at the 0% BW loading condition the right hip extension moment was the smallest and the left hip flexion moment was the largest. The results show that there are not any significant changes in the biomechanics of the lower limbs under loading conditions up to 20% BW. Further investigations including more loading conditions with more weights and more additional steps analyzed are needed.
