Tapia's Syndrome after Posterior Cervical Spine Surgery under General Anesthesia

  • We present a case report to remind surgeons of this unusual complication that can occur in any surgery, even posterior cervical spine surgery under general anesthesia and discuss its causes, treatment methods, and the follow-up results in the literature. The peripheral Tapia's syndrome is a rare complication of anesthetic airway management. Main symptoms are hoarseness of voice and difficulty of tongue movement. Tapia's syndrome after endotracheal general anesthesia is believed to be due to pressure neuropathy of the vagus nerve and the hypoglossal nerve caused by the endotracheal tube. To our knowledge, no report has been published or given an explanation for Tapia's syndrome after posterior cervical spine surgery. Two patients who underwent posterior cervical surgery complained hoarseness and tongue palsy postoperatively. There is no direct anatomical relation between the operation, the vagus nerves and the hypoglossal nerves, and there is no record of displacement or malposition of the endotracheal tube. After several months, all symptoms are resolved. To avoid this problem in posterior cervical spine surgery, we suggest paying special attention to the position of the endotracheal tube to avoid excessive neck flexion before and during the positioning of the patient.

  • Piriformis Syndrome is characterized by pain of the low back, groin, perineum, buttock, hip, posterior thigh, leg and foot. Symptoms are aggravated by sitting, prolonged combination of hip flexion, adduction, the medial rotation, or by activity. In addition, patient may complain of painful swelling of the limb and sexual dysfunction-dyspareunia in female, and impotence in male. It currently appears that three specific conditions may contribute to the piriformis syndrome: (a) myofascial pain; (b) nerve and vascular entrapment; (c) dysfunction of the sacroiliac joint. The important keys of diagnosis are history and physical examination. There is no known objective diagnostic method. We described the clinical features of four cases of piriformis syndrome and reviewed foreign literature.

A Comparison of Shoulder Stabilizer Muscle Activities of Therapeutic Climbing and Isometric Exercise in Patients with Shoulder Impingement Syndrome

  • Purpose: The purpose of this study was to compare therapeutic climbing exercise and general isometric exercise in patients with shoulder impingement syndrome. Methods: Among 20 adults, study subjects were arbitrarily classified into an experimental group of 10 and a control group of 10. The control group performed general isometric exercise (ISE) and the experimental group performed therapeutic climbing exercise (TC) (3 sets, 3 times per week for 8 weeks). To evaluate the effects of exercise, subjects were evaluated using a Disabilities of the arm, shoulder and hand score (DASH), a goniometer for range of motion, and shoulder activity measured serratus anterior, upper trapezius, and lower trapezius. Independent and paired t-test were used for comparison of the effect between groups. Results: DASH scores showed a significant decrease in both groups after 8 weeks of treatment (p<0.001) and significant difference was observed between the TC groups (p<0.01). Flexion and abduction were significantly increased after 8 weeks of treatment in the ISE group (p<0.001) and flexion, abduction, external and internal rotation were significantly increased after 8 weeks of treatment in the TC group (p<0.001). Serratus anterior and lower trapezius activity were significantly increased after 8 weeks of treatment (p<0.001) and upper trapezius activity was not significantly increased after 8 weeks of treatment in the ISE group (p>0.05). Serratus anterior, lower trapezius, and upper trapezius activity were significantly increased after 8 weeks of treatment in the TC group (p<0.001). Conclusion: Scapular stabilizing exercise using a therapeutic climbing exercise increases range of motion and decreases DASH, and increases activity of shoulder muscles in patients with impingement syndrome.

  • Purpose: The purpose of this study was to compare modified sling exercise and general isometric exercise in patients with shoulder impingement syndrome. Methods: Twenty subjects were studied. The control group, n1 = 10, received instructions for doing general isometric (ISO) exercise. An experimental group, n2 = 10, received instructions for doing push-ups from standing and sitting positions and modified scapular exercises using a sling (3 sets, 3 times per week for 6 weeks). To evaluate the effects of exercise, subjects were evaluated using a visual analog scale for pain, a goniometer for range of motion, and electromyography for onset time of muscle contraction. Statistical analysis was done using the Wilcoxon Signed rank and Mann-Whitney U tests. Results: Pain in the sling group was significantly decreased after 6 weeks of treatment (p<0.05) pain in the general ISO exercise group was not significantly decreased (p>0.05). Flexion and external rotation were significantly increased after 6 weeks of treatment in both groups (p<0.05) and the change in the Sling group was greater than in the ISO group (p<0.05) in the flexion test. Time of onset of contractions in the Sling group for the upper trapezius, lower trapezius and serratus muscle were significantly decreased after 6 weeks of treatment (p<0.05), but the onset time for the middle trapezius did not significantly decrease (p>0.05). Conclusion: Scapular stabilizing exercise using a sling increases range of motion and decreases pain, and onset time of muscle contraction in patients with impingement syndrome.

그라인딩 작업시 손목자세별 국소진동 전달특성 분석 (An Analysis of Transmitted-Vibration Characteristics by Different Wrist Posture during Grinding Tasks)

  • This study was performed to evaluate the characteristics of transmitted vibration to hand-arm system under different work posture while operating a light-weighted powered hand grinder. For the experiment, 8 different types of wrist posture (natural, unlar-flexion, radual-flexion, flexion, extension, complex posture, and etc.) and 3 types of feed force (20[N], 50[N], 70[N]) were considered. 10 male subjects were employed to polish metal plate with a hand grinder. All of them were normal and healthy with no history and symptom of the work related musculoskeletal disorders in the dominant hand. Vibration acceleration data were recorded with sampling rate, 2048[Hz]. In addition, unweighted overall R.M.S. acceleration at the tool and wrist, and transmissibility between them were used to evaluate factors from the recorded tri-axial vibration acceleration. The results indicate that transmissibility of natural wrist posture was significantly higher than others. In addition, as the feed force becomes larger, the vibration was transmitted in large quantities to hand-arm system through radius.

  • Objective : The therapeutic exercise on low back found In the literatures mostly have adopted methods that are applied to only certain muscles. The purpose of this paper to classify various low pains and to Investigate an active physical treatment can be applied to certain low back pain. Methods : By exploring the journals and medical publications. Results and Conclusions 1. A goal of low back exercise is reinforcement and extension of muscles in order to control pains. 2 Low back exercise which causes the movement of the spinal joint and disk can control pains. 3. Flexion exercise of lumbar spine can be generally applied to any low back pains except kyposis. 4. Extension exercise of lumbar spine can be applied to any low back pains except facet joint syndrome or hyperlordosis. 5. Rotation exercise of lumbar spine can be applied to any low back pains except facet joint syndrome. 6. Lateral bending exercise of lumbar spine can be applied to HNP. facet Joint syndrome, scoliosis.

Piriformis Syndrome in Knee Osteoarthritis Patients after Wearing Rocker Bottom Shoes

  • Background: Rocker bottom shoes (RBS) are popular among patients with different foot, leg, or back problems in Korea. Patients with knee osteoarthritis concurrent weakness in the quadriceps femoris muscle, who wear these shoes, are often assumed to develop piriformis syndrome (PS). This study was performed to improve the understanding about the effect of wearing such shoes on duration of the syndrome in knee osteoarthritis. Methods: We randomly assigned 150 patients with PS, who had used RBS daily for at least 6 months, to 2 groups, the S (stopped wearing) and K (kept wearing) groups. Both the groups were subdivided into the O and N groups, comprising patients with and without knee osteoarthritis, respectively. The effects of the treatment, including piriformis muscle injections and a home exercise program, were compared between the 2 groups by using a flexion-adduction-internal rotation (FAIR) test, a numeric rating scale (NRS), and the revised Oswestry disability index (ODI) during the 12-week follow-up. Results: The positive FAIR test ratios, mean NRS scores, and revised ODIs were higher in the KO group than the SN group from 4-12 weeks after treatment. Conclusions: RBS may extend duration of the PS in osteoarthritis patients.

Effect of Physiotherapeutic Intervention Using TECAR Therapy on Pain Self-Awareness and Hip Joint Function in Hip Impingement Syndrome: A Case Study

  • PURPOSE: The current case study focuses on identifying the effects of the independent application of TECAR therapy and physiotherapeutic intervention using TECAR therapy on pain self-awareness and hip joint function in patients with hip impingement syndrome caused by nonstructural changes. Subjects: The research subject was a 34-year-old woman struggling with acute pain in her left hip, difficulty in actively moving the hip, and a problem in its overall function. METHODS: The subject's pain awareness and hip joint function were measured using a Visual Analog Scale (VAS) and passive range of motion (PROM), respectively. The experimental intervention was carried out in 24 sessions of 16 minutes each, three times a week, for eight weeks. RESULTS: The VAS score decreased to 0 cm on the post-test from 4.3 cm, 6.5 cm, and 7.2 cm in the pre-test at the rest, standing, and gait positions, respectively. The index of PROM measured hip joint flexion, extension, abduction, adduction, internal rotation, external rotation, and passive straight leg raise. The values increased to 122.5°, 24.5°, 78°, 33°, 65°, 42°, and 96.5° in the post-test compared to 88.5°, 15°, 39°, 21.5°, 23°, 22°, and 46.5° in the pre-test, respectively. CONCLUSION: TECAR therapy and physiotherapeutic intervention using TECAR can help reduce pain and enhance the hip joint function in patients with hip impingement syndrome.

McConnell 테이핑과 Kinesio 테이핑이 무릎넙다리통증증후군 환자의 계단 올라가기 시 통증과 다리관절 각도에 미치는 영향 (Effects of McConnell Taping and Kinesio Taping on Pain and Lower Extremity Joint Angles During Stair Ascent in People with Patellofemoral Pain Syndrome)

  • Purpose: The purpose of this study was to investigate the effect of McConnell taping and Kinesio taping on pain and lower extremity joint angles when patients with patellofemoral pain syndrome (PFPS) ascend stairs. Methods: Fifty young adults who were experiencing anterior knee pain due to PFPS were selected as participants. Then, 25 patients were randomly assigned to the McConnell taping group and 25 to the Kinesio taping group. Pain and lower extremity joint angle were measured while ascending stairs before and after the intervention. A paired t-test was performed to evaluate the amount of change in the parameter values after the intervention within the groups, and an independent t-test was used to compare the results of the groups. Results: In the within-group comparisons, a statistically significant difference was found in both groups between the anterior knee pain scale scores recorded before and after the intervention (p < 0.05). A statistically significant difference was also found between the groups (p < 0.05). Comparison of the lower extremity joint angles at initial contact, loading response, terminal stance, and pre-swing within the groups showed that there were statistically significant differences in the hip, knee flexion, abduction, lateral rotation, and dorsiflexion angles in both the McConnell and Kinesio taping groups (p < 0.05). There was also a statistically significant difference in all angles between the groups during the following events (p < 0.05): (1) at initial contact, (2) at loading response (except hip flexion angle), (3) at terminal stance (except hip flexion and lateral rotation angles), and (4) at pre-swing (except hip, knee abduction, and inversion angles). Conclusion: McConnell taping and Kinesio taping both effectively improved the occurrence of knee pain and the lower extremity joint angles during stair ascent in patients with PFPS. However, McConnell taping had a significant impact on pain reduction and lower extremity joint angles compared to Kinesio taping.

허리 부위 동태손상증후군의 진단을 위한 설문 문항 개발 (Development of Diagnostic Questionnaires for Lumbar Movement System Impairment Syndromes)

  • Objectives: The aim of this study is to develop diagnostic questionnaires for the lumbar movement system impairment syndrome. Methods: We had reviewed the previous study and literature and organized various checkable items for differential diagnosis of five different lumbar movement system impairment syndromes. Next, we selected items which can be developed as questionnaire items. Finally, we conducted face validity study with eight Korean medicine doctors and survey research for the importance score of the items with three experts. Results: We developed a diagnostic questionnaire as followings: 6 items for lumbar extension syndrome; 5 items for lumbar flexion syndrome; 6 items for lumbar rotation syndrome. With the 2 rounds of survey research, we could bridge the differences of the importance score of each items. Conclusions: Questionnaire for the diagnosis of movement system impairment syndrome was developed. With this questionnaire, objective diagnosis of lumbar movement system impairment syndrome can be expected. This tool may also be used for detecting sub-health status of musculoskeletal systems.
