The Influence of Hamstring Length on the Flexion-relaxation Phenomenon in Relation to the Erector Spinae Muscle During Trunk Flexion (몸통 굽힘 동안 뒤넙다리근 길이가 척추세움근의 굽힘-이완 현상에 미치는 영향)

  • Background: The flexion-relaxation phenomenon (FRP) refers to a sudden onset of activity in the erector spinae muscles that recedes or fades during full forward flexion of the trunk. Lumbar spine and hip flexion are associated with many daily physical activities that also impact trunk flexion. Shorter hamstring muscles result in a reduction of pelvic mobility that eventually culminates in low back pain (LBP). Many studies have explored the FRP in relation to LBP. However, few studies have investigated the influence of hamstring muscle length on the FRP in relation to the erector spinae muscles during trunk flexion. Objects: This study aimed to investigate the influence of hamstring muscle length on the FRP in relation to the erector spinae muscles during trunk flexion. Methods: Thirty subjects were divided into three groups according to hamstring length measured through an active knee extension test. The 30 participants consisted of 10 subjects who had a popliteal angle of 20° or less (Group 1), 10 subjects who had a popliteal angle of 21°-39° (Group 2), and 10 subjects who had a popliteal angle of 40° or more (Group 3). A one-way analysis of variance was used to compare the difference in muscle activity of the erector spinae muscles during trunk flexion. Results: The subjects with a shorter hamstring length had significantly higher muscle activity in their erector spinae muscles during trunk flexion and full trunk flexion (p < 0.05). The subjects with a shorter hamstring length also had a significantly higher flexion-relaxation ratio (p < 0.05). Conclusion: The results of this study demonstrate that differences in hamstring muscle length can influence the FRP in relation to the erector spinae muscles. This finding suggests that the shortening of the hamstring might be associated with LBP.

The Effect of Pain on Muscle Performance of Knee Extension and Flexion in Elite Athletes

  • Objective: In this study, we compared the key indices of isokinetic tests for knee joint extension and flexion according to the presence or absence of pain in elite athletes with past knee and thigh injuries. Design: Crossed-sectional study Methods: This study was conducted on 33 elite athletes. The elite athletes who participated in this study were divided into a group with pain and a group without pain due to past knee and thigh injuries. The subjects in each group performed an isokinetic muscle strength test for knee joint extension and flexion. After the isokinetic muscle strength test, the main indices of the isokinetic test were compared between the pain group and the non-pain group. Results: In the comparison of the pain group and the non-pain group for 60°/s knee flexion and extension in elite athletes, there was a significant difference only in the torque max average/kg for knee extension. However, there was no statistically significant difference in all other variables. The comparison of the pain group and the non-pain group for 240°/s also showed a significant difference in the torque max average/kg for knee extension, and there was no statistically significant difference in all other variables. Conclusions: In subjects who have experienced previous knee or thigh injuries and have mild pain during strong isokinetic strength tests, torque average/kg is affected, but other strength variables are not affected. Therefore, it would be good to consider this aspect and use it as a basis for testing and training elite athletes.

The Effect of Isometric Exercise and Active Stretching on Joint Function in Patient with Osteoarthritis (슬관절염 환자의 능동신장과 등척성 운동이 관절 기능에 미치는 영향)

  • Purpose: This study was to investigate the effect of isometric exercise and active stretching on joint function in patient with osteoarthritis. Methods: 30(M=1, F=29) subjects with osteoarthritis were divided in three groups: control group, quadriceps isometric exercise group, and hamstring active stretching group. After 6 weeks treatment, ROM(range of motion) and LSS(lysholm scoring scale) were measured. Results: There was a significant increase in knee flexion, extension in post-treat of quadriceps isometric exercise group and hamstring active stretching group(p<0.05). There was a significant increase in LSS in post-treat of quadriceps isometric exercise group and hamstring active stretching group(p<0.05). Conclusion: This study shows that both the active stretching exercise and the quadriceps isometric exercise effectively promote the range of knee extensions for osteoarthritis patients. Also, as measuring the ROM of knee flexion and extension by exercise methods, there is significant increase from knee flexion and extension in both hamstring stretching exercise group and quadriceps isometric exercise group. The increase of the range of knee is more effective in the exercise of knee extension with hamstring stretching exercise groups. And it is found that there are some difference between the experimental group and controlled group in statistics. As it is concerned with the function of knee extension, supporting and squatting are more effective to promote the both knee extension and flexion in its range. Therefore, this shows that the hamstring stretching exercise is required in general with enforcing the quadriceps at a sickbed in the present.

Application Effects of Various Solidity of Mouth Guard on Torque and Power at Isokinetic Contraction in Shoulder : A Pilot Study (다양한 경도의 마우스가드의 착용이 어깨관절의 등속성 수축 시 토크 및 파워에 미치는 영향 : 예비 실험)

  • PURPOSE: This study find out the effect of torque and power through various solidity of mouth guard on shoulder flexion and extension. METHODS: This study was conducted with healthy adults in their 20s Korean. Cybex system was used to measure the torque and power fo shoulder joint flexion/extension during isokinetic contraction when the various solidity mouth guard was used and no mouth guard was used. RESULT: The results of this study were summarized as follows: relative torque and average power was showed significantly difference between non applying mouth guard and applying hard type mouth guard during shoulder flexion and extension at isokinetic contraction. CONCLUSION: The application of hard type mouth guard was increased torque and power in joint of body. which may be clinically useful.

  • The isometric area of the anterior cruciate ligament was calculated during knee flexion-extension. Flexion-extension motion data of the joint were obtained using Fastrak and a three-dimensional motion measurement system. A total of five subjects were seated on a flat table and the tibia sensor position was measured with the femur fixed on the table. A three-dimensional knee model was constructed using a graphic tool to simulate the knee motion. Twenty seven positions of the tibia region and forty two positions of the femur region were selected and the distances between the determined tibial and femoral points were calculated. Highly isometric areas were found and displayed as three dimensional aspects.

  • There is no line of demarcation between the shoulder and elbow regions. Pain In the arm may originate at the shoulder with reference downwards or less often at the elbow with reference upwards. Most pains indicated by the patient at the elbow or forearm have a local origin, since at the more distal part of the upper limb the capacity for correct localization is good. Once it is clear that the elbow region is at fault, the joint and the muscles about it are tested by ten movements. 1. Four. Passive extension, flexion, pronation, supination-full range, LOM, painful, painless. 2. Four. Resisted extension, flexion, pronation, supination-strong, weak, painful, painless. 3. Two. Resisted flexion, extension at the wrist-painful, painless. The muscles that perform theses two movements arise from the humeral epicondyles and a lesion in either often causes pain felt at the elbow although the tissuse affected is not functionally a part of the elbow (i. e. Tennis elbow and Golfer's elbow).

Repetitive Dorsiflexion Exercises in Ankles have Effects on the Active Range of Flexion and Extension Motion through Lumbar, Cervical Spine and Ankle, Wrist Joints (족관절의 반복적 배측굴곡 운동이 요.경추와 족.수관절의 능동 굴신 가동범위에 미치는 영향)

  • Background: This study was designed to analyze Repetitive dorsiflexion exercises in ankles have effects on the active range of flexion and extension motion through lumbar, cervical spine and ankle, wrist joints. Methods: 30 female college students in their twenties who frequently wear high heels participated the number of the experimental group was 15 persons and the number of the control group was 15 persons. They did exercise at the physical therapy room in M college, from the 8th of March to the 11th of April 2007. The experimental group had used the model of dorsiflexion repetitive exercise three times per week, for 4 weeks, but the control group did not exercise at all. In the sagittal plane active ROM of the these spine and joints were measured before and after the experiment using a digital goniometer. The results of two groups were compared and analyzed using paired T-test. Results: The active range of flexion and extension motion of the vertebra(especially lumbar flexion) and distal joints were significantly different in exercise group(p<.05). Conclusion: The model of repetitive dorsiflexion exercise of the ankle joint had positive effects on improving the active range of flexion and extension motion of the lumbar vertebra and distal joints of limbs. The results suggest that the repetitive dorsiflexion exercise is useful and also effective therapy for improving motion in women usually wearing high-heel.

Comparison of the Electromyographic Activity of the Tibialis Anterior and Isometric Dorsiflexor Strength during Dorsiflexion According to Toe Postures in Individuals with Ankle Dorsiflexor Weakness

  • Purpose: This study compared the electromyographic activity of the tibialis anterior (TA) and isometric dorsiflexor strength during dorsiflexion according to the toe postures in individuals with ankle dorsiflexor weakness. Methods: Twenty subjects with ankle dorsiflexor weakness participated in this study. The electromyographic activity of the TA and isometric dorsiflexor strength during dorsiflexion between with toe flexion, extension, and neutral postures were measured using an electromyography device and a hand-held dynamometer in individuals with ankle dorsiflexor weakness. One-way repeated measured analysis of variance, and a Bonferroni post hoc test was used. The level of statistical significance was set to α=0.01. Results: The electromyographic activity of the TA was greater with toe flexion during dorsiflexion than with toe extension and neutral postures (p<0.01). The isometric dorsiflexor strength was smaller with toe flexion during dorsiflexion than with toe extension and neutral postures (p<0.01). Conclusions: In individuals with ankle dorsiflexor weakness, the dorsiflexion with toe flexion can help improve the TA electromyographic activity. The toe posture during dorsiflexion for selective TA activation should be considered, especially in individuals with ankle dorsiflexor weakness.

Pain and Weakness on Unilateral Upper Extremity Diagnosed as Brachial Plexopathy after Herpes Zoster Infection (대상포진후 상완신경총병증으로 진단된 편측 상지의 통증과 위약)

  • Motor paralysis is a less common neurologic complication of herpes zoster. Until now, a few cases have been reported, and most of these cases showed brachial plexopathy involving one or two segments. We report a patient with pain and weakness on upper extremity diagnosed as brachial plexopathy after herpes zoster infection. An 88-year-old female patient complained not only tingling sense, pain, and swelling on right whole arm, but also weakness on this right upper extremity. On physical examination, weakness is seen in right shoulder abduction·shoulder flexion·elbow flexion·elbow extension· wrist extension (grade 4), finger flexion·finger abduction·finger extension·finger DIP flexion (grade 3). In electrodiagnostic study and magnetic resonance imaging study, she was diagnosed as the brachial plexopathy, whole branch involved. This is the only case of post-herpetic brachial plexopathy involving whole branch in domestic.

  • 본 논문에서는 이처럼 중요한 역할을 하는 무릎의 굽힘(flexion)과 폄(extension)에 대하여 측정을 한다. 무릎과 엉덩이를 펴고 골반을 중립상태로 두고 엎드려 무릎을 구부리고 편다. 일반인의 도수 측정 각도는 구부렸을 때 $110^{\circ}$에서 $135^{\circ}$ 사이의 값이 나온다. 여기서 구부렸을 때 고통을 호소하면 무릎 관절 및 근육에 이상이 있는 것이다.
