A Case of Soft Tissue Metastasis in Contralateral Submandibular Space by Regional Recurrence of Lower Lip Cancer (하구순암의 구역 재발로 반대편 악하 공간에 발생한 연조직 전이 1예)

  • The most common cause of treatment failure in oral cavity cancer is when it is found to have local recurrence, usually occurring in the ipsilateral cervical lymph node. On the contrary, it is extremely rare to find local recurrence in soft tissue metastasis (STM) in the contralateral neck. Furthermore, lung cancer and malignant lymphoma are most commonly confined to their primary sites. The poor general condition increases the likelihood of STM, which indicates bad prognosis. A 72-year-old man with a hard and fixed mass on the right submandibular space visited our clinic. He had received a wide excision with local flapreconstruction for squamous cell carcinoma in the left corner of lower lip 18 months ago. We performed the wide excision with bilateral selective neck dissection (I-III), and he was finally diagnosed as STM from contralateral lip cancer. We report this unique and rare disease entity with a literature review.

Effect of Pile Head Constraint on Lateral Behavior of Single Flexible Pile in Non-homogeneous Sand (비균질 사질토 지반에서 단일 휨성말뚝의 수평거동에 대한 말뚝 두부 구속효과 연구)

  • This paper shows the results of a series of model tests on the behavior of single flexible pile, which is subjected to lateral load, in non-homogeneous Nak-Dong River sands, consisting of two layers. The purpose of the present paper is to investigate the effects of ratio of lower layer thickness to embedded pile length, ratio of soil modulus of upper layer to lower one, and pile head constraint condition on the characteristics of lateral behavior of single pile. These effects can be quantified only by the results of model tests. Based on the results of model tests, in non-homogeneous sand, it was found that the lateral behavior depends upon the ratio of soil modulus of upper layer to lower one. And, in respect of deflection, it was found that the relationship between the deflection ratio of non-homogeneous to homogeneous sand and the ratio of lower layer thickness to embedded pile length can be fitted to exponential function of H/L and lateral load by model tests results. Also, in respect of maximum bending moment, it was found that the relationship H/L and $MBM_{fixed-head}/MBM_{free-head}$ can be fitted to linear function of H/L by model test results.

  • We present a novel head tracking system for stereoscopic displays that ensures the viewer has a high degree of movement. The tracker is capable of segmenting the viewer from background objects using their relative distance. A depth camera is used to generate a key signal for head tracking application. A method of the moving parallax barrier is also introduced to supplement a disadvantage of the fixed parallax barrier that provides observation at the specific locations.

The Tectonic Characteristics of ′KAN′ in the head residence of Uisong Kims (의성 김씨종택에 있어서 간의 건축적 특성)

  • The purpose of this study is to explain the tectonic characteristics of 'Kan' which is an unit of composition in the head residence of Uisong Kims. Here, the 'Kan' is assumed as a conceptual framework to organize a house and also a maneuvering tool to control the spatial order with relationship between the whole and the part. Not limited to the simple conceptual aspects, but the interrelated elements such as the site conditions, modules, functions, structure and aesthetic factors. Through the process of the study, the role of 'Kan' is proved to be a tectonic tool for self-regulating architectural order by relation matrix of elements. 'Kan' is not only a fixed measuring tool for functions and material of a house but also a flexible device to control the building composition. 'Kan' is certainly a relative subject for measurement of form, space and aesthetic quality.

  • Head fixed type multi-focus display system using Galvano-scanner and DMD(Digital Micro-mirror Device), which is able to perfectly accommodate, can solve eye fatigue due to conflict between convergence eye movement and accommodation action in stereoscopic display. This system is able to accommodate through making convergence point about each view point and offering it in front of observer's pupil by using laser scanning method. In this paper, we analyzed laser scanning method of this multi-focus display system. And multi-focus display system based on this analysis was made, which showed that focus adjustment was possible through video camera. As a result, formation principle of view point of multi-focus system by laser scanning method was verified.

  • A lot of researches have been done to improve the energy efficiency of Wireless Sensor Networks. But all the current researches are based on the idea of direct communication between cluster head and sink node. Previous results assume that node can intelligently regulate signal energy according to the distance between nodes. It is difficult to implement algorithms based on this assumption. We present a multi-level routing algorithm from the sink node to all other nodes which have fixed radio wave radius. We also show the energy saving efficiency and the implementation in real WSN using the simulation result.

  • Freiberg's infarction involving the fifth metatarsal head is very rare condition. The authors experienced a case of avascular necrosis of the fifth metatarsal head and base of the proximal phalanx in a 27-year-old female patient. Involved bones were excised and internally fixed with 2 K-wires after autogenous cancellous bone grafting. Pain was relieved, and an excellent clinical result was obtained at the follow-up examination six months after the surgery.

A Case of Co-Existence of Diffuse Large B cell Lymphoma in Parotid Gland and Follicular Lymphoma in Upper Neck (동측의 이하선과 상경부에 공존하여 발생한 미만성 거대 B세포 및 여포성 림프종 1예)

  • Primary non-Hodgkin's lymphoma (NHL) rarely involves the parotid gland and its incidence is only 1.7% to 3.1% of all salivary gland neoplasms. The mucosa associated lymphoid tissue (MALT) is the most common subtype of NHL, followed by follicular lymphoma (FL) and diffuse large B cell lymphoma (DLBCL). However, two distinct types of lymphomas occurring synchronously in the parotid gland and cervical lymph node have not been reported earlier. A 72-year-old man with rubbery-hard and fixed mass on the left parotid area came to our clinic. We performed the left total parotidectomy with ipsilateral excision of lymph node (level II), and he was finally diagnosed as DLBCL in parotid gland and FL in upper neck. It is presumed that the DLBCL appeared to be a transformation from FL. We report the unique and rare disease entity with brief literature review.

A Case of Bilateral Tension Pneumothorax after the Successful CO2 Laser-assisted Removal of a Bronchial Foreign Body in a Child

  • Bronchial foreign body aspiration (BFA) is a common but emergent condition in infants and children. Furthermore, it can result in various complications such as atelectasis, pneumonia, bronchiectasis, and pneumothorax. Among these, pneumothorax is a very rare complication. However, it can be fatal without the swift implementation of appropriate treatment. We experienced a case of 16-month-old girl with an aspirated peanut. The foreign body was fixed in her left main bronchus. A CO2 laser was used to safely cut and break the foreign body. Removal was successful after breaking it. But after the process, inflammatory tissue of the tracheal mucosa was ruptured. Bilateral tension pneumothorax followed after the rupture. The patient was treated with bilateral chest tube insertion. Here we present this BFA case with a rare and unexpected complication. We also review the appropriate literature.

Removal of a Bronchial Foreign Body by Bronchoscopic Cryotherapy: A Case Study

  • A foreign body in the airway can be a potentially life-threatening event. The diagnosis and treatment of foreign bodies in the airway are a challenge for otolaryngologists. Despite the improvements in medical care and public awareness, approximately 3,000 deaths occur each year from foreign body aspiration. A high degree of vigilance is required to ensure prompt treatment and avoid the complications of foreign body aspiration. The author encountered a case of a 77-year-old female patient who had aspirated an unknown foreign body that was fixed in her main bronchus. An initial attempt was made to remove it with a flexible bronchoscope but failed due to the patient's hypoxemic state during the procedure. Under general anesthesia, a rigid bronchoscopic examination was performed, but it was difficult to approach the object due to the bronchus curvature. Instead, a cryotherapy instrument of bronchoscopy was applied. The foreign body was frozen and removed to the carina, where a laryngoscope and laryngeal forceps were used to remove it.
