치과위생사의 근무환경 연구 -근무기관·경력·지역을 중심으로- (A study on work environments for dental hygienists: - focusing on kind of workplace. career and service area)

  • The purpose of this study was to examine the work environments of dental hygienists, to find out about what problems there were with their work environments and ultimately to help improve their work environments. It's basically intended to pave the way for furthering the welfare and interests of dental hygienists. The subjects in this study were dental hygienists who were selected by random sampling from among the members of Korean Dental Hygienists Association. Approximately 20 percent of the members each were selected from every region across the nation, and their work environments were investigated in consideration of the kind of their workplaces, service area, career and field of duties. As for the demographic characteristics of the dental hygienists investigated, there were differences between those who worked in the field of health care and the clinical workers. More of the former were older and married, and the former was ahead of the latter in career and education as well. Regarding working hours and leave of absence by kind of workplace, the number of regular average holidays was different according to their place of employment. Dental hospitals(6.66 days) and dental clinics(6.81 days) gave their employees less days off on the whole, whereas public dental clinics(19.29 days) granted the dental hygienists the longest leave of absence. Also, there was a broad gap in the number of regular average holidays among different regions in the nation. The dental hygienists who worked in Gangweon province enjoyed the longest holidays(10.88 days), while those on Jeju Island took the shortest vacation(4.46 days). Concerning monthly mean pay by place of employment, those who worked in public dental clinics were paid the best, and the dental hospital employees received the smallest pay. Their monthly mean pay significantly varied with the kind of their workplaces. As to connections between service area and pay level in the event of the dental hygienists with a four-year career, those who served in Seoul were paid the best(1,820,800 won), followed by Gyeonggi province(1,795,800 won), Gyeongsang province(1,604,200 won), metropolitan cities(1,424,800), Gangweon province(1,300,000 won) and Jeolla province(1,016,700 won). In regard to the starting pay in the different areas, the starting pay was largest in Seoul(1,501,800 won) and smallest in Jeolla province(904,000 won). Concerning work environments by place of employment, the dental hygienists in public dental clinics, general hospitals and university hospitals were far older than the others, and the career of the former was much larger than that of the latter. As to the number of regular leave of absence, public dental clinics, general hospitals and university hospitals were different from dental hospitals and clinics in that regard as well. Concerning monthly pay, public dental clinics paid their employees the best, and dental hospitals and clinics were ahead in terms of pay raise. But the reason seemed that public dental clinics and general hospitals increased the pay of their employees based on a fixed wage system and according to a fixed rate at the same time. As for relations between career and work environments, the pay of the dental hygienists differed with their career. The amount and rate of pay raise were largest for those whose career was between four years and less than six years, and smallest for those whose career was between seven years and less than nine years. The above-mentioned findings of the study suggested that in order to give dental hygienists better treatment, pay and welfare benefits should urgently be improved, and that it's required to take actions to boost their job satisfaction. Besides, they should be given more chances to receive education or to take training courses in pursuit of self-development, and how to narrow gaps in work environments among different regions or fields should carefully be considered.

하반신 마비환자의 FES 독립보행을 위한 근육 강화 프로그램 (FES Exercise Program for Independent Paraplegic Walking)

  • 본 연구에서는 엑서사이즈 프로그램이 FES를 이용한 하반신 마비환자의 일어서기 및 보행에 미치는 영향을 고찰하였으며, 또한 주요 다리근육들의 전기자극에 대한 수축특성과 피로특성에 주안점을 두었다. 정상인 10명과 완전 하반신 마비환자 4명의 대퇴사두근에 연속적 혹은 간헐적으로 전기자극을 가하였고, 자극주파수는 20Hz와 80Hz로 하였다. 또 근육의 길이에 따른 피로현상을 살펴보기 위하여 무릎의 각도를 90$^{\circ}$와 150$^{\circ}$로 각각 고정한 뒤 무릎신근 토크를 측정하였다. 그 결과를 바탕으로 남자 하반신 마비환자의 대퇴사두근과 장딴지근에 지난 2년간 FES엑서사이즈를 시행하였다. 무릎신근의 근력이 체중을 지지하기에 충분하다고 판단되었을 때 FES 일어서기를 시작하였으며, 자세교환 연습을 거친 뒤 평행봉 혹은 워커를 잡고 정전압 자극기와 표면전극을 사용한 4 또는 6채널 자극으로 보행하도록 하였다. 마비된 근육은 정상인과는 반대로 최적길이 부근에서 상대적으로 급격한 피로를 나타내었고, 저주와 자극과 간헐 자극이 피로를 지연시키는 것을 확인할 수 있었다. 본 실험에 참가한 환자는 FES 엑서사이즈 프로그램을 실시한 결과, 근력이 초기의 10배 정도로 증가하였고, 피로지수는 초기의 절반 정도로 감소하였으며, 엑서사이즈 횟수를 매주 6일에서 7일로 바꾼 후 근력이 눈에 띄게 향상되었다. 환자 자신의 잔존능력도 향상되어 양쪽 무릎을 10cm정도 들어올릴 수 있게 됨으로써 보행시 스윙 단계에서 이 능력을 최대한 활용할 수 있었다. 현재 환자는 워커를 잡고 스스로 자극기의 스위치를 조작하면서 4채널 자극에 의하여 10m/min의 속도로 최대 약 2분 40초의 보행이 가능하다.

사적소득이전과 노후소득보장 (Private Income Transfers and Old-Age Income Security)

  • 본 연구는 그동안 사회적 안전망이 미흡한 가운데 우리나라의 고령인구 부양에 중요한 역할을 해온 사적소득이전에 대한 미시적 분석을 통해 향후 노후소득보장정책에 시사점을 제공하고 있다. 한국노동패널 자료에 따르면, 만 60세 이상 노인가구주 세대의 다섯 가구 중 두 가구는 매월 자식들로부터 생활비 등의 경제적 도움을 받고 있는 것으로 조사되었으며, 공공부조 등 공적소득이전은 사적소득이전을 구축하는 효과를 가진 것으로 나타났다. 또한 지금까지는 사적이전이 공적이전보다 빈곤완화효과가 큰 것으로 나타났지만, 외환위기 후 복지지출의 확대와 함께 공적이전의 비중이 대폭 높아져, 공적이전을 주 소득원으로 살아가는 만 60세 이상 고령자는 2003년 기준으로 약 4분의 1에 달하는 것으로 조사되었다. 그러나 같은 해 기준으로 국민기초생활보장제도의 보호지정을 실질적으로 필요로 하면서도 수급권에서 배제된 것으로 추정되는 노인가구주 세대는 약 12%로서, 이들 가구의 빈곤 해소를 위해서는 예산 확보와 함께 사각지대를 없애기 위한 전달체계 개선이 필요할 것으로 보인다. 이처럼 여전히 광범한 빈곤노인계층이 존재하는 한편, 고령인구에 대한 사적 부양이 공적 부양으로 전환되면서 재정건전성이 우려되고 있는 현 상황에서 소득재분배의 효과가 적고 경직적 비용부담이 큰 보편급여의 확대보다는 취약노인계층에 대한 집중적인 지원을 강화할 필요가 있을 것이다.

정적 후처리방식에 의한 GPS의 측위정도 개선 (Improvement of GPS positioning accuracy by static post-processing method)

  • 기선장에 따른 GPS 위치 정도 및 분포를 측정하기 위하여 2002년 5월 29일부터 6월 2일까지 2대의 GPS수신기 (L1, 12채널)를 이용하여 고정점에서 30분에서 24시간동안 관측하는 실험을 수행하였다. 기준국(PKNU)과 이동, 국립지리원의 GPS 상시관측소에서 수신한 GPS 데이터는 동적 및 정적 후처리방식으로 처리하였으며, 그 결과를 요약하면 다음과 같다. 1. 2002년 5월 31일 차단각 (cut-off angle) $15^{\circ}$에서 6시간 이상 관측 가능한 위성은 16개였고, 위성은 주로 동서방향으로 배치되어 있었다. 10분마다 평균한위성 수와 GDOP는 각각 8개, 3.89이었다. 2. 후처리 전후의 GPS 평균위치는 모두 기준점에서 남서쪽으로 편위 (단독 측위 : 1.17m, 후처리 : 평균 0.43m)되었다. 확율원 오차는 단독 측위가 6.65m이었고 이를 동적 후처리하면 단독 측위의 약 33.8%(표준편차 0=17.2), 정적 후처리하면 단독 측위의 약 5.3% (0=2.2)로 감소시 킬 수 있었다. 3. 정적 후처리방식으로 구한 기선장 x(Km)와 확률원 오차 y(m) 의 관계식은 y = 0.0016x + 0006 $(R^2=0.87)$이었다. 본 연구에서 사용한 1주파용 GPS 수신기를 이용하여 정적 후처리방식으로 위치를 측정하는 경우, 기선장을 100Km 이내로 하고 GPS 수신시간을 30분 이상으로 하면 확율원 오차 20 Cm 이내의 측위가 가능함을 알 수 있었다.

호흡동조방사선치료를 위한 Trigger mode 투시영상 획득 시 호흡 속도에 따른 정확성 평가 - Phantom Study (Evaluation of the Accuracy and usability of Trigger mode in Respiratory Gated Radiation Therapy)

  • 목 적 : 본 연구를 통해 호흡동조방사선치료(Respiratory Gated Radiation Therapy, RGRT)시 환자 호흡 속도에 따른 Trigger mode의 정확성과 유용성을 평가하고자 한다. 대상 및 방법 : 호흡 속도에 따른 Trigger mode의 정확성을 평가하기 위해 QUASARTM 호흡 움직임 팬텀에 3 mm의 기준 표지자(Fiducial marker, gold marker)를 삽입하여 본원 한달 동안 환자의 평균 호흡인 20 bpm(Breath per minute)을 기준으로 4DCT 촬영 후 정중앙(Median)에 위치한 표지자에 윤곽 묘사(Contouring)를 하였다. OBI(On Board Imager)가 장착된 Truebeam STxTM를 이용해 방사선조사 구간인 Gating window를 Lower threshold는 2.0 mm로 모든 측정 조건에서 고정시키고, Upper threshold를 최고 위상으로부터 각각 1.0 mm, 1.5 mm, 2.0 mm, 2.5 mm, 3.0 mm로 바꿔가며 측정하였다. 위와 같은 조건에서 평균 호흡 속도인 20 bpm을 기준으로 10 bpm, 30 bpm, 40 bpm, 50 bpm, 60 bpm 호흡속도를 바꿔가며 방사선이 끊기는 순간인 'Once at beam off'로 5회 촬영하였다. 같은 방법으로 3일간 반복 촬영 후 각 속도 별 오차율을 비교하였다. 결 과 : 기준 호흡 속도 20 bpm에서 최고 위상으로부터 각각 1.0 mm, 1.5 mm, 2.0 mm, 2.5 mm, 3.0 mm Upper threshold에서 Trigger mode의 beam off시 차이는 3일간의 평균값으로 0.68±0.05 mm, 0.91±0.03 mm, 1.23±0.03 mm, 1.42±0.04 mm, 1.66±0.06 mm이다. 기준 호흡 속도(20 bpm)대비 호흡 속도 변화에 따른 측정 결과는 최대 절대차이(Absolute Difference)의 경우 1일차, 2일차, 3일차 모두 3 mm Upper threshold에서 평균 0.81±0.08 mm로 차이가 확인되었다. 호흡 속도와 절대차이의 편차(Variation)에 대한 상관관계를 평가하기 위한 결정계수 R2는 3일 평균 수치로 각각 0.838, 0.887, 0.770, 0.850, 0.906로 확인되었다. 3일간의 Threshold 모든 변수에서 p-value는 설정 유의수준 0.05 이하로 차이유의를 확인하였다. 결 론 : 호흡동조방사선치료 시 Trigger mode를 이용하여 영상유도를 할 경우 기준 호흡 속도(20 bpm)에서의 Trigger mode의 오차율이 평균 ±0.04 mm 값으로 정확성과 유용성을 확인 할 수 있었다. 그러나 호흡 속도에 따른 부정확성(Uncertainty) 또한 발생할 수 있다는 것을 알 수 있었으며, 특히, 기준 호흡 속도 대비 느려지는 경우(< 20 bpm)보다 빨라지는 경우(> 20 bpm) 영상획득에 대한 부정확성은 커졌다. 따라서 사전 모의치료시의 호흡을 선별하고 호흡을 유지하기 위한 호흡교육과 치료 중 적극적인 실시간 모니터링(Monitoring)이 필요하다고 사료된다.

금강하구언 담수방류와 춘계 식물플랑크톤 군집의 단주기 변동 (Semiweekly variation of Spring Phytoplankton Community in Relation to the Freshwater Discharges from Keum River Estuarine Weir, Korea)

  • 1988년에 완공하여 1994년 8월부터 수문을 작동한 금강 하구언으로 인해 금강 하구역 환경은 크게 변하였다. 수문을 통해 담수를 불규칙하게 방류하면, 극심한 염분 저하와 함께 수중 무기영양염 농도가 급변한다. 이와 같이 환경 스트레스가 큰 금강 하구 내측에서 춘계 식물플랑크톤 군집의 단주기 변동을 연구하기 위하여, 2004년 2월 중순부터 4개월 동안 2개의 고정점에서 약 2일 간격으로 고조 때에 현장조사와 시료채취를 실시하였다. 하구언에 근접한 정점에서는 정점2(도선장 부근 정점)에 비해 염분 측정치의 변동계수가 2배에 이르러, 정점1에서 시간에 따른 염분의 변동이 극심함을 반증하였다. 용존 무기영양염 가운데, 질소계 영양염($NO_3^-,\;NO_2^-$$NH_4^+$)의 농도가 정점2에 비해 정점1에서 뚜렷이 높게 나타나, 수문 작동에 따라 그 농도가 부정기적으로 급변할 수 있는 환경임을 확인하였다. 식물플랑크톤 군집을 구성하는 주요 분류군은 규조류로서 출현종의 수나 생물량 모두 최고치를 기록하였다. 이 가운데 단독성 중심 규조 Cyolotella meneghiniana와 사슬형 중심 규조 Skeletonema costatum은 조사기간 전반부에 차례대로 5-6주 동안씩(천이 구간 I 및 II)절대 우점하여, 수온 $10^{\circ}C$의 시점을 중심으로 뚜렷한 천이현상을 보여 주었다. 천이구간 III(4월 중순-5월 말)은 대량 방류된 담수와 함께 하구역에 유입된 담수 녹조종인 남세균 Aphanizomenon flos-aquae와 Phormidium sp. 등이 매우 높은 구성비를 나타낸 시기이다. 이 기간에는 극저염-고염의 극심한 변동으로 인하여 낮은 수준의 생물량을 기록한 규조 Cyclotella meneghiniana와 Skeletonema costatum 외에는 담수종들이 전적으로 우점하였다. 천이구간 IV에서는 수온이 $18^{\circ}C$ 이상으로 상승하면서, 규조 Guinardia delicatuia가 단일 시료 내 최고 우점도 $75^{\circ}C$를 기록하며 약 일주일간 우점하였다. 즉, 조사기간 중에는 수온 $18^{\circ}C$ 미만의 극심한 염분 변동기(천이구간 III)를 제외한, 전 기간 중에 규조가 절대 우점하며 춘계 식물플랑크톤 우점종의 천이를 주도하였다. 천이구간 III에서도, 만약 담수방류의 규모와 빈도를 적절히 조절하였더라면, 주요 우점종이 담수 남세균이 아닌 하구성 규조로 바뀔 수도 있었을 것이다. 본 연구에 적용한 격일간 현장조사는 불규칙한 담수방류가 불가피한 반일주조 하의 하구역 환경에서 식물플랑크톤 종의 천이현상을 규명하기 위한 최소한의 필요조건으로 판단된다.

조선왕조(朝鮮王朝) 왕릉(王陵) 문인석상(文人石像)의 복식형태(服飾形態)에 관한 연구 (A Study on the Costume Style of Civil Servants' Stone Images Erected at Tombs of the Kings for Yi-dynasty)

  • A costume reveals the social characteristics of the era in which it is worn, thus we can say that the history of change of the costume is the history of change of the living culture of the era. Since the Three States era, the costume structure of this country had been affected by the costume system of the China's historical dynasties in the form of the grant therefrom because of geographical conditions, which affection was conspicuous for the bureaucrat class, particularly including but not limited to the Kings' familities. Such a grant of the costume for the bureaucrat class (i.e., official uniform) was first given by the Dang-dynasty at the age of Queen Jinduck, the 28th of the Shilla-dynasty. Since then, the costume for the bureaucrats had consecutively been affected as the ages had gone from the unified Shilla, to the Koryo and to the Yi-dynasty. As the full costumes officially used by government officials (generally called "Baek Gwan") in the Yidynasty, there existed Jo-bok, Gong-bok and Sang-bok. Of such official costumes, Gong-bok was worn at the time of conducting official affairs of the dynasty, making a respectful visit for the expression of thanks or meeting diplomatic missions of foreign countries. It appears no study was made yet with regard to the Gong-bok while the studies on the Jo-bok and the Sangbok were made. Therefore, this article is, by rendering a study and research on the styles of costumes of civil servants' stone images erected at the Kings' tombs of the Yi-dynasty, to help the persons concerned understand the Gong-bok, one of the official costume for Baek Kwan of that age and further purports to specifically identify the styles and changes of the Gong-bok, worn by Baek Gwan during the Yi-dynasty, consisting of the Bok-doo (a hat, four angled and two storied with flat top), Po (gown), Dae (belt), and Hol (small and thin plate which was officially held by the government officials in hand, showing the courtesy to and writing brief memorandums before the King) and Hwa (shoes). For that purpose, I investigated by actually visiting the tombs of the Kings of the Yi-dynasty including the Geonwon-neung, the tomb of the first King Tae-jo and the You-neung, the tomb of the 27th King Soon-jong as well as the tombs of the lawful wives and concubines of various Kings, totalling 29 tombs and made reference to relevant books and records. Pursuant. to this study, of the 29 Kings' tombs the costume styles of civil servants' stone images erected at the 26 Kings' tombs are those of Gong-bok for Baek-gwan of the Yi-dynasty wearing Bok-doo as a hat and Ban-ryeong or Dan-ryenog Po as a gown with Dae, holding Hol in hand and wearing shoes. Other than those of the 26 tombs, the costume styles of the Ryu-neung, the tomb of the Moon-jo who was the first son of 23rd King Soon-jo and given the King's title after he died and of the You-neung, the tomb of the 27th King Soon-jong are those of Jobok with Yang-gwan (a sort of hat having stripes erected, which is different from the Bok-doo), and that of the Hong-neung, the tomb of the 26th King Go-jong shows an exceptional one wearing Yang-gwan and Ban-ryeong Po ; these costume styles other than Gongbok remain as the subject for further study. Gong-bok which is the costume style of civil servants' stone images of most of the Kings' tombs had not been changed in its basic structure for about 500 years of the Yi-dynasty and Koryo categorized by the class of officials pursuant to the color of Po and materials of Dae and Hol. Summary of this costume style follows: (1) Gwan-mo (hat). The Gwan-mo style of civil servants' stone images of the 26 Kings' tombs, other than Ryu-neung, Hong-neung and You-neung which have Yang-gwan, out of the 29 Kings' tombs of the Yi-dynasty reveals the Bok-doo with four angled top, having fore-part and back-part divided. Back part of the Bok-doo is double the fore-part in height. The expression of the Gak (wings of the Bokdoo) varies: the Gyo-gak Bok-doo in that the Gaks, roundly arisen to the direction of the top, are clossed each other (tombs of the Kings Tae-jong), the downward style Jeon-gak Bok-doo in that soft Gaks are hanged on the shoulders (tombs of the Kings Joong-jong and Seong-jong) and another types of Jeon-gak Bok-doo having Gaks which arearisen steeply or roundly to the direction of top and the end of which are treated in a rounded or straight line form. At the lower edge one protrusive line distinctly reveals. Exceptionally, there reveals 11 Yang-gwan (gwan having 11 stripes erected) at the Ryu-neung of the King Moon-jo, 9 Yang-gwan at the Hong-neung of the King Go-jong and 11 Yang-gwan at the You-neung of the King Soon-jong; noting that the Yang-gwan of Baek Kwan, granted by the Myeong-dynasty of the China during the Yi-dynasty, was in the shape of 5 Yang-gwan for the first Poom (class) based on the principle of "Yideung Chaegang" (gradual degrading for secondary level), the above-mentioned Yang-gwans are very contrary to the principle and I do not touch such issue in this study, leaving for further study. (2) Po (gown). (a) Git (collar). Collar style of Po was the Ban-ryeong (round collar) having small neck-line in the early stage and was changed to the Dan-ryeong (round collar having deep neck-line) in the middle of the: dynasty. In the Dan-ryeong style of the middle era (shown at the tomb of the King Young-jo); a, thin line such as bias is shown around the internal side edge and the width of collar became wide a little. It is particularly noted that the Ryu-neung established in the middle stage and the You-neung in the later stage show civil servants in Jo-bok with the the Jikryeong (straight collar) Po and in case of the Hong-neung, the Hong-neung, the tomb of the King Go-jong, civil servants, although they wear Yang-gwan, are in the Ban-ryeong Po with Hoo-soo (back embroidery) and Dae and wear shoes as used in the Jo-bok style. As I could not make clear the theoretical basis of why the civil servants' costume styles revealed, at these tombs of the Kings are different from those of other tombs, I left this issue for further study. It is also noted that all the civil servants' stone images show the shape of triangled collar which is revealed over the Godae-git of Po. This triangled collar, I believe, would be the collar of the Cheomri which was worn in the middle of the Po and the underwear, (b) Sleeve. The sleeve was in the Gwan-soo (wide sleeve) style. having the width of over 100 centimeter from the early stage to the later stage arid in the Doo-ri sleeve style having the edge slightly rounded and we can recognize that it was the long sleeve in view of block fold shaped protrusive line, expressed on the arms. At the age of the King Young-jo, the sleeve-end became slightly narrow and as a result, the lower line of the sleeve were shaped curved. We can see another shape of narrow sleeve inside the wide sleeve-end, which should be the sleeve of the Cheom-ri worn under the Gong-bok. (c) Moo. The Moo revealed on the Po of civil servants' stone images at the age of the King Sook-jong' coming to the middle era. Initially the top of the Moo was expressed flat but the Moo was gradually changed to the triangled shape with the acute top. In certain cases, top or lower part of the Moo are not reveald because of wear and tear. (d) Yeomim. Yeomim (folding) of the Po was first expressed on civil servants' stone images of the Won-neung, the tomb of the King Young-jo and we can seemore delicate expression of the Yeomim and Goreum (stripe folding and fixing the lapel of the Po) at the tomb of the Jeongseong-wanghoo, the wife of the King Young-jo, At the age of the King Soon-jo, we can see the shape of Goreum similar to a string rather than the Goreum and the upper part of the Goreum which fixes Yeomim was expressed on the right sleeve. (3) Dae. Dae fixed on the Po was placed half of the length of Po from the shoulders in the early stage. Thereafter, at the age of the King Hyeon-jong it was shown on the slightly upper part. placed around one third of the length of Po. With regard to the design of Dae, all the civil servants' stone images of the Kings' tombs other than those of the Geonwon-neung of the King Tae-jo show single or double protrusive line expressed at the edge of Dae and in the middle of such lines, cloud pattern, dangcho (a grass) pattern, chrysanthemum pattern or other various types of flowery patterns were designed. Remaining portion of the waist Dae was hanged up on the back, which was initially expressed as directed from the left to the right but thereafter expressed. without orderly fashion,. to the direction of the left from the right and vice versa, Dae was in the shape of Yaja Dae.
