119구급대원의 응급구급활동과 관련한 형법적 책임 (A Criminal Responsibility of Aid by 119 Rescuer)

  • 119구급대원의 응급구급활동과 관련하여 형사법적으로 문제되는 영역은 크게 세 가지이다. 첫째는 구급 요청을 거절하는 행위이며, 둘째는 요구호자를 이송하지 않는 미이송 행위이고, 셋째는 응급처치행위이다. 이러한 일련의 구급대원의 행위들이 '응급의료에 관한 법률'이나 관련 법규의 규정에 위반되어 이루어진 경우에는 이들 법률들에 규정된 범죄행위가 성립한다. 특히 정당한 이유 없이 구급요청을 거절한 행위는 응급의료법상 응급의료거부죄가 성립하고, 적법한 절차에 의하지 아니한 응급처치는 무단 응급처치죄가 성립한다. 또한 구급요청거절과 미이송 행위 그리고 응급처치나 미응급처치 등으로 인해 요구호자가 사망하거나 상태가 악화된 경우에는 형법상 살인죄나 업무상과실치사죄 또는 중상해죄가 성립할 수 있다. 이처럼 구급대원의 경우에는 구급활동과 관련하여 형사적 책임으로부터 자유로울 수가 없다. 즉 구급대원들도 직무의 소홀과 과실로 인해 발생하는 법익침해적 결과에 대해서는 당연히 형사적 책임을 져야 한다. 그러나 긴급상황에서 불가피하게 국민의 생명과 건강을 침해한 경우에는 이들을 보호할 수 있는 법적 방안도 마련되어야 하며, 특히 열악한 근무환경에서 구급대원에게만 그 책임을 전가할 수 없는 게 현실이다. 현재 구호자보호법 제정문제가 논의되고 있는데, 이 법률에 구급대원의 형사면책부분이 합리적으로 규정되어 궁극적으로 구급대원들이 국민의 생명과 건강을 지키는 파수꾼으로서 그 역할에 충실할 수 있도록 하여야 한다.

A Study on the Restorement Meaning In Life of First aider in the Disaster Activities

  • Many first aiders suffer from the stress of working in uncertain disaster sites. The stress of responding to disasters leads to an increase in post-traumatic stress disorder (PTSD) and a decrease in psychological well-being. It also has many negative effects on the meaning in life for first aiders. Reduced meaning in life will result in many societal opportunity costs due to decreased health and work engagement. To address this issue, this study conducted a survey to analyze the level of meaning in life of first aiders. The results indicated that the overall level of meaning in life of first aiders is very low. This issue needs to be addressed sooner rather than later, given the increasing trend of emergency response. Based on the results of the study, measures were proposed to reduce PTSD and psychological stability by restoring the meaningfulness of first aiders' lives.

심폐소생술 구조자의 무릎 높이 정도가 흉부압박의 질에 미치는 효과 (Effects of Knee Height of CPR Rescuer on the Quality of Chest Compression)

  • 본 연구는 심폐소생술 시행 시 구조자의 무릎이 바닥으로부터 10 cm 높은 위치와 바닥 위치 간에서 흉부압박의 질에 차이가 있는지를 알아보고자 시행하였다. G광역시의 G대학 응급구조과 1학년 재학생으로 심폐소생술 교육 과정을 이수한 66명 중, 무작위추출과정을 통해 2011년 11월 8일부터 9일까지 실험군 31명, 대조군 32명으로 선정하였다. 바닥으로부터 10 cm 위치(재질: B4 Copy Paper)와 바닥 위치에는 공통으로 매트리스(재질: PVC, 사이즈: $185{\times}125{\times}0.65cm$)를 깔고 2분 동안 흉부압박 만을 시행하였다. 실험 처치는 Resusci Anne 마네킨 1대를 사용하였으며, Laerdal PC SkillReporting System으로 시행결과를 기록하였다. 수집된 자료는 SPSS 14.0 for Window으로 $x^2$-test와 Fisher's exact probability test, Mann-Whitney U-test, Wilcoxon signed rank test를 사용하였다. 연구결과, 심폐소생술 구조자의 무릎이 바닥으로부터 10 cm 높은 위치가 바닥 위치보다 흉부압박 적절한 깊이(회), 평균 흉부압박 깊이(mm)가 더 효과적이었는데, 신장 170 cm 이하 그룹, 몸무게 65 kg 이하 그룹에서 흉부압박의 적절한 깊이(회), 평균 흉부압박 깊이(mm)가 효과적 이었다.

대형화재에 대비한 응급의료체계에 관한 분석과 개선방안 (A Study on the Analysis and Improvement methods of Emergency Medical Service Systems for Large Scaled Fire)

  • In Korea, Crowds, as well as more complex and larger structures, have been caused more victims in the event of fire. In that situation, EMSS should have done triage the patients according to their severity in the field, treat with standard guidelines as like advanced burn life support and transfer to appropriate facility. But in many cases, they didn't practice like that. The purpose of this study is to give basic data for proper emergency medical services by analysing EMSS of large scaled fire in Korea and suggesting some improvement methods. The improvment methods are like this ; First, developing of protocols about burn patient is needed for EMT and dispatcher. Second, Legal approval for advanced treatment is needed for EMT. Third, Fire-helicoptors are needed in Daejeon and Jeju. And Emergency Medical Helicoptors are needed in EMSS. Forth, more advanced emergency medical centers of burn must be established. Sixth, more pocket-mask and burn sheet are needed for 119 rescuer. Finally, building owners must change inflammable materials as non-inflammable materials and educate fire-prevention and first-aid to employees.

119 구급대의 후송활동 및 개선방안 (Transport Activities and Improvement strategies of 119 Rescue)

  • This study was performed to investigate transport activities and strategize improvement of 119 rescue. We reviewed emergency care records of users who were transported by 119 rescue of six agencies in Chungnam from July 13, 1998 to August 8, 1998. The results were as follows ; 1. In sex distribution of users, the male was 65.0%. And the highest age group among users was above sixties(21.6%), then thirties in second order(19.3%). Accident was 50.8% as occupied first cause of transport, and then acute disease 22.8%. The highest requester for 119 rescue call was patient's families(47.1%) and average number of 119 rescue users per day was 20.9. 2. The nonurgent state of users was 58.9%. The frequency of users was 26.0 persons at sunday in most frequently, weekend and holiday was more common than ordinary day, and most frequent weather state was cloudy(23.8 persons). 3. Total running distance of 119 ambulance was 7.0km in average. Call time by users was 20-24 hours most commonly(21.9%). In then running time by each transport stage, 8 minutes were taken from 119 call receipt to scene arrival, 13 minutes from scene arrival to hospital. The kinds of pre-hospital care by 119 rescuer was vital sign check(81.2%), wound dressing or fracture fixation(41.2%), airway maintenance and O2 supply(30.4%).

기본 소생술 e-러닝(e-Learning) 멀티미디어 컨텐츠 개발 연구 (A study on e-Learning multimedia contents develop focus basic resuscitation)

  • This study was designed to develop e-Learning multimedia contents to provide the first aid rescuer with the basic resuscitation, members of the public, corporations and any other institutions who wish to learn this skills. It is carried out from October to December in 2001. This program was constructed on the basic of the network-instructional system Design model, this model has severalprogressive steps, which includes the planning, analysis of the contents, development of the contents, instructional design, development of web-based and multimedia, pilot test, implement, and evaluation. The URL of this site is http://www.cyber.hc.ac.kr named as the cyber education program for the basic resuscitatingskill. This contents consisted of 8 chapters providing as follows : Introduction, anatomy and physiology of heart-ling, Adult Cardio-Pulmonary Resuscitation, Adult Foregin-body Air-Obstruction, Child Cardio-Pulmonary Resuscitation, Child Foregin-body Air-Obstruction, Infant Cardio-Pulmonary Resuscitation, Infant Foregin-body Air-Obstruction. To make the learning more interesting, as much animation and videos were integrated. In conclusion, this e-Learning multimedia contents will be useful for student as well as members of the public. It significantly increases the chances of saving the life of person who has collapsed.

119 구급대원 구급활동의 근골격계 증상 및 위험실태 (A Survey of Musculoskeletal Symptoms & Risk Factors for the 119 Emergency Medical Services (EMS) Activities)

  • Fire service personnel and ambulance paramedics suffer musculoskeletal disorders as they lift and carry patients while performing Emergency Medical Services (EMS). The objective of the current study was performed to examine the association between working environment and musculoskeletal disorders of 119 paramedics and to analysis the EMS activities for them through basic survey (including task characteristics, risk factors, symptoms and illnesses). Observational job analysis of EMS activities indicated the squatting posture during first-aid performed on floor and the abrupt use of force during carrying heavy load including stretcher with patients on as hazard factors, and excessive low back twisting and bending during stairway transfer was observed. In addition, work-physiological assessment revealed various but rather high lumbar muscle usage rate among the study subjects, being 14.6~32.8% compared with Maximum Voluntary Contraction (MVC) during patients transfer work. Resting heart rate showed 65/min, on the other hand, heart rate on mobilization indicated maximum 124~156/min. Therefore, the results of analysis to the EMS activities, rescuer activities and medical tasks were accompanied with high possibility of accident and musculoskeletal disorders. Also, EMS activities indicated high muscle fatigue and energy consumption, and accumulated muscle fatigue with during continued work.

홍수의 재해 초기 구호활동에서 효율적 자원분배를 위한 의사결정 (Decision Mking for Efficient Resource Allocation in Initial Disaster of Flood)

  • This study is for decision making on distribution of resources so as to improve the effectiveness of initial disaster relief efforts. It is very important that relief efforts should be accomplished appropriately at the initial disaster. Furthermore, efficient allocation of relief resources such as rescuer, shelter, relief goods, relief funds, medical and relief equipments is also the first step to achieve main objective of relief efforts when disaster occurs. For this purpose, this study establishes flood as a imaginery disaster and develops a model for efficient distribution of resources when flood outbreaks. This model fixes initial 72 hours, which is subdivided into three intervals, as a initial disaster range. The model is to set a prioity against alloction of relief resources by each time zone which is related to damaged degree( Red Tag, Yellow Tag, Green Tag). Experts in this field input their experience into this model, and these are analyzed by Analytic Hierachy Process(AHP)/Expert Choice(EC) software. Therefore, we can decide a prioity against distribution of resources by each time zone which is in accordance with damaged degree. The result of this study would be helpful to a person who is in charge of relief from calamity in order him to make a decision toward distribution of resources.

IoT notification system for marine emergencies

  • Minimization of human casualties in disaster situations is of paramount importance. In particular, if a marine disaster occurs, it can be directly connected to human casualties, so prompt action is needed. In the event of a marine disaster, the route and location of movement should be identified and life tubes should be used to float on the water. This paper designs and proposes an emergency IoT notification system that can quickly rescue drowning people. The maritime emergency IoT notification system consists of four main types. First, an emergency IoT device that detects the expansion of the life tube and delivers location and situation information to the emergency IoT notification server. Second, an emergency IoT web server that manages emergency information and provides notification. Third, a database server that stores and manages emergency IoT notification information. And finally, an emergency notification app that can receive and respond to emergency notification information. The emergency IoT device consists of a TPMS(Tube Pressure Monitoring System) device that checks the pressure value of the TPMS in real time and sends it to the IoT device, and an IoT device that sends the rescuer's voice information and emergency information to the emergency IoT server. Emergency information is delivered using the MQTT(Message Queuing Telemetry Transport) protocol, and voice information is delivered to the IoT server as HTTP FormData.

Chest compression quality, exercise intensity, and energy expenditure during cardiopulmonary resuscitation using compression-to-ventilation ratios of 15:1 or 30:2 or chest compression only: a randomized, crossover manikin study

  • Objective Our aim was to compare the compression quality, exercise intensity, and energy expenditure in 5-minute single-rescuer cardiopulmonary resuscitation (CPR) using 15:1 or 30:2 compression-to-ventilation (C:V) ratios or chest compression only (CCO). Methods This was a randomized, crossover manikin study. Medical students were randomized to perform either type of CPR and do the others with intervals of at least 1 day. We measured compression quality, ratings of perceived exertion (RPE) score, heart rate, maximal oxygen uptake, and energy expenditure during CPR. Results Forty-seven students were recruited. Mean compression rates did not differ between the 3 groups. However, the mean percentage of adequate compressions in the CCO group was significantly lower than that of the 15:1 or 30:2 group ($31.2{\pm}30.3%$ vs. $55.1{\pm}37.5%$ vs. $54.0{\pm}36.9%$, respectively; P<0.001) and the difference occurred within the first minute. The RPE score in each minute and heart rate change in the CCO group was significantly higher than those of the C:V ratio groups. There was no significant difference in maximal oxygen uptake between the 3 groups. Energy expenditure in the CCO group was relatively lower than that of the 2 C:V ratio groups. Conclusion CPR using a 15:1 C:V ratio may provide a compression quality and exercise intensity comparable to those obtained using a 30:2 C:V ratio. An earlier decrease in compression quality and increase in RPE and heart rate could be produced by CCO CPR compared with 15:1 or 30:2 C:V ratios with relatively lower oxygen uptake and energy expenditure.
