Duplication of the Esophagus -Report of A Case- (식도중복증의 1례)

  • Duplication of the alimentary tract, especially of the esophagus, have been regarded as rare cong- enital anomalies. However, they are being reported with increasing frequency in the literature. In the . ,- past they have been described by a variety of names, such as "enteric cysts", "intestinal cysts", "giant diverticula", "`mediastinal cysts of foregut origin" "enterogenous cysts" and other descriptive terms. . Most authorities now agree that these anomalies are best described by the term "duplications of the alimentary tract. The duplications [of the alimentary tract] are spherical or tubular structures which poses a well developed smooth muscle layer and are lined with a mucous membrane from any part of the alimentary tract. They may occur at any place in the digestive tube from the tongue to the rectum and usually are intimately attached to some portion of the alimentary tube. We have experienced a duplication of the esophagus in 14 years old middle schoolboy. He complained dysphagia, eructation and substernal pain associated with intermittent high fever and chilliness, increasing in severity for recent three weeks. Routine chest X-ray film revealed nore markable abnormal finding but esophagogram. revealed marked narrowing of the esophagus throughout with a large blind pouch in lower half with fistulous communication at mid portion of the esophagus. On thoracotomy, a large infected blind pouch communicating with the lumen of normal esophagus proximally, Was extended from the level of 5th to 10th thoracic spine. The duplicated segment of the esophagus has a common muscular .wall and proximal communication with the adjacent esophagus. The infected, duplicated esophagus was segmentally resected, and esophagogastrostomy with pyloroplasty was done by displacing the stomach into the right thoracic cavity through midline laparotomy. His Postoperative course was uneventful and discharged without complication.

The benefit analysis of constructing the visual conservation center (영상자료센터 건립사업의 경제적 편익)

  • Visual conservation center is a facility for the preservation of visual materials such as film and digital image data, and its construction project is currently being considered. This study evaluates the economic benefit of the project using CVM (Contingent Valuation Method), the main method of nonmarket evaluation in the both domestic and foreign literature. Survey data used in the analysis was collected in seven major metropolitan cities using person-to-person interviews and a logit model is used for the econometric estimation. The economic benefit measured by a household's average willing to pay (WTP) per year is shown to amount to 8,958 won.

Development of Tablet PC-Based Multimedia Educational Contents for Patients and Their Family Undergoing Thoracic Surgery (흉부수술 환자와 가족을 위한 태블릿 PC기반 멀티미디어 교육자료 개발)

  • Purpose: This study aims to develop Tablet PC-based multimedia educational contents on the procedures from surgery preparation to recovery for hospitalized patients who are going to undergo thoracic surgery and their families. Methods: The educational contents were created based on literature review and a survey of needs in patients and their families who were admitted to a hospital in Seoul from Dec. 2013 to May 2014. The contents were developed in conjunction with a film production company. The educational materials including texts, images, sounds, and videos were developed appling the ADDIE (analysis, design, development, implementation, and evaluation) model. Results: Two multimedia educational contents were developed for the patients with coronary artery bypass graft or lung surgery. Each content consisted of six subdivisions of self-learning for 20 minutes. The developed educational contents were completed after testing content validity by 30 medical experts. Conclusion: This study recommends that the Tablet PC-based educational contents for thoracic and cardiovascular patients and their families be used in general wards and intensive care units to relieve their anxiety before surgery and reduce postoperative complications.

Patent ductus arteriosus associated with cardiovascular anomalies and severe pulmonary hypertension: Preoperative hemodynamics and surgical observation in 51 patients. (개방성동맥관의 술전혈역학적상태와 외과적치료에 대하여)

  • During the years 1959 to 1974, 99 patients with patent ductus arteriosus were admitted to National University Hospital. These includes 5 patients with additional cardiovascular and 5 patients with severe pulmonary hypertension. All were operated upon except three refused operation. In all instances, the diagnosis was made by history and physical, roentgenological and electrographic examinations. In addition, in 53 patients, special diagnostic procedures were carried out either for diagnosis or for evaluation of pulmonary hypertension and associated cardiovascular anomalies. Right cardiac catheterization was resorted to in 51 patients. In one of these patients catheterization was incorrectly interpreted [ventricular septal defect]. Retrograde aortogram was performed in two patients. In both cases the ductus itself was visualized on the x-ray film. An additional vascular anomaly, namely the persistent left superior vena cava, was confirmed by retrograde angiogram in one of them. In 5 cases the pulmonary arterial pressure was elevated well over 80 mmHg. In these instances,the operative mortality was 80% [4 out of 5 patients]. The management of patent ductus arteriosus when associated with severe pulmonary hypertension. and/or other cardiac anomalies is controversial. Opinions differ as to how to close the ductus and to repair the cardiac anomalies as well as to whether a one-staged or two-staged procedure should be resorted to. The author is of the that each case must be evaluated individually before any specific surgical treatment is ou.tlined. The literature on the subject is reviewed in this paper.

Fluid Simulation Control for Effective VFX Underwater Explosion Effects (효과적인 VFX 수중 폭발효과 구현을 위한 유체 시뮬레이션 제어)

  • The explosion effect of computer graphics Visual Effects(VFX) used in films and animations is an important element that determines the completeness of the film, and its usage is getting extended. The realistic explosion effect of VFX should be made according to observations and analysis of various factors of actual explosion in real world. This experimental research would suggest the efficient production guideline for the technical characteristics of underwater explosion of VFX. For this research process, first, the comparison of actual explosion and VFX explosion effect, classification of actual explosion, and characteristics of underwater explosion effect will be addressed. Second, based on the literature reviews, the four steps of experimental production analysis tool will be derived. Third, the experimental research will be processed in along with technical factors four steps of the underwater explosion effect, (1)realistic creation and emission of fluid, (2)fluid expansion control by water pressure, (3)bubble effect, and (4)motion of bubble & dissipation of fluid. The effective method of fluid simulation production will be verified through experimental studies based on the characteristics of the actual explosion process. This experimental study suggested the VFX production technique is expected to be used as the basic data for related research field.

Analysis of Tilting Pad Journal Bearings Considering Pivot Stiffness (피봇 강성을 고려한 틸팅 패드 저널 베어링의 해석)

  • This study set out to predict the load capacity and rotordynamic coefficients of tilting-pad journal bearings, taking the pivot stiffness into account. The analysis uses rocker-back (cylindrical) and ball in socket (spherical) pivot models, both of which are based on Hertzian contact stress theory. The models ascertain the non-linear elastic deformation of the pivots according to the applied load, pivot geometry, and material properties. At present, the Reynolds equation for an isothermal, isoviscous, and incompressible fluid is used to calculate the film pressure by using the finite-element method, after which the Newton-Raphson method is used to simultaneously find the journal center location, pad angles, and pivot deflections. The bearing analysis, excluding the pivot models, is validated using predictions those are readily available in the literature. As the rotor speed increases, the predicted journal eccentricity and damping coefficients decrease, but the stiffness coefficients increase, as expected. Most importantly, the implementation of the pivot models increases the journal eccentricity but significantly decreases the stiffness and damping coefficients of the tilting-pad journal bearings.

  • Viewing the larynx as a part of a thorough physical examination has been universally recommended but yet largely ignored. To see the larynx in its hidden anatomic position, it has been necessary to use an angled laryngeal mirror and an indirect light source, a technique requiring training, artistry and patience. We explored the concept of using a suitable right angle endoscopic laryngoscope as a tool in the out patient department, to replace the unused mirror, in a series of trials during 5 years. The whole procedure takes approximately one minute. It is simple, requires no anesthesia and the larynx and its surrounding structures clearly into view. We were able to record by Camera film and compare with before and after treatment. So we report them with literature review.

Analysis of Gender Identity Expressed in the Movie based on Judith Butler's Gender Theory

  • is based on a true story of the first transgender individual. It portrays psychological changes visually during the protagonist's confusion with self-identity based on gender. This study analyzes gender identity in contemporary fashion intensively based on gender images and costumes appearing in the film . In the society lacking a fixed gender image, this study provides a timely insight into gender identities by analyzing the fashions depicted in the movie. The movie is a true story of the first transgender person working hard to determine his or her own gender identity. As a research method, the theoretical basis of genderless approach was established via literature review. The characteristics of genderless identity were determined by dividing the movie into established and ambiguous gender periods to analyze the comprehensive changes in costumes for comparison. Einer Wagner representing male identity portrays men's fashion whereas Lily Elbe representing female identity depicts women's fashion. While the two different genders find their places in a single body, the confusion creates genderless fashion. By dividing these phases into femininity, masculinity and genderless categories, each costume was analyzed comprehensively, and the images of relatively changing fashion were studied by altering the gender identity. Four characteristics including androgyne, rebellion, pleasure and balance were derived from the gender identity based on Fashion in .

A Case of Endobronchial Aspergilloma (기관지내 아스페르길루스종 1예)

  • Pulmonary aspergillosis presents as the following three different types depending on the immune status of the host: invasive aspergillosis, allergic bronchopulmonary aspergillosis (ABPA), and aspergilloma. Aspergilloma develops as a result of an aspergillus growth inside a pre-existing lung cavity. However, endobronchial aspergilloma without a lung parenchymal lesion is quite rare. We encountered a case of endobronchial aspergilloma that developed in a healthy 75 year-old woman that led to necrotizing pneumonia of the right lower lobe. The chief complaints were fever, cough and yellowish sputum. The chest film revealed haziness with cavity-like shadows on the right lower lobe, and the chest CT scan showed endobronchial calcified density in the basal bronchus of the right lower lobe with peribronchial lymph node enlargement. Bronchoscopy revealed an obstruction of the basal orifice of the right lower lobe by blackish stone-like material, and the aspergilloma was confirmed by the bronchoscopic biopsy. The pneumonia improved after bronchoscopic removal of this lesion. We report this case along with a review of the relevant literature.

A Study on Typography Style by Deconstructionism (해체주의(Deconstructionism) 경향의 타이포그래피 스타일에 관한 연구)

  • It has been known that deconstruction has affected various fields as literature, architecture, an, film, industrial design, and so on. Especially, deconstruction has applied into a wide scope of the design related fields through diverse ways and the results also show that there is a great potentiality in the ways of it's expression with different styles. Deconstruction design has been approached in connection with post-modernism, deconstruction theory of J. Derrida and pluralistic post industrial society. In particular, it is obvious that deconstruction philosophy has deeply affected the fields of visual design and typography which are mostly communicated with symbols and texts. This study intends to examine how the deconstruction theory has been practically applied into the typography by analyzing the change of deconstruction typographic styles and exploring the distinctiveness of the change.
