Surgical Resection of Solitary Fibrous Tumor in the Anterior Mediastinum -Report of a Case- (종격동에 발생한 고립성 섬유종의 외과적 치험 1례)

  • A solitary fibrous tumor in the pleura has been reported occasionally, but extremely rare in themediastinum. The histogenesis of this tumor has been still in controverse, but recent studies paid attention to it a mesenchymal origin by the immunohistochemical and ultrastructural bases. A few cases, reported in the literature, suggest that the mediastinal solitary fibrous tumor occurs more commonly in adult and shows slightly higher incidence in women. Its aggressive behavior such as recurrence rate and distant metastasis, is more prominant than reported in solitary fibrous tumor of the pleura. No single histologic feature allows an assumption definite prognosis. Surgical resection of this tumor is usually curative although the recurrence or distant metastasis are reported in about half of the patients. This report is a case of solitary fibrous tumor in the mediastinum in a 16-year-old female patient.She underwent surgical resection and her postoperative result was satisfactory.

  • Solitary fibrous tumor is rare benign mesenchymal neoplasm. The spinal solitary fibrous tumor is extremely rare. The authors experienced a case of intramedullary solitary fibrous tumor of cervicothoracic spinal cord in a 48-year-old man with right lower extremity sensory disturbance. Spinal MRI showed intradural mass lesion in the level of C7-T1, the margin between the spinal cord and tumor was not clear on MRI. A Left unilateral laminectomy and mass removal was performed. Intra operative finding, the tumor boundary was unclear from spinal cord and it had intramedullary and extramedullary portion. After surgery, patient had good recovery and had uneventful prognosis. Follow up spinal MRI showed no recurrence of tumor.

  • 흉막의 고립성 섬유성 종양은 중피하에 존재하는 미분화 중배엽성 기원의 섬유 세포에서 기원하는 종양으로 알려졌다. 이 종양은 대부분 양성으로 보고되고 있으나 병리학적으로 높은 세포 밀도, 유사분열의 수가 많고(10개의 고배율 시야에서 4개 이상), 핵의 다양성, 출혈, 괴사등이 있으며 악성의 판단기준으로 보고된다. 환자는 62세 여자로 호흡곤란 및 기침을 주소로 본원에 입원하였으며 단순 흉부 방사선 및 전산화 단층 촬영상 우측 흉강내에 거대한 종양소견이 보였다. 이 종양은 23$\times$18$\times$12 cm, 2 kg의 크기와 무게를 가졌으며 우측폐 하엽과 중엽은 압박되어있었으나 종양 절개 후 재 팽창됨을 확인하였다. 또한 횡경막과 심하게 유착되어 박리도중 손상이 동반되어 봉합술이 필요하였다. 병리학적으로 종괴는 세포밀도가 높았고 방추형세포의 다발로 이루어져 있었으며 유사분열의 수가 많이(27 mitosis/10HPF)보였다. 면역 조직 화학 검사상 vimentin과 CD34에 양성 반응을 보였다. 이에 악성 고립성 섬유성 종양으로 확진되었다. 본 교실에서는 흉막에 발생한 악성 고립성 섬유성 종양을 치험하였기에 문헌 고찰과 함께 보고한다.

  • Myxoid solitary fibrous tumor (SFT) is very rare soft tissue neoplasm. It is microscopically composed of spindle cells which is individually separated by delicate band of collagen fibers. And this tumor cells are immunohisto-chemistrically highlighted by CD34. Myxoid SFT has indolent clinical course and a good prognosis, so it is important to make a diagnosis because of its morphological similarities to myxoid spindle cell neoplasms that have different prognoses and treatment. We report the case of a 20-year-old female with a myxoid SFT found in the left temporo-parietal scalp. This case report appears to be the first reported scalp occurrence of this rare tumor.

  • Solitary fibrous tumor is an uncommon submesothelial mesenchymal neoplasm that primarily arises from the pleura. Most solitary fibrous tumors have a benign course, and the single most important predictor of the clinical outcome is the ability to excise the entire lesion. We experienced a case of CSF leakage through a subarachnoid-pleural fistula after resection of a malignant solitary fibrous tumor and the involved rib. We detected CSF leakage via performing CT myelography and we treated this case with hemilaminectomy and dura repair.

  • A 44-year-old man was transferred to our department for mediastinal mass. He had suffered from only an easily fatiguable condition for 1 month. A physical examination and laboratory finding of the patient disclosed no abnormality. A chest radiograph showed a soft tissue tumor in the posterior mediastinum. It was well circumscribed and ovoid. Invasions to adjacent organs were not seen. Therefore it was though the 5th intercostal space. The tumor mass was attached to the visceral pleura of the right upper lobe by a pedicle and this pedunculated tumor laid entirely within the pleural cavity. Excision of the tumor which measured 7$\times$7$\times$3cm was done easily. Pathologic studies confirmed the diagnosis of localized fibrous tumor of the pleura. Localized fibrous tumor of the pleura is rare, This tumor along ith the evidence from ultrastructural and immunohistochemical studies has led most researchers to conclude that localized fibrous tumor is not of the mesothelial origin but arises in the submesothelial connective tissue.

  • Malignant fibrous histiocytoma (MFH) of the spermatic cord represents an uncommon location for the most common soft tissue tumor in adults. MFH of the spermatic cord is extremely rare. No case report has been described in the Korean literature. We report a case of malignant fibrous histiocytoma, myxoid type, arising from left spermatic cord. A 77-year-old male presented with a painless left upper scrotal mass for 5 months. Simple mass excision was performed for a diagnosis. Grossly, the mass closely contacted with the left spermatic cord. Since a myxoid type of malignant fibrous histiocytoma was diagnosed from histopathological findings, left radical orchiectomy with high ligation of the spermatic cord was performed additionally. There were no evidence of local recurrence or metastasis at 6 months after surgery.

  • Solitary fibrous tumor on the pleura is rare but should be included in the differential diagnosis on a peripheral pulmonary nodule. Cytologic features of solitary fibrous tumor of the pleura is not familar to the pathologist and may be misdiagnosed as malignancy. We report fine needle aspiration cytologic(FNAC) findings of a case of solitary fibrous tumor misdiagnosed as adenocarcinoma in a 48-year-old woman. The FNAC displayed a mixture of bland-looking spindle cells and clusters of epithelioid cells, which have hyperchromatic nuclei with prominent nucleoli. The helpful finding to distinguish It from other circumscribed benign and malignant lesions is the presence of fibromyxoid matrix admixed with blood vessels and thin collagen fibers. Familiarity with these features is essential to avoid misdiagnosis and overtreatment.

  • Malignant fibrous histiocytoma, a type of sarcoma, is a malignant neoplasm with uncertain origin that arises in both the soft tissues and the bone. The occurrence of primary malignant fibrous histiocytoma of the pleura is extremely rare. We report a case of a 65-year-old Korean man who is being diagnosed with primary malignant fibrous histiocytoma of the pleura.

  • A solitary fibrous tumor is a relatively uncommon neoplasm that usually occurs in the pleura but occurs extremely rarely in the oral cavity. Reported herein is a rare case of a solitary fibrous tumor in the buccal cheek mucosa. A 50-year-old man visited the authors' hospital due to a buccal cheek mass whose size had increased. Excisional biopsy was done under local anesthesia. After the excisional biopsy, the patient was diagnosed to have a solitary fibrous tumor. In immunohistochemistry, the patient's solitary fibrous tumor was characterized by the expression of CD34 and CD99 on the neoplastic cells, and negativity for Bcl-2 and S-100. No recurrence or complication occurred for a period of 5 years. The growth of a primary solitary fibrous tumor in the buccal cheek mucosa is extremely rare and has been rarely reported in the South Korean medical literature. A solitary fibrous tumor must be distinguished from other spindle cell tumors. Presented herein is a case of primary solitary fibrous tumor in the buccal cheek mucosa. The relevant literature is briefly reviewed.
