Meningioma in the Lateral Cerebellomedullary Cistern without Dural Attachment

  • A 59-year-old female presented with headache and dizziness for one year. Magnetic resonance imaging revealed a $52{\times}28$ mm, wellcircumscribed, homogenously enhancing mass lesion without dural attachment located in the left lateral cerebellomedullary cistern. The tumor was excised, and a histological diagnosis was a mixed pattern meningioma of meningothelial and fibroblastic type. A meningioma in the posterior fossa without dural attachment is quite rare. We report a rare case of lateral cerebellomedullary cistern meningioma without dural attachment with literature review.

  • Accidental or suicidal fatalities of paraquat(Gramoxon) poisong are occasionally seen in the emergency room or intensive care unit in this country. In most cases, respiratory symptoms and eventual death by respiratory distress occur within several days. The most striking pathologic change is fibrosis of the lung due to widespread proliferation of fibroblastic cell. We experience a 21-year-old woman with huge bulla on left lung and diffuse fibrosis in other site, who ingested paraquat 10 months ago. After thoracoscopic removal of bulla, the patient survive without progression of pulmonary complication till now.

하인두에 발생한 섬유종증 1예 (A Case of Fibromatosis of Hyphopharynx)

  • Fibromatosis is a broad group of benign fibroblastic proliferation that shows locally aggressive growth but never metastasize. Common anatomic sites include abdominal wall, extremity, and mesentery. Little is reported about clinical features and outcome of fibromatosis of the head and neck. The treatment of choice is wide excision, which is often difficult. Postoperative recurrence rates are high. We recently confirmed a unique case of fibromatosis occurred on the hypopharynx of 44-year-old male patient and report this interesting case with review of literature.

  • Desmoplastic small round cell tumor (DSRCT) is a rare malignant mesenchymal neoplasm. It mainly involves the abdominal or pelvic peritoneum of male adolescents. We report here the imprint cytologic features of a case of DSRCT occurring in the intraabdominal cavity of a 21-year-old man. A microscopic examination showed moderate cellularity. The tumor cells were singly arranged and arranged in clusters. The cells had round to oval nuclei with finely granular chromatin, inconspicuous nucleoli and scanty cytoplasm. Some tumor cells showed nuclear molding, and some cells had an epitheloid appearance with a large amount of lightly eosinophilic cytoplasm. A rosette-like pattern was present. Spindle-shaped, fibroblastic stromal cells were occasionally found. The tumor cells were immunoreactive for the markers cytokeratin (AE1/AE3), epithelial membrane antigen (EMA), desmin, vimentin and neuron specific enolase (NSE).

  • Normal cells proliferate generally a limited number doublings in culture and only rarely have they been shown to overcome cellular senescence and crisis stages, and immortalize spontaneously. I have established a number of non-chemically and non-chemically immortalized embryo fibroblastic (EF) cell lines in continuous cell culture. These include the spontaneously immortalized cell line, DF-1 and several immortal EF cell lines derived from various embryonic tissues. I have previously demonstrated that all of the immortal EF cells established have rapid cell proliferation capacity compared to primary EF cells, presumably due to the deregulation of cell cycle regulators such as p53, E2F-1 and the numerous cyclins. DF-1 cells, in particular, were shown to proliferate more rapidly under normal culture conditions compared to other immortal EF cells, implicating other mechanisms may be important for regulating their growth. The possible mechanism(s) underlying the accelerated growth of DF-1 cells will be addressed in this study. (omitted)

  • The author has observed the tissue reaction of the absolute alcohol infection of rat oral mucosa. 0.5ml absolute alcohol was injected subcutaneously on the mucobuccal fold of rat. And the rats were sacrifieced at intervals of one day, 3rd, 1 week, 2 week and 4 week after alcohol injection. The microscopic tissue sections were made and stained with hematoxylin and eosin. The results were are as follows; 1. Degeneration and shrinkage of fibroblasts and coagulative necrosis were observed one day to and three day after alcohol injection. 2. Although coagulative necrosis and tissue degeneration occurred, the inflammatory infiltration was not prominent especially there were scarcely any polymorphonuclear leukocytes in that field. 3. Granulation tissue with moderate small round cell infiltration were replaced the necrotic area at one week after injection and the fibroblast proliferate into the granulation tissue at two week group. 4.   • Calcifying aponeurotic fibroma (CAF) is an uncommon benign soft-tissue fibroblastic tumor with characteristic histological features. It mainly occurs in the distal part of the extremities, such as the hands and feet, in children and adolescents. Males are twice as commonly affected as females. CAF exhibits local invasiveness, and hence, its recurrence rate is also high. Therefore, complete surgical excision is both diagnostic and therapeutic. The occurrence of CAF in the maxillofacial region, especially the temporomandibular joint (TMJ), is very rare, and this necessitates its differentiation from other TMJ neoplasms. The differential diagnosis of CAF requires microscopic examination. Herein, we report a rare case of CAF located at the left mandibular condyle, which was confirmed by histopathological analysis.

Two-stage Surgical Treatment of a Giant Solitary Fibrous Tumor Occupying the Thoracic Cavity

  • A solitary fibrous tumor (SFT) is a mesenchymal fibroblastic tumor inside the pleura, for which complete surgical resection is the standard treatment. For large SFTs, preoperative identification of tumor-feeding vessels using angiography is important for achieving complete resection without unexpected operative bleeding. Extensive adhesions can make resection difficult in a limited operative window, and pulmonary resection may be required to achieve complete SFT resection. Herein, we report successful resection of a large pleural SFT in a 39-year-old man without any complications using a 2-stage approach, in which ligation of the feeding vessels through small another operative window was the first step.

  • 석회화건막섬유종은 매우 드문 양성 연부조직 종양으로 소아와 청소년기에 팔다리, 특히 손과 발에 발생한다. 저자들은 오른쪽 엄지손가락에 발생한 석회화건막섬유종 1예를 경험하였기에 문헌 고찰과 함께 보고한다. 14세 남자가 오른쪽 엄지손가락의 종괴를 주소로 내원하였다. 방사선 소견에서 연부조직 병변이 오른쪽 엄지손가락의 손허리손가락관절 부위에 관찰되었다. 종괴에 대한 절제가 시행되었다. 절제된 종괴는 $2.0{\times}1.5cm$ 크기의 회백색을 띠는 섬유성 조직이었다. 조직학적으로 종괴는 섬유모세포의 증식과 주위조직으로 침윤성 성장을 보였다. 종괴 내부에 부분적인 석회화와 연골 분화가 관찰되었다.

  • This study was undertaken to observe the histologic changes of surrounding maxillary sutures to the widening of midpalatal suture, using two adult dogs, weighing about 10 kg, for experimental, and one for control group. After widening of the midpalatal suture with expansion screw for ten days was performed, and followed by sacrifice of experimental animals. Tissues were excised from 5 surrounding maxillary suture portions such as, internasal, interfrontal, midsagittal, zygomatrco-temporal, and midpalatal sutures. After that, the specimens were fixed and decalcified in $10\%$ formalin sol. and $5\%$ nitric acid. Embedding in paraffin and serial sections at a thickness of 5 micron was done, After Hematoxylin-Eosin staining and light microscopic examination, Following results were obtained: 1. In midpalatal suture area, which showed most prominent histologic change, High degree of fibronlastic and osteoblastic proliferation lining the bony trabeculae with osteoid tissue projection into the fibrous connective tissue were observed. 2. In interfrontal suture area, moderate degree of fibroblastic and osteoblastic proliferation was observed. 3. In internasal suture area, active osteoblastic and osteoclastic proliferation lining the bony trabeculae was observed, and separation of fibrous connective tissue was also observed. 4. In midsagittal and zygomatico-temporal suture areas, no histologic changes can be observed.
