A Study on the DNA Polymorphisms at $\beta$fibrinogen Loci and Plasma Fibrinogen Concentration

  • Many prospective studies have confirmed the predictive value of plasma fibrinogen levels for vascular diseases, including ischemic heart disease. Several polymorphisms of the $\beta$fibrinogen gene have been investigated in relation to plasma fibrinogen levels. The blood samples for DNA were collected from 109 healthy Koreans who have no relationship by blood (67 males and 42 females) in due consideration of some other factors such as gender, age, and smoking status. Four polymorphisms of the $\beta$fibrinogen gene that consist of HaeIII, AluI, MaII and BcII restriction fragment length polymorphisms (RFLPs) were investigated to examine the associations between RFLPs and plasma fibrinogen levels. In conclusion, the significant associations between HaeIII, AluI, MnII RFLPs(H$_1$H$_2$, M$_1$M$_2$, $A_1$A$_2$) and the concentration of plasma fibrinogen were shown by the smokers as well as by the old people more than 50, whereas the association between BcII and plasma fibrinogen were shown no connection with the status of age and smoking. The concentration of plasma fibrinogen was significantly shown higher by the old people ($\geqq$50) by the younger people ($\leqq$49) in male and also higher by the smokers than by the nonsmokers.

피브리노겐의 수치 및 중요한 아미노산 변형 돌연변이가 뇌중풍에 미치는 영향 (Effects of Fibrinogen Level and Genetic Variation in FGA Gene on Korean Stroke Patients)

  • Backgrounds : Stroke is characterized by loss of brain functions due to a disturbance in the blood vessels supplying blood to the brain, and classified into hemorrhage and ischemia. Stroke is known to be affected by genetic factors and other diseases such as hypertension and cardiovascular diseases. However, the distinctive association between stroke and genetic variations has not discovered yet. Objectives : This study investigated the effects of fibrinogen level and genetic variations in FGA (Fibrinogen alpha chain) gene on stroke in Korean stroke patients and controls. Methods : DNA samples from 674 stroke patients diagnosed by Oriental medical hospitals and 267 controls were used in this study. Two common single nucleotide polymorphism(SNP) with high minor allele frequency(MAF), rs2070011G/A of promoter region and nonsynonymous rs6050A/G of exon 5 in FGA gene, were targeted for Taqman genotyping. Because the TOAST classification is important to the factors and symptoms of stroke, ischemic patients were further classified into five subtypes using diagnosis and clinical data. One-way ANOVA and chi-square test were used for clinical data and genetic association, respectively. Haploview v4.1 program was used for linkage disequilibrium(LD), haplotype and haplotype block analysis. Results : The levels of red blood cells and fibrinogen from clinical data were shown to be significant factors for the sub-groups of TOAST classification. No significant associations of stroke, hemorrhage, ischemic and subtypes of TOAST with rs2070011 and rs6050 of FGA gene were found(P > 0.05). However, rs2070011 in promoter region and nonsynonymous rs6050 in exon 5 which produce the amino acid change from threonine to alanine showed a haplotype block and three haplotypes of A-G, G-A, A-A, suggesting that rs2070011 and rs6050 might be co-segregated in generic recombination. Although A-A haplotype of stroke patients showed 64-69% low frequency compared to controls, there was no significant association between stroke and haplotype(P > 0.05). Conclusion : This study showed that there was no significant association between stroke and two SNP of rs2070011G/A and nonsynonymous rs6050A/G in FGA gene. However, these two SNP compose a haplotype block and three haplotypes of A-G, G-A, A-A. This finding suggests that rs2070011 and rs6050 are so close as to be positioned as linkage disequilibrium. Nevertheless, no significant association between haplotypes and stroke was found.

Association between Single Nucleotide Polymorphisms of the Fibrinogen Alpha Chain (FGA) Gene and Type 2 Diabetes Mellitus in the Korean Population

  • Fibrinogen alpha chain (FGA), a subunit of fibrinogen, might be a potential player for type 2 diabetes mellitus (T2DM), since the plasma levels of fibrinogen is known to be related to the incidence of T2DM. To elucidate the potential role of FGA in T2DM, we investigated whether FGA genetic variations are relevant in T2DM in the Korean population. Seven FGA single nucleotide polymorphisms (SNPs) were genotyped in Ansung and Ansan cohorts (474 T2DM subjects and 470 normal controls) in Korea. The association between SNPs and T2DM was determined by logistic regression analysis. Genetic relevance of SNPs to T2DM-related phenotypes was investigated by multiple linear regression analysis. Statistical analysis revealed that among seven FGA SNPs, significant associations with T2DM were observed in FGA rs2070011 (p=0.013-0.034, OR=0.72${\sim}$0.79), rs6050 (p=0.026${\sim}$0.048, OR=1.24${\sim}$1.37), and rs2070022 (p=0.016${\sim}$0.039, OR=0.70${\sim}$0.72). Two SNPs, rs2070011 and rs6050, also showed significant association with T2DM-related phenotypes such as triglyceride (p=0.005${\sim}$0.011 for rs2070011 and p=0.003${\sim}$0.008 for rs6050), total cholesterol (p=0.01 for rs2070011 and p=0.024 for rs6050) and fasting glucose (p=0.035${\sim}$0.036 for rs2070011 and p=0.048 for rs6050) in 470 normal controls. Our association study implies that FGA might be an important genetic factor in T2DM pathogenesis in the Korean population by affecting plasma lipid and glucose levels.

소아 신증후군 환자에서 Plasminogen Activator Inhibitor Type 1 유전자 다형성 (Plasminogen Activator Inhibitor Type 1 Gene Polymorphism in Patients with Minimal Change Nephrotic Syndrome)

  • 목 적 : 소아 신증후군 환자에서 과응고성 경향을 가지고 있으며 Plasminogen activator inhibitor-1(PAI-1)은 강력하게 섬유소의 용해를 감소시키는 당단백으로 최근 몇몇 연구에 의하면 신증후군에서 증가된 PAI-1과 사구체내의 섬유소원이나 섬유소와 관련된 항체의 침착이 연관되어 있음을 시사하고 있다. 저자는 PAI-1 유전자 다형성 중에서 A-844 G 대립유전자 다형성의 빈도와 유전형을 소아의 신증후군에서 임상경과와의 연관성을 비교 평가하였다. 방 법 : 2001년 10월부터 2003년 1월까지 경희대학교 부속병원 동서신장병연구소에 방문한 146명의 신증후군 환아와 249명의 대조군을 대상으로 하였고 신증훈군 환아는 infrequent relapser(IR)와 frequent relapser(FR)로 다시 나누었다. 이들에 대해 PAI-1 promoter gene의 A-844 G에 대한 중합효소 연쇄반응-제한효소절편길이 다형현상(PCR-RFLP)을 이용하여 유전자형을 A/A, A/G, G/G로 분류하여 비교 분석하였다. 통계는 Graph Pad Prism 통계분석 소프트웨어 version 2.0을 사용하였고 95% 신뢰구간과 P value는 0.05보다 작은 것을 의미 있게 보았다. 결 과 : PAI-1 promoter gene의 A- 844G 다형성의 분포는 대조군애서 G/G 81(32.5%), A/A 42(16.9%), G/A 126(50.6%)이였고 신증후군 그룹 중 IR 그룹은 G/G 29(34.1%), A/A 15(17.7%), G/A 41(48.2%)이였으며 FR 그룹에서는 G/G 17(27.9%), A/A 18(29.5%), G/A 26(42.6%)이었다. 단지 PAI-1 gene의 A-844G의 다형성중 A/A 유전자형이 신증후군 환아 중 FR군에서만 대조군에 비하여 유의하게 증가하였다(16.9% vs 29.5%, OR=2.06, P=0.0251). 반면 A/G 유전자형(OR=0.73, P=0.2639)이나 G/G 유전자형(OR=0.80, P=0.4828)은 통계학적인 의의가 없었고 IR군에서 대조군에 비하여 A/A(OR=1.06, P=0.8690), A/G(OR=0.91, P=0.7063), G/G(OR=1.07, P=0.7880)으로 모두 통계학적으로 유의하지 않았다. 결 론 : A-844G AA 유전자형을 가진 신증후군 환자와 FR와 통계적인 상관관계가 있음을 알수 있었다. 이는 향후 PAI-1 유전자와 과응고성과 관련된 생화학적 검사와의 연관성에 대한 추가적인 조사와 특히, 신증후군의 과응고성과 신증후군 재발과의 연관성이 있는지에 관한 연구가 더 필요할 것으로 사료된다.
