A Study on Correlation between the Blood Pressure and Bone Mineral Density or Body Mass Index (혈압에 따른 골밀도와 체질량 지수와의 상관관계 연구)

  • Objectives : This study was conducted to investigate how Bone Mineral Density(BMD) and bone mass index(BMI) differ according to classification of blood pressure which JNC 7(The seventh report of the joint national committee on prevention, detection, evaluation and treatment of high blood pressure) indicated. Methods : We measured BMD and BMI of lumbar spine($L_2-L_4$) and femoral neck of 9816 people, and then we analyzed them according to classification of blood pressure. Results : The number of prehypertension group was the most, and Stage 2 hypertension group was the least. As the hypertension was increasing, BMD of lumbar and femoral neck were decreasing, and BMI was increasing. In men, as the blood pressure was increasing, BMI was increasing. But the BMD was irrelevant. In women, the distribution was similar to the total. Conclusions : As the hypertension was increasing, BMI was increasing and BMD was decreasing. And it was more remarkable in women.

Effects of Dietary Calcium, Protein, and Phosphorus Intakes on Bone Mineral Density in Korean Premenopausal Women (우리나라 폐경전 여성에서 칼슘, 단백질, 인의 섭취상태가 골밀도에 미치는 영향)

  • Effects of dietary calcium(Ca), protein, and phosphorus(P) intake on bone mineral density (BMD) were investigated in 129 Korean premenopausal women(age 31-54 years) without diagnosed disease. BMD was measured at the spine(vertebrae L2-4) and femur(neck, Ward's triangle and trochanter). By stepwise multiple regression analysis it was shown that protein, Ca, and P intakes affected most significantly on BMD at the vertebrae L2-4, protein and P intakes affected most significantly on BMD at the femoral neck and Ward's triangle, and body mass index(BMI) affected most significantly on BMD at the trochanteric region. When ate-matched BMD % at the vertebrae L2-4 and all femoral sites was grouped by three levels(<90%, 90-99%, >=100%), only at the vertebrae L2-4>=100% and 90-99% groups had higher Ca intakes than <90% groups. When Ca, protein and P intakes of the recommended level for Korean(RDA) were grouped by three levels (Ca or P ; <=650mg/d, 650-750mg/d, >=750mg/d, Protein ; <=55g/d, 55-60g/d, >=65g/d), only at the vertebrae L2-4>55g/d of protein intake had higher age-matched BMD % than <=55g/d intake, >=750mg/d of Ca and P intakes, age-matched BMD % than <=650mg/d. In RDA range of Ca, protein, and P intakes, age-matched BMD % of the vertebrae L2-4 and all femoral sites was greater than 90%. Correlation between Ca intake and vertebral BMD was examined closer. There was more significant linear correlation between vertebral BMD and Ca intake below 800mg/d(r=0.346, p<0.0001)than above(r=0.376, p<0.019), implying a threshold effect and vertebral BMD was better expressed as a function of the logarithm of calcium intake(r=0.3881, p<0.0001). These results suggest that Ca, protein, and P intakes greater than RDA help to maintain proper BMD in middle-aged prementopausal women. Especially dietary Ca have important role in increasing the vertebral BMD and 800mg/d of Ca intake is optimum amount.

Research on Relation of Nutrients Intake, Health Status, and Bone Mineral Density in Middle-aged Women (장년기 여성의 영양섭취상태, 건강상태와 골밀도와의 관련성에 관한 연구)

  • This study was to investigate the relation of nutrients intake, health status, and bone mineral density in middle-aged women. Daily nutrients intake were analyzed by convenient method. The BMD of subjects were measured by Dual Energy X-ray Absorptiometry(DEXA). BMD was measured at the spine(vertebrae L2-L4) and femur(neck, Ward's triangle and trochanter). The nutrients intake of subjects were higher than recommended dietary allowances(RDA) except for calorie, iron, calcium, vitamin B1. Nutrients intake of vitamin A(p<0.05), iron(p<0.01), vitamin $B_2$(p<0.05), niacin(p<0.05), and vitamin C(p<0.05) between $\leq$49 yr group and $\geq$50 yr group. Mean daily intake of calcium was much less than the Korean RDA, 78.5% in $\leq$49 yr group and 77.3% in $\geq$50 yr group. 35.8% of the subjects under 50 years of age($\leq$49 yr group) and 77.4% of the subjects from 50 years up($\geq$50 yr group) were classified as osteopenia or osteoporosis. BMD of L2-L4 were positively correlated with height(p<0.05), weight(p<0.05) and BMI(p<0.05). BMD of femoral neck was positively correlated with BMI(p<0.05), BMD of Ward's triangle was positively correlated with weight(p<0.001). But BMD of L2-L4 and femoral neck were negatively correlated with age(p<0.05), menarche(p<0.05). BMD of Ward's triangle was negatively correlated with age(p<0.001). The BMD of L2-L4 were positively correlated with animal protein(p<0.05), calcium(p<0.05) and iron(p<0.05). The BMD of femoral neck was positively correlated with animal protein(p<0.05). The BMD of Ward's triangle was positively correlated with animal protein(p<0.001) and iron(p<0.001). The above results suggest that it should be difficult to prevent middle-aged women's bone destruction through nutrients intake. Further investigation is necessary to prove the mutual relations between BMD, exercise, and calcium intake. Therefore, middle-aged women will need proper exercise as well as Ca supplementation in order to prevent osteoporosis with aging.

A Comparative Study of Influencing BMD Factors in Postpartum and General Women in Their Twenties and Thirties ($20{\sim}30$대 여성의 골밀도 영향 인자에 대한 출산 후 여성과 일반여성과의 비교 연구)

  • This study is to evaluate the factors that affect to the bone mineral density(BMD) of postpartum women by comparing BMD between postpartum and general women who are ages from 20 to 39, and to identify correlation between various factors weight, height, body mass index(BMI), hemoglobin(Hb) and BMD. Study subjects were postpartum(n=159) and general(n=180) women. We checked the BMD of femoral neck and lumbar spine by using dual energy X-ray bone densitometry. The mean age was $30.69{\pm}3.32$ in postpartum care group and $31.22{\pm}5.66$ in general women group. In postpartum care group, the mean BMD of femoral neck and lumbar spine were -0.187 and -0.076. In general women group, the mean BMD of femoral neck and lumbar spine were -0.029, and -0.169. According to BMI level, each group was divided into two subgroups. One was 23 or more BMI subgroup, and the other was under 23 BMI subgroup. There was a significant difference in BMD of femoral neck and lumbar spine between two subgroups in general women group, but only a significant difference in BMD of lumbar spine is noted between two subgroups of postpartum group. According to Hb level, each group was divided into two subgroups. One was 11g/dl or more subgroup, and the other was under 11g/dl subgroup. There was not a statistically significant difference of BMD in all subgroups. BMD of femoral neck and lumbar spine showed a significant correlation with weight, height, BMI in both groups. There was a significant correlation between BMD and Hb level in postpartum care group, but a significant correlation was not noted in general women group. The postpartum women showed a significant correlation between BMD and various factors such as weight, height, BMI just like in general women. There was a significant correlation between BMD and Hb in postpartum women, but not in general women.

  • Craniopharyngiomas are rare primary intracranial tumors. Despite their benign histological appearance, they are often associated with an unfavorable prognosis. The typical manifestations upon diagnosis are headache, visual impairment, polyuria/polydypsia, growth retardation, disturbance of pubertal development, and significant weight gain. The treatment options include radical surgery or radiotherapy, or a combination of these modalities. Slipped capital femoral epiphysis (SCFE) is the most common adolescent hip disorder. SCFE occurs when the capital femoral epiphysis displaces posteriorly on the femoral neck at the level of the physis. The etiology of SCFE is thought to be multifactorial and may include obesity, growth surges, and less common endocrine disorders. The related endocrine disorders include hypothyroidism, growth hormone supplementation, hypogonadism, and panhypopituitarism. Reported herein is a case of panhypopituitarism caused by craniopharyngioma combined with SCFE.

Bone Mineral Density of Korean Mother-daughter Pairs : Relations to Anthropometric Measurement, Body Composition, Bone Markers, Nutrient Intakes and Energy Expenditure (모녀간의 골밀도 : 신체 측정치 및 체조성, 골지표, 영양소 섭취량 및 에너지 소비량과의 관계)

  • The objectives of this study were to obtain normative data for 45 mother-daughter pairs on spine, femur(neck, ward's triangle, trochanter) and total body BMD (bone mineral density) measured by dual energy X-ray absorptiometry, anthropometric measurement, body composition, biochemical markers, nutrient intakes and energy expenditure, to determin the interrelations of these factors within each group, to measure familial resemblance for each variable. We observed significantly positive correlations between height, weight, head, hip and calf circumferences, tricep, femur and calf skinfold thickness, total lean body mass(=weight-total fat body mass-bone mineral content), protein and fat intakes, Ca index, serum total protein and albumin of monter-daughter pairs(p<.05-p<.001). Among mothers, age, osteocalcin, higher, weight, Ca and energy intakes were predictors of BMDs. Among daughters, weight and energy intake were predictors of BMDs. The BMD in lumbar spine(r=.48, p<.01), femoral neck(r=.38, p<.05), ward's triangle(r=.36, p<.05) of the mothers were significantly correlated with those of the daughters, after adjustment for mother's age, hight, weight, osteocalcin, Ca and energy intakes and daughter's weight, energy intake. In regression analyses, mother's BMD also were positively associated with daughter's BMD in lumbar spine, femoral neck, ward's triangle. Our findings support that mothers with low BMD tended to have daughters with low BMD. In the age groups studied, as well genetic factors as environmental factors may have an important role in determining BMD. This study suggests that women may successfully enhance their genetically determined BMD through adequate nutrient intakes and weight bearing exercise.

The Influence of Diet, Body Fat, Menstrual Function, and Activity upon the Bone Density of Female Gymnasts (신체구성성분, 영양상태 및 월경기능이 여자체조선수의 골밀도에 미치는 영향(제2보))

  • This study was conducted with 20 female gymnasts and 23 age-matched controls to examine the relationship of diet, menstrual function and bone mineral density (BMD). The results obtained are summarized as follows : Energy intake of gymnasts was 968.9$\pm$421.4kcal, and energy expenditure was 2091.4$\pm$361kcal showing negative energy balance(-1,122.5$\pm$534.6kcal). The average intakes of calcium, iron, vitamin A, thiamin, riboflavin and niacin did not meet the Recommended Dietary Allowances for their age groups. Mean age at menarche in gymnasts is 15.8$\pm$1.2 years compared with 11.8$\pm$2.8 years in age-matched controls. The profile of estradiol, progesterone, and luteinizing hormone was lower than age-matched controls but not significant. Athletic amenorrheic gymnasts(n=12) have the menstrual irregularity(n=10) and amenorrhea(n=2). A number of variables as such nutritional deficiency in diet, negative energy blasnce and hypogonadotropic hormonal status were included. The bone mineral density (BMD) of female gymnasts were significantly higher than controls for the lumbar neck(p<0.001), trochanter(p<0.01), and Ward's triangle(p<0.001), but there were no significant differences for the lumbar spine and forearm. The lumbar spine BMD had a positive correlation with age and lean body weight. The femoral neck BMD was significantly associated with age, group and lean body mass. The trochanter BMD had significant relationship with group, body mass index, energy expenditure and follicular stimulating hormone. Ward's triangle BMD were related to body mass index and follicular stimulating hormone. The significant association was deterced between forearm BMD and age and lean body weight. The major finding of this investigation is that the BMD of gymnasts were higher than age-matched controls despite the fact that gymnasts as a group had inadequate dietary calcium and a higher propensity to have an interruption of their menstrual cycle. These data indicate that grymnsts involved in sports producing significant impact loading on the skeleton had greater femoral neck, trochanter and Ward's triangle bone density than age-matched controls.

Effects of Weight-Bearing Exercise on Bone Metabolism in College Women (체중부하운동이 여대생의 골대사에 미치는 효과)

  • Purpose: This study was done to identify the effects of weight-bearing exercise(WBE) on bone metabolism. Method: WBE was performed for 12 weeks by healthy college women. Bone-related parameters were measured four times during this period by evaluating the immunoradiometric assay and enzyme immunoassay. Bone mineral densities(BMDs) were measured by dual energy x-ray absorptiometry before and after the WBE program. Data was analyzed using t-test, paired t-test, $x^2$-test, and repeated measures ANOVA. Result: Osteocalcin, a bone formation marker, increased more in the experimental group than in the control group based on the interaction between time and group(F=3.29 p=.024). Little difference between the two groups was found for the other parameters: urinary deoxypyridinoline, insulin-like growth factorI, parathormone, serum calcium, and serum phosphorus without showing any time interaction between the groups. The femoral trochanter BMD rose in the experimental group while that of the control group fell, showing a significant difference for BMD(t=3.06 p=.005). However, there was no significant difference between the two groups for changes in BMD of the forearm, lumbar spine, femoral neck, and femoral ward's triangle. Conclusion: These findings supported the WBE is beneficial for increasing bone formation in college women and long-term application is needed to substantiate the effects of WBE as a intervention in promotion of bone-health.

  • The accurate measurement of the femoral anteversion is very important to the practice of orthopedic and osteotomy. It is measured by means of the axis of head and neck of the femur and the knee axis. At the present time, widely used computed tomography method of measuring anteversion on femoral necks of patients. Measurement by the manual method and image viewer of computed tomography to determine the anteversion of femoral head were carried out on both femurs. In September and October 2002, 5 patients 28 to 36 years of age were randomly selected from Seoul National University Hospital. The purpose of this paper was to introduce a new method to measure femoral anteversion angle utilizing PACS image viewer program in the MR imaging. Significant difference was observed between the right and left side the image viewer measurement of femoral anteversion. In conclusion, MR imaging very usefulness in the measured the angle of the femoral anteversion.

Correlations of Lumbar and Femoral Bone Mineral Densities with Calcaneal Speed of Sound in Osteoporotic Woman (골다공증 여성에서 요추골 밑 대퇴골 부위의 골밀도와 종골 음속 사이의 상관관계)

  • In this in vivo study, correlations of lumbar and femoral bone mineral densities (BMDs) with calcaneal speed of sound (SOS) were investigated in 36 osteoporotic women. Areal BMDs of the L2-L4 lumbar spine and the right femoral neck were measured by using dual energy X-ray absorptiometry (DEXA). SOS of the right calcaneus was measured by using ultrasound bone densitometry. Pearson's correlation coefficient (r) and level of significance (p) were used to evaluate the correlations between measurements. Lumbar BMD was highly correlated with femoral BMD (r=0.81). Lumbar and femoral BMDs exhibited similar comparable negative correlations with age (r=-0.52 and r=-0.55). A moderate negative correlation was found between calcaneal SOS and age (r=-0.45). Calcaneal SOS was significantly correlated with lumbar and femoral BMDs, with a higher correlation with femoral BMD rather than with lumbar BMD (r=0.54 and r=0.62). However, calcaneal SOS may not be an optimum index for the estimation of BMD of the most important fracture sites, such as the lumbar and the femur, because it showed lower correlations with lumbar and femoral BMDs compared to that with calcaneal BMD. Therefore, the development of a quantitative ultrasound technology for the direct measurement of acoustic properties at the lumbar and the femur is required to estimate BMD of these sites more accurately.
