20대와 60대 남성의 발 유형 비교 연구 - Size Korea의 3차원 측정 데이터를 이용하여 - (A Study on the Comparison of Foot Type for the Men in Their 20's and 60's - With the Usage of Size Korea's 3-Dimensional Measurement Data -)

  • In this study, foot shapes were compared between the men in their 20's and 60's, with the application of 3-dimensional measurement data, and the classification was established in consideration of each of their features. The results are as follows: 1. The investigation into the differences between the groups for the 3-dimensional measurement items of the men in their 20's and 60's indicated that those in their 20's had larger values than those in the 60's age group for all of the length items, while those in the 60's age group had larger values for all of the angle items. 2. The analysis of the constituting factors for the feet of those in their 60's and 20's resulted in the 7 items for the 20's age group and 8 items for those in the 60's age group. 3. The comparison of foot classifications for the 60's age group and 20's age group presented 3 types each for both groups, but the features of each type was different. To examine each characteristic by each type and age group, following can be determined of the 60's age group. Type 1 had a thick instep, high malleolus position and heel and a thick formed ankle. Type 2 the entire foot was small, and the big toe was formed in such a way that it tened to bend toward the inside. When the whole size compared with similar Type 1, Type 3 had a low instep, a low heel, the big toe did not bend and ankle form was thin. The following can be determined of the 20's age group. Type 1 had narrow width a thin ankle and the big toe was formed in such a way that it bended toward the inside. Compared with length, Type 2 had wide width and a thick ankle. Type 3 had a larger overall size, thick instep and wide and the toe was big, but the characteristic of inside and outside of a malleolus position was low and the big toe's form showed no tendencies to bend.

8세 초등학생의 편평족 조사 (Flat Foot Survey in 8 Year Old Primary School Children)

  • Purpose: To estimate the prevalence of flat foot in the 1st grade primary school children in the Kang-Dong Gu, Seoul, Korea. Materials and Methods: We examined 1336 8 year old the first grade children (2672 feet) in primary school at 5 primary school for prevalence of flat foot and the associating factors in Kang-Dong Gu in April 2003. The group was examined with 2 mm-pannel in physical examination at erect position to put diagnosis of flat foot which showed no plantar arch. The height, weight, foot length and foot width were estimated in all children. We used obesity grading system of Korean Pediatric Association for overweight evaluation. Results: We enrolled 728 boys and 608 girls, and prevalences of each gender were 20.8% and 14.9%. The children who had flat foot were 243 and the prevalence of flat foot was 18.2%. The foot length ranges between 152 mm and 300 mm (mean value 183.6 mm, SD 10.6), and the foot width were between 50 mm and 107 mm (mean value 2.16 mm, SD 4.8). Of the 1336 children group, 1215 children (90.0%) were in range of normal weight, 72 children (5.4%) were in grade-1 overweight, 38 (2.8%) were in grade-2 overweigh and, 11 (0.8%) were in grade-3 overweight. The prevalence of flat foot of each overweight group were 16.3%, 34.7%, 39.5% and 45.5%. Overweight in children effected increased prevalence of flat foot. But there were no significant relationship with flat foot in other factors. Conclusion: Over all prevalence of flat foot of 8 year old children was 18.2% and most of patients were flexible flat foot. The prevalence of flat foot was influenced by overweight remarkably.

유연성 편평족에 대한 내측 전위 종골 절골술의 단기 추시 결과 (Short-term Results of Medial Displacement Calcaneal Osteotomy for Flexible Flatfoot)

  • Purpose: To evaluate the short-term results of medial displacement calcaneal osteotomy without flexor digitorum longus transfer for flexible flatfoot deformity. Materials and Methods: Twenty four patients (25 feet) who had undergone medial displacement calcaneal osteotomy without flexor digitorum longus transfer for flexible flatfoot between July 2004 and May 2007 were included. The mean age was 43.6 years (16~78 years). The mean follow-up was 26 months (14~50 months). Clinical outcomes were assessed using American Orthopaedic Foot and Ankle Society (AOFAS) score and visual analogue scale (VAS). Six radiographic parameters were measured from weightbearing radiographs to assess the difference between preoperative and postoperative radiographs. Results: The mean AOFAS score improved from 57.9 points preoperatively to 89.2 points at latest follow-up (p=0.000). The mean VAS improved from 62 points preoperatively to 23 points at latest follow-up (p=0.000). The mean talonavicular coverage angle on anteroposterior (AP) view changed from 20.2 degrees to 16.0 degrees (p=0.002). The mean calcaneal pitch angle on lateral view changed from 12.6 degrees preoperatively to 14.5 degrees at latest follow-up (p=0.001). Regarding these radiographic parameters, the difference between interobserver measurements was larger than that between pre- and post-operative measurements. The calcaneus was transferred medially by average 11.8 mm (p=0.003), which was 27.9% of the width of calcaneal tuberosity (p=0.000). The mean talo-first metatarsal angle on AP and lateral views, and navicular height on lateral view were not significantly changed. Conclusion: Medial displacement calcaneal osteotomy without flexor digitorum longus transfer for flexible flatfoot could lead to improve the clinical outcomes, although the restoration of medial longitudinal arch was not clinically significant.

지능적 족형 보행 재활 보조 로봇의 개발 (Development of an Intelligent Legged Walking Rehabilitation Robot)

  • 본 논문에서는 하지 보행 장애를 가진 환자가 사용하는 크러치에 로봇 기술이 적용된 새로운 타입의 착용식 족형 보행 재활 로봇에 대해 서술하였다. 개발된 로봇의 주된 특징은 크게 세 가지로 나뉜다. 첫 번째, 착용 방식의 특징으로서, 팔꿈치에 착용하여 환자를 보조하는 엘보 크러치와 달리 환자의 가슴부에 족형 로봇을 착용함으로써 보행 시 넓은 가슴 면적으로부터 하중을 분산시키고 팔에 작용하는 집중 하중을 제거하였으며, 팔과 손의 움직임을 자유롭게 하였다. 두 번째, 사용자의 보행 의지를 파악하는 방법으로서, 족형 로봇의 발에 부착된 3축 힘센서를 이용하여 지면 반발력의 크기와 방향으로부터 사용자의 보행의도를 추정하는 알고리즘을 제안하였고 이에 따라 가슴 착용형 로봇을 보행시켰다. 마지막으로, 계단 보행을 위한 근력 강화 방법으로서, 둔턱을 오르내리는 근력 훈련이 가능하도록 바닥 높낮이에 따라 상하 운동 궤적이 변화할 수 있는 둔턱 보행 기능을 구현하였다. 최종적으로 개발된 로봇이 보행 의도를 인지하고 로봇 다리로부터 보행 보조를 효과적으로 수행할 수 있다는 것을 실험을 통하여 확인하였고, 하지 근전도 측정을 통하여 보행 시 로봇이 하지 근력을 보조하는 성능을 정량적으로 검증하였다.
