Purpose: To investigate the association between urinary neutrophil gelatinase-associated lipocalin (uNGAL) and leukocyte differential count in children with urinary tract infections (UTIs). Methods: A retrospective chart review was performed in children undergoing uNGAL measurements between June 2018 and September 2019. Patients with suspected or diagnosed UTIs were included. The relationship between uNGAL and blood leukocyte differential count was investigated in children. Results: A total of 197 children were included in this study, 119 of whom (60%) had UTIs. The non-UTI patients (n=78) were diagnosed with pneumonia, acute gastroenteritis, viral upper respiratory infection, and others. After adjusting for age, gender, and fever duration, the leukocyte count, monocyte count, and uNGAL levels were higher in the UTI group than in the non-UTI group (P<0.05). uNGAL showed positive correlations with neutrophil counts, monocyte counts, the neutrophil-to-lymphocyte ratio, and the monocyte-to-lymphocyte ratio in the UTI group (P<0.05). uNGAL levels were only associated with the neutrophil-to-lymphocyte ratio in the non-UTI group (P<0.05). In a multivariable logistic regression analysis, only uNGAL was associated with the presence of UTI (P<0.05). The area under the receiver operating characteristic curves for uNGAL and monocyte counts to identify UTI were 0.89 (95% confidence interval (CI): 0.824-0.939; P=0.025) and 0.7 (95% CI: 0.627-0.774; P=0.038), respectively. Conclusions: In children with UTIs, uNGAL levels may be associated with blood leukocyte differential counts. uNGAL measurements and monocyte counts can be helpful in children with suspected UTIs.
Purpose : Coxiella burnetii is a well-known causative agent of granuolmatous inflammation and an inducer of morphological change and transformation of human B lymphocyte in vitro. Coxiella burnetii manifests with several clinical symptoms depending upon the organs that are involved. We therefore undertook to clarify the association of Kawasaki disease and Coxiella burnetii. Subjects and Methods : The patient's sera were tested for antibodies specific for Coxiella burnetii, using indirect fluorescent antibody technique(IFA). We compared Coxiella burnetii infection with 3 groups of patients, group 1 (Kawasaki disease), group 2 (other febrile disease) and group 3 (control group). Results : 1) In children with Kawasaki disease (group 1), 93% of the patient tested positive for Coxiella burnetii. 2) In group 2 children, 20% of the patient tested positive for Coxiella burnetii. 3) In group 3 children, 10% of the patient tested positive for Coxiella burnetii 4) There were significant higher positive rate for Coxiella burnetii in Kawasaki disease than group 2 and group 3 (p<0.05). Conclusions : We concluded that our cases of Kawasaki disease were associated with Coxiella burnetii infection. Further studies will be needed to understand the precise role of Coxiella infection in Kawasaki disease.
The present study determined and compared the genetic diversity of Plasmodium falciparum strains infecting children living in 2 areas from Gabon with different malaria endemicity. Blood samples were collected from febrile children from 2008 to 2009 in 2 health centres from rural (Oyem) and urban (Owendo) areas. Genetic diversity was determined in P. falciparum isolates by analyzing the merozoite surface protein-1 (msp1) gene polymorphism using nested-PCR. Overall, 168 children with mild falciparum malaria were included. K1, Ro33, and Mad20 alleles were found in 110 (65.5%), 94 (55.9%), and 35 (20.8%) isolates, respectively, without difference according to the site (P>0.05). Allelic families' frequencies were comparable between children less than 5 years old from the 2 sites; while among the older children the proportions of Ro33 and Mad20 alleles were 1.7 to 2.0 fold higher at Oyem. Thirty-three different alleles were detected, 16 (48.5%) were common to both sites, and 10 out of the 17 specific alleles were found at Oyem. Furthermore, multiple infection carriers were frequent at Oyem (57.7% vs 42.2% at Owendo; P=0.04) where the complexity of infection was of 1.88 (${\pm}0.95$) higher compared to that found at Owendo ($1.55{\pm}0.75$). Extended genetic diversity of P. falciparum strains infecting Gabonese symptomatic children and high multiplicity of infections were observed in rural area. Alleles common to the 2 sites were frequent; the site-specific alleles predominated in the rural area. Such distribution of the alleles should be taken into accounts when designing MSP1 or MSP2 malaria vaccine.
목적 : 발열성 요로감염 환아의 ${99m}Tc-DMSA$ 신주사에서 상대적 및 절대적 신섭취율을 비교하여 진단적 가치를 규명하고자 한다. 방법 : 요로감염으로 진단받은 117례(남아 70례, 여아 47례)에서 $^{99m}Tc-DMSA$ 정맥주사 3시간 후에 좌우 신장의 상대적 및 절대적 신섭취율을 측정하였다. 고열과 $^{99m}Tc-DMSA$ 신주사상 결손 부위를 보인 환아는 총 68례였고 이중 일측성 부분 결손을 보인 35례, 양측성 부분 결손을 보인 13례, 양측성 미만성 결손을 보인 20례의 절대적 및 상대적 신섭취율을 측정하여 비발열성 요로감염이면서 $^{99m}Tc-DMSA$신주사가 정상인 대조군 49례와 비교하였다. 결과 : 1) 대조군에서 절대적 신섭취율은 우측 $21.8{\pm}3.9%$, 좌측 $22.2{\pm}3.9%$, 평균 $22.0{\pm}3.9%$, 전체 $44.2{\pm}7.8%$로 좌우측간에는 유의한 차이가 없었다. 연령에 따른 변화로 좌우측 평균과 전체는 0-6개월에 $18.3{\pm}4.3%,\;36.7{\pm}8.7%$, 7-12개월에 $20.8{\pm}3.2%,\;41.6{\pm}6.4%$, 13-24개월에 $22.0{\pm}1.3%,\;45.7{\pm}3.5%$, 25-36개월에 $22.7{\pm}0.7%,\;44.5{\pm}1.5%$, 36개월이상 $22.7{\pm}3.2%,\;43.1{\pm}6.6%$로서 영아기동안 연령에 따라 점차 증가하여 1세경에 성인과 비슷하였다. 2) 일측성 급성 신우신염시 $^{99m}Tc-DMSA$의 상대적 및 절대적 신섭취율을 보면 이환신의 경우 $41.2{\pm}9.7%,\;16.5{\pm}5.4%$로 대조군의 $50.0{\pm}2.6%,\;22.0{\pm}3.9%$(좌우 평균)에 비해 모두 유의하게 감소하였으며(p<0.01) 반대측의 정상신은 $58.8{\pm}9.7%,\;23.2{\pm}5.2%$로 대조군에 비해 유의한 차이가 없었다. 3) 양측성 급성 신우신염시 상대적 신섭취율은 우측 $49.9{\pm}2.9%$, 좌측 $50.1{\pm}2.9%$로 대조군의 우측 $49.4{\pm}2.6%$, 좌측 $50.2{\pm}2.5%$에 비해 유의한 차이가 없었으나(p>0.01) 절대적 신섭취율은 우측 $17.3{\pm}5.3%$, 좌측 $17.4{\pm}5.3%$, 전체 $34.7{\pm}10.3%$로 대조군의 우측 $21.8{\pm}3.9%$, 좌측 $22.2{\pm}3.9%$, 전체 $44.2{\pm}7.8%$보다 유의하게 감소되었다(p<0.01). 4) 양측성 미만성 결손을 보인 급성 신우신염시 상대적 신섭취율은 우측 $48.9{\pm}1.9%$, 좌측 $51.0{\pm}1.9%$로 대조군의 우측 $49.4{\pm}2.6%$, 좌측 $50.2{\pm}2.5%$에 비해 유의한 차이가 없었으나 절대적 신섭취율은 우측 $18.1{\pm}3.9%$, 좌측 $18.4{\pm}3.8%$, 전체 $36.8{\pm}7.6%$로 대조군의 우측 $21.8{\pm}3.9%$, 좌측 $22.2{\pm}3.9%$, 전체 $44.2{\pm}7.8%$에 비해 유의하게 감소되었다(p<0.01). 결론 : 양측성 부분 결손이나 미만성 결손을 보이는 급성 신우신염 환아의 $^{99m}Tc-DMSA$신주사 에서 절대적 신섭취율은 상대적 신섭취율에 비해 진단적 가치가 높았다.
Purpose: Dengue fever occurs in many popular tourist destinations and is increasingly imported by returning travelers in Korea. Since Korea is not an endemic country for dengue fever, pediatricians do not usually suspect dengue fever in febrile children even with typical presentation and exposure history. This study was performed to describe the international travel experiences and dengue fever in Korean children. Methods: Travel histories were collected based on questionnaires completed by all patients' guardians who visited the pediatric infectious diseases clinic at Samsung Medical Center from January 2008 to December 2008. For patients who were suspected of dengue fever, a serological test was performed. Results: Five hundred and seventeen children visited the pediatric infectious diseases clinic for the first time during this period. About 30% of patients who responded to the questionnaire (101/339) had experienced international travel within the last 2 years. Four patients were diagnosed with dengue fever by serological test. Conclusion: Increasing numbers of Korean children visit dengue endemic areas and they may return home with dengue fever. Dengue fever should be suspected in patients who have a travel history to endemic areas.
Purpose: We aimed to study the pattern of liver-injury in children with dengue fever (DF) and validate serum aminotransferase ${\geq}1,000IU/L$ as a marker of severe DF. Methods: Children admitted with DF were included. DF was defined by presence of clinical criteria and positive serological or antigen tests in absence of other etiology. DF severity was graded as dengue without or with warning signs and severe dengue. Liver-injury was defined as alanine aminotransferase (ALT) more than twice the upper limit of normal (boys, 30 IU/L; girls, 21 IU/L). Results: Of 372 children with DF, 144 (38.7%) had liver-injury. Risk of liver-injury and aminotransferase levels increased with DF severity (p<0.001). Recommended ALT and aspartate aminotransferase (AST) cut-off at ${\geq}1,000IU/L$ had sensitivity 4.8% (5/105), specificity 99.3% (265/267) for detection of severe DF. In children with ALT and AST <1,000 IU/L (n=365), the area under receiver operating curves for prediction for severe DF, were 0.651 (95% confidence interval [CI], 0.588-0.714; p<0.001) for ALT and 0.647 (95% CI, 0.582-0.712; p<0.001) for AST. Serum ALT at 376 IU/L and AST at 635 IU/L had sensitivity and specificity comparable to ${\geq}1,000IU/L$ for defining severe DF. Conclusion: Liver-injury is common in DF. The ALT and AST levels increase with DF severity. ALT and AST levels of ${\geq}1,000IU/L$ could be lowered to 376 IU/L and 635 IU/L respectively for defining severe DF.
결론적으로 출력 도플러 신초음파는 급성신우신염의 병변을 찾는데에 특이도나 민감도가 그리 높지 않았으나 하부요로감염과 급성신우신염을 감별하는데에 어느 정도 유용하였으며, 특히 DMSA 스캔을 같이 실시하는 경우에는 신손상을 찾는데 더 도움이 될 수 있었다. 소아들의 상부요로감염의 진단을 위하여 출력 도플러 신초음파가 DMSA 스캔을 대체하는 영상검사로서는 한계가 있어 향후 보다 많은 시술로 진단을 위한 검사의 질을 높이는 방법을 찾아보아야 할 것이다.
목적: 소아의 요로감염은 적절히 관리 되지 않는 경우에 여러 문제를 일으킬 수 있어 다양한 관리 지침이 계속 개발되고 있고, 근래에는 근거 중심의 지침들이 만들어져서 진료에 적용되고 있다. 하지만 소아과 의사들의 요로감염 관리 실태에 대한 조사는 매우 드문 실정이다. 이 연구에서는 소아과 의사들의 요로감염에 대한 인식을 알아보고, 적절하게 관리가 이루어지는지를 평가하고 개선점을 찾고자 하였다. 방법: 대구시 지역의 78명의 소아청소년과 전문의들을 대상으로 요로감염의 진단, 영상검사 및 치료와 예방을 위한 환자교육을 어떻게 하는지를 설문조사로 알아보았다. 결과: 대부분의 응답자들은 발열의 원인을 찾을 수 없는 경우에 요로감염을 감별하기 위하여 소변검사를 하였다. 환자가 소변을 가릴 수 없는 경우 상당수의 응답자가 무균채뇨백을 사용하여 채뇨를 하였으며, 소변배양 검사를 하지 않는 경우도 많았다. 가장 많이 시행하는 영상검사는 초음파였다. 항생제는 입원한 경우에만 주사로 투여하고 외래 통원 시는 경구투여를 하였다. 요로감염의 중요성, 재발과 예방 등에 대한 환자 교육을 선택적으로 하고 있었다. 방광 요관역류의 관리 지침을 가지고 있는 경우는 28.6%로서 많지 않았다. 결론: 요로감염을 진단하기 위한 시도는 적절하게 이루어지고 있었으나, 무균채뇨백을 사용하여 소변배양을 하는 경우가 많아 위양성진단율이 높아져서 과잉치료가 우려되었다. 또한 영상검사를 하지 않는 경우가 많아 동반된 요로 기형의 진단이 조기에 되지 않을 수 있다는 것도 우려되었다. 근거-중심의 요로감염 관리 지침에 따른 보다 적절한 관리를 하기 위한 노력이 필요하다.
목적 일 지역 소아청소년과 외래를 방문한 부모들의 자녀의 발열에 대한 발열공포의 수준 및 부모의 발열염려 정도와 대상자의 특성 간의 관련성을 파악하고자 함이다. 방법 소아청소년과 외래를 방문한 부모 151명을 대상으로 구조화된 자기기입식 설문지를 이용하여 자료를 수집하였다. 수집된 자료는 기술통계 및 카이제곱 검정을 통해 분석하였다. 결과 약 50%의 대상자들이 $37.8^{\circ}C$를 발열의 최저기준 체온으로 그리고 $38.9^{\circ}C$를 고열의 최저기준 체온으로 정의했다. 약 3/4의 대상자들이 발열의 해로운 효과로 경련과 뇌손상을 언급했으며, 발열에 대해 '매우 염려'를 하였고, 자녀의 발열 시 1시간 이내에 체온을 다시 측정하였고, 미온수 목욕을 제공했으며, 열성질환이 있는 경우 잠자는 아동을 해열제를 주기 위해 깨웠다. 부모의 자녀 발열에 대한 염려는 이전 열성 경련 경험 여부와 한 자녀 가정의 부모와 통계적으로 유의한 관련성이 있었다. 결론 소아청소년과 외래를 방문한 부모들에게도 발열공포가 광범위하게 존재하고 있었다. 부모들을 위한 발열 및 발열 관리 교육 프로그램의 개발과 평가가 요구된다. 의사와 간호사가 부모들의 발열관련 정보의 1차적 근원임을 고려할 때 건강관리제공자들은 부모들의 발열에 대한 비현실적인 공포를 경감시키는 데 중요한 역할을 하여야 할 것이다.
