뇌졸중 환자의 재발염려에 영향을 미치는 요인 (Factors related to Fear of Recurrence in Stroke Patients)

  • Purpose: Recently, the number of post-stroke survivors has increased. The physical deficits following stroke have been well studied, but there is little information on fear of recurrence in stroke patients. Objective: The purpose of this study was to investigate the health risk behaviors, health motivation and sense of control on fear of recurrence after a stroke. Method: Data were collected through a questionnaire that included items on general characteristics. Participants were recruited from the convalescent centers and outpatients clinics. Participants completed the mastery scale, health motivation scale, and a fear of stroke recurrence scale. The data were analyzed using hierarchial multiple regressions analysis with SPSS version 18.0. Results: Levels of fear of recurrence, health motivation and sense of control were moderate with means of 19.76(SD=5.15), 26.85(SD=5.10), and 16.69(SD=4.65), respectively. Health motivation and sense of control contributed to fear of recurrence. The variables explained 30.5% of variance in fear of recurrence. Conclusions: Results indicate that interventions for fear of recurrence management after stroke should take into account health motivation and sense of control.

Concept Analysis of Fear of Recurrence in Breast Cancer Survivors

  • Breast cancer incidence continues to increase, and survival rates are also increasing compared to the past. An increase in breast cancer survivors means an increase in the number of women who return to their life after treatment. These patients feared cancer recurrence, which makes it an important aspect to be studied among breast cancer survivors. Therefore, this study was aimed at analyzing the concept of breast cancer survivors' fear of recurrence. The procedure of concept analysis developed by Walker and Avant (2011) was used to clarify and describe the concept. Studies published from 2007 to 2017 were searched through domestic and foreign electronic databases. Finally, 15 studies were selected and included in analysis. Through concept analysis, the scope of use of the fear of recurrence among breast cancer survivors was confirmed, and concept analysis was performed to confirm the antecedent, consequences, attributes and empirical criteria. Based on the analysis, the attributes of the fear of recurrence among breast cancer survivors included 1) worry about health status, 2) ineffective coping, and 3) possibility of changes. The antecedents of the fear of recurrence among breast cancer survivors were 1) healthcare-related activity, 2) perception of cancer, and 3) perceived severity; the consequences were 1) increase in distress, 2) difficulty in role function, and 3) decrease in the quality of life. This study provides a clear definition of the fear of recurrence among breast cancer survivors, and the results can be applied to improve the understanding of breast cancer survivors who have finished treatment and to help them return to daily life.

허혈성 뇌졸중 환자의 뇌졸중 지식과 재발염려가 건강행위에 미치는 영향 (Impact of Stroke Knowledge, Fear of Recurrence on Health Behavior in Patients with Ischemic Stroke)

  • Purpose: The purpose of this study was to identify the following: knowledge of stroke, fear of recurrence and health behaviors among patients with ischemic stroke. Further, factors influencing health behavior will be described. Methods: Data were collected from 180 patients with ischemic stroke at a general hospital. The study instruments included items about general and health related characteristics, a Stroke Knowledge Scale, a Stroke Fear of Recurrence Scale, and a Health Behavior Scale. Hierarchical regression method was conducted to examine predictors of health behavior. Results: The mean age of the participants was $63.62{\pm}11.10years$, and 57.8% of the sample was men. The mean score for stroke knowledge (possible range=0~17) was $14.99{\pm}1.76$, the mean score for fear of recurrence (possible range=0~32) was $23.16{\pm}3.75$, and the mean score for health behavior (possible range=20~80) was $54.69{\pm}6.46$. Stroke knowledge and fear of recurrence were associated with health behavior in patients with ischemic stroke (F=9.98, p<.001, Adjusted $R^2=.43$). Conclusion: The results demonstrated that stroke knowledge and fear of recurrence impacts the health behavior among patients with ischemic stroke. Thus, nursing interventions which focused on fear of recurrence as well as enhancing stroke knowledge could help health behavior in patients with ischemic stroke.

Psychometric Characteristics of the Fear of Cancer Recurrence Inventory-Severity Subscale Among Korean Cancer Survivors

  • Objectives: Despite the importance of choosing and using a valid assessment tool for fear of cancer recurrence (FCR) for early detection and interventions, the validity of the FCR inventory has yet to be thoroughly investigated in Korea. This study explored the psychometric properties of the Fear of Cancer Recurrence Inventory-Severity (FCRI-S) subscale and assessed its applicability to cancer survivors in Korea. Methods: The survey involved 93 Korean individuals who had survived cancer. The reliability of the FCRI-S subscale was assessed using Cronbach's α and composite reliability (CR). Confirmatory factor analysis (CFA), along with tests for discriminant and convergent validity, was conducted to evaluate the construct validity of the FCRI-S subscale. Results: The FCRI-S subscale showed excellent internal consistency (Cronbach's α=0.88; CR=0.89). CFA showed a good factor structure for the FCRI-S subscale, and the correlations of the FCRI-S subscale with FCR-related measures (r=0.69 to 0.80) and other psychosocial measures (r=-0.23 to 0.37) confirmed both the convergent and discriminant validity of the FCRI-S subscale. Conclusions: This study confirmed the robust psychometric characteristics of the FCRI-S subscale among cancer survivors in Korea. The use of the FCRI-S subscale would be helpful for health professionals to rapidly screen FCR levels in clinical settings.

젊은 뇌졸중 환자의 우울감이 재발염려와 건강관련 삶의 질에 미치는 영향 (The Effects of Depression on Fear of Recurrence and Health-related Quality of Life in Young Adults with Stroke)

  • 본 연구는 젊은 뇌졸중 환자의 우울감이 재발염려와 건강관련 삶의 질에 미치는 영향을 파악하고자 실시한 서술적 조사연구로 2018년 7월 24일부터 2018년 11월 22일까지 C도 내 일개 상급종합병원과 J도 내 일개 대학교 병원 신경과 외래를 방문하여 젊은 뇌졸중 환자 240명을 대상으로 조사하였다. 수집된 자료는 SPSS 24.0과 SmartPLS 3.0를 이용하여 통계적인 유의성과 가설검증을 위한 분석을 실시하였다. 우울에 대한 탐색적 요인분석 결과, 죄책감, 정신적 우울, 신체적 우울의 3개의 요인으로 나누어져 추후 분석에 사용되었다. 가설 검증 결과, 우울감 중 죄책감과 정신적 우울이 재발염려에 영향을 미치는 것으로 나타났으며, 정신적 우울이 건강관련 삶의 질에 통계적으로 유의한 영향을 미치는 것으로 나타났다. 또한 재발염려가 건강관련 삶의 질에도 유의한 영향을 미치는 것으로 나타났다. 본 연구 결과를 통해 환자의 죄책감과 정신적 우울 정도를 경감시키기 위한 중재뿐만 아니라 우울감의 조기 사정 및 중재를 통하여 젊은 뇌졸중 환자의 뇌졸중 재발염려를 극복하는 것이 건강관련 삶의 질 향상에 크게 기여할 수 있을 것으로 사료된다.

암환자가 체험한 화학요범의 의미 (The Meaning of Chemotherapy in Cancer Patients)

  • This study purports to find out the meanings of chemotherapy among cancer patients. The subjects of this study were selected from those patients who have taken chemotherapy at least twice at a hospital affiliated with a university. The twelve subjects agreed to join the present study and had no problem in communication with others. The data were collected through observations and interviews by the researcher. The phenomenological analysis method proposed by Giorgi was adopted for analyzing the data. The experiences of the subjects to chemotherapy were classified into 24 atributes. These attributes were also categorized into four groups such as hope, pain, fear, and ordea according to their meanings. The subjects expressed hope through the attributes such as "the wish for a new life." "the wish for healing." "the plasure form improvement of cancer." "the wish for being discharged form a hospital." "the interest in dietectic treatment." and "the trust in medical staff." Pain was represented by such attributes as "the physical pain", "the suffering from intravenous injection." "the discomfort of hospital environment." and "the economic burden." As for the attributes represention fear, "fear of being hospitalized." "tehr obscurity of uncertain situations." "the fear of side effects." "the fear of recurrence of cancer," "the lack of knowledge of the disease." Finally, nine attributes werw frouped to ordeal "the will endeavouring to recover cancer" "the adaptation to the present situation." "the giving up of being healthy," "the regret of the past life." "the recognition of significant others." "the physical changes." "the emotional changes." "the social changes." "the recollection of illness experiences." The above findings indicate that chemotherapy means hope, pain, fear, and ordeal to those cancer patinests under treatment. Hence, the nursing interventions for those cancer patients need to be directed to maintaining hope, alleviating pain and fear, and overcoming ordeal. There are some suggestions in achieving these goals : (1) the nurses caring for cancer patients need to understand the meaning of chemotherapy experienced by those patients, (2) a nursing specialty of intravenous injection needs to be developed, (3) interventions for providing emotional support should be devised, (4) nursing care should also be available to those cancer patients being dischaged at home.

입원 치료중인 유방암 재발 환자의 삶의 경험 (Life Experience of Inpatients with Recurrent Breast Cancer)

  • Purpose: Understanding daily life experiences of patients admitted to hospital with recurrent breast cancer. Methods: The grounded theory method was used for this study. Results: Consistent comparative analysis was used throughout the study to obtain the results. Results showed that inpatients with recurrent breast cancer experience 'a co-existence of life suffering and fear of death'. The causal condition of this result was determined to be 'patient's response to cancer recurrence (acceptance/despair)', including contextual conditions such as, 'previous experience with cancer treatment', 'patient's current physical condition', and 'treatment methods for recurrent cancer'. Intervening conditions, such as 'a strong will to live', 'family support', 'moral support providers', and action/interaction strategies were found to provide patients with 'a strength to live'. Shown in these results, inpatients with recurrent breast cancer were seen to have a simultaneous 'hope for life and fear of death'. Conclusion: When providing nursing services to inpatients with recurrent breast cancer, people must recognize there is a notable difference between individual patients' contextual conditions and interactive strategies. Henceforth, proper cognitive nursing must be provided which encourages patients to maintain a strong will to overcome the many hardships of treatment as well as physical nursing, such as management of side effects caused by chemotherapy.

알코올 사용 장애 환자의 재발경험: Q 방법론적 접근 (Relapse Experience of Patients with Alcohol Use Disorder: Q Methodological Approach)

  • Purpose: The purpose of this study was to identify the subjective relapse experiences of patients with alcohol use disorder in one's life context. Methods: A Q methodology was used to analyze the subjectivity of relapse experiences among 55 participants with alcohol use disorder. Fifty-five Q-statement were derived from interviews and literature review. Q-statements were classified into normally distributed shapes using a 9-point scale. Data were analyzed using the QUANL program. Results: Four types of relapse experiences were identified: failure to self-regulation drinking cravings, fear of relapse and awareness of the need for treatment, drinking as a coping mechanism and defensive coping, and lack of motivation to change drinking behavior. Conclusion: The results of this study suggest that patients with alcohol use disorder need a differential approach based on four types of relapse experiences in the recovery process.

폐업 소상공인의 재무적자본·시장경쟁력·사회적자본 손실지각이 재기의도에 미치는 영향 : 실패두려움의 매개효과와 창업자기효능감의 조절효과를 중심으로 (The Effect of Perceived Loss of Financial·Market·Social Capital Based on Recurrence Intention of Failed Small Business : Focusing on the Mediating Effect of Fear of Failure and the Moderating of Entrepreneurial Self-Efficacy)

  • 본 연구는 소상공인이 사업운영 시 경험했던 손실지각이 실패에 대한 두려움을 통해 재기에 어떤 영향을 미치는지를 확인하고자 폐업을 경험한 소상공인 413명을 대상으로 설문조사를 했다. 분석 결과는 다음과 같다. 첫째, 재무적자본, 시장경쟁력, 사회적자본의 지각된 손실이 클수록 실패에 대한 두려움은 컸다. 둘째, 실패두려움이 클수록 재창업 의지는 적었으나, 취업의지에는 영향을 미치지 못하였다. 셋째, 재무적자본, 시장경쟁력, 사회적 자본에 대한 지각된 손실이 실패두려움을 키우고, 이는 재창업 의지에도 부정적 영향을 미쳤다. 그러나 취업의지에 대해서는 시장경쟁력에 대한 손실지각만이 실패두려움을 통해서 취업의지를 강화했다. 이는 시장경쟁력으로 인해 폐업했다고 생각하면 창업보다는 취업을 선택할 가능성이 높다는 것을 시사한다. 넷째, 창업자기효능감이 높은 사람은 낮은 사람보다 손실지각이 실패두려움에 미치는 영향이 적었다. 즉, 창업효능감이 높은 사람은 재창업을 할 가능성이 높다고 볼 수 있다. 한편 연구결과, 사회적자본·재무적자본·시장경쟁력 손실지각 순으로 실패두려움에 영향을 미쳤는데, 소상공인들의 경우 창업시 상권내 경쟁과 위험도에 대한 이해부족으로 실패하는 경향이 큼에도 이에 대한 지각이 가장 낮았다. 또한 지인이나 친척 등 사회적 네트워크를 통해서 자금이나 영업활성화에 대한 지원을 받으려 하기 때문에 사회적 자본손실에 대한 지각이 실패에 대한 두려움에 미치는 영향이 가장 컸다고 추측된다. 본 연구는 이상의 결과를 바탕으로 정책실무 제언을 통하여 창업준비 시경쟁우위를 확보하기 위한 충분한 시장조사가 필요하며, 정교한 사업계획의 수립을 통해 재무적 손실을 줄일 수 있는 방안을 제시하고 있다.

관상동맥 중재술후 추적 관상동맥조영술 실천에 대한 조사연구 (A survey on Patients' Compliance with Follow-up Coronary Angiogram after Coronary Intervention)

  • Coronary intervention is now a well established method for the treatment of coronary artery disease. Coronary restenosis is one of the major limitations after coronary intervention. So medical teams advise the patients to get the follow-up coronary angiogram in 6 months after coronary intervention to know if the coronary artery stenosis recurs or not. This study was done in order to know how many patients complied with the advice, and to identify the relative factors to the compliance with getting the follow-up coronary angiogram. The subjects were 101 patients (male: 58 female: 22, mean age: $61{\pm}15$), who received coronary interventions from Jan. 1st to Mar. 31st 1997, and their data were collected from them by questionnaires one year after intervention. The questionnaires consisted of family support scale, self efficacy scale and compliance with sick role behavior scale. The result may be summarized as follows. 1. The number of patients who complied with getting the follow-up coronary angiogram were 37 people(36.6%) and did not comply with it were 64 people(63.4%). All scores of family support(t=5.56, p<.0001), self efficacy (t=4.13, p<.0001) and compliance with sick role behavior(t=5.66, p<.0001) were significantly higher in the patients who got the follow-up coronary angiogram than in those who did not get it. But there was not any relative factor in demographic variables (p>.05). 2. The major motivations for getting follow-up coronary angiogram were recurrence of subjective symptom(40.5%), the advice of medical team(32.4%), and fear of recurrence (27.1%). The restenosis rate in patients who got the follow-up coronary angiogram was 37.8%. 3. The restenosis rate was higher in the patients who had subjective symptoms than in those who did not have any subjective symptom. So subjective symptom and restenosis rate showed a high positive correlation(r=39.9, p<.001). However, 27.2% of the patients who did not have any subjective symptom showed coronary restenosis. 4. The reasons why they did not get the follow-up coronary angiogram were economic burden(37.5%), improved symptom(34.4%), busy life schedule(10.9%), fear of invasive procedure(9.4%), negative reaction of family member(3.1%), no helper for patient(3.1%) and worry about medical team's mistake (1.6%). The relative fators on compliance with getting the follow-up coronary angiogram after coronary intervention were family support, self-efficacy and Compliance with sick role behavior. And the most important reason why the patients did not get the follow-up coronary angiogram after coronary intervention was an economic burden.
