Psychiatric Characteristics According to Tinnitus Severity (이명 심각도에 따른 정신의학적 특징)

  • Objectives The purpose of this study was to examine the psychiatric characteristics of outpatients with tinnitus. Methods A total of 88 subjects were included in this study. According to Tinnitus Handicap Inventory (THI) scores, the subjects were classified into two group ; a mild tinnitus symptoms (mild-tinnitus) group and a severe tinnitus symptoms (severe-tinnitus) group. A questionnaire was used for an assessment of demographic characteristics, and the THI, the Visual Analogue Scale (VAS) about tinnitus, the Beck Depression Inventory (BDI), and the Beck Anxiety Inventory (BAI) are applied for evaluation of other clinical psychiatric characteristics. Results Higher THI scores were positively correlated with tinnitus course, the number of accompanying symptoms, and the VAS. BDI total scores, BDI factors, and BAI total scores were significantly higher in the severe-tinnitus group than in the mild-tinnitus group. Also suicidal ideation, interpersonal problems, sleep problems, occupational impairment, and fatigue were significantly higher in the severe-tinnitus group than in the mild-tinnitus group. Conclusions Tinnitus is a common disorder of hearing which is associated frequently with psychiatric problems. This study suggests that psychiatric interventions should be taken into consideration in the treatment of patients suffering from tinnitus.

Vibration based damage detection in a scaled reinforced concrete building by FE model updating

  • The traditional destructive tests in damage detection require high cost, long consuming time, repairing of damaged members, etc. In addition to these, powerful equipments with advanced technology have motivated development of global vibration based damage detection methods. These methods base on observation of the changes in the structural dynamic properties and updating finite element models. The existence, location, severity and effect on the structural behavior of the damages can be identified by using these methods. The main idea in these methods is to minimize the differences between analytical and experimental natural frequencies. In this study, an application of damage detection using model updating method was presented on a one storey reinforced concrete (RC) building model. The model was designed to be 1/2 scale of a real building. The measurements on the model were performed by using ten uni-axial seismic accelerometers which were placed to the floor level. The presented damage identification procedure mainly consists of five steps: initial finite element modeling, testing of the undamaged model, finite element model calibration, testing of the damaged model, and damage detection with model updating. The elasticity modulus was selected as variable parameter for model calibration, while the inertia moment of section was selected for model updating. The first three modes were taken into consideration. The possible damaged members were estimated by considering the change ratio in the inertia moment. It was concluded that the finite element model calibration was required for structures to later evaluations such as damage, fatigue, etc. The presented model updating based procedure was very effective and useful for RC structures in the damage identification.

A Case Report on Papillary Thyroid Cancer for the Recurrence of Regional Cervical Lymph Nodes improved by Korean medicine (갑상선 유두암으로 수술 후 좌우 림프절 전이 환자의 장기간 복합 한의약 치료 증례보고)

  • Objective : This study assessed the effect of a combination of Korean medicine on a thyroid papillary cancer patient who was diagnosed with local lymph node metastasis after thyroidectomy and lymph node dissection but did not want surgery. Methods : Gami-Palmultang administration and moxibustion(large Bmoxa cautery) were performed for six years. Treatment outcomes were evaluated with Brief Fatigue Inventory (BFI), Numerical Rating Scale (NRS), Insomnia Severity Index (ISI), Functional Assessment of Cancer Therapy-General (FACT-G), blood test/ CT imaging results, and patient's statements. Results : After the treatment, all symptoms have been alleviated, the quality of life has increased, and it has been maintained without further metastasis of tumors for six years. Conclusion : Korean medicine treatment along with active observation can be an alternative to patients who do not want surgical treatment after recurrence of local lymph nodes in thyroid papillary cancer, and can have positive results in improving the quality of life.

A Case Report of Persistent Postural-Perceptual Dizziness Treated with Korean Medicine (한방 치료로 호전된 지속체위 - 지각어지럼에 대한 증례 1례)

  • Objectives: The purpose of this study was to report the effectiveness of Korean medicine in the treatment of persistent postural-perceptual dizziness. Methods: A patient with dizziness was treated with Banhabackchulchunma-tang and Gongjin-dan herbal medicines. The severity of symptoms was assessed with a daily visual analog scale (VAS) for dizziness and the frequency of subjective trembling feelings (over 30 minutes). Results: After the treatment, the severity of dizziness was reduced from VAS 5 to VAS 2, and fatigue was improved from VAS 5 to VAS 1. The frequency of subjective trembling feelings was reduced from once every 3-4 days to once every 10 days. Other symptoms, such as dyspepsia, sleep disturbance, and hot flashes, also showed improvement. Conclusion: Treatment with Korean medicine may be an effective option for the treatment of persistent postural-perceptual dizziness.

Binocular Vision Corrective Spectacle Lenses Reduce Visual Fatigue in 3-D Television Viewing (양안시 교정안경의 3차원 텔레비전 시청 중 발생한 안정피로 감소)

  • Purpose: Three-dimensional (3D) displays are very useful in many fields, but induce physical discomforts in some people. This study is to assess symptom type and severity of asthenopia with their habitual distance corrective spectacle (HDCS) and their binocular vision corrective spectacle lenses (BVCSL) in people who feel physical discomforts. Methods: 35 adult subjects (ages $32.2{\pm}4.4$ yrs) were pre-screened out of 98 individuals to have the highest symptom/asthenopia scores following 65 minutes of 3D television viewing with HDCS. These 35 individuals were then retested symptom/asthenopia scores during they watched 3D television for 65 minutes at a distance of 2.7 m with wearing BVCSL of horizontal, vertical or base down yoked prisms. A 4-point symptom-rating scale questionnaire (0=no symptom and 3=severe) was used to assess 11 symptoms (e.g., blur, diplopia, etc.) related to visual fatigue/visual discomfort. Distance and near lateral phoria were measured using Howell phoria card and vertical phoria were measured using Maddox rod. Symptoms induced by watching 3D TV were compared between wearing HDCS and BVCSL. Results: Asthenopia in watching 3D TV with wearing BVCS was significantly lower than wearing HDCS at 5, 25, 45, and 65 minutes (all p < 0.001, paired t-tests). In only refractive error power correction power group, all asthenopia was not significantly different between HDCS and BVCSL (all $p{\geq}0.05$, paired t-tests). In prism correction groups for binocular imbalance, symptoms of asthenopia, however, was significantly lower for when wearing BVCSL than when wearing HDCS (all p < 0.05). Conclusions: Correction of phoria/vergence-based binocular vision imbalance can reduce asthenopia during 3D television viewing. An individual with binocular vision imbalance need corrected/compensated glasses with appropriate prisms prior to prolonged viewing of 3D television displays to reduce asthenopia/visual fatigue.

Morningness-Eveningness Affects the Depressive Mood and Day Time Sleepiness of Obstructive Sleep Apnea Syndrome Patient (폐쇄성 수면무호흡증 환자에서 일주기 리듬 특성에 따른 주간 졸음과 우울감의 차이)

  • Objectives: Recent studies have reported a correlation between obstructive sleep apnea syndrome (OSA) and depression. In attempt to verify the suggestion that eveningness is related to depression, we examined the effect of morningness-eveningness on their depressive mood in patinets with OSA. Methods: The examination was based on the medical records and polysomnography reports of 211 OSA patients. Information was gathered from the patients who filled out the H$\ddot{o}$rne and Ostberg questionnaire (HOQ), profile of mood states-Korean version (K-POMS), and Epworth sleepiness scale (ESS). We compared mean values of K-POMS total, subscales of K-POMS, ESS, and OSA severity variables among the 3 morningness-eveningness groups (morningness, eveningness, and neither groups). Partial correlation analysis was performed between variables and ANCOVA was performed among the 3 groups after adjustment with age and weight. Results: There were significant negative correlations between HOQ and the followings : K-POMS total, POMS-T (tension-anxiety), POMS-D (depression-dejection), POMS-A (anger-hostility), POMS-F (fatigue-inertia), POMS-C (confusion-bewilderment), spontaneous arousal index, average O2 saturation. There were significant positive correlations between HOQ and the followings : POMS-V (vigor-activity), apnea-hypopnea index, respiratory arousal index, snore time. There were significant negative correlations between POMS-D and the followings : HOQ, POMS-V, stage 1 sleep (%), AHI, TAI (total arousal index), oxygen desaturation index, respiratory arousal index, neck circumference, average O2 desaturation, snore time (%). There were significant positive correlations between POMS-D and K-POMS total, POMS-T, POMS-A, POMS-F, POMS-C, sleep latency, stage 2 sleep (%), heart rate, spontaneous arousal index. There were significant differences in K-POMS total, POMS-T, POMS-D, POMS-F, POMS-C, spontaneous arousal index among the three HOQ groups in ANCOVA. Conclusion: The depressive correlates of OSA patients might be affected, not by excessive daytime sleepiness or OSA severity indexes, but by eveningness circadian characteristics. It would be important to take into account the morningness-eveningness tendency when we manage the depressive mood of OSA patients.

The Epidemiologic and Clinical Study of Pulmonary Tuberculosis - Among Students at One Women's University for Recent 10 Years - (일개 여자대학교 학생의 최근 10년간 폐결핵 동태 및 임상경과에 관한 고찰)

  • Background: The prevalence of pulmonary tuberculosis has decreased progressively after the control of the tuberculosis began as national control. But as diabetes, malignancy, immunodeficiency disease recently tend to be increased, the tuberculosis become to the important national health problem. So, this study was designed to observe the state and the change of the prevalence and the clinical status of pulmonary tuberculosis for recent 10 years at one women's university. Method: We retrospectively investigated the epidemiology and the clinical status of 612 patients who were registered at the Ewha Womans University Health Center by analyzing records from 1983 to 1992. Results: 1) The prevalence rate had been steadily decreased from 0.63% in 1983 to 0.11 % in 1992. The prevalence of freshman and the incidence rate according to the entrance year while in the university significantly decreased since 1989. 2) In classifying by registered source, 45.6% of students were detected by annual periodical health examination, 34.5% by entrance physical examination, 12.0% by hospital, 5.4% by health center clinic, 2.5% by reentrance physical examination, sequentially. 3) The students with past history of tuberculosis were 70(11.4%) and 61(10%) suffered from pulmonary tuberculosis. The patients with family history of tuberculosis were 142(23.3%). 4) There were 530(86.6%) with minimal disease, 79(12.9%) with moderate and only 3(0.5%) with far advanced, when classified by the severity of disease. 5) The initial symptoms were mild breathing difficulty in 30.1%, sweating in 14.9%, fatigue in 14.3%, febrile sense in 11.7%, hemoptysis in 8.2%, sequentially. 6) The duration of treatment was $10.6{\pm}3.6$ months in mild group, $14.9{\pm}5.2$ months in the moderate group(P<0.05). 7) The side reactions of the drug were GI trouble in 7.2%, hepatitis in 1.8%, skin rash in 0.8% and streptomycin side in used patients in 9.1%. Conclusion: The prevalence of pulmonary tuberculosis among the students in one women's university was significantly lower than that of university students and 20-24 year-old age group announced in tuberculosis survey on a national scale, and significantly decreased since 1989. The treatment effect was desirable in student's group managed by university health center.
