This study classified subjects aged 30 to 64 into normal group, impaired fasting glucose group, and diabetes mellitus patient group based on data from the 6th period of the National Health and Nutrition Examination Survey (2013-2015). Skipping breakfast, lack of physical activity, inadequate sleep time, inadequate weight, excessive drinking, and current smoking are classified as a low health risk group when three or less items are present, and a high health risk group when three or more items are included. By classification, each item included in the physical measurement and biochemical analysis factors and health risk behavior factors was comparatively analyzed. As a result, in the normal group, impaired fasting glucose group, and diabetes group, the average age was higher in the group with high health risk factors than the group with low risk factors, and the male ratio was higher. Body mass index, waist circumference, blood pressure, triglyceride showed a significantly higher result. In the normal group, the fasting blood glucose level and total cholesterol level were also higher in group 2. Therefore, it is thought that it is necessary to control health risk behaviors through lifestyle changes in the normal group, fasting glucose disorder group, and diabetes group.

Background: Antihyperglycemic effects of Panax ginseng berry have never been explored in humans. The aims of this study were to assess the efficacy and safety of a 12-wk treatment with ginseng berry extract in participants with a fasting glucose level between 100 mg/dL and 140 mg/dL. Methods: This study was a 12-wk, randomized, double-blind, placebo-controlled clinical trial. A total of 72 participants were randomly allocated to two groups of either ginseng berry extract or placebo, and 63 participants completed the study. The parameters related to glucose metabolism were assessed. Results: Although the present study failed to show significant antihyperglycemic effects of ginseng berry extract on the parameters related to blood glucose and lipid metabolism in the total study population, it demonstrated that ginseng berry extract could significantly decrease serum concentration of fasting glucose by 3.7% (p = 0.035), postprandial glucose at 60 min during 75 g oral glucose tolerance test by 10.7% (p = 0.006), and the area under the curve for glucose by 7.7% (p = 0.024) in those with fasting glucose level of 110 mg/dL or higher, while the placebo group did not exhibit a statistically significant decrease. Safety profiles were not different between the two groups. Conclusion: The present study suggests that ginseng berry extract has the potential to improve glucose metabolism in human, especially in those with fasting glucose level of 110 mg/dL or higher. For a more meaningful benefit, further research in people with higher blood glucose levels is required.
Diabetes mellitus is associated with a variety of adverse health outcomes, including metabolic syndrome and cardiovascular diseases. Recently, diabetes has been receiving increased scientific attention because of the rapid increase in the diabetic population. One of the features of type-2 diabetes mellitus is an elevated glucose level in blood. Fasting glucose level, which is the most basic test, is widely used as a diagnostic indicator of diabetes. Several previous studies reported that TLR4 expression is relatively high in the heart. However, few studies have investigated the association between TLR4 variants and fasting blood glucose to date. Thus, this study tested the association between single nucleotide polymorphisms (SNPs) in the TLR4 gene and fasting glucose in the Korean population. A total of 994 subjects recruited from Seoul were used for the present study. When compared to fasting blood glucose, the TLR4 gene region was shown as a linkage disequilibrium owing to the relatively large gene range. This region also presented as several LD blocks. We found that specific SNPs in the TLR4 gene were associated with the mean fasting glucose (p<0.01). The minor allele frequency of rs1329067 was 16.4%, and individuals with the AA genotype had a higher fasting blood glucose level than those with the GG genotype, suggesting that genetic variants in TLR4 influence glucose levels in Korean adults.

Purpose : The purpose of this study was to investigate the effects of physical activity on impaired fasting glucose in adults aged 20 years or older. Methods : This study utilized raw data from the 8th National Health and Nutrition Examination survey (2019~2021). The subjects of this study were 5,344 adults aged 20 years or older who were confirmed to be free of diabetes. The control variables in this study model are health behavior characteristics (subjective health status, smoking, drinking), anthropometric characteristics (body mass index), and personal background characteristics (gender, age, income level, education level, marital status). As for the analysis method, the degree of physical activity was made into a dummy variable, and a probit model was used. Results : As a result of this study, compared to quartile 1 of the relative grip strength value obtained by dividing the grip strength by the body mass index (body mass index, kg, m2), fasting blood glucose levels were significantly higher in quartile 2 (.05, p<.01), quartile 3 (.04, p<.01), and quartile 4 (.04, p<.01). It was found that the probability of belonging to the normal category was higher than that of impaired fasting glucose. In addition, in the group of adults aged 20 or older who had a lot of aerobic and anaerobic physical activity, fasting blood sugar was more likely to be in the normal category. Conclusion : Based on the results of this study, it was suggested that diabetes should be managed through physical activity in the pre-diabetic stage, as prevention is important as well as treatment. From a practical point of view, muscle strength, such as grip strength, can be identified as a reliable indicator for identifying impaired fasting glucose.
Purpose: The purpose of this study was to evaluate the effect of antioxidant vitamins and magnesium supplementation on fasting blood glucose and lipids in patients with type 2 diabetes. Methods: This study is a unequivalent control group pretest-posttest design. Seventyone subjects with type 2 diabetes who were recruited from home visiting clients of a public health center, completed the trial. The experimental group entered a 12-week treatment period with antioxidant vitamins and magnesium and the control group with no antioxidant vitamins and magnesium. Results: Serum level of fasting blood glucose decreased from $134.7mg/d{\ell}$ to $125.0mg/d{\ell}$ and total-cholesterol decreased from $215.5mg/d{\ell}$ to $198.2mg/d{\ell}$ in the experimental group. No changes in fasting blood glucose and total-cholesterol were demonstrated in the control group. Conclusions: A short-term supplementation with antioxidant vitamins and magnesium can reduce fasting blood glucose and total-cholesterol in patients with type 2 diabetes. The continuous effect of this supplementation and the beneficial effect on the prevention of diabetes complication still needs to be demonstrated.
Objectives : Diabetes has been reported as a risk factor for several cancers. However, the association between diabetes and gastric cancer has been inconsistent. The aim of this study was to evaluate the association between the fasting serum glucose level and gastric cancer risk in Korea. Methods : Among the members of the Korean Multi-Center Cancer Cohort (KMCC) from 1993 to 2004, a total of 100 incident gastric cancer cases were ascertained until December 31, 2002 and 400 controls were matched according to age, sex, and year and area of enrollment. Of the eligible subjects, those without fasting serum glucose level information were excluded, with a total of 64 cases and 236 controls finally selected. On enrollment, all subjects completed a baseline demographic and lifestyle characteristics questionnaire, and had their fasting serum glucose level measured. The Helicobacter pylori infection status was determined by an immunoblot assay using long-term stored serum. The odds ratios (ORs) were estimated using conditional and unconditional logistic regression models adjusted for the H. pylori infection status, smoking, drinking, education, follow-up period and matching variables. Results : The ORs for risk of gastric cancer according to the serum glucose level were 1.33 [95% CI=0.50-3.53] and 1.66 [95% CI=0.55-5.02] for the categories of 100-125 and 126 mg/dL or greater, respectively, compared to the category of less than 100 mg/dL. No increased risk of gastric cancer according to the serum glucose level was found (p-trend=0.337). Conclusions : This study provides no evidence for an association of the serum glucose level with gastric cancer.
In this study, body weight levels of glucose, insulin and triglyceride in blood and glucosidase activity of the small intestine were investigated to determine the effect of Sangbaegpitang and Supungsungiwhan on the glucose metabolism of db/db mice. The GLUT4 mRNA of muscle tissue and the Acetyl CoA Carboxylase and the activation rate of GLUT2 mRNA of liver tissue were measured by the reverse transcription-polymerase chain reaction method and by the vitro transcription. The results were obtained as follows: 1. In the Sangbaegpitang administration group, (1) The level of triglyceride was decreased significantly and the glucosidase activity of the small intestine was inhibited remarkably, (2) The amounts of the GLUT4 mRNA in muscle tissue and Acetyl CoA Carboxylase mRNA in liver tissue were increased significantly. (3) Though glucose level in both fasting and non-fasting, were decreased and the insulin level in blood was increased, the results showed no statistical significance. 2. In the Supungsungiwhan administration group, (1) The levels of glucose and triglyceride were decreased significantly in the blood of non-fasting animals. (2) The glucosidase activity of small intestine was inhibited markedly and the amounts of GLUT4 mRNA of muscle tissue and GLUT2 mRNA of liver tissue were increased significantly. (3) The glucose levels in the fasting group were reduced, while insulin level was increased but showed no statistical significance, Based on the above results, our conclusions are as follows: Sangbaegpitang & Supungsungiwhan are thought to be capable of inhibiting the activity glucosidase, the enzyme which influences carbohydrate metabolism in the small intestine of db/db mice(the experimental diabetic model) and delaying the absorption of carbohydrate, thus proving effective on inhibiting the increase of non-fasting glucose level effectively. Futhermore Sangbaegpitang and Supungsungiwhan are though: to be capable of preventing the composition of free fatty acids by restoring the production of GLUT4 mRNA of muscle tissues and GLUT2 mRNA of liver tissues. Those results suggests that above prescriptions can be applied to non-insulin dependent diabetes mellitus in order to improve insulin resistance.
High insulin level is known to be a risk factor of coronary heart disease. High insulin level with normal glycemic control is known to be an indicator for insulin resistance. This study was aimed to find out the influencing factors for the fasting serum insulin levels in elderly females with normal glycemic control. One hundred thirty-eight older females aged over 60 years without diabetes medication and high blood HBA1c level were examined on the serum fasting insulin concentration, anthropometry and asked about nutrient intake and exercise habits. The elderly were categorized into 3 group according to the fasting insulin level. The high insulin group was in a state of hyperinsulinemia. Except vitamin C, the nutrient intakes showed no difference according to fasting serum insulin level. But the intakes of calorie and protein per kg body weight were significantly lower in the high insulin group. The intakes of most nutrients except vitamin B$_1$, C and niacin were lower than the korean RDA in all the insulin group. Especially, the intakes of vitamin B$_1$ and Ca were below 75% of the korean RDA. Weight, BMI, body fat percent, body fat mass and fat-free mass, circumferences of waist and hip, WHR of elderly females were significantly higher in the group with the highest insulin level. The body fat percent in the highest insulin group was 35.8%. showing a state of obesity. The high insulin group showed higher proportion of low exercise frequencies per week and short exercise duration. Therefore, the mean energy expenditure for exercise were lower in this group, showing a state of very low exercise activity. Age and waist circumference in elderly females could explain to the 14.5% variances of the fasting insulin level according to multiple stepwise regression. It can be concluded that aging and central body fat deposition influence independently the serum fasting insulin level in elderly females with normal glucose level.
This study was carried out to determine whether a short-tenn zinc supplementation contributes to beneficial changes in glycemic control among type 2 diabetic patients. Seventy-six diabetic subjects and 72 normal adults participated in this study. Subjects were divided into supplemented and control groups. Forty-four diabetic patients and 34 normal subjects were supplemented with 50 mg zinc daily as zinc gluconate for 4 weeks. Zinc status was assessed from fasting plasma levels and urinary excretion. The effects of zinc supplementation on fasting blood glucose, $HbA_{1c}$, insulin, and C-peptide were measured at the beginning of the study and after 4 weeks of supplementation. The changes in glycemic control indicators were compared between diabetic groups, classified by baseline $HbA_{1c}$ levels, and by diabetic duration. At baseline, the incidence of marginal zinc deficiency in the diabetic group, as determined by plasma zinc level, was approximately twice as high as in the normal adult group. The changes of $HbA_{1c}$ concentration, and fasting blood glucose following supplementation were not statistically significant in diabetic subjects. In normal subjects, a significant decrease of $HbA_{1c}$ occurred only in the zinc supplemented group. No significant changes were observed for serum insulin and C-peptide in diabetic as well as normal subjects. However, when the changes were compared by baseline $HbA_{1c}$ level, we found that diabetic subjects with $HbA_{1c}\;{\geq}\;7.5%$ showed significantly improved levels of $HbA_{1c}$ and fasting glucose after Zn supplementation. While such improvement in fasting blood glucose was significant among diabetics with shorter diabetic duration, significant levels of increase in serum insulin and C-peptide were observed in zinc supplemented subjects with longer diabetic duration. Fasting blood glucose was significantly decreased, whereas serum insulin and C-peptide were increased in diabetics with marginal zinc status. Therefore, we suggest that Zn supplementation for a short-term period may improve glycemic control in diabetic patients with higher $HbA_{1c}$ levels and marginal zinc status.
