Objectives : This study was performed to evaluate the relationship between C-reactive protein(CRP) and carotid intima-media thickness(carotid IMT) in a population of middle-aged Koreans. Methods : A total of 1,054 men and 1,595 women(aged 40-70 years) from Kanghwa County, Korea, were chosen for the present study between 2006 and 2007. We measured high-sensitivity CRP and other major cardiovascular risk factors including anthropometrics, blood pressure, blood chemistry, and carotid ultrasonography. Health related questionnaires were also completed by each study participant. Carotid IMT value was determined by the maximal IMT at each common carotid artery. The relationship between CRP level and carotid IMT was assessed using multiple linear and logistic regression models after adjustment for age, body mass index, menopause(women), systolic blood pressure, total/HDL cholesterol ratio, triglyceride level, fasting glucose, smoking, and alcohol consumption. Results : Mean carotid IMT values from the lowest to highest quartile of CRP were 0.828, 0.873, 0.898, and 0.926 mm for women(p for trend<0.001), and 0.929, 0.938, 0.949, and 0.979 mm for men(p for trend=0.032), respectively. After adjustment for major cardiovascular risk factors, the relationship between CRP and carotid IMT was significant in women(p for trend=0.017), but not in men(p for trend=0.798). Similarly, adjusted odds ratio of increased IMT, defined as the sex-specific top quartile, for the highest versus lowest CRP quartiles was 1.55(95% CI=1.06-2.26) in women, but only 1.05(95% CI=0.69-1.62) in men. Conclusions : CRP and carotid IMT levels appear to be directly related in women, but not in men.

Parainfluenza virus infection is one of the causes of fatal rhabdomyolysis. Rhabdomyolysis can be aggravated by mitochondrial fatty acid ${\beta}$-oxidation disorders during prolonged periods of fasting. Moreover, in patients with late-onset isovaleric acidemia, hyperammonemia may occur following catabolic stress. In the present report, we describe a case of a 4-year-old boy with parainfluenza virus infection and late-onset isovaleric acidemia that rapidly progressed to coma, seizures, and cardiorespiratory collapse. His serum ammonia and creatinine kinase (CK) levels were $385{\mu}Mol/L$ and 23,707 IU/L, respectively. Continuous renal replacement therapy (CRRT) was initiated using continuous venovenous hemodiafiltration, after which the ammonia and CK levels returned to normal. Thus, we recommend the immediate initiation of CRRT in the management of patients with life-threatening rhabdomyolysis and hyperammonemia.

The purpose of this study was to investigate the use of fad dieting as a method to lose weight among male and female college students. A total of 710 students participated in this study. Fad dieting was surveyed using a questionnaire about 11 fad diet methods. The mean age of the subjects was 21.4 years for males and 20.4 years for females. The proportion of students who had experience trying to lose weight was 52.8% for males and 78.6% for females (p<0.001). The diet method employed by the highest number of participants was the half meal diet (27.6% for males, 43.9% for females), followed by the cereal diet (8.8% for males, 43.9% for females), fasting (9.9% for males, 21.4% for females), and the chicken breast diet (13.1% for males, 9.2% for females). The female students were more likely than the males to try fad diets other than the chicken breast diet and the Atkins diet. The most frequent side effect of fad dieting was the yo-yo effect, followed by binge eating, weakness, anemia, and so on. The proportion of students who indicated that nutrition education is needed for healthy dieting was 39.1% for males and 45.4% for females. From these results, we concluded that a high percentage of college students attempt to lose weight through fad diets, with some gender differences. Therefore, in the future, nutrition education programs should focus more attention on providing healthy dieting methods to young college students to help them lose weight.
The purpose of this study was to assess the energy value of breakfast and its relation to total daily nutrient intake and serum lipid. Dietary intakes were evaluated through the 3-day dietary recalls(interview for 1day and self-report for 2 days) from 333 Korean urban adults aged 20-49 year. Serum lipids in fasting blood samples were measured form 98 of those 333 adults. Low energy breakfast(<15% of daily energy intake) was consumed by 22.6% of males and 18.5% of females. 59.4 of males and 43.3% of females consumed a breakfast of average energy intake(15-25% daily energy intake) ; and 18.1 of males and 38.2% of females consumed a significant contribution to a total daily nutrient intake. The daily nutrient intake except crude fiber and vitamin A, B, B, and C in males and except vitamin C in females significantly increased as the energy value of breakfast increased. The differences in energy and nutrient intakes at breakfast were not made up for by other meals. To make matters worse, the satisfactory-energy breakfast group took more energy at dinner compared with the low-energy and average-energy breakfast group took more energy at dinner compared with the low-energy and average-energy breakfast groups in females. Among serum lipid parameters, TG was negatively correlated with energy provided at breakfast, and total cholesterol and LDL-cholesterol had a negative correlation with energy and carbohydrates provided at breakfast in males. For females TG was positively correlated with the ratio of carbohydrates to energy at breakfast but negatively correlated with the ratio of fat to energy at breakfast. These results suggest that satisfactory energy intakes at breakfast have positive effects on the adequacy of daily nutrient intake and may positively affect the serum lipid status.
Pregnant women in South Korea are a highly risk group fur iron deficiency anemia. Previous studies indicated that the 24-hour recall method was insensitive in distinguishing iron deficiency anemic women from normal women. This method is also impractical to when used at community health centers where no public health dietitians are employed. The objective of this study was to develop a convenient tool to evaluate the usual iron (Fe) intake of pregnant women. The study participants were 115 pregnant women (age 23 to 37 years) at gestational stage of 13 to 24 weeks. Anemic subjects were classified on the basis of their serum ferritin < 12.0 ${\mu}$g/L and hemoglobin < 12.0 g/dL levels. Food frequency questionnaires with 46, 29, and 15 commonly consumed food items were used to measure the usual intake of iron of the subjects. Hemoglobin and serum ferritin were measured from fasting blood samples. Nutrients intake was assessed on three consecutive days using the 24-hour recall method and the food record method. The iron index score calculated using the food frequency method showed a significantly positive correlation with iron intake for the three days dietary intake. The iron index showed a significantly difference (p < 0.05) between the normal and anemic groups. However, there was no significant difference in the iron intake between the anemic and the normal women as measured by the 24-hour recall and food record method. Our study indicated that the 29-food items questionnaire could be used as a screening tool to identify poor dietary intake of iron. (Korean J Community Nutrition 8(2) : 160170, 2003)
The nutritional status of middle-aged overweight and control normal subjects were evaluated for the purpose of providing the background information of the degenerative disease control. A survey was conducted with 293 healthy subjects (121 males and 172 females) between the ages 40-64. The average BMI (body mass index) of male subjects was 24.5 $\pm2.75$, and that of females was 23.5 $\pm2.87$. The average values of WHR (waist-hip ratio) were 0.88 $\pm0.04$, for males and 0.82 $\pm0.07$, for females. The normal BMI group (BMI 18.5-22.9) comprised 28.9% of males and 47.1% of females. The percentage of overweight subjects (BMI 23-24.9) was 26.4% of males and 30.8% of females. The obese group (BMI > 25) was 44.6% of males ana 22.1% of females, showing the greater rate of obese state among male subjects. The average energy intakes were 76.6 $\pm14.9$% for males and 77.8 $\pm12.6$, % for females, protein intakes were 108.0 $\pm24.6$% for males and 111.2 $\pm22.7$, % for females of the RDA levels. The average intakes of other nutrients were above the 75% of RDA levels except calcium. The average nutrient intakes of the three subgroups according to their BMI values were not different for both males and females. There were weak correlations between obesity and blood biochemical indices. There were positive correlations between BMI or WHR and hemoglobin, hematocrit, fasting glucose, total cholesterol, triglyceride, AST or ALT. There were negative correlations between BMI or WHR and HDL-cholesterol. These results suggest that the obesity rate of middle-aged is an influential factor of chronic disease. The middle-aged subjects of this study with higher educational and socioeconomic background tend to have desirable nutrition knowledge and attitude, but the application of their knowledge or attitude was relatively poor.
This study was performed to determine the prevalence and associated risk factors of osteoarthritis in the elderly using the Korea National Health and Nutrition Examination Survey (2013~2014 KNHANES VI). A total of 2,148 (936 males, 1212 females) elderly participants aged 65 years or older were selected. Prevalence of osteoarthritis was higher in 460 female (80.7%) and 167 rural (29.1%) groups, than in 110 male (19.3%) and 404 urban (70.9%) groups. While the mean values of LDL-cholesterol, triglyceride level, and waist circumference were significantly higher in the osteoarthritis group than the normal group, other parameters including height, HDL-cholesterol, fasting blood glucose, hemoglobin and hematocrit values were not. Quality of life (EQ-5D) was also significantly lower in the osteoarthritis group. The results of logistic regression analysis showed that smoking, alcohol intake, obesity, hypercholesterolemia and energy intakes were significantly different in regards to the prevalence of osteoarthritis. This study suggests that obesity and energy intakes were associated with osteoarthritis, whereas risk factors of chronic disease, such as hypertension, Hypo-high density lipoprotein, hypertrigly-ceridemia, anemia, diabetes, vigorous physical activity, moderate physical activity and walking were not. Long-term prospective study is necessary to investigate the effects of these factors on osteoarthritis.
The purpose of this study was to compare nutrient intake, health-related factors, and risk factors for metabolic syndrome and to investigate the prevalence odds ratio according to fruit and vegetable intake among middle-aged Korean men. Subjects included 1,677 men aged 40 - 64 years. The average intake of fruits and vegetables was $578.48{\pm}5.90g/day$. Forty-nine percent of the subjects were categorized into the deficient group based on the intake of fruits and vegetables, and the other subjects were categorized into the adequate group. There was a significant difference in the nutrient density per 1,000 kcal of energy, minerals, vitamins, and dietary fiber between the fruit and vegetable intake groups (p<0.001). In terms of health-related factors, only the current smoking rate in the deficient group (57.9%) was higher than that in the adequate group (45.4%) (p<0.001). There was no significant difference in the risk factors for metabolic syndrome between the fruit and vegetable intake groups. Proportion of serum triglycerides (42.6%) was the highest at a rate that exceeded the criteria for risk factors for metabolic syndrome, but systolic blood pressure (21.7%) showed a very low rate. The fasting blood glucose, serum triglycerides, and diastolic blood pressure were significantly higher in the deficient group than in the adequate group (p<0.05). Prevalence of metabolic syndrome was 31.2%. OR for prevalence of metabolic syndrome related to fruit and vegetable intake was 1.270 times higher in the deficient group than in the adequate group (p<0.05). Also, when the daily fruit intake was increased by 100 g, the prevalence of metabolic syndrome was reduced by 0.948 times but it was not related to the vegetable intake. The results of this study can be used as the basic data to establish the guidelines for fruit and vegetable intake for prevention of metabolic syndrome in middle-aged men.
In the atherosclerotic subjects, arterial endothelial cell injury and plaque formation are suspected to be strong causable factors in developing acute coronary syndrome, and it was revealed that platelets have a very important role in this case. Women are exposed to atherosclerosis at a different degree after menopause or oral contraception. The purpose of this study was to determine the effects of endogenous and exogenous estrogen on the degree of platelet aggregation in platelet rich plasma(PRP) in twenty nonsmoking healthy Korean women for 12 weeks. The subjects were assigned to three groups: (1) eight women aged 49 to 60(yr) for the postmenopausel(Pst) group, (2) eight, aged 22 to 30(yr) for the premenopausa(Pre) group, (3) four, aged 23 to 30(yr) for the oral contraceptive (OC) group which used triphasic OC formulation. Fasting blood sample were obtained from the subjects, (1) once per 6 weeks in the Pst group, (2) every phase of the menstrual cycle in the Pre group, (3) each once during and after OC administration in the OC group. ADP, collagen and epinephrine were used as aggregating reagents, and platelet aggregation and time(Δt: time reaching to the maximum point of aggregation) in PRP were measured at the maximum point of aggregation in five minutes. All the data were adjusted for dietary effects, personality type and body mass index(BMI) by using analysis of covariation(ANCOVA). Platelet aggregation to ADP and collagen(MADP and MCOLL) were not significantly different among the three groups, and Δt to ADP and collagen(TADP and TCOLL) were not either. But maximum platelet aggregability and Δt to epinephrine(MEPIN and TEPIN) were significantly different among the three groups, and the OC group showed the lowest value (p<0.01). Maxtimum platelet aggregability and Δt during the menstrual cycle were not significantly different in the Pre group. Any other significant differences in the maximum platelet aggregability and Δt were found between oral contraception phase and washing out phase(menstruation) in the OC group. In results, maximum platelet aggregability and aggregation time to ADP and collagen seemed not to be affected by endogenous and exogenous estrogen, even though MEPIN and TEPIN showed significantly low in the OC group among the three groups.
This study was intended to examine whether dehydroepiandrosterone (DHEA) and dietary fat level or source could modulate glutathione utilizing detoxifying system activity and the cytosolic NADPH generation in rat liver. Male Sprague-Dawley rats were fed semipurifed diet containing either 2%(w/w) corn oil (low level of corn oil diet: 5 ca% of fat) 15% corn oil (high level of corn oil diet: 31 cal% of fat) or 13% sardine oil plus 2% corn oil(high level of fish oil diet: 31 cal% of fat) for 9 weeks. Half of the rats in each diet group were fed a diet supplemented with 0.2% DHEA (w/w). DHEA administration increased plasma total cholesterol level in low corn oil diet-fed rats. The high fish oil diet significantly decreased plasma total cholesterol level compared to the high corn oil diet. Plasma triglyceride level was not significantly changed by DHEA administration and dietary fat level and source. Fasting plasma glucose level was increased by DHEA administration and fish oil diet. Glucose 6-phosphate dehydrogenase activity in liver tissue was significantly increased by DHEA administration and high fat diet, especially fish oil diet. Malic enzyme activity in liver tissue was significantly increased by DHEA administration and high fat diet, especially fish oil diet. Malic enzyme activity in liver tissue was significantly increased by DHEA administration. DHEA suppressed the glutathione peroxidase, glutathione-dependent enzymes compared to the low corn oil diet, while fish oil diet elevated the activity of glutathione peroxidase and glutathione reductase compared to corn oil diet. These results suggest that DHEA administration and high level of corn oil diet may suppress the cellular detoxifying system activity through reduction of glutathione utilization, while the fish oil diet did not show these effects.
