  • We present a fatal case of cerebral arterial thrombosis after corona virus disease 19 (COVID-19) vaccination with ChAdOx1 nCOV-19. The deceased was a 63-year-old woman with no relevant medical history. She presented symptoms of nausea, fatigue, and headache immediately after vaccination. Ten days after vaccination, she suddenly started vomiting and developed high blood pressure. The patient eventually died 23 days after vaccination. Autopsy findings showed that the cerebral arteries and internal carotid arteries were fully enlarged and were compacted with thrombi. The brain stem showed ischemic necrosis, and extravasation from this necrotic lesion led to focal subarachnoid hemorrhage around the brain stem where large blood clots still remained. No aneurysms or atherosclerotic changes were found in these arteries. We note the following three facts. Firstly, all symptoms occurred immediately after vaccination; secondly, the main cause of death was consistent with known side effects of the vaccine; and lastly, the mechanism of thrombus formation in this case goes beyond the general category of thrombogenesis known so far. While the authors know that this case does not fall into known categories of vaccine side effects, we presenting this case to demonstrate that a comprehensive review of various possibilities related to vaccine side effects is needed to establish a COVID-19 defense system.

  • The main purpose of this study is to investigate the effects of dietary lysine deficiency on protein turnover of porcine muscles. There were 18 LYD three-breed-crossing postweanling barrows from six litters cannulated with gastric tubes through the esophagus at approximate 10 kg of body weight and allocated into three treatment groups. When their body weights reached over 12 kg, one group was sacrificed for determining the initial protein masses of m. masseter, m. longissimus dorsi, m. adductor and m. biceps femoris from the right body side. The others received a diet containing 100% or 61.4% (calculated values) of the lysine requirement (NRC, 1998) multiplied by 1.103 for a period of 17 days. Daily feed provision was computed for each pig according to body weight at the same day. All pigs were infused a flooding dose of $^2$H$_5$-phenylalanine to determine the fractional protein synthesis rates (FSR) of the aforementioned muscles in the end. Their four muscles from the right body side were also dissected for measuring the fractional rates of protein accretion (FAR). As for protein degradation, fractional rates (FDR) were calculated by differences between synthesis and accretion. Results showed that the lysine deficiency resulted in, significantly (p<0.05), lighter body weights, smaller muscles and a slower growth rate. The protein mass, accreted by the muscles, of the deficient group was only 54% averaged of the pigs fed adequately (p<0.05). The FAR of these muscles in the deficient group was significantly lower (p<0.05) and only achieved 61.1% averaged of the control; there was no significant difference (p>0.05), nevertheless, in the amino-acid composition of muscles between two groups. The lysine deficiency reduced significantly (p<0.05) the FSR of m. longissimus dorsi but did not influence its FDR. The m. biceps femoris also presented an inhibited FSR while its FDR reduced only exhibited a very high tendency (p = 0.055) compared to the adequately-fed pigs. As for the m. masseter and m. adductor, both of the FSR and FDR were depressed significantly (p<0.05) by the lysine deficiency, and changes in the FSR were severer than those in the FDR, so that their FAR were significantly slower (p<0.05) in comparison with the control group. The lysine deficiency also inhibited the RNA translation activity of the muscles while the effects on RNA capacity were not significant (p>0.05). In conclusion, the FAR of muscle protein was changed by the current lysine deficiency through the alterations in the FSR and/or FDR.

  • For ten firn cores, from both the eastern and the western side of Lambert Glacier basin (LGB), snow accumulation rate and isotopic temperature were measured far the recent 50 years. Results show that snow accumulation for five cores over the eastern side of LGB (GC30, GD03, GD15, DT001, and DT085) at Wilks Land and Princess Elizabeth Land increases, whereas it decreases at the western side (Core E, DML05, W200, LGB 16, and MGA) at Dronning Maud Land, Mizuho Plateau and Kamp Land. For the past decades, the increasing rate was $0.34-2.36kg\;m^{-2}a^{-1}$ at the eastern side and the decreasing rate was $-0.01\;-\;-2.36kg\;m^{-2}\;a^{-1}$ at the western side. Temperatures at the eastern LGB were also increased with the rate of $0.02%o\;a^{-l}$. At the western LGB it was difficult to see clear trends, which were confirmed by Instrumental temperature records at coastal stations. Although statistic analysis and modeling results display that both surface temperature and accumulation rate has increased trends in Antarctic ice sheet during 1950-2000, the regional distributions were much more different for different geographic areas. We believe that ice-core records at Wilks Land and Princess Elizabeth Land reflect the real variations of SST and moisture change in the southern India Ocean. For the Kamp Land and Dronning Maud Land, however circulation pattern was different, by which the climate was more complicated. The International Trans-Antarctic Scientific Expedition (ITASE) aimed to reveal an overall spatial pattern of climate change over Antarctic ice sheet for the past 200 years. This study points the importance of continental to regional circulation to annual-decadal scale climate change in Antarctica.

  • Background Free tissue transfer is considered the gold standard option for the reconstruction of distal leg defects. Free tissue transfer using recipient vessels in the contralateral leg (cross-leg bridge) is a potential option to supply the flap if there are no suitable recipient vessels in the injured leg. Most studies have described this technique using end-to-end anastomosis which sacrifices the main vessel in the uninjured leg. This study evaluated the use of a cross-leg free latissimus dorsi muscle flap for the reconstruction of defects in single-vessel legs, using end-to-side anastomosis to recipient vessels in the contralateral leg without sacrificing any vessel in the uninjured leg. Methods This is a retrospective study that included 22 consecutive patients with soft tissue defects over the lower leg. All the reconstructed legs had a single artery as documented by CT angiography. All patients underwent cross-leg free latissimus dorsi muscle flap using end-to-side anastomosis to the posterior tibial vessels of the contralateral leg. Results The age at surgery ranged from 12 to 31 years and the mean defect size was 86 cm2. Complete flap survival occurred in 20 cases (91%). One patient had total flap ischemia. Another patient had distal flap ischemia. Conclusion Cross-leg free latissimus dorsi muscle flap is a reliable and safe technique for the reconstruction and salvage of mutilating leg injuries, especially in cases of leg injuries with a single artery. As far as preservation of the donor limb circulation is concerned, end-to-side anastomosis is a reasonable option as it maintains the continuity of the donor leg vessels.

  • Currently, the only established criteria is on the location of bus stops on principal roads where uninterrupted flow mainly occurs. There are no clear guidelines on any method to locating bus stops considering the characteristics of bus operation and pedestrians. If the location or exterior of a bus stop is inappropriate, road users including bus drivers and pedestrians will be caused serious dangerous and inconvenience. In this study, the research below was performed in order to propose rational criteria for the location of bus stops integrated with or separated from speed-change lanes at signalized intersections considering smooth traffic flow and the characteristics of bus operation and pedestrians as well as traffic safety : First, the appropriate length of each of the near-side and far-side bus stops was calculated by categorizing bus stops to be constructed into those integrated with speed-change lanes and those separated from speed-change lanes. Secondly, the appropriate length of each of the bus stops divided into near-side bus stops and far-side bus stops and integrated with or separated from speed-change lanes was selected by considering the characteristics of pedestrians. Thirdly, whether the construction locations of bus stops were appropriate or not was determined based on the appropriate length of bus stops integrated with or separated from speed-change lanes, which was calculated and selected by considering traffic flow and the characteristics of pedestrians and considering traffic safety. The method for locating bus stops considering traffic flow, the characteristics of pedestrians, and traffic safety will be able to help suggestion criteria of bus stop and the location of safe and pleasant bus stops.

  • The wind pressure distributions were analyzed to provide fundamental criteria for the structural design on the two-span arched house according to the wind directions through the wind tunnel experiment. In order to investigate the wind force distributions, the variation of the wind force coefficients, the mean wind force coefficients, the drag force coefficients and the lift force coefficients were estimated using the experimental data. The results obtained are as follows : 1. The variation of the wind force with wind directions on the side walls was the greatest at the upwind edge of the walls. 2. The maximum negative wind force along the length of the roof appeared at the upwind edge at the wind direction of 60$^{\circ}$. 3. The maximum negative wind force along the width of the roof appeared at the width ratio and wind direction of 0$^{\circ}$ and 0.4 in the first house and 0.6 and 30$^{\circ}$ in the second house, respectively. 4. The mean negative wind force on the side walls of the first house at the wind direction of 0$^{\circ}$ was far greater than that of the second house, and the maximum negative wind force on the roof occurred at the wind direction of 30$^{\circ}$. 5. The maximum lift force appeared on the second house at the wind direction of 30$^{\circ}$, but the lift force on the first house was far greater than that on the second house at the wind direction of 0$^{\circ}$. 6. The parts to be considered for the local wind forces were the edges of the walls, and the edges of the x-direction and the width ratio, 0.4 of the y-direction in the roofs.

  • We report the results of our analysis of far ultraviolet (FUV) observations made for the broad region around the ${\alpha}$ Vir (Spica) including the interaction zone of the Loop I and the Local Bubble. We employed the datasets of the GALEX and the FIMS, which made observations at similar FUV wavelengths. First, we noted that the GALEX image was enhanced in the southern region where the interaction zone exists. We attribute this enhanced FUV emission to dust scattering of the stellar photons, mostly from the background field stars with small contributions from the central star Spica. While the region is optically thin in general, the FUV intensity did not correlate well with the dust extinction level, indicating that the local radiation field has significant fluctuations. On the other hand, the GALEX FUV intensity well with the $H{\alpha}$ intensity as well as the dust extinction level in the northern part. In fact, the neutral hydrogen column density correlated very well with the dust extinction level throughout the whole region in consideration. The relationship between the neutral hydrogen column density and the color excess was estimated to be ${\sim}7{\times}10^{21}atoms\;cm^{-2}$, which is a little higher than the previous observations made for a diffuse interstellar medium. The spectral analyses of the FIMS observations showed the enhanced C IV emission throughout the whole region, indicating that the C IV emission arises by the interaction of the hot gases with the shell boundaries. A simple model showed that a large portion of the C IV emission comes from the Loop I side of the interaction zone, compared to the Local Bubble side. The FIMS spectrum also showed indications of the molecular hydrogen fluorescence lines for the interaction zone.

  • A clinical investigation was reported on 17 cases of spontaneous pneumothorax requiring surgical mana-gement. Males outnumbered females 15:2. Determination of the etiology in this series showed that the majority were pulmonary tuberculosis and paragonimiasis. Several others had pneumonia, lung abscess, cyst and blebs. It is of particular interest that the acute inflammation of respiratory system was younger age group, pulmonary tuberculosis & paragonimiasis were between 2 nd and 3 rd decades, and lung abscess, cyst, blebs were above 4 th decade. Pulmonary tuberculosis was far advanced bilateral and active. The ratio of right to left side was 13:6 and both side involved in 2 cases. In about half cases of patients, above 50%-collapsed lung associated with mediastinal shifting developed. The complications were pleural effusion and bronchopleural fistula. The former was 13 cases [76.4%] in which 3 cases combined with mixed infection, and latter was 5 cases. As the management, 11 cases were subjected to intercostal or rib resection drainage with continuous suc-tion. Among 11 drainage cases, 8 cases were successful in acute stage and 3 cases failed in chronic stage. This faiure was due to interference with re-expansion of collapsed lung for peel formation and broncho-pleural fistula. The open thoractomy was applied in 9 cases, among which primary operation were 5 cases and drainage failure were 4 cases. Among 11 cases subjected to the open thoracotomy, wedged resection was performed in 3 cases including paragonimiatic cyst, and pneumonectomy in 1 case-tuberculosis, and decortication only was performed in 2 cases in paragonimiasis. Decortication & lung resection was carried out in 2 patients among which ruptured lung abscess 1 case and ruptured multiple blebs 1 case. There was no case of death but prognosis of the tuberculosis may be poor because of far advanced bilateral and active pulmonary tuberculosis.

  • Because stow net being used in now is doing throwing net and hauling net through a ship's side, the work is very complicated and the fishing boat needs many seamen and it could cause a loss of lives and ship in stormy weather. We are now using small mesh size 36~500mm and it even catches young fish, so we call it the fishing gear of resource reduction type. Therefore we must make manpower reduction in automatic operation, safe operation of throwing net and hauling net in the stern and the stern-typed stow net of resource management using large mesh. And we performed three-typed model tests to examine the fishing gear. The obtained results are as follows;1. The fishing gear being used in the ship's side type stow net has inappropriate standard and arrangement of the net, resistance increase of the fishing gear and frequent breakdown of the net.2. To supplement the fault of A-typed stow net, we schemed fishing gear developed as both B-type(12-seamed net) and C-types(8-seamed net) of the stern-typed stow net. 3. In model tests, C-typed model net(mesh size 40~1,600mm) was proved good fishing gear because the resistance in accordance with the flowing speed was comparatively small and it's mouth area was broad. 4. A-typed stow net had the spreading device attached to side panel of the net, but the stern-typed stow net had the spreading device consisted of 4 lines far behind about 6m from side panel of the net mouth. In the flowing speed 2knot, the spreading condition of fishing gear was proved batter than the former.

  • Objective : Cases of a ruptured pericallosal artery aneurysm with a high risk of intraoperative premature rupture and technical difficulties for proximal vascular control require a technique for the early and safe establishment of proximal vascular control. Methods : A combined pterional or subfrontal approach exposes the bilateral A1 segments or the origin of the ipsilateral A2 segment of the anterior cerebral artery (ACA) for proximal vascular control. Proximal control far from the ruptured aneurysm facilitates tentative clipping of the rupture point of the aneurysm without a catastrophic premature rupture. The proximal control is then switched to the pericallosal artery just proximal to the aneurysm and its intermittent clipping facilitates complete aneurysm dissection and neck clipping. Results : Three such cases are reported : a ruptured pericallosal artery aneurysm with a contained leak of the contrast from the proximal side of the aneurysm, a low-lying ruptured pericallosal artery aneurysm with irregularities on its proximal wall, and a multilobulated ruptured pericallosal artery aneurysm with the parasagittal bridging veins hindering surgical access to the proximal parent artery. In each case, the proposed combined pterional-interhemispheric or subfrontal-interhemispheric approach was successfully performed to establish proximal vascular control far from the ruptured aneurysm and facilitated aneurysm clipping via the interhemispheric approach. Conclusion : When using an anterior interhemispheric approach for a ruptured pericallosal artery aneurysm with a high risk of premature rupture, a pterional or subfrontal approach can be combined to establish early proximal vascular control at the bilateral A1 segments or the origin of the A2 segment.
