Risk Factors for Rectal Cancer and Methylenetetrahydrofolate Reductase Polymorphisms in a Population in Northeast Thailand

  • Background and Aim: Polymorphisms in methylenetetrahydrofolate reductase (MTHFR) are known to be associated with predisposition for certain cancers. This study aimed to evaluate the effects of lifestyle factors, family history and genetic polymorphisms in MTHFR C677T and A1298C on rectal cancer risk and possible interactions with lifestyle factors in Northeast Thailand. Methods: A hospital-based case-control study was conducted during 2002-2006 with recruitment of 112 rectal cancer cases and 242 non-rectal cancer patient controls. Information was collected using a structured-questionnaire. Blood samples were obtained for assay of MTHFR C677T and A1298C genotypes by polymerase chain reaction with restriction fragment length polymorphism (PCR-RFLP) techniques. Associations between lifestyle factors, family history and genetic polymorphisms v.s. rectal cancer risk were assessed using logistic regression analysis. Results: Subjects with frequent and occasional constipation had a higher risk ($OR_{adj.}$=14.64; 95%CI=4.28-50.04 and $OR_{adj.}$=2.15; 95%CI=1.14-4.06), along with those who reported ever having hemorrhoids ($OR_{adj.}$=2.82; 95%CI=1.36-5.84) or a family history of cancer ($OR_{adj.}$=1.90; 95%CI=1.06-3.39). Consumption of a high level of pork was also associated with risk ($OR_{adj.}$=1.82; 95%CI=1.05-3.15). Interactions were not observed between MTHFR and other risk factors. Conclusions: This study suggested that the risk factors for rectal cancer in the Thai population are bowel habits, having had hemorrhoids, a family history of cancer and pork consumption.

Contralateral Breast Cancer: a Clinico-pathological Study of Second Primaries in Opposite Breasts after Treatment of Breast Malignancy

  • Background: Breast cancer is by far the most frequent cancer of women (23 % of all cancers), ranking second overall when both sexes are considered together. Contralateral breast cancer (CBC) is becoming an important public health issue because of the increased incidence of primary breast cancer and improved survival. The present communication concerns a study to evaluate the role of various clinico-pathological factors on the occurrence of contralateral breast cancer. Materials and Methods: A detailed analysis was carried out with respect to age, menopausal status, family history, disease stage, surgery performed, histopathology, hormone receptor status, and use of chemotherapy or hormonal therapy. The diagnosis of CBC was confirmed on histopathology report. Relative risk with 95%CI was calculated for different risk factors of contralateral breast cancer development. Results: CBC was found in 24 (4.5%) out of 532 patients. Mean age of presentation was 43.2 years. Family history of breast cancer was found in 37.5% of the patients. There was statistically significant higher rate (83.3%) of CBC in patients in age group of 20-40 years with RR=11.3 (95% CI: 1.4, 89.4, p=0.006) seen in 20-30 years and RR=10.8 (95% CI:1.5-79.6, p=0.002) in 30-40 years as compared to older age of 60-70 years. Risk of development was higher in premenopausal women (RR=8.6, 95% CI: 3.5-21.3, $p{\leq}0.001$). Women with family history of breast cancer had highest rate (20.9%) of CBC (RR=5.4, 95% CI: 2.5-11.6, $p{\leq}0.001$). Use of hormonal therapy in hormone receptor positive patients was protective factor in occurrence of CBC but not significant (RR=0.7, 95% CI: 0.3-1.5, p=0.333). Conclusions: Younger age, premenopausal status, and presence of family history were found to be significant risk factors for the development of CBC. Use of hormonal therapy in hormone receptor positive patients might be protective against occurrence of CBC but did not reach significance.

노인의 고혈압에 영향을 주는 건강행태요인 - 제8차 국민건강영양조사를 활용하여 (Health-related Behavior Affecting Hypertension in the Elderly Using Data from the 8th Korea National Health and Nutrition Examination Survey)

  • 본 연구는 한국 노인의 고혈압에 영향을 미치는 건강행태 요인을 평가하기 위해 수행되었다. 본 연구는 제8차 국민건강영양조사(2019)의 65세 이상 피험자 1,404명의 데이터를 분석하였다. 남성의 경우 고혈압 발병률은 연령(OR=1.056, P=0.004), 만성질환 가족력이 없는 경우(OR=0.662, P=0.024), 저체중(OR=0.142, P<0.001), 정상체중(OR=0.390; P<0.001), 비음주자(OR=0.552; P=0.001)와 관련이 있었다. 여성의 경우 고혈압 발생률은 연령(OR=1.091; P<0.001), 만성질환 가족력이 없는 경우(OR=0.424; P<0.001), 정상체중(OR=0.532; P<0.001), 좋지 않은 주관적 건강 수준(OR=1.640; P=0.018)이 관련이 있었다. 결론적으로, 남성에서는 고혈압에 영향을 미치는 요인은 연령, 비만, 만성질환 가족력, 음주습관 등이었고, 여성에서는 연령, 비만, 만성질환 가족력, 주관적 건강수준 등이었다. 따라서, 노인의 고혈압 발생률을 낮추기 위해 성별에 맞는 건강증진 및 중재 프로그램과 건강정책을 수립하는 노력이 필요하다고 사료된다.

당뇨환자의 자기간호행위 영향요인 분석 (A Study of Factors Predicting Self-care Behavior in Diabetics)

  • To determine factors affecting self-care behavior of diabetics, the relationships of hardiness, family support, demographic and medical variables to self-care behavior were investigated in 180 samples with non-insulin dependent diabetes mellitus. In stepwise multiple regression analysis, 26.76% of the variance in self-care behavior was accounted for by family support(15.52%), age(7.76%), and clinical history (2.07%). To compare the magnitude of predictor's significance by gender, stepwise multiple regression was conducted seperatively by gender group. In the male sample 25.22% of the variance in self-care behavior was accounted for by family support, age, and challenge. In the female sample family support, age, and committment were significant predictors in self-care behavior with 28.82% of the variance. The results highlight the value of family support in self-care behavior in diabetics regardless of gender difference. According to the finding of this study, family support is the most significant predictor of self-care behavior in NIDDM. This implicates that in future diabetic care, a family member should be encouraged to participate in the patient education process. Also as hardiness is not supported by a unidimensional construct, more empirical studies are recommended to differentiate the conceptual traits for the three subconcepts of hardiness.

Factors Affecting Adherence to Antihypertensive Medication

  • Background: Hypertension is a major contributor to the global disease burden of cardiovascular and cerebrovascular disease. The aim of this study was to determine demographic and clinical factors associated with adherence to antihypertensive medication. Methods: From August 2012 to February 2015, we recruited 1,523 Korean patients with hypertension who visited family physicians. The study was conducted in 24 facilities located in urban and metropolitan areas. Of these facilities, two were primary care clinics and 22 were level 2 or 3 hospitals. Adherence was assessed using the pill count method; a cut-off value of 80% was used as the criterion for good adherence. Sociodemographic and lifestyle factors were compared between the adherent and nonadherent groups using the chi-square test for categorical variables and t-test for continuous variables. Binary logistic regression analysis was performed with medication adherence as the outcome variable. Results: Of the 1,523 patients, 1,245 (81.7%) showed good adherence to antihypertensive medication. In the multivariate logistic analysis, age ${\geq}65$ years, exercise, treatment in a metropolitan-located hospital, being on ${\geq}2$ classes of antihypertensive medication and concomitant medication for diabetes, and a family history of hypertension or cardiovascular diseases were associated with good adherence. Patients who had a habit of high salt intake were less adherent to medication. Conclusion: Multiple classes of antihypertensive medications, concomitant medication, and exercise were associated with good adherence to antihypertensive medication, and high salt intake was associated with poor adherence to antihypertensive medication. These factors should be considered to improve hypertension control.

조선 인조(仁祖)의 질병기록에 대한 고찰 - 승정원일기 기록을 중심으로 - (A review on disease records of King-Injo of Chosun Dynasty - based on the records from The Daily Records of Royal Secretariat of Chosun Dynasty -)

  • 'The Daily Records of Royal Secretariat of Chosun Dynasty' is a record created in Seung-jeong-won, a secretariat for kings of Chosun, and is a government record which holds conversations between kings and their vassals as it is. General affairs in terms of the royal family and national administration are recorded, but what is more important is the records on diseases of kings and how they were treated. This study is to look into diseases from which King Injo(1959-1649) had suffered based on the records written during the time of his reign, which was from 1623 to 1649. Also, the "curse incident" and the death of prince Sohyeon, son of King Injo, both of which had significant influence on the health of the king, were reviewed in relation to the disease records.

"A Very Sudden Thing": Recapturing Cold War History in Philip Roth's American Pastoral

  • As the first of Philip Roth's recent series of novels that delve into American Cold War history deeply entwined with the post-war Jewish American experience, American Pastoral traces the tragic fall of a third-generation Jewish American named Seymour "Swede" Levov, whose dream of complete assimilation to the post-ethnic American paradise is irrecoverably disrupted when his young daughter blows up the local post office to protest against the Vietnam War. This essay proposes to examine Swede Levov's interrupted pursuit of the American dream by locating it within specific Cold War contexts and national imaginaries propagated particularly during the years from John F. Kennedy to Lyndon B. Johnson. In so doing, I will argue that Roth presents a paradoxical vision of Jewish American identity that could be acquired by performing perpetual self-effacement and submergence into the non-place of anonymity and doubleness, a mythic location of the post-ethnic Cold War American family. Levov's life becomes true part of the mythic narrative of American history when he realizes that his life, just like the nation's history, is a series of temporalities radically discontinued without any manageable detour ot divine bypass to cross over. Rather than indicating Roth's retraction from the postmodern understanding of subjectivity, the novel's historical realism, I will argue, serves to illuminate the postmodern conditions of American Cold War history and ethnic identity.

가족콘텐츠 스토리족보(가계도) 저작시스템 설계 및 구현에 관한 연구 (A Study on Implementation of Family Tree Authoring System for Family Contents)

  • 본 연구는 앞서 가족이야기(familyStory)를 중심으로 인터렉티브 스토리텔링을 지원하는 데이터의 서술구조연구를 바탕으로 가족콘텐츠 스토리족보 저작시스템 설계 및 구현에 대한 연구이다. 한 인간이 태어나서 가지게 되는 이름, 생년월일, 가계도와 같이 정체성 있는 1차적 정보와 본인과 가족구성원이 경험하는 업적, 에피소드, 경험 등의 2차적 정보를 '가족콘텐츠'라고 정의한다. 본 저작 시스템에서 제시하는 '스토리가 있는 가계도(족보)'를 한 줄로 정의하면 가계도에 스토리텔링을 적용하여 가족스토리를 만드는 것으로, 현존하는 1세대가 생존기간 동안 한 가족의 광범위한 역사를 이후 세대들에게 남기는 것으로서 매우 가치 있고 중요한 일이다. 가족콘텐츠 스토리족보 저작도구는 1,2세대의 경험적 지식의 이야기를 텍스트, 이미지, 영상 등 디지털 매체를 사용하여 쉽게 저작할 수 있으며, 저작된 다양한 장르의 가족이야기를 가지고 3세대와의 소통을 위한 도구로 활용하거나, 이야기를 재구성하여 창작이야기를 작성하는데 활용 가능하다.

영조가 복용한 죽(粥)에 대한 고찰 - 『승정원일기(承政院日記)』의 영조 기록을 중심으로 - (A Research on Porridge that King Yeongjo Had Been Served - Based on The Daily Record of Royal Secretariat of Joseon Dynasty during King Yeongjo period -)

  • Objectives : Many curative methods are used utilized in order to rid human body of disease when people become sick. Traditional Korean medicine generally prescribe methods that involve acupunture, moxibustion, or herb formulae. However, different types of foods are sometimes used as well. While wondering the history and efficacy of Qi-elevation foods that were consumed by Koreans in the past, the author discovered that a record from the Joseon Dynasty, called The Daily Record of Royal Secretariat of Joseon Dynasty, henceforth the Records, contained information about different types of porridge. Hence, the author looked through the records of porridge as written in the historical material in an attempt to learn the examples and efficacy of medicine-porridge consumed in Joseon's royal palace. Methods : After searching for the keyword, 'porridge', in the Records as provided by the National Institute of Korean History, the author extracted the porridges recorded during the Yeongjo period that each has its own special name. Results : Different types of porridge were recorded in the Records as following: arrowroot porridge, bean-leaf porridge, mung bean porridge, bean porridge, malt-rice porridge, oriental arborvitae seed porridge, crucian porridge, lotus seed porridge, adlay porridge, red bean porridge, welsh onion porridge, milk porridge, seashell porridge, ginko nut porridge, black sesame porridge, and mandarin porridge. Each porridge was used for the purpose of alleviating any disease that afflicted the king Yeongjo or his royal family members in relation with the ingredient herb's medicinal function. Conclusions : These porridges consumed by the king Yeongjo and his royal family members were used not only with a purpose of aiding their body's recovery from disease, but with the goal to actively curing them of ailments.
