A Meta-analysis of the Risk Factors related to Falls among Elderly Patients with Dementia (치매노인의 낙상위험요인에 관한 메타분석)

  • Purpose: The purpose of this study was to provide data about the risk factors related to falls among elderly patients with dementia using meta-analysis. Methods: Key words used for search through electronic database (CINAHL, PubMed, Ovid-MEDLINE, RISS, KISS, DBPIA, National Assembly Library) included 'dementia', 'Alzheimer', 'fall'. Twenty studies met the inclusion criteria for the meta-analysis and 'R' version 3.2.2 was used to analyze the correlated effect size. Results: Study results showed that risk factors related to falls were identified as the demographic (age, gender, education), dementia-related (disease duration, cognition), physical (body mass index, walking, balance, activity of daily living, use of walking aids, number of medications including psychotropic drugs, musculoskeletal problems, parkinsonism, comorbidity), psychological (neuropsychiatric symptom, depression), environmental (Physical environment), and fall-related (fall history, high risk group of fall) factors. The effect size of risk factors such as high risk group of fall (r=.35), use of walking aids (r=.33), depression (r=.31), psychotropic drugs (r=.27), Musculoskeletal problems (r=.25) were higher than the other risk factors. Conclusion: Based on the findings of this study, strategies to improve elderly patient's depression, intensive care for high risk group of fall, and adequate training with walking aids are needed for prevention of falls in elderly patients with dementia.

Development of Fall Assessment Scale in Adult Inpatients (한국형 성인 환자 낙상위험 사정도구 개발)

  • Purpose: The purpose of this study was to develop a valid Adult Fall Assessment Scale Korean version (FAS-K) for inpatients in general hospitals in Korea. Methods: This a 14 item FAS-K was developed based on a review of national and international research and expert opinions in related field. Interrater reliability of FAS-K was analyzed using the ICC (Intraclass Correlation Coefficients). To examine the validity of FAS-K, a CVI (Content Validity Index), the Pearson correlation coefficients between FAS-K and two widely used scales, the Morse Fall Scale (MFS) and the Johns Hopkins Hospital Fall Risk Assessment Tool (JHFRAT), and the Mann-Whitney test were used. Results: The ICC was .958. The CVI ranged from .70 to 1.00. The FAS-K was significantly associated with the MFS (r=.74, p<.001) and with the JHFRAT (r=.87, p<.001), verifying the concurrent validity of FAS-K. As for the construct validity of FAS-K, significant differences between the fall and non-fall groups were found not only in the MFS (Z=-3.23, p=.001) but also in the FAS-K (Z=-3.10, p=.002). Conclusion: The findings indicate that the FAS-K is a reliable fall risk assessment tool with high validity.

Falls in Patients of Medical Institutions in South Korea: A Literature Review

  • Background: Like many other countries, falls and related injuries in older adults are great concerns in South Korea. In particular, falls are common in medical institutions, often causing the increase of the length of hospitalization. Objects: The purpose of this review was to help understand and address falls in hospitalized individuals in South Korea. Methods: The review was conducted on literature published in Korean from 2010 to 2022, searched in the Korea Citation Index and PubMed. Keywords used for the search were as follows: falls, fall risk, fall risk assessment, hospital, inpatient, intervention, Korea, and prevention. Results: A total of 54 articles were found and reviewed. The most common place of fall accidents was the inpatient room, where there were many cases of falls while walking. Loss of balance was the most common cause of falls, and many falls occurred in patients admitted to the internal medicine. Furthermore, a risk of falling increased with the type of medications taken. In terms of tools to assess patients' fall risk, the Morse Fall Scale (MFS) was commonly used. Patient-specific fall prevention activities were common to address falls, and they decreased the frequency of falls and the fear of falling. Factors influencing the effectiveness of the fall prevention activities included attitudes toward falls, education, environmental factors, patient safety culture, and self-efficacy in preventing falls. Conclusion: Our results should help understand and address falls and injuries in medical institutions.

Development of New Balance Evaluation Index through Biomechanical Verification for Healthy Elderly (운동역학적 검증을 통한 건강한 노인의 새로운 균형평가지표 개발)

  • Objective: The purpose of this study is to develop new balance evaluation index that can discriminate fall risk factors and provide effective interventions for healthy elderly. In order to conduct this study, the balance assessment tools (TUG, mCTSIB, OLST, FRT and BBS) currently used in clinic were re-evaluated using biomechanical analysis. Method: The participants were healthy elderly people over 65 years old, n=26, age: 69.31±3.13 years; height: 154.00±4.12 cm, body weight: 56.13±6.04 kg. The variables are length of CoM-BoS, length of CoP-BoS, range of CoP, mean distance of CoP, mean frequency of CoP, root mean square of CoP, joint angle, ASM (%SL), CoP-CoM angle. Results: As a result of this study, the following items were included in the list of new balance evaluation index for the healthy elderly, showing differences in the biomechanical evaluation based on the clinical evaluation (Inclusion list: TUG, OLST, 8th assessment item of BBS (reaching forward with outstretched arm), 11th item (turning 360 degrees), 13th item (standing with one foot in front), 14th item (standing on one foot)). Conclusion: Based on the results, the new balance evaluation index for the healthy elderly determined through this study can be used to prevent the fall by evaluating the balance ability in various situations that can be experienced in the normal daily life of the healthy elderly.

A Study on the Atmospheric Clearness of Major Cities in Korea (우리나라 주요 도시의 대기청명도 평가)

  • Since the atmospheric clearness index is main factor for evaluating atmosphere circumstance, it is necessary to estimate its characteristics all over the cities in Korea. We have began collecting clearness index data since 1982 at 16 different cities and considerable effort has been made for constructing a standard value from measured data at each city. The new clearness index data will be extensively used by evaluating atmospheric circumstance analysies as well as by solar application system designer or users. From the results, we can conclude that 1) Yearly mean 63% of the atmospheric clearness index was evaluated for clear day all veer 16 cities in Korea. 2) Atmospheric clearness index of spring and summer were 63% and 64% and for fall and winter their values wre 61% and 59% respectively. So, spring and summer were higher, and fall and winter were lower then the yearly value. 3) A significant difference of atomospheric clearness index is observed between $1982{\sim}1987$ and $1988{\sim}1993$ through 16 different cities in Korea.

Fall experience and dual-task during gait performance for community-dwelling persons with stroke

  • Objective: The purpose of this study was to investigate the effects of fall experience and task complexity on gait performance in community-dwelling persons with chronic hemiparetic stroke. Design: Cross-sectional study. Methods: Thirty-three persons who had a history of stroke participated in this study. The participants included 18 persons (aged mean 54.0, mean score of 24.6 points on the Montreal Cognitive Assessment, MoCA) with fall experience (faller group) and 15 persons (aged mean 53.7, mean score of 24.7 points on the MoCA) without fall experience (non-faller group) in the previous six months. This study measured balance and gait performance at two different conditions (with/without 70% of water filled in a 200 cc cup). The participants were clinically assessed using the 10-meter walk test (10MWT), 6-minute walk test (6MWT), Berg Balance scale (BBS), Dynamic Gait Index (DGI), and Timed Up-and-Go (TUG) test. Results: After analyzation, persons in the faller group performed significantly better on the 10MWT, 6MWT, BBS, DGI, and the TUG test in the no-cup-carrying condition than those in the cup-carrying condition (p<0.05). The persons in the non-faller group also performed significantly better in all outcome measures with the no-cup-carrying condition than those in the cup-carrying condition (p<0.05). However, there was no interaction between fall experience and task complexity in the two groups. Conclusions: Our results showed that balance and gait performance depended on fall experience and task complexity but fall experience did not interact with task complexity. Clinicians should consider fall prevention and task complexity during therapeutic approaches in persons with hemiparetic stroke.

Analysis about Effect for Stock Price of Korea Companies through volatility of price of USA and Korea (미국과 한국의 가격변수 변화에 따른 한국기업 주가에 대한 영향분석)

  • The result of variance decomposition through yield of Treasury of 30 year maturity of USA, S&P 500 index, stock price of KEPCO has 76.12% of impulse of KEPCO stock price at short-term horizon, but they have 51.40% at long-term horizon. After one year, they occupy 13.65%, and 33.25%. So their effects are increased. By the way, S&P 500 index and yield of Treasury of 30 year maturity of USA have relatively more effect for forecast of stock price oi KEPCO at short-term & long-term. The yield of Treasury of 30 year maturity of USA more than S&P 500 index have more effect for stock price of KEPCO. It is why. That foreign investors through fall of stock price of USA invest for emerging market is less than movement for emerging market of hedge funds through effect of fall of yield of Treasury of 30 year maturity of USA, according to relative effects for stock price of Korea companies. The result of variance decomposition through won/dollar foreign exchange rate, yield of corporate bond of 3 year maturity, Korea Stock Price index(KOSPI), stock price of KEPCO has 81.33% of impulse of KEPCO stock price at short-term horizon, but they have 41.73% at long-term horizon. After one year, they occupy 23.57% and 34.70%. So their effects are increased. By the way, KOSPI and won/dollar foreign exchange rate have relatively more effect for forecast of stock price of KEPCO at short-term & long-term. The won/dollar foreign exchange rate more than KOSPI have more effect for stock price of KEPCO. It is why. The recovery of economic condition through improvement of company revenue causes of rising of KOSPI. But, if persistence of low interest rate continues, fall of won/dollar foreign exchange rate will be more aggravated. And it will give positive effect for stock price of KEPCO. This gives more positive effect at two main reason. Firstly, through fall of won/dollar foreign exchange rate and rising of credit rating of Korea will be followed. Therefore, foreign investors will invest more funds to Korea. Secondly, inflow of foreign investment funds through profit of won/dollar foreign exchange rate and stock investment will be occurred. If appreciation of won against dollar is forecasted, foreign investors will buy won. Through this won, investors will do investment. Won/dollar foreign exchange rate is affected through external factors of yen/dollar foreign exchange rate, etc. Therefore, the exclusion of instable factors for foreign investors through rising of credit rating of Korea is necessary things.

Seasonal Variations of Marine Water Quality and Eutorphication Index in Mokpo Harbour (목포항의 수질 및 부영양도의 계절 변화)

  • The in situ observations and the seawater analyses were conducted for 3 years from August 1996 to August 1999 in order to elucidate and evaluate seasonal variations of marine water quality and trophic state in Mokpo harbour of Korea. Compared the seasonal seawater qualities of Mokpo harbour with the OECD standards of trophic classification in parameters such as Secchi depth, dissolved inorganic nitrogen, phosphate phosphorus and chlorophyll-a, the trophic level of seawater in Mokpo harbour was evaluated to be in mesotrophic or eutrophic state through all 4 seasons and to be in eutrophic state, particularly In summer. The estimation of pollution index by eutrophication showed the seawater quality of Mokpo harbour to deteriorate and fall under the regular grades through all 4 seasons, although the seawater quality of Mokpo harbour was evaluated to be equivalent to the second or third class of the Korean seawater quality standards in view of COD values. The results of eutrophication index estimation showed the high potentiality of red tide occurrence In Mokpo harbour, particularly in summer or fall. In the light of the average atomic ratio of N/P in seawater, the limiting nutrient factor against the growth of phytoplankton was concluded to be phosphorus rather than nitrogen in Mokpo Harbour.

Effect of trunk training in wheelchair on fall efficacy, fall risk, activities of daily living in acute stroke patients: Randomized controlled pilot study (휠체어 몸통 훈련이 급성기 뇌졸중 환자의 낙상 효능감, 낙상 위험도, 일상생활활동에 미치는 영향: 무작위 대조군 예비 연구)

  • Background: The aim of this randomized controlled pilot study was to determine the effect of trunk training in wheelchair on fall efficacy, fall risk and activities of daily living in acute stroke patients. Design: Randomized controlled pilot trial. Methods: The study included 18 patients with acute stroke who were randomly allocated to an experimental group (EG) (n=9) and a control group (CG) (n=9). Patients in the EG group received general rehabilitation therapy combined with trunk training in wheelchair for 20min, whereas CG group received general rehabilitation therapy combined with bicycle training for 20min. Both groups performed the exercise 5 times a week for 3 weeks. Outcomes were assessed using Korean-Fall Efficacy Scale (K-FES), Fullerton Advanced Balance Scale (FAB) and Korean-Modified Barthel Index (K-MBI). Results: After 3 weeks of training, both groups showed significantly improved K-FES, FAB and K-MBI (p<.05 in both groups). However, the K-FES, FAB and K-MBI in the experimental group was significantly improving than in the control group (p<.05). Conclusion: These findings indicate that trunk training in wheelchair may be effective at decreasing fall risk and improving activities of daily living in acute stroke patients. Therefore, trunk training in wheelchair may be recommended as an intervention in reducing the incidence of fall risk in acute stroke patient.

Estimation of Degree of Weathering in Residual Soil Using Water Content from Fall Cone Test Result (Fall cone test의 함수비를 이용한 잔적토의 풍화도 측정)

  • Weathered soils appear from the rock and its weathering result. In accordance with the degree of weathering the roch may become soft rock, weathered rock and residual soil. In general, classification method for determining the degree of weathering are performed by chemical method and N-value. But these method have some problems. So, this research is to suggest an appropriate physical method to determine the degree of weathering of weathered soils. A new classification method for determining the degree of weathering is suggested, based upon the results from fall cone test. According to the proposed physical method using fall cone apparatus, the measured values of the samples from the same area show distinctive difference of weathering. For the checking, we selected two areas. As a result, the relationship between CWI and water content according to penetration is expressed as an equation in Ilsan and Incheon area. And it proved to be a good method to determine the degree of weathering.
