Purpose: This study was done to identify effects of a fall prevention program on physical fitness and psychological functions in community dwelling elders. Methods: A quasi-experimental study was carried out with a nonequivalent control group pre & post-test design. The program, which included exercises and education, consisted of a 12-week group program and an 8-week self-management program using a health calendar. An experimental group (32) and a control group (21) participated. Results: There were significant differences in SPPB (t=-3.92, p<.001), TUG test (t=4.94, p<.001), standing with right leg (t= -3.60, p=.001), standing with left leg (t=-3.74, p<.001), front and rear maximum step length test (t=-4.34, p<.001), right-left maximum step length test (t=-2.65, p=.011), and fall efficacy (t=-2.42, p=.019). Fall efficacy, fear of falling and depression showed significant differences following the 12-week exercise program and 8-week self-management program in the experimental group. Conclusion: Study findings indicate that the fall prevention program is an effective nursing intervention to enhance physical fitness and psychological functions for elders. Using a health calendar, the self-management program was more effective for psychological functions compared to only the group program. Therefore, health providers should develop diversified fall prevention programs which include motivation plans to encourage clients in participating.
This study was a quasi-experimental study of non-equivalent control group pretest and posttest design. The purpose of this study was to determine the effect of Tai Chi exercise program on physical functions, psychological functions, and fall among the fall-prone elderly. The data were collected from September 19, 2001 to January 31, 2002. The study, conducted at two facilities located in Kwang-ju, was targeted to the ambulatory aged 60 years or older who had at least one of the key fall risk factors. Experimental group participated in Tai Chi exercise for 40 minutes per one time and three times a week for 12weeks at an auditorium. Fifty nine fall-prone elderly were assigned to 12-week Tai Chi exercise program (n=29) and control group (n=30). They underwent tests of lower muscle strength, time for chair stand, balance, flexibility, depression, falls efficacy, fear of falling, and numbers of fall at the baseline and at the 12th week. Numbers of fall and fall injuries were monitored for 16 weeks(12-weeks intervention plus 4-week follow-up periods) using fall calendar. Each participant was given a calendar to record the numbers of fall per day for a month. The calendars were collected at the last week of each month. 1. Tai Chi exercisers showed significant improvement in the strength of knee flexors, and ankle dorsiflexors and plantarflexors compared to the control group. The experimental group had improvement in the strength of knee extensors while the control group did not, with no statistical significance. 2. Tai Chi exercisers reported positive change in the average time of chair stand as compared to the control group. 3. Tai Chi exercisers had significant improvement in flexibility as compared to the control group. 4. There was no significant difference in the depression between the two groups, even though the Tai Chi exercisers maintained depression score in the same level while the others were increased. 5. Tai Chi exercisers showed significant improvements in the falls efficacy as compared to the control group. The falls efficacy was significantly improved among the experimental group while the opposite was identified among the control group. 6. The experimental group reported the significant reduction of the fear of fall, whereas control group reported the opposite. 7. Of the 59 subjects for 16weeks(12weeks intervention period and 4weeks follow up), 9 (31.0%) of the 29 in exercise group and 15 (50%) of the 30 in the control group fell (relative risk=0.62. 95% CI 0.32-1.19), even with no statistical difference. The results suggest that the Tai Chi exercise program can improve the strength of knee flexors, ankle dorsiflexors and plantarflexors, chair stand. flexibility, falls efficacy, and fear of falling for the fall-prone elderly.
Fall is one of the most intimidating health conditions in elders. Comprehensive assessment is necessary to understand the individual and environmental aspects of the falls such as balancing abilities, depression, and quality of life. The purpose of this study was to compare the balancing ability, depression, and quality of life between elderly fallers and elderly non-fallers. Thirty-two community-dwelling elders (fifteen males and seventeen females between 65 and 83 years old), who have experienced fall on walking during last twelve months, were involved in the elderly fallers group. And twenty-four males and twenty-two females between 65 and 83 years old of community-dwelling elders, who have no experienced fall on walking during last twelve months, were involved in the elderly non-fallers group. Berg balance scale (BBS), timed up and go test (TUG), and functional reach test (FRT), were used to evaluate the ability of the physical balance. 'Beck depression scale in Korean' questionnaire was used to assess the depression. 'Korean version of World Health Organization Quality of Life Assessment Instrument-Bref' questionnaire was used to assess the quality of life. The results were as follows: 1) Balancing abilities measured by the BBS, TUG in the elderly fallers group were meaningfully lower than that of the elderly non-fallers group (p<.05), whereas no significant difference in the FRT was found (p>.05). 2) Depression level in the elderly fallers group was significantly higher than that of the elderly non-fallers group (p<.05). 3) Quality of life in the elderly fallers group was significantly lower than that of the elderly non-fallers group, excluding environment domain (p<.05). Therefore, in order for clinical evaluation of the community-dwelling elders those with reduced balancing ability, it is necessary to evaluate and understand the fall experience, depression, and quality of life.

Purpose : The home-visiting exercise guidance service is a component of the community-based integrated care policy in District J of Busan Metropolitan City. This study aims to investigate the effects of the service provided by physical therapists on depression, pain, and balance among the elderly. Methods : Three elderly individuals were selected as study participants. Depression was assessed using the beck depression inventory (BDI) and the short geriatric depression scale-Korea (SGDS-K). Pain was evaluated using the visual analog scale (VAS). Balance was assessed using the berg balance scale (BBS), functional reach test (FRT), and timed up and go test (TUGT). The evaluation data were compared by averaging the values from the first and eighth sessions. Additionally, the overall satisfaction with the home-visiting exercise guidance service was surveyed. Results : In the depression assessment, the beck depression inventory score decreased (by 16.67±5.69 points), and the short geriatric depression scale score decreased (by 4.67±2.52 points). The visual analog scale score for pain decreased (by 2.67±0.58 points). In the balance assessment, the berg balance scale score increased (by 7.67±2.52 points), the functional reach test distance extended (by 4.67±0.58 cm), and the timed up and go test time decreased (by 5.33±0.58 s). Overall satisfaction with the home-visiting exercise guidance service was high (4.26±0.14 points). Conclusion : The home-visiting exercise guidance service received high satisfaction ratings from the participants. It was effective in reducing depression and pain while improving balance, potentially reducing fall risk factors among the elderly. These findings suggest that the home-visiting exercise guidance service, delivered by physical therapists, could significantly enhance the quality of life for elderly individuals by addressing issues related to depression, pain, and fall prevention within the community. Consequently, it is recommended that this service be continued and expanded as part of government policy.
The aim of this study was to understand the factors related to depression according to gender in vulnerable elderly. We conducted a cross-sectional study with 1,709 elderly beneficiaries of the visiting health care program in Seoul. Depression was measured using the Geriatric Depression Scale Short Form Korea Version(GDSSF-K) questionnaire. Depression score was mild level in vulnerable elderly. There was no gender difference in depression level. Factors related to depression differ by gender. In the elderly men, higher depression showed significant correlation with a lower level of health-related quality of life(HRQOL) and self-rated health(SRH). In addition, higher depression was observed for elderly men living alone than for those living with family. These factors explained depression by 23.1%. In the elderly women, higher depression showed significant correlation with a lower level of HRQOL, SRH and a higher dependence of instrumental activities of daily living. In addition, higher depression was observed for elderly women who has fall experience. These factors explained depression by 22.8%. Gender-specific nursing strategies may be required for prevention of depression in vulnerable elderly.
Purpose: The purpose of this study was to determine the effects of a fall prevention program on gait ability, one leg standing-right, one leg standing-left, fear of falling, fall efficacy, and depression in the elderly. Methods: A fall prevention program was consisted of fall preventing exercise with laughter therapy. Quasi-experimental design with one group pre- and post-test was used. Twenty-two elderly were participated in this study from a community health center located in a rural area, South Korea. SPSS/WIN 18.0 was used for descriptive analysis and paired t-test. Results: After 12 weeks with a fall prevention program, gait ability (t=3.44, p=.002), one leg standing-right (t=-3.73, p=.001), one leg standing-left (t=-4.17, p<.001) and fear of falling (t=2.12, p=.046) in the elderly were significantly improved. Conclusion: A fall prevention program was effective on physical and psychological functions. This study suggested that control groups and a larger sample should be included in order to validate the effects of a fall prevention program for the elderly. This study aimed to determine the effect of depression between the health-related quality of life and pain in elderly persons with physical disabilities. A total of 111 patients who were treated at hospital B (Seoul, Daejeon, Daegu, Gwangju, and Busan) participated in the Survey. The SF-36 Health Assessment was used to determine the quality of life of subjects, the Korean version of the Geriatric Depression Scale-Short form to assess the level of depression, and Numeric Rating Scale to determine the level of pain. To determine the moderating effect of depression on the relationship between quality of life and pain, simple regression analysis, and the Sobel test were performed. There was a significant negative correlation between health-related quality of life and pain (r<-.3, p<.05), and a significant positive correlation between depression and pain (r=.251, p<.05). Thus, health-related quality of life had a simple regression relationship with depression and pain. Depression also showed a mediating effect between health-related quality of life and pain. The results of this study suggest that depression mediates between pain and quality of life.
Purpose: The purpose of this study was to examine the mediating and moderating effects of fall-related efficacy and social support on the relationship between frailty and health-related quality of life among rural community-dwelling elderly. Methods: A cross-sectional survey was conducted with a convenient sampling method, and data of 438 elderly residents living in a rural community was used. The structured questionnaire included items from the Euro Quality of life-5 Dimensions (mobility, self-care, usual activities, pain/discomfort, anxiety/ depression), fall-related efficacy and social support. Results: Analysis of the mediating effect of fall-related efficacy and social support showed that there was significant mediating influence of fall-related efficacy on the relationship between frailty and health-related quality of life. There were no moderating effects of fall-related efficacy and social support. Conclusion: The findings suggest that fall-related efficacy may play a role in reducing the effect of frailty on health-related quality of life and underscore the need to consider ways of enhancing fall-related efficacy in interventions for rural community-dwelling frail elderly.
Purpose: This study examined the circumstances, risk factors, and the predictors of fall incidents among patients in the small and medium-sized hospitals. Methods: Fifty patients with any fall experiences were matched by gender, age, and medical departments with 100 patients without fall incident at the same hospital. Data were collected from 5 small and medium-sized hospitals. Data were analyzed using descriptive statistics, a Chi-square test, a Fisher's exact test, and a logistic regression with the SPSS/WIN 21.0 program. Results: In the patients with falls, the largest number of falls occurred during the day shift, in the patients' rooms, and while they were walking. Further 74.0% of the patients had physical injuries, and 34.0% had to take further medical diagnostic tests. Significant differences were found between the patients with falls and the others on 14 variables (cardiovascular disease, anemia, sedative-hypnotics, vasodilators, narcotic analgesics, dizziness, general weakness, unstable gait, walking aids, anger, anxiety, depression, orientation, and fear of fall). Narcotic analgesic use, dizziness, walking aids, and cardiovascular disease were identified as the predictors of fall incidents. Conclusion: These findings are hoped to be used in developing a fall risk assessment tool and fall prevention nursing programs for small and medium-sized hospitals.
Purpose: The purpose of this study was to explore the effects of the lavender fragrance on sleep and depression in women college students, Method: Forty-two women college students who complained of insomnia were studied during a four-weekprotocol( control treatment week, $60\%$ lavender fragrance treatment week, washout week, $100\%$ lavender fragrance treatment week), All subjects were in the department of nursing in 'K' college and the study was a single blind repeated measurements experiment For the duration of the study, weekly evaluations of sleep, patterns of sleep disturbance, severity of insomnia scale, self satisfaction with sleep, and severity of depression were performed. Result: Among sleep variables, length of time taken to fall asleep, severity of insomnia, and self satisfaction with sleep were improved for the $60\%$(p=.000, p=.000, p=.000) and $100\%$(p=.000, p=.000, p=.000) week while the severity of depression was improved only for the $100\%$(p=.002)week. Conclusion: According to the study results, it can be concluded that the lavender fragrance had a beneficial effect on insomnia and depression in women college students. Repeated studies are needed to confirm effective proportions of lavender oil and carrier oil for insomnia and depression.
