투석 치료중인 만성 신부전 소아에서의 지질 및 지질단백 이상 (Lipoprotein and Lipid Abnormalities in Uremic Children with Maintenance Dialysis)

  • 목적 : 말기 신부전 환자에서 심혈관계 질환은 유병율 및 사망율의 중요한 원인중 하나이다. 이들 환자에서 흔히 동반되는 고지질단백 혈증은 심혈관계 질환의 위험인자로 작용하며, 특히 lipoprotein(a)[Lp(a)]는 관상동맥 질환과 혈전증의 위험 인자로 알려져 있다. 소아에서도 만성 신부전시 고지질 혈증이 보고되어 있다. 저자들은 투석 치료중인 만성 신부전 소아를 대상으로 혈중 지질, 지질단백 및 Lp(a)의 농도 변화를 분석하였다. 방법 : 서울대학교 어린이병원 소아과에서 유지 혈액 투석을 받고 있는 환아 10명과 지속성 외래 복막 투석을 받고 있는 환아 14명을 대상으로 정맥 혈청중 지질, 지질단백 및 Lp(a)농도를 측정하여 건강 대조군과 비교하였으며, Lp(a)의 혈중 농도에 따른 다른 지질단백의 혈중 농도를 비교하였다. 혈액투석(hemodialysis, HD)환아군의 평균 연령이 $162{\pm}59$ 개월, 남녀비는 7:3 이었으며 복막투석(peritoneal dialysis, PD)환아군의 평균 연령은 $123{\pm}69$개월, 남녀비는 6:8 이었다. 결과 : 1) 연령, 성별, 비만지수, 투석기간은 HD군과 PD군 간의 차이가 없었으나 혈중 단백과 알부민치는 PD군에서 유의하게 낮았다. 2) HD군과 PD군 모두에서 혈중 콜레스테롤, 중성지방, 저비중 지질단백 콜레스테롤(low density lipoprotein-cholesterol, LDL-콜레스테롤)과 고비중 지질단백 콜레스테롤(high density lipoprotein-cholesterol, HDL-콜레스테롤)의 비는 대조군에 비해 증가되어 있었으며, HDL-콜레스테롤과 콜레스테롤의 비는 대조군에 비해 유의하게 낮았다. 3) 혈중 아포단백 B치는 대조군에 비해 상승되어 있었지만 아포단백 Al과 아포단백 B의 비는 차이가 없었고, 혈중 Lp(a)치는 PD군에서만 대조군보다 상승되어 있었다. 4) 혈중 Lp(a)치가 30 mg/dl 미만인 환아는 13명, 30 mg/dl 이상인 환아는 11명으로 이들 간에 연령, 비만지수, 투석방법, 투석기간은 차이가 없었으나 혈중 알부민은 Lp(a)가 상승된 환아들에서 감소되어 있었다. 5) 혈중 Lp(a)농도와 다른 지질 단백과의 비교에서 아포 단백 B만이 유의한 차이를 보였다. 결론 : 투석 치료중인 만성 신부전 소아에서 혈중 지질단백 이상이 관찰되었으며 특히 PD군에서는 혈중 Lp(a)치가 증가되어 있었다. 따라서 만성 신부전 소아에서 관상 동맥 질환의 위험도가 높을 것으로 사료되며 이에 대한 장기적이고 지속적인 관찰이 필요하다.

국내 민간병원에서 치료한 다제내성결핵 환자의 치료 결과 및 예후 인자 (Treatment Outcomes and Prognostic Factors in Patients with Multidrug-Resistant Tuberculosis in Korean Private Hospitals)

  • Background: The increasing rate of drug-resistant tuberculosis (TB) is a threat to the public health and TB control. In Korea, about 75~80% of TB patients are treated in private hospitals and the rate has been continuously increasing since 2000. Methods: On a retrospective basis, we enrolled 170 newly diagnosed with or retreated for multidrug-resistant TB (MDR-TB) in 2004 from 21 private hospitals. We extracted the following demographics and treatment history from patient medical records: initial treatment outcomes, cumulative survival rates, treatment outcomes, and prognostic factors. Results: Of the 170 patients, the majority were male (64.1%), the mean age was 44.5 years old, and mean body-mass-index was $20.2kg/m^2$. None of the patients tested positive for HIV. Eleven (6.5%) were confirmed to have extensively drug-resistant TB (XDR-TB) at treatment initiation. Treatment success rates were not different between XDR-TB (36.4%, 4/11) and non-XDR MDR-TB (51.6%, 82/159). Default rate was high, 21.8% (37/170). Far advanced disease on X-ray was a significant negative predictor of treatment success; advanced disease and low BMI were risk factors for all-cause mortality. Conclusion: In private hospitals in Korea, the proportion of XDR-TB in MDR-TB was comparable to previous data. The treatment success rate of MDR-/XDR-TB remains poor and the failure rate was quite high. Adequate TB control policies should be strengthened to prevent the further development and spread of MDR-/XDR-TB in Korea.

압박 긴장대 방법을 이용한 구상 돌기 골절의 견고한 고정과 조기 운동을 통한 주관절 기능의 향상 (Improvement of the Elbow Function with Early Mobilization and Rigid Fixation of Coronoid Fracture by Tension Band Technique)

  • 목적: 압박 긴장대 방법을 이용한 작은 구상 돌기 골절의 견고한 고정과 주관절의 조기 운동을 통한 기능의 향상 및 그 결과를 분석하고자 한다. 재료 및 방법: 주관절 내측접근법으로 K-강선과 철선을 이용하여 고정된 8예의 구상돌기 골절을 대상으로 하였다. 분류상 8예 모두 Regan-Morrey type 2였으며 O'Driscoll 분류12)로는 tip형(subtype 2)가 예, anteromedial 형 (subtype 2, 1예 그리고 subtype 3, 2예)3예였다. 동반 손상으로는 측부인대 파열이 6예, 요골두 및 경부골절이 4예였으며 모두 함께 치료하였다. 술 후 5~7일간의 고정 치료 후 경첩 보조기 (hinge brace)을 착용한 상태에서 술후 6주까지 능동적 운동을 허용하였다. 술 후 통증의 여부, 관절 운동 범위, Mayo elbow performance score(MEPS)을 이용한 기능적 평가 방법을 이용하여 치료 결과를 분석하였다. 평균 11 (6~28)개월 추시 하였으며 척골 신경 증상에 대해서도 함께 조사하였다. 결과: 모든 구상돌기 골절은 내고정용 도구의 실패 없이 모두 유합되었다. 평균 2.2 (2~4)개의 K-강선이 사용되었다. 관절운동 범위에 대해 신전은 평균 $3^{\circ}(0^{\circ}\sim25^{\circ})$, 굴곡는 평균 $137^{\circ}(130^{\circ}\sim140^{\circ})$, 회내전은 평균 $69^{\circ}(45^{\circ}\sim90^{\circ})$, 회외전은 $78^{\circ}(45^{\circ}\sim90^{\circ})$이였다. MEPS는 평균 96 (65~100) 이였다. 척골 신경 증상은 terrible triad 1예에서 첫 수술 (index operation)후 요골두를 절제하고 내측 측부 인대의 기능 부전이 있는 1예에서 관찰되었다. 결론: 쉽게 구할 수 있으며 가격이 저렴한 K-강선과 철선을 이용한 압박 긴장대 방법은 충분히 견고하여 조기에 능동적 운동을 허용할 수 있어 주관절의 기능 향상을 가져올 수 있었다. 이 방법은 특히 다발성 소형 구상 돌기 골절의 경우 유용한 하나의 고정 방법으로 생각된다.

초기 시청시간 패턴 분석을 통한 대흥행 드라마 예측 (Prediction of a hit drama with a pattern analysis on early viewing ratings)

  • TV 드라마는 타 장르에 비해 시청률과 채널 홍보 효과가 매우 크며, 한류를 통해 산업적 효과와 문화적 영향력을 확인시켜줬다. 따라서, 이와 같은 드라마의 흥행 여부를 예측하는 일은 방송 관련 산업에서 매우 중요한 부분임은 주지의 사실이다. 이를 위해서 본 연구에서는 2003년부터 2012년까지 10년간, 지상파 채널을 통해 방송된, 총 280개의 TV 미니시리즈 드라마를 분석하였다. 이들 드라마 중 평균 시청률 상위 45개, 하위 시청률 45개를 선정하여 흥행 드라마의 시청시간 분포 (5%~100%, 11-Step) 모형을 만들었다. 이들 기준 모형과 신규 드라마의 시청시간 분포와의 이격 거리를 Euclidean/Correlation으로 측정한 유사도(Similarity)를 통해, 시청자의 초기(1~5회) 시청시간 분포로 신규 드라마의 성패 여부를 예측하는 모델을 만들었다. 또한 총 방송 시간 중 70% 이상 시청한 시청자를 열혈 시청층(이하 열혈층) 으로 분류하고, 상위/하위 드라마의 평균값과 비교하여, 신규 드라마의 흥행여부를 판별할 수 있도록 설계하였다. 연구 결과 드라마의 초반 시청자 충성도(시청시간)는 드라마의 대흥행 여부를 예측하는데 중요한 요소임을 밝혔으며, 최대 75.47%의 확률로 대흥행 드라마의 탄생을 예측할 수 있었다.

슬관절 부분 치환술의 장기 생존 분석 (Long-Term Survival Analysis of Unicompartmental Knee Arthroplasty)

  • 목적: 본 연구의 목적은 단일 술자가 시행한 슬관절 부분 치환술의 장기 임상적, 방사선적 결과 및 생존율을 분석하는 것이다. 또한 치환물의 생존에 영향을 미치는 인자를 분석하고 그에 따른 생존 곡선을 비교하는 것이다. 대상 및 방법: 1982년 12월부터 1996년 1월까지 시행된 슬관절 부분 치환술 99예를 대상으로 하였다. Modular II 10예, Microloc 44예, Allegretto 45예였다. 평균 추시기간은 16.5년이었다. 임상적 평가로 술 전, 술 후 1년, 최종 추시 hospital for special surgery(HSS) 점수와 관절운동범위를 조사하였다. 방사선적 평가로 술 전, 술 후 2주, 최종 추시 대퇴 경골각을 조사하였다. Kaplan-Meier 생존 분석을 이용하여 생존율을 분석하였다. Cox 비례위험모형을 이용하여 나이, 성별, 체질량 지수, 술 전 진단, 치환물의 종류 중 생존에 영향을 미치는 요인을 조사하였다. 생존에 영향을 미치는 인자에 따라 군을 나누어 생존 곡선을 비교하였다. 결과: 전체 HSS 점수는 평균 술 전 57.7점, 술 후 1년 92.7점, 최종 추시 79.1점이었다(p<0.001). 관절운동범위는 평균 술 전 134.3도, 술 후 1년 138.4도, 최종 추시 129.3도였다(p<0.001). 전체 대퇴 경골각은 평균 술 전 내반 0.8도, 술 후 2주 외반 4.1도, 최종 추시 외반 3.0도였다(p=0.003). 전체 생존율은 5년 91.8%, 10년 82.9%, 15년 71.0%, 20년 67.0%였다. 부분 치환술의 생존에 영향을 미치는 요인은 환자의 나이와 치환물의 종류였다. 나이가 높을수록 실패 위험이 낮았으며(hazard ratio=0.933), Microloc 사용 시 Modular II와 Allegretto에 비해 위험률이 높았다(hazard ratio=0.202, 0.430, respectively) 60세 미만군의 생존 곡선이 60세 이상 군에 비해 의미 있게 낮았다(p=0.003). Microloc 사용군의 생존 곡선이 Modular II와 Allegretto 사용군에 비해 낮았다(p=0.025). 결론: 고정형 치환물을 사용한 슬관절 부분 치환술의 장기 임상적, 방사선적 결과 및 생존율은 만족스러웠다. 부분 치환물의 장기 생존을 위해 적절한 환자와 치환물의 선택이 중요할 것이다.

슬관절 주위 재건물 감염 후유증 시 슬관절 상하부 종양인공관절을 이용한 사지 구제술 (Limb Salvage Using a Combined Distal Femur and Proximal Tibia Replacement in the Sequelae of an Infected Reconstruction on Either Side of the Knee Joint)

  • 목적: 슬관절 주위 악성 골연부 종양 치료 시 종양의 도약전이나 슬관절 내 침범, 종양인공관절 치환술 후 반복적 감염 치료를 위한 인접골 절제, 국소재발 및 기계적 파괴가 발생한 경우 슬관절 상하부 전치환술은 하지와 슬관절 기능을 보존하는 한 방법이다. 이 중 반복된 감염 치료를 위해 슬관절면 반대측 골까지 절제 후 한시적 슬관절 고정술을 한 환자에서 가동관절로 재치환술 시 적응증, 합병증, 치환물의 생존율에 대하여 알아보고자 하였다. 대상 및 방법: 본 연구는 슬관절 상하부 전치환술 환자 34예 중 슬관절 주위 종양인공관절 치환술 후 반복적인 감염으로 슬관절면 반대측 골까지 절제한 후 한시적 슬관절 고정술이 불가피했던 13예를 대상으로 하였다. 진단, 원발병소의 위치, 슬관절 상하부 전치 환술을 받기 전까지 수술 횟수 및 기간, 재 재건술 후 치환물의 생존율, 합병증, 기능적 결과를 분석하였다. 결과: 슬관절 상하부 종양인공관절 치환물의 Kaplan-Meier 법에 의한 5, 10년 생존율은 각각 69.0%±12.8%, 46.0%±20.7%였다. 총 13예 중 6예(46.2%)에서 주 합병증이 발생하여 3예는 내고정물을 제거 후 슬관절 고정술을, 2예는 내고정물의 부분교체를, 나머지 1예는 감염된 육아조직만 제거하였다. 최종 추시상 가동관절을 유지한 10예의 Musculoskeletal Tumor Society 기능평가 점수는 평균 24.6점(21-27점)이었다. 슬관절 상하부 종양인공관절 치환술이 실패하여 슬관절 고정술로 재치환 한 3예의 기능평가 평균 점수는 12.3점(12-13점)이었다. 가동관절을 유지한 10예의 슬관절 가동 범위는 평균 67°였다(0°-100°). 슬관절 능동적 신전제한은 평균 48° (20°-80°)였다. 결론: 슬관절 주위 종양인공 치환술 후 반복적인 감염으로 슬관절면 반대측 골까지 절제 후 한시적 슬관절 고정술이 불가피했던 환자에서 슬관절 상하부 종양인공관절 치환술은 합병증의 위험성은 높으나 슬관절 고정술에 비해 기능적 결과가 월등하므로 시도할 가치가 있는 술식이다. 고정된 슬관절을 가동관절로 치환 시 반흔 조직을 철저히 제거하고 연부조직을 확보하여 종양인공관절을 삽입 후 굴곡 및 신전이 가능할 정도의 공간을 확보하는 것이 술식의 성공에 중요하다고 생각된다.

On-X$^{circledR}$ 기계판막을 이용한 대동맥판 치환술의 조기 임상 경험 (Early Clinical Experience in Aortic Valve Replacement Using On-X$^{circledR}$Prosthetic Heart Valve)

  • On-X 기계판막은 1997년 처음으로 사용된 이후 전 세계적으로 사용빈도가 증가하고 있다. 본 연구는 이 새로운 기계판막에 대한 유용성과 안전성, 그리고 술 후 혈역학적 변화에 대해 알아보고자 시행되었다. 대상 및 방법: 1999년 4월부터 2002년 8월까지 전남대학교병원 흉부외과에서 On-X 판막을 사용하여 대동맥판막 치환술을 시행했던 52명의 환자를 대상으로 분석하였다. 환자 중 52%가 남자였으며 평균연령은 50$\pm$13세였다. 연구는 판막수술 후 결과보고에 대한 AATS/STS 지침에 따라 분석하였다. 술 전 뉴욕 심장병학회의 기능분류상 class III 이상의 환자가 32명(61.5%)이였으며, 2명의 환자는 이전에 대동맥판막 수술을 받았던 환자였다. 71.1%의 환자에서 동반된 질환에 의해 대동맥판막 치환술과 함께 동반 수술을 시행하였고, 이식된 판막의 크기는 19 mm 13명, 21 mm 26명, 23 mm 10명과 25 mm 3명이었다 평균 외래 추적관찰 기간은 16.6$\pm$10.5 (1∼39)개월이었다. 수술 전과 수술 직후 그리고 3, 6, 12개월에 심초음파를 반복 시행하여 술 후 혈역학적 변화양상을 분석하였다. 결과: 평균 체외순환시간과 대동맥 차단시간은 각각 191$\pm$94.7분과 142$\pm$51.7분이었다. 술 후 조기 및 만기 사망은 없었다. 술 후 12개월간 event-free rate는 혈전색전증 95.6$\pm$6%; 출혈성 합병증 90.2$\pm$4%: 판막 주위누출 92.3$\pm$4%이었으며 전체적인 event-free rate는 76.6$\pm$3%이었다. 수술 후 판막혈전이나 심내막염, 구조적 판막부전은 발생하지 않았다. 술 후 12개월의 평균심박출량은 62.7$\pm$9.8%로 수술 전(55.8$\pm$15.9%)에 비해 의미 있는 증가를 보였다(p=0.006). 좌심실 체적지수와 최대 판막압력차는 수술 전에 247.3$\pm$122.3 g/$\textrm{cm}^2$, 62.5$\pm$38.0 mmHg을 보였으나 술 후 12개월에 각각 155.5$\pm$58.2 g/$\textrm{cm}^2$(p=0.002), 18.0$\pm$10.8 mmHg (p<0.0001)로 감소하는 소견을 보였다. 결론: On-X 기계판막을 이용한 대동맥판막 치환술은 임상적 및 혈역학적으로 만족스러운 조기 결과를 보여주었다. 그러나 대상 환자들에 대한 장기간의 외래 추적관찰과 함께 기존 판막과의 비교가 이루어짐으로써 이 새로운 판막에 대한 유용성과 안정성이 확립될 수 있을 것으로 생각한다.

패혈증환자에서 APACHE III Scoring System의 예후적 가치 (The Prognostic Value of the First Day and Daily Updated Scores of the APACHE III System in Sepsis)

  • 연구배경: 중환자의 예후를 정량화하는 체점체계 중 APACHE III system은 중환자실 제 1병일 점수는 물론 일갱신 점수도 환자의 위험도를 갱신하는 것이 알려진 바 있어 패혈증 환자들에서 APACHE III score의 예후 지표로서의 가치를 알아 보고자 하였다. 방법: 48명의 패혈증 환자들에서 후향적으로 중환자실 제 1병일, 2병일 및 3병일의 APACHE III score를 조사하여 패혈증 생존자와 비생존자 간의 차이를 분석하였다. 결과: 패혈증 생존자는 비생존자에 비해 APACHE III system에 따른 중환자실 제 1병일 정수가 유의하게 낮았으며, 제 2일 및 제 3일의 점수도 각각의 전날 점수에 비해 유의하게 감소하였다. 폐혈증 비생존자의 경우 제 1병일 점수가 생존자에 비해 높았고, 제 2병일 및 제 3병일 점수 역시 각각의 전날 점수에 비해 증가하는 경향을 보였다. 결론: APACHE III system에 따른 제 1병일 점수 및 제 3병일까지의 점수의 일변동은 패혈증 환자의 예후를 반영하는 것으로 사려된다.

국방 온톨로지를 통한 지능형 의사결정지원시스템 구축 및 활용 - 공군 군수상황관리체계 적용 사례 (Construction and Application of Intelligent Decision Support System through Defense Ontology - Application example of Air Force Logistics Situation Management System)

  • 제 4차 산업혁명의 초연결 환경에서 발생하는 많은 양의 데이터는 제 4차 산업혁명을 기존의 생산 환경과 구분지어 주는 주요한 요소이다. 이러한 환경은 데이터를 필요로 하는 동시에 데이터를 생산하는 양면적인 특징을 가진다. 때문에 앞으로의 정보 시스템은 기존의 정보시스템보다 양적인 측면에서 더 많은 데이터를 처리해야 하며, 질적인 측면에서는 많은 데이터 중 사용자의 목적에 부합하는 목표 데이터만을 추출하는 능력이 요구된다. 작은 규모의 정보 시스템에서는 사람이 그 시스템을 정확히 이해하고 필요한 정보를 획득하는 것이 가능하지만, 시스템에 대해 정확한 이해가 어려워진 다양하고 복잡한 시스템에서는 원하는 정보를 획득하는 것이 점점 더 어려워진다. 이러한 문제는 데이터를 사람뿐 아니라 컴퓨터가 이해할 수 있는 온톨로지로 표현하여 다양한 정보처리가 가능하도록 하는 시맨틱 웹(Semantic Web) 구축이 해결책이 될 수 있다. 군에서도 현재 대부분의 업무가 정보 시스템을 통해 이루어지고 있는데, 정보의 입력이나 가공 등 단순처리 중심으로 구축된 기존 시스템이 점점 더 많은 양의 데이터를 포함하게 되면서 시스템을 쉽게 활용하기 위한 노력이 필요한 상황이다. 본 연구에서는 온톨로지를 통한 지능형 의사결정지원시스템의 예로 온톨로지 기반 군수상황관리체계를 제안하고자 한다. 온톨로지 기반 군수상황관리체계는 기존의 군수정보체계의 복잡한 정보를 직관적으로 보여주기 위해 구축된 군수상황관리체계를 온톨로지를 통해 구축하였으며, 성과기반군수지원 계약관리, 부품사전 등의 유용한 기능을 추가 식별하여 온톨로지에 포함하였다. 또한 구축된 온톨로지가 의사결정지원에 활용할 수 있는지를 확인하기 위해 시맨틱 웹 기술을 통해 기본적인 질의응답은 물론 추론 및 함수를 통한 분석기능을 구현하였다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers.
