  • Background: Golf is a popular sport involving overhead activity and engagement of the rotator cuff (RC). This study aimed to determine to what level golfers were able to return to golf following RC repair, the barriers to them returning to golf and factors associated with their failure to return to golf. Methods: Patients preoperatively identifying as golfers undergoing RC repair at the study centre from 2012 to 2020 were retrospectively followed up with to assess their golf-playing status, performance and frequency of play and functional and quality of life (QoL) outcomes. Results: Forty-seven golfers (40 men [85.1%] and 7 women [14.9%]) with a mean age of 56.8 years met the inclusion criteria, and 80.1% were followed up with at a mean of 27.1 months postoperatively. Twenty-nine patients (76.3%) had returned to golf with a mean handicap change of +1.0 (P=0.291). Golf frequency decreased from a mean of 1.8 rounds per week preinjury to 1.5 rounds per week postoperatively (P=0.052). The EuroQol 5-dimension 5-level (EQ-5D-5L) index and visual analog scale (EQ-VAS) score were significantly greater in those returning to golf (P=0.024 and P=0.002), although functional outcome measures were not significantly different. The primary barriers to return were ipsilateral shoulder dysfunction (78%) and loss of the habit of play (22%). Conclusions: Golfers were likely (76%) to return to golf following RC repair, including mostly to their premorbid performance level with little residual symptomatology. Return to golf was associated with a greater QoL. Persistent subjective shoulder dysfunction (78%) was the most common barrier to returning to golf.

  • In this paper, an experimental investigation is carried out to assess the inherent self-compacting properties of geopolymer mortar and its impact on flexural strength of thin-walled ferro-geopolymer box beam. The inherent self-compacting properties of the optimal mix of normal geopolymer mortar was studied and compared with self-compacting cement mortar. To assess the flexural strength of box beams, a total of 3 box beams of size 1500 mm × 200 mm × 150 mm consisting of one ferro-cement box beam having a wall thickness of 40 mm utilizing self-compacting cement mortar and two ferro-geopolymer box beams with geopolymer mortar by varying the wall thickness between 40 mm and 50 mm were moulded. The ferro-cement box beam was cured in water and ferro-geopolymer box beams were cured in heat chamber at 75℃ - 80℃ for 24 hours. After curing, the specimens are subjected to flexural testing by applying load at one-third points. The result shows that the ultimate load carrying capacity of ferro-geopolymer and ferro-cement box beams are almost equal. In addition, the stiffness of the ferro-geoploymer box beam is reduced by 18.50% when compared to ferro-cement box beam. Simultaneously, the ductility index and energy absorption capacity are increased by 88.24% and 30.15%, respectively. It is also observed that the load carrying capacity and stiffness of ferro-geopolymer box beams decreases when the wall thickness is increased. At the same time, the ductility and energy absorption capacity increased by 17.50% and 8.25%, respectively. Moreover, all of the examined beams displayed a shear failure pattern.

  • Health monitoring of civil infrastructures, in particular, old bridges that are still in service, has become more than necessary, given the risk that a possible degradation or failure of these infrastructures can induce on the safety of users in addition to the resulting commercial and economic impact. Bridge integrity assessment has attracted significant research efforts over the past forty years with the aim of developing new damage identification methods applicable to real structures. The bridge of Ouled Mimoun (Tlemcen, Algeria) is one of the oldest railway structure in the country. It was built in 1889. This bridge, which is too low with respect to the level of the road, has suffered multiple shocks from various machines that caused considerable damage to its central part. The present work aims to analyze the stability of this bridge by identifying damages and evaluating the damage rate in different parts of the structure on the basis of a finite element model. The applied method is based on an inverse analysis of the normal stress responses that were calculated from the corresponding recorded strains, during the passage of a real train, by means of a set of strain gauges placed on certain elements of the bridge. The results obtained from the inverse analysis made it possible to successfully locate areas that were really damaged and to estimate the damage rate. These results were also used to detect an excessive rigidity in certain elements due to the presence of plates, which were neglected in the numerical reference model. In the case of the continuous bridge monitoring, this developed method will be a very powerful tool as a smart health monitoring system, allowing engineers to take in time decisions in the event of bridge damage.

  • Successive wetting and drying cycles of concrete due to weather changes can endanger the safety of engineering structures over time. Considering wetting and drying cycles in concrete tests can lead to a more correct and reliable design of engineering structures. This study aims to provide a model that can be used to estimate the resistance properties of concrete under different wetting and drying cycles. Complex sample preparation methods, the necessity for highly accurate and sensitive instruments, early sample failure, and brittle samples all contribute to the difficulty of measuring the strength of concrete in the laboratory. To address these problems, in this study, the potential ability of six machine learning techniques, including ANN, SVM, RF, KNN, XGBoost, and NB, to predict the concrete's tensile strength was investigated by applying 240 datasets obtained using the Brazilian test (80% for training and 20% for test). In conducting the test, the effect of additives such as glass and polypropylene, as well as the effect of wetting and drying cycles on the tensile strength of concrete, was investigated. Finally, the statistical analysis results revealed that the XGBoost model was the most robust one with R2 = 0.9155, mean absolute error (MAE) = 0.1080 Mpa, and variance accounted for (VAF) = 91.54% to predict the concrete tensile strength. This work's significance is that it allows civil engineers to accurately estimate the tensile strength of different types of concrete. In this way, the high time and cost required for the laboratory tests can be eliminated.

  • Purpose: Autologous osteochondral transplantation (AOT) is indicated for patients with a large osteochondral lesion of the talus (OLT), accompanying subchondral cyst, and the failure of bone marrow stimulation (BMS) procedures. Despite the many reports on the clinical results of surgical treatment for medial osteochondral lesions, those of lateral lesions are rare. This paper reports the intermediate-term clinical outcomes after AOT for lateral OLTs. Materials and Methods: Twenty-one patients with lateral OLTs were followed up for at least three years after AOT. The clinical evaluations comprised the Foot and Ankle Outcome Score (FAOS) and Foot and Ankle Ability Measure (FAAM). The radiographic assessment included the irregularity of the articular surface (subchondral plate), the progression of degenerative arthritis, and the changes in talar tilt angle and anterior talar translation. Results: The mean FAOS and FAAM scores improved significantly from 42.1 to 89.5 and 39.5 to 90.6 points, respectively, at the final follow-up (p<0.001). The radiological evaluation revealed two cases (9.5%) of articular step-off ≥2 mm and 1 case (4.8%) of progressive arthritis. The mean talar tilt angle and anterior talar translation improved significantly. As postoperative complications, there was one case of a local wound problem, one case of superficial peroneal nerve injury, and one case of donor site morbidity. At a mean follow-up of 62.3 months, no patient showed a recurrence of instability or required reoperation for OLT. Conclusion: AOT for the lateral OLTs demonstrated satisfactory intermediate-term clinical outcomes, including daily and sports activity abilities. Most OLT could be accessed through lateral ligament division and capsulotomy, and the incidence of iatrogenic complications, such as recurrent sprains or chronic instability, was minimal. AOT appears to be an effective and relatively safe treatment for patients with large lateral osteochondral lesions unresponsive to conservative therapy, with subchondral cysts, or with failed primary BMS.

  • Purpose: A variety of short Exeter stems designed specifically for use in performance of total hip arthroplasty (THA) in primary and revision settings have recently been introduced. Some have been used 'off label' for hip reconstruction. The aim of this study is to report clinical and radiological results from the Exeter V40 125 mm stem in performance of primary THA and revision THA. Materials and Methods: This study had a retrospective design. Insertion of 58 (24 primary, 34 revision) Exeter V40 125 mm stems was performed between 2015 and 2017. The minimum follow-up period was two years. Assessment of the Oxford hip score (OHS), EuroQol-5 Dimension (EQ-5D), and radiological follow-up was performed at one and two years. Results: In the primary group, the preoperative, mean OHS was 13.29. The mean OHS was 32.86 and 23.39 at one-year and two-year post-surgery, respectively. The mean EQ-5D-3L scores were at 0.14, 0.59, and 0.35, preoperatively, at one-year follow-up and two-year follow-up, respectively. In the revision group, the mean preoperative OHS was 19.41. The mean OHS was 30.55 and 26.05 at one-year and two-year post-surgery, respectively. The mean EQ-5D-3L scores were 0.33, 0.61, and 0.48 preoperatively, at one-year follow-up and two-year follow-up, respectively. No progressive or new radiolucent lines were observed around any stem at the time of the final follow-up in all patients in both groups. Conclusion: Encouraging results regarding use of Exeter V40 125 mm stems have been reported up to two years following surgery in primary and revision THA settings.
