Composite Midface Allotransplantation Model with Sensory and Motor Reinnervation (감각과 운동 신경의 재생을 동반한 중안모 동종이식 모델)

  • Face transplantation has been reported over the last six years, and it started in 2004 with the announcement of Cleveland Clinic granting the world's first IRB approval to proceed with human face transplantation. Composite tissue allografts (CTAs) in the facial region are considered to be more immunogenic than other solid organ transplants, so these kinds of animal experiments were also considered as challengeable activities to the facial reconstructive surgeons. For the better understanding of CTAs in the oral and maxillofacial fields, we reviewed several recent articles about facial composite transplantation animal model, and summarized some knowledges of composite midface allotransplantation model with sensory and motor reinnervation in this review article.

A Study on the Measurement of Korean Women′s Head for Headgear Pattern Making (모자제작을 위한 여자 머리 계측에 관한 연구)

  • The purpose of this study is to set measurement parts which are important to identify the size and shape of the head in order to produce tightly fitted hats, and to identify detailed sizes and the major factors of head shape classifications of Korean women. A total of 285 Korean women's in the age group of 18 to 35 years old. It were measured through the direct measurement method by selecting 67 measurement items. Materials were analyzed by SPSS Ver.10 and technology statistics and factor analysis were performed according to the agenda. An attempt was made to conduct factor analysis of the measured region of the head in women's. Here, this study drew the head and the facial parts horizontal size as Factor 1, the head and facial parts vertical size as Factor 2, the circumference and width item as Factor 3, the factor representing the form of head height as Factor 4, the factor expressing the proportion of the facial form as Factor 5 and the factor about the frontal and back head form on the plan of the middle as Factor 6. Through this study, we will be able to systemize head measurement materials that can differentiate Korean's head from other peoples' and can use the results in developing head shape models according to Korean's head shape by selecting major head parts needed to identify the sizes and shapes.

Bimaxillary orthognathic surgery and condylectomy for mandibular condyle osteochondroma: a case report

  • Osteochondroma is rarely reported in the maxillofacial region; however, it is prevalent in the mandibular condyle. This slowly growing tumor may lead to malocclusion and facial asymmetry. A 39-year-old woman complained of gradual development of anterior and posterior unilateral crossbite, which resulted in facial asymmetry. A radiological study disclosed a large tumor mass on the top of the left mandibular condyle. This bony tumor was surgically removed through condylectomy and the remaining condyle head was secured. Subsequently, bimaxillary orthognathic surgery was performed to correct facial asymmetry and malocclusion. Pathological diagnosis was osteochondroma; immunohistochemistry showed that the tumor exhibited a conspicuous expression of BMP-4 and BMP-2 but rarely expression of PCNA. There was no recurrence at least for 1 year after the operation. Patient's functional and esthetic rehabilitation was uneventful.

Analysis of Kidney Function States Based on Face Color Observation by Kidney Ear Acupuncture Point Irritation (신장 이혈 상응점 자극에 따른 얼굴 색상 기반의 신장 기능 상태 분석)

  • As indicators of life quality have recently shown great improvement, alternative medicine is becoming a mainstream health care pattern, in which early stage medical examinations and treatments are usually preformed before diseases occur. In this context, the ear-acupuncture method is increasingly used these day as one of important alternative medicine treatments. In this paper, we performed an experiment of facial region color analysis with kidney ear acupuncture point irritation. For this, we collected facial image samples before and after kidney ear acupuncture point irritation under the same condition. From the samples, the change rates of K, L values of CMYK color system and Lab color system, respectively, on kidney related facial parts were extracted and analyzed to investigate kidney function improvement.

  • Condylar hyperplasia is a self-limiting condition characterized by a slowly progressing, enlargement of the mandible that results in facial asymmetry and a crossbite malocclusion. The facial asymmetry, open bite or crossbite, and radiographic evidence of an enlarged condyle confirm the diagnosis of condylar hyperplasia. The etiology of the condition is unknown. This condition usually first becomes apparent during the second decade of life, when one condyle continues to grow while the other is no longer active. Radiographically, the condyle may appear enlarged or the neck of the condyle maybe elongated or both may occur, Sometimes, however, no radiographically demonstrable condylar abnormality will be noted. Surgical correction with subcondylar osteotomy is the treatment of choice. We have observed two cases of condylar hyperplasia occurred in the left mandibular condyle of 24-year-old and 35-year-old women. We obtained that two cases were shown the followed results; 1. Clinically, both cases was unilaterally developed on C/C area, with temporomandibular disorders and pain, facial asymetry and malocclusion. 2. Radiographically, hyperplastic mass confined to the condyle. 3. Histopathologically, these cases shown increased hypertrophic region in parts, and lamellated bone with irregular trabeculae.

A Clinical Analysis on 250 cases of Inpatients with Facial Paralysis (말초성 안면마비 입원환자 250례에 대한 임상적 고찰)

  • Objectives : This study was performed to analysis the effect of oriental medical care for inpatients with facial paralysis that had visited Dept. of Otolaryngology Oriental medical hospital Dong-eui university. Methods : From January 2008 to September 2010, a clinical study was done on 250 inpatients who were treated as facial nerve paralysis at the Dept. of Otolaryngology Oriental medical hospital Dong-eui university. This study was assessed using the chart analysis. Results : 1. The distribution of sex : female 54.8%, male 45.2%. The distribution of age was pregented that fifty to sixty was the most in 67 cases(26.8%). 2. The distribution of the period of admission : female 12.5 days, male 9.9 days. 3. The distribution of past history : hypertention(18.8%), diabetes-mellitus(10.85%), facial paralysis(9.25%), cerebrovascular disease(4.4%), liver disease(5.6%), hyperlipidemia(1.2%), otitis media(1.2%), herpes zoster(1.2%), cardiac disease(2.45%), thyroid disease(1.2%). 4. The distribution of the region of facial paralysis : Rt(55.36%), Lt(56.52%). 5. Check the mastoid pain : 66.8%(female 73.91%, male 58.04%). 6. Out of prescription(Ko-Bang, 古方), Galgeun-Tang(葛根湯) and Gaejigeogaegayoungchul-Tang was used most in each 34 cases, Daesiho-Tang(大柴胡湯) 30 cases, Galgeungabanha-Tang(葛根加半夏湯) 27 cases, Sihogaeji-Tang(柴胡桂枝湯) 14 cases, Hwanggigaejiomul-Tang 12 cases, Odu-Tang(烏頭湯) 10 cases, Chijadaehwangsi-Tang 10 cases, Gaejigagalgeun-Tang(桂枝加葛根湯) 7 cases, Banhasasim-Tang(半夏瀉心湯) 5 cases, Injinho-Tang(茵蔯蒿湯) 5 cases in order. 7. The distribution of herb group : Mahwang-Jae(麻黃劑) 31.72%, Gaeji-Jae(桂枝劑) 26.00%, Siho-Gae(柴胡劑) 20.70%, Chija-Gae(梔子劑) 7.49%, Buja-Jae(附子劑) 4.41%, Banhahwanggeum-Gae(半夏黃芩劑) 3.08%, Daehwang-Gae(大黃劑) 2.64%, Bockryeong-Gae(茯笭劑) 1.76%, Jisil-Gae(枳實劑) 1.32%, Insam-Gae(人蔘劑) 0.88% in order. 8. The distribution of House-Brackmann grade of admission : Gr Ⅳ 74.85%, Gr.III 13.6%, Gr.V 11.6% in order. 9. The distribution of House-Brackmann grade of discharge : Gr.III 56%, Gr.IV 38.4%, Gr.II 5.6% in order. 10. The average number of OPD follow up is 6.46. Conclusion : This results indicated that oriental medical treatment with Ko-bang(古方) can be an effective way to treat facial paralysis. The more patients we treat with Ko-bang(古方), the more clinical report is accumulated. Then it would be helpful to map out a systematic treatment on facial paralysis.

Standardization Study of Thermal Imaging using the Acupoints in Human Body (적외선 체열 영상의 표준화 연구 부위별 대표 혈위를 중심으로)

  • Objective: The purpose of this study was to invigorate the use of infrared thermal imaging apparatus as a diagnostic tool in Oriental medicine by providing standard temperature on specific acupoints. Methods: Subjects for the study was recruited through an advertisement in the school homepage(www.sangji.ac.kr) explaining the objectives and methods. 100 volunteers agreeing to terms were selected and measured the full body thermal image. Common acupoints used in the clinical surrounding were chosen and final 63 acupoints were selected for the measurement. Male/female and right/left readings were obtained for the analysis. Results: Following results were obtained from analyzing the body temperature of 50 male subjects and 50 female subjects 1. Subjects participating in the study ranged from 19 years of age to 44 years. Median male age at $26.86{\pm}6.02$ and female age at $22.88{\pm}2.74$, respectively. 2. For all acupoints, no significant differences were witnessed between the gender and right, left side of the body. 3. 10 acupoints from the facial region(18 bilateral), 8 acupoints from the chest/abdomen region(10 bilateral), 6 acupoints from the back region(11 bilateral), 17 acupoints from the upper extremity(34 bilateral), and 22 acupoints from the lower extremity(44 bilateral) were chosen. 4. In the facial region, BL2 showed the highest temperature and GV26 showed the lowest. 5. In the chest/abdomen region, CV22 showed the highest temperature and CV6 showed the lowest. 6. In the back region, GV14 showed the highest temperature and BL23 showed the lowest. 7. In the upper extremity region, jianqian(extra point) showed the highest temperature and baxie(extra point) showed the lowest. 8. In the lower extremity region, KI1 and bafeng(extra point) shoed the highest temperature and BL40 showed the lowest. Conclusions: Based on the standard body temperature measured on specific acupoints throughout the body, we hope these findings can trigger further studies on applications of infrared thermal imaging and clinical usage in the field of oriental medicine.

Multiple Face Segmentation and Tracking Based on Robust Hausdorff Distance Matching

  • This paper describes a system fur tracking multiple faces in an input video sequence using facial convex hull based facial segmentation and robust hausdorff distance. The algorithm adapts skin color reference map in YCbCr color space and hair color reference map in RGB color space for classifying face region. Then, we obtain an initial face model with preprocessing and convex hull. For tracking, this algorithm computes displacement of the point set between frames using a robust hausdorff distance and the best possible displacement is selected. Finally, the initial face model is updated using the displacement. We provide an example to illustrate the proposed tracking algorithm, which efficiently tracks rotating and zooming faces as well as existing multiple faces in video sequences obtained from CCD camera.

Adenocarcinoma of the parotid gland with calcification (석회화를 수반한 이하선내 선양암종)

  • A 78-year-old woman was referred to Chonbuk National University Dental Hospital complaining of facial palsy and palpable mass on the right parotid gland area. Clinical examination showed non-specific findings of the intraoral region, but showed asymmetrical facial appearance. Panoramic view showed a large amorphous calcified mass on the posterior to the mandibular ramus and thin cortical plate of the posterior ramus. Sialogram showed constriction of the main duct and no further filling of striated, intercalated ducts and parenchymal areas. CT scans demonstrated an irregular, infiltrating mass with slight enhancement in the right parotid gland. The mass showed necrotic areas and calcifications. Bone scan showed marked accumulation of /sup 99m/Tc-MDP on the right posterior maxilla. Microscopic findings demonstrated the minimal morphologic alterations and rare mitotic figures within tumor cells, and diagnosed as adenocarcinoma (NOS, Grade II). This report could be aid in the diagnosis of calcified lesions of the salivary gland.

Online Face Avatar Motion Control based on Face Tracking

  • In this paper, a novel system for avatar motion controlling by tracking face is presented. The system is composed of three main parts: firstly, LCS (Local Cluster Searching) method based face feature detection algorithm, secondly, HMM based feature points recognition algorithm, and finally, avatar controlling and animation generation algorithm. In LCS method, face region can be divided into many small piece regions in horizontal and vertical direction. Then the method will judge each cross point that if it is an object point, edge point or the background point. The HMM method will distinguish the mouth, eyes, nose etc. from these feature points. Based on the detected facial feature points, the 3D avatar is controlled by two ways: avatar orientation and animation, the avatar orientation controlling information can be acquired by analyzing facial geometric information; avatar animation can be generated from the face feature points smoothly. And finally for evaluating performance of the developed system, we implement the system on Window XP OS, the results show that the system can have an excellent performance.
