Febrile urinary tract infection in children: changes in epidemiology, etiology, and antibiotic resistance patterns over a decade

  • Background: Understanding the epidemiology and prevalence of febrile urinary tract infection (fUTI) in children is important for risk stratification and selecting appropriate urine sample collection candidates to aid in its diagnosis and treatment. Purpose: This study aimed to analyze the epidemiology, etiology, and changes in antibiotic susceptibility patterns of the first fUTI in children. Methods: This retrospective observational cohort study included children younger than 19 years of age who were diagnosed and treated for their first fUTI in 2006-2016. Electronic medical records were analyzed and radiologic images were evaluated. Results: A total of 359 patients (median age, 5.1 months; interquartile range, 3.0-10.5 months) fit the inclusion criteria; of them, 78.0% (n=280) were younger than 12 months old. The male to female ratio was 5.3:1 for patients aged 0-2 months, 2.1:1 for those 3-5 months, and 1.6:1 for those 6-11 months. Beyond 12 months of age, there was a female predominance. Escherichia coli was the leading cause (83.8%), followed by Enterococcus species (6.7%), and Klebsiella pneumoniae (3.6%). Significant yearly increases in the proportions of multidrug-resistant strains (P<0.001) and extended-spectrum beta-lactamase (ESBL) producers (P<0.001) were observed. In patients with vesicoureteral reflux (VUR), the overall recurrence rate was 53.6% (n=15). A significantly higher recurrence rate was observed when the fUTI was caused by an ESBL versus non-ESBL producer (75.0% vs. 30.0%, P=0.03). Conclusion: fUTI was most prevalent in children younger than 12 months of age and showed a female predominance in patients older than 12 months of age. The proportion of ESBL producers causing fUTI is increasing. Carbapenems, rather than noncarbapenems, should be considered for treating fUTI caused by ESBL-producing enteric gram-negative rods to reduce short-term recurrence rates in children with VUR.

Reaction Rates for the Oxidation of Pitch based Carbon Fibers in Air and Carbon Dioxide Gas

  • Two types of carbon fiber based high modulus- and isotropic-pitch were exposed to isothermal oxidation in air and $CO_2$ gas and the weight change was measured by TGA apparatus. The kinetic equation was introduced $f=1-{\exp}(-at^b)$ and the constant b was obtained in the range of 1.02~1.68 for the isotropic fiber and obtained 0.91~1.93 for the high modulus fiber respectively. In considering the effect of the atmosphere for isothermal oxidation, the value of the constant b obtained in the carbon dioxide was higher than that obtained in the air. Therefore, it was found that the pitch based carbon fiber shows sigmoidal characteristic when it is oxidized in the carbon dioxide. In addition, it was also found that $k_f = 0.5$, which was reaction constant at f = 0.5, was a very useful parameter for evaluation of the oxidation reactivity of pitch based carbon fibers. According to the consideration, it is suggested that the conversion-time curves of the pitch based carbon fibers are correlated by normalized equation $f=1-{\exp}(-A{\tau}^B)$, where ${\tau}=t/t_f= 0.5$.

Study on Effect of Fluorine Content on the Synthesis of Machinable Glass-ceramics Based on Fluorophlogopite Crystals (플루오르함량이 Fluorophologopite 결정들을 함유하고 있는 기계 가공성 결정화유리의 합성에 미치는 영향에 관한 연구)

  • The crystallization behaviour and the machinability of mica glass-ceramics with the content of F1 were studied. The material was made from the $K_2O-MgO-Al_2O_3-B_2O_3-SiO_2-F$ glasses by the heattreatment at 80$0^{\circ}C$-110$0^{\circ}C$ where the content of F-1 was changed in the range from 1, 3wt% to 6.1wt%. X-ray diffraction phase analysis and optical observation were adopted to study the crystallization behaviour. The machinability was measured by a manual sawing test and MOR. The crystal phases of these glass-ceramics identified by XRD were chondrodite fluoborite and norbergite at low temperature but fluorophlogopite at high temperature. The crystallization of glasses containing 1.3wt% -2.5wt% F-1 were predominately controlled by surface crystallization while the crystallization of glasses containing 3.8 wt% -6.1wt% F-1 were controlled by volume crystallization. Among the test the best machinability and strength value were obtained from those specimens contained fluoride 4.2wt% -4.4wt% and when the heattreatment was performed at 95$0^{\circ}C$-110$0^{\circ}C$ for 2 hours.

Development of LTCC Materials for RF Module (RF 모듈용 LTCC 소재 개발)

  • In this study, new LTCC materials of $ZnWO_4$-LiF system were developed for the application to RF Module fabrication. Pure $ZnWO_4$ must be sintered above $1050^{\circ}C$ in order to obtain up to 98% of full density. The measured dielectric constant ($\epsilon_r$)quality factor ($Q{\times}f0$), and temperature coefficient of resonant frequency ($\tau_f$ were 15.5, 74000 GHz, and $-70ppm^{\circ}C$, respectively. LiF addition resulted in a liquid phase formation at 81$0^{\circ}C$ due to interaction between ZnWO$_4$ and LiF. Therefore, ZnWO$_4$ with 0.5∼1.5 wt% LiF could be densified at $850^{\circ}C$. In the given LiF addition range, the sintering shrinkage increased with increasing LiF content. Addition of LiF slightly lowered the dielectric constant from 15.5 to 14.2∼15 due to lower dielectric constant of LiF. Qxfo value decreased with increasing LiF content. This can be explained in terms of the interaction between LiF and $ZnWO_4$, and inhomogeneity of grain structure.

Comparative Study on the Acoustic Characteristics of the Korean Vowel /a/ before and after LMS (후두미세수술 전후 /아/의 음향적 특성 비교)

  • The aim of this study is to show the differences in acoustic parameters between a pathological voice /a/ caused by vocal polyp and a normal voice /a/ produced after LMS (Laryngeal Microscopic Surgery). It was expected that voices of two kinds could be analyzed effectively in terms of HNR in specific frequency bands than in all frequency bands. For this study, 10 patients' voice were recorded before and after LMS and then were manipulated in terms of four acoustic parameter. It was found out that (a) frequency bands of 500Hz in the range of 1,000Hz to 4,000Hz were very useful to obtain HNR values; (b) frequency bands in the range of 1,248Hz to 5,500Hz on a log scale were very useful to obtain HNR values; (c) F0 dropped after LMS but not significantly; (d) the bandwidth of the second formant (B2) decreased significantly after LMS, while that of the first formant (B1) decreased after LMS but not significantly.

  • We have reported to make nanostructured cobalt oxide electrode that have large capacitance over than 400 F/g (specific capacitance) and good cycleability. But, it had serious demerits of low voltage range under 0.5 V and low power density. Therefore, we need to increase voltage range of cobalt oxide electrode. We report here on the electrochemical properties of sol-gel-derived nanoparticulate cobalt xerogel in 1M KOH solution and aqueous polymeric gel electrolyte. In solution electrolyte, cobalt oxide electrode had over 250 F/g capacitance consisted of EDLC and pseudocapacitance. In gel electrolyte, cobalt oxide electrode had around 100 F/g capacitance. This capacitance was only electric double layer capacitance of active surface area. In solution electrolyte, potassium ion as working ion reacted with both of layers easily. However, In gel electrolyte, reacted with only surface-active layer. Itis very hard to reach resistive layer. So, we have studied on pretreatment of electrode to contain working ions easily. We'll report more details.

Kinetics of Thermal Dehydration of Ha-dong Kaolin (하동카오린의 열분해속도에 관한 연구)

  • The kinetics of the dehydration of Ha-dong kaolin was studied isothermally at various temperatures. Dehydration rate was measured by thermogravimetry method in the temperature range of 440~50$0^{\circ}C$ and the particle size range of 170~325mesh. The general equation f($\alpha$)=kt, where $\alpha$ is the fraction reacted in the time and the function f($\alpha$) depends on the reaction mechanism, was applied to this reaction. The function, f($\alpha$) was obtained by application of reduced-time plot and plot of lnln (1-$\alpha$) vs. ln (time), and expressed as (1-$\alpha$) ln (1-$\alpha$)+$\alpha$=kt. The dehydration followed the diffusion-controlled reaction model and gave activation energy of 30Kcal/mole.

Comparison of temperature dependance between short and long period fiber gratings (단주기 광섬유 격자(Fiber Grating)와 장주기 광섬유 격자의 온도 의존성 비교)

  • An optical fiber short period grating of 0.7 nm as a 3 dB wavelength linewidth was fabricated using a Gaussian distributed KrF Eximer laser and a phase mask. This grating has temperature dependancy of 0.01 nm/$^{\circ}C$ over the range of -10 $^{\circ}C$ ~ 70 $^{\circ}C$and no difference between temperature directions. An optical fiber long period grating of 14.22 nm as a 3 dB linewidth was also fabricated using a amplitude mask and has dependancy of 0.01 nm/$^{\circ}C$ over the same range.
