전방십자인대 재건술 환자에 대한 비손상측과 손상측 간의 하지 근수축 특성 비교 (Comparison of the Muscle Contractile Properties on Lower Extremity Between Non-injury and Injury in Subjects with Anterior Cruciate Ligament Reconstruction)

  • Background: The purpose of this study was to identify the comparison of the muscle contractile properties on lower extremity between non-injury and injury in subjects with anterior cruciate ligament reconstruction (ACLR). Methods: Twenty-four subjects on the post-operative ACLR participated in this study. Measurement method were using tensiomygraphy to analyze the displacement maximum(Dm) and contraction time(Tc) on the both quadriceps. Results: Compared difference of the Dm between non-injury and injury, the rectus femoris (RF) and vastus lateralis (VL) increased significantly (p<.01). However, vastus medialis (VM) no significantly difference between non-injury and injury. Compared difference of the Tc between non-injury and injury, the RF increased significantly (p<.01) but VL and VM no significantly difference between non-injury and injury. Conclusions: These findings suggest that occurred muscle atrophy of the RF and VL and change properties of muscle fibers on the RF from fast muscle fibers to slow muscle fibers on the injury side of post-operative ACLR. Therefore, when apply to rehabilitation of lower extremity for post-operative ACLR, we should consider the enhance of RF and VL on injury side.

좌측 하지의 관절구축을 동반한 작열통의 치험 (A Case of Causalgia with Contracture Deformity in Lower Extremity)

  • Causalgia is a symptom complex usually consisting of burning pain, hyperesthesia and atrophy of the involved extremity. The pain may be aggravated by contact, motion of extremity or emotional excitement. It occurs following incomplete nerve injury. The patient was a 58-year-old male with a 3-year history of causalgic pain of left lower extremity. He had multiple fractures with degloving injury of left lower extremity in an automobile accident. The pain was exacerbated by movement, and he experienced knee joint and ankle joint contracture. The patient's pain decreased after continuous epidural block with 1% lidocaine and 0.25% bupivacaine. He also received lumbar sympathetic ganglion alcohol block resulting in much improvement of level of pain and walking disturbance.

  • Small Overlap test decided on IIHS (Insurance Institute for Highway Safety) evaluates a robustness of the car body and a passenger injury protection. A current vehicle is satisfied with a robustness of the car body for Small Overlap test but not meet the requirements of passenger injuries because it is difficult to inquire into the actual state of injury mechanism for Small Overlap test. The study shows the new injury mechanism which represents the relation between a structure of the vehicle placed with the lift foot and the low extremity injury.

국내 하지손상의 발생현황에 대한 분석 (Incidences of Lower Extremity Injuries in Korea)

  • Purpose: We conducted this retrospective epidemiological study to assess the incidence and severity of lower extremity injuries in Korea Methods: For this study, we retrospectively reviewed nationwide lower-extremity injury data compiled from 2001 to 2003 based on the National Injury Database, what included National Health Insurance Corporation (NHIC), Car Insurance, and Industry Insurance data. Data were standardized in terms of demographic characteristics, region, and socioeconomic status by using NHIC data. To assess the degree of the injuries, we used the Modified Abbreviated Injury Scale (MoAIS), what has been changed from the International Classification of Disease-10 (ICD-10) code. By using the Excess Mortality Ratio-adjusted Injury Severity Score (EMR-ISS), we classified the degree of severity into four categories: mild, moderate, severe and critical. Results: From 2001 to 2003, lower extremity injuries increased slightly, with a yearly average of 2,437,335. Insurance data should that lower-extremity injuries were the most common, followed by upper-extremity injuries. Significant difference were seen in the numbers of lower extremity injuries based on gender and age. As for provinces, Seoul and Gyeongi provinces had the highest numbers of cases. Junlabukdo had the highest rate of 55,282 cases per 1 million people for standardized gender and population. The annual incidence of the insured patients with lower extrimity injuries was higher than the employer's medical insurance contributions to the medical insurance program. Daily cases occur most often in May and June, with the lowest occurrences being in January and February. Conclusion: The result of this study shows that lower extremity injuries comprised common cause of all injuries. In addition, differences associated with gender, location and socioeconomic status were observed. Further studies are needed to find reasons and then this knowledge will allow strategies to prevent the lower extremity injuries.

상지말초신경손상에 대한 침술치료의 효과: 체계적 문헌고찰 (Acupuncture for Upper Extremity Peripheral Nerve Injury: A Systematic Review)

  • Objectives The purpose of this study is to systematically explore the effects and safety of acupuncture treatment for upper extremity peripheral nerve injury and to review the methodology of clinical trials. Methods We searched 9 electronic databases(3 international, 1 Chinese, 5 Korean) including English, Korean and Chinese, up to December 2017 for randomized controlled trials which evaluated the effects of the acupuncture in patients with upper extremity peripheral nerve injury. We abstracted the designs of the randomized clinical trials and the method of acupuncture treatment according to the Standards for Reporting Interventions in Clinical Trials of Acupuncture(STRICTA). Results A total of 8 papers were reviewed. All randomized clinical trials were conducted in China. Of them, five studies(62.5%) were electro-acupuncture as intervention. All randomized clinical trials reported favorable effects of acupuncture treatments compared to baseline or control group with outcomes of efficacy rate. However risk of bias seemed high. LI4, LI11, SI3, PC3, PC6 were most frequently used for acupoints to treat upper extremity peripheral nerve injury. Conclusions These results suggest that it is recommended to develop more detailed reporting standards for acupuncture treatment method. In the future, well designed randomized clinical trials which evaluate the effects and safety of acupuncture treatment for upper extremity peripheral nerve injury is highly needed.

The effect of neuropathic pain on quality of life, depression levels, and sleep quality in patients with combat-related extremity injuries

  • Purpose: There is limited research on the effects of neuropathic pain (NP) on quality of life, depression levels, and sleep quality in patients with combat-related extremity injuries. This study evaluated whether patients with combat-related extremity injuries with and without NP had differences in quality of life, sleep quality, and depression levels. Methods: A total of 98 patients with combat-related extremity injuries, 52 with NP and 46 without, were included in this cross-sectional study. The presence of NP was determined using the Leeds Assessment of Neuropathic Symptoms and Signs questionnaire. The outcome measures were a visual analogue scale (VAS), the 36-Item Short Form Survey, the Beck Depression Inventory, and the Pittsburgh Sleep Quality Index (PSQI). Results: The VAS subparameter scores for pain (all P<0.05), PSQI sleep dur ation subscale scores (P=0.025), PSQI sleep disturbance subscale scores (P=0.016), and PSQI total scores (P=0.020) were significantly higher in patients with NP than those without. Logistic regression analysis showed that VAS scores of 5 and above for average pain during the previous 4 weeks contributed independently to the prediction of NP. Conclusions: Patients with combat-related extremity injuries with NP had more pain and poorer sleep quality than those without NP. Sleep quality should be evaluated as part of the diagnostic work-up in patients with combat-related extremity injury with NP, and interventions to improve sleep quality may help manage NP in this patient group.

운동심상이 만성 경수 손상 환자의 근활성도와 일상생활에 미치는 영향 (Effect of a Motor Imagery Program on Upper Extremity Strength and Activities of Daily Living of Chronic Cervical Spinal Cord Injury Patients)

  • Purpose: The purpose of this study is to determine the effect of motor imagery training on residual upper extremity strength and activities of daily living of chronic cervical spinal cord injury patients. Methods: Twelve ASIA A B patients, who had more than a 12-month duration of illness and C5 or 6 motor nerve injury level, were randomly divided into experimental group (n=6) and control group (n=6). Patients in the experimental group performed motor imagery training for five minutes prior to general muscle strengthening training, while those in the control group performed general muscle strengthening training only. The training was performed five times per week, 30 minutes per day, for a period of four weeks. General muscle strengthening training consisted of a progressive resistive exercise for residual upper extremity. Motor imagery training consisted of imagining this task performance. Before and after the training, EMG activity using BTS Pocket Electromyography and Spinal Cord Independent Measure III(SCIM III) were compared and analyzed. Results: The residual upper extremity muscle strengths showed improvement in both groups after training. Comparison of muscle strength improvement between the two groups showed a statistically significant improvement in the experimental group compared to the control group (p<0.05). SCIM III measurements showed significant improvement in the scores for Self-care and Transfer items in the experimental group. Conclusion: Motor imagery training was more effective than general muscle strengthening training in improving the residual upper extremity muscle strength and activities of daily living of patients with chronic cervical spinal cord injury.

Epidemiologic study of hand and upper extremity injuries by power tools

  • Background Hand injuries caused by chain saws, electric saws, and hand grinders range from simple lacerations to tendon injuries, fractures, and even amputations. This study aimed to understand the distribution of various types of hand and upper extremity injuries caused by power tools, in order to help prevent them, by investigating the incidence and cause of power tool injuries treated over a 4-year period at a single institution in Korea. Methods We reviewed the medical records of patients who visited a single institution for power tool-induced injuries from 2011 to 2014. The distribution of sex, age, injured body part, type of injury, and mechanism of injury sustained by patients who received hand and upper extremity injuries from using an engine saw, electric saw, or hand grinder was evaluated. Results Among 594 subjects who were injured by power tools, 261 cases were hand and upper extremity injuries. The average age was 53.2 years. Tendon injury was the most common type of injury. An electric saw was the most common type of power tool used. More injuries occurred in non-occupational settings than in occupational settings. Conclusions In this study, power tool-induced hand and upper extremity injuries were mostly caused by direct contact with electric saw blades. More injuries occurred due to non-occupational use of these tools, but the ratios of amputations and structural injuries were similar in the non-occupational and occupational groups.

Effect of core training on dynamic posture control, lower extremity injury, and joint position sense in ski athletes

  • 이 연구의 목적은 8주간의 코어 훈련이 스키선수의 동적자세조절, 하체 손상 및 고유수용성 관절위치감각에 미치는 영향을 보고자 하였다. 이 연구에 참여한 대상자는 스키선수 20명으로 무작위로 코어 운동군 10명과 통제군 10명으로 분류하였다. 코어 훈련프로그램은 벤치, 사이드웨이 벤치, 프랜크, 사이드 브리지 및 서파인 브리지로 구성하였고, 8주간 주 3회 실시하였다. 연구 결과 동적자세조절 검사는 좌·우측 후방안쪽 뻗기에서 유의한 효과가 있었고, 하체 손상 준거 검사는 좌·우측 종합점수에서 통계적으로 유의한 효과가 있었다. 또한 고유수용성 관절위치감각은 왼발 15°과 왼발 45°에서 유의한 감소가 있었다. 결론적으로 8주간의 코어 훈련은 스키선수의 동적자세조절 능력, 하체 손상 감소 및 관절위치감각을 향상시키는 것으로 나타났다.

삼두 근 견열 손상 - 3례 보고- (Avulsion of the Triceps Brachii - 3 cases report -)

  • Avulsion of the triceps brachii is an uncommon injury. The usual mechanism of injury is fall onto an outstretched hand but can occur after direct contact injuries. Diagnosis is critical and dependent on history, physical examination, and radiological findings. Rupture of triceps brachii occurs most frequently at the tendo-osseous insertion and may be complete or partial, and could be associated with radial head fracture. Treatments include surgical repair of the complete rupture and immobilization of partial triceps rupture. We report 3 cases of triceps avulsion fractures treated by open surgical repair with literature reviews.
