Effect of Asymmetrical Tonic Neck Reflex on Weight Bearing of the Extremities (비대칭성 긴장성 경반사가 사지의 체중지지에 미치는 영향)

  • The purposes of this study were 1) to compare weight distribution and ratio of the extremities between normal and hemiplegic children in quadruped position and 2) to compare the effect of ATNR on weight bearing of the extremities between normal and hemiplegic children. The subjects fer the study were 48 children(24 normal, 24 hemiplegic) between the ages 3 to 6. They were teated weight distribution and ratio of the extremities in the neutral position of head and by passive right and left rotation of the head in the quadruped position. The data wert analized by paired t-test. The results were as follows: 1. In the neutral position of head, normal group was not difference on weight distribution and ratio of the extremities and hemiplegic group was more weight bearing on the sound upper extremity than the affected upper extremity(p<.01). 2. When the head rotated to the dominant side or sound side passively, there was not a significant difference between normal and hemiplegic group. 3. When the head rotated to the nondominant side or affected side passively, there was a significant difference between nondominant upper extremity nf normal and affected upper extremity of hemiplegic group(p<.05).

Clinical study for the one case complaining both lower extremities weakness after parturition (산후 하지무력 환자 1례 임상경과 보고)

  • Purpose: The purpose of this study is to report the effect of TKM to both lower extremities weakness after parturition who is diagnosed as Subacute inflammatory demyelinating peripheral neuropathy. Methods: The patient was a 31-year-old woman who couldn't walk at all by herself because of both lower extremities weakness after parturition. The patient was demonstrated as shin-yang-heo, and treated by Pal-mi-ji-hwang-hwan, Acupuncture treatment of Shin-jeong-gyeok, and Moxa treatment of Gwan-won. And the progress of symtoms was evaluated by observation of her walking status, cheking the change of VAS, and inspecting the DITI. Result: The patient could walk by herself. The feeling of both lower extremities weakness improved to VAS 2. The paraesthesia of the soles of feet improved to VAS 4. And the DITI state slightly got better. Conclusion: TKM is expected to have positive effect on puerperium disease especially the symptoms of pain, weakness, paraesthesia, etc.

The Effect of Wrist and Trunk Weight Loading using Sandbags on Gait in Chronic Stroke Patients (모래주머니를 이용한 팔목과 몸통의 무게 증가가 만성 뇌졸중 환자들의 보행에 미치는 영향)

  • Objective: This study aimed to determine the effect of wrist and trunk weight loading using sandbags in stroke patients in order to provide the quantitative data for enhancement of gait movement. Method: Twelve stroke patients, who have been diagnosed with hemiplegia over a year ago, were participated in this study. All subjects were asked to perform normal walking [N], wrist sandbag walking [W], wrist & trunk sandbag walking [WT], and both wrist sandbag walking [B] and both wrist & trunk sandbag walking [BT], respectively. Eight infrared cameras were used to collect the raw data. Gait parameters, arm swing, shoulder-pelvic kinematics, and lower extremity joint angle were calculated to examine the differences during walking. Results: As a result, there were no significant differences in the gait parameters, shoulder-pelvis, and lower extremities joint angles, but significant differences were found in the range of motion and the anteversion in arm swing. Conclusion: Wrist and trunk weight loading using sandbags affected the movement of the upper extremities only while it did not affect the movement of the lower extremities. It implies that it can reduce the risk of falling caused by a sudden movement change in lower extremities. In addition, the wrist and trunk weight loading using sandbags can induce changes in movement of the upper extremities independently and contribute to functional rehabilitation through resistance training.

  • Benign monomelic amyotrophy(BMA) is an uncommon cause of progressive mildly disabling atrophy and weakness of a limb. It predominantly affects the distal upper limb of young men. I present two men with BMA of the lower extremities. Although the disorder seemed clinically confined to a arm or leg, I confirmed by electromyography evidence of denervation in the other extremities. I reviewed the literatures and discuss the differential diagnosis. BMA is a diagnosis of exclusion that requires consideration in men with unilateral arm or leg atrophy and weakness.

Reliability of the Anterior Thigh Free Flap for Reconstruction of the Extremities (상하지 재건을 위한 유리피판 공여부로서 전측대퇴부의 신뢰성)

  • With the advent of microsurgery, perforator free flap is nowadays considered the first choice for reconstruction of the extensive defect of the extremities because of their moderate thickness. Among them, anterior (anterolateral and anteromedial) thigh perforator free flaps provide the first choice for reconstruction of various soft tissue defects of the extremities with many advantage such as its large, uniform thickness, long vascular pedicle with proper vessel size and minimal donor site morbidity. But, it has still some criticism of unreliable perforators which makes us very careful in elevating the flap. Between March of 2006 and February of 2007, we treated 7 patients of soft tissue defects in the hand and lower extremities with anterior thigh perforator free flap at Hallym and DongGuk University Hospital. We performed 6 anterolateral thigh perforator free flaps based on the descending branch of lateral circumflex femoral artery (LCFA) and 1 anteromedial thigh perforator free flap based on the innominate branch of the LCFA. While approaching for the anterolateral thigh free flap, we happen to meet the cases which we should change into the anteromedial thigh free flap uneventfully on the operating field. In contrast to the original design of anterolateral thigh free flap, we had to harvest the anteromedial thigh perforator free flap in 1 case. All the anterior thigh perforator free flaps survived completely except 1 case of partial necrosis due to venous congestion. Donor sites were closed primarily and healed uneventfully within 2 weeks. Patients were satisfied with the functionally and aesthetically acceptable results. Although doppler sonography is strongly recommended preoperatively in planning the anterior thigh perforator free flaps, we should always remember the variation in vascular anatomy and be ready to change the flap choice from the anterolateral to anteromedial intraoperatively. we provide a review of the literature and present our series of anterior thigh perforator free flaps for reconstruction of the extremities.

The Effect of Massage to the Different Regions of the Body on the Relaxation in Healthy Women (부위별 마사지가 건강한 성인여성의 신체이완에 미치는 영향)

  • This study was to determine whether the massage would enhance the physical relaxation in healthy women, and whether the effects of massage on the relaxation would be different by the applied regions of the body. Five healthy young women participated in the two test sessions of back and lower extremities massage in a chamber controlled at an air temperature of $28^{\circ}C$ and a relative humidity of $50{\pm}5%$. The slow stroke massage with lavender aroma oil for 30 minutes was applied to the subjects with a 10-min. and a 20-min rest time before and after the application, respectively. Rectal temperature decreased and the chest and thigh skin temperatures increased with the massage application, and the changes of the temperatures were greater with lower extremities massage than with back massage. Heart rate also decreased during massage period and tended to be lower with lower extremities massage. Systolic and diastolic blood pressure decreased with lower extremities massage. These results show that the slow stroke massage for 30 minutes can induce physiological responses related with the enhanced relaxation in healthy women, and lower extremities massage is more effective on the physical relaxation rather than back massage.

Indications and Prognostic Factors of Groin Flap for Reconstruction of the Extremities (사지 재건에 있어 서혜부 유리 피판술의 적응증 및 예후 인자)

  • There are several advantages for groin flap, but its small and unpredictable vessels of pedicle have made it to lose its initial popularity. Although it would be ideal flap when it is focused on its useful advantages such as relative larger size, low donor site morbidity and possible bone graft, there have been few studies for prognostic factors for successful groin flap. Authors intended to determine prognostic factors which are relative with success of free groin flap. From January 1985 to December 2007, 107 patients who underwent groin flap for reconstruction of extremities were selected consecutively. Univariate and multivariate analysis were performed to determine prognostic factors which were related with success of groin flap. Eighty of 107 (74.8%) flaps survived. There was significant difference in success rate according to the recipient site. Nineteen of 20 cases (95%) survived in upper extremities, but 61 of 87 cases (70.1%) survived in lower extremities, which was statistically significant (p=0.022). Univariate analysis showed that mean diameter of donor veins was significantly larger in success group (p=0.021). Groin flap is recommended for reconstruction of upper extremities than lower extremities. It is thought to be critical that surgeons try to match vessel diameters between donor and recipient site.

Effect of Vestibular Sensory Stimulation Exercise on Limit of Stability, Dynamic Weight Shift, and Upper and Lower Extremities Reaction Time in Adult Women (안뜰감각 자극운동이 성인여성의 안정성한계, 동적체중이동 및 팔다리 반응시간에 미치는 영향)

  • Purpose : The purpose of this study was to investigate the effect of vestibular sensory stimulation exercise on the limit of stability, dynamic weight shift, and upper and lower extremities reaction time in adult women. Methods : This study was conducted with 30 female. All subjects were randomly and equally assigned to an experimental group and a control group of 15 each. Subjects assigned to the experimental group received vestibular sensory stimulation training for 6 weeks. For the intervention, vestibular sensory stimulation exercises were conducted by referring to the Hamid exercise method and the Cawthorne-Cooksey exercise method, and the control group did not receive any intervention. All subjects were tested for limit of stability, dynamic weight shift, and upper and lower extremities reaction time before and after the intervention. Results : The results of this study showed significant differences between groups in reaction time, moving velocity, and directional control in the limit of stability test after intervention. In the dynamic weight shift test after intervention, there was a significant difference between the groups in the slow directional control of left and right. And in the upper and lower extremities reaction time test after intervention, both scores and reaction time showed significant differences between groups. Conclusion : As a result, the three vestibular sensory stimulation exercises applied in this study improved the limit of stability, dynamic weight shift, and upper and lower extremities reaction time.

  • When the eldery with limited mobility and disabled use a wheelchairs to move, it can cause decreased exercise ability like decline muscular strength in upper limb and lower extremities. The disabled people suffers with spinal cord injuries or post stroke hemiplegia are easily exposed to secondary problems due to limited mobility. In this paper, We designed intelligent wheelchair robot system for upper limb and lower extremities exercise/rehabilitation considering the characteristics of these severely disabled person. The system consists of an electric wheelchair, biometrics module for Identification characteristics of users, upper limb and lower extremities rehabilitation. In this paper, describes the design and configurations and of developed robot. Also, In order to verify the system function, conduct performance evaluation targeting non-disabled about risk context analysis with biomedical signal change and upper limb and lower extremities rehabilitation over wheelchair robot move. Consequently, it indicate sufficient tracking performance for rehabilitation as at about 86.7% average accuracy for risk context analysis and upper limb angle of 2.5 and lower extremities angle of 2.3 degrees maximum error range of joint angle.
