Early Results of Mitral Valve Replacement - Clinical analysis of 158 cases - (승모판막 치환의 조기 성적;158 임상보고례)

  • The results of the clinical observations on the 158 cases of the mitral valvular heart disease treated at the Department of Thoracic and Cardiovascular Surgery, Masan Koryo General Hospital from June, 1986 to December, 1991 were as follows; 1. There were 50 men and 108 women with sex ratio 1: 2.2. 2. The age of patient varied widely from 13 years to 65years. 3. The preoperative functional level accoridng to the NYHA classification were class II, III, IV in 18%, 67%, 15%. 4. All 158 patients were operated on under direct vision using extracorporeal circulation, mitral valve replacement in 118 cases, mitral and aortic valve replacement 40 cases. 5. The follow up period was between 2 months and 65 months postoperatively. [mean 30.1 months] 6. The early operative motality was 5.7%[9 cases], late motality was 4%[6 cases]. 7. The cardiothoracic ratio in the chest X-ray decreased at the 6th month postoperation. [pre-op: 0.60$\pm$0.07, post-op: 0.56$\pm$0.06 p<0.01]

Triple Valve Replacement -A report of two cases- (삼판막 이식수술 (2례 보고))

  • Simultaneous triple valve replacements were performed in two patients on January and April 1980 at Seoul National University Hospital. The first case was 17 years old male patient with a history of exertional dyspnea for 7 years. He was in class III by the NYHA functional classification and diagnosed as aortic insufficiency, mitral steno-insufficiency and tricuspid insufficiency. The second case was 46 years old male patient suffered from exertional dyspnea for 5 years, He was in class IV and diagnosed as aortic stenoinsufficiency, mitral stenoinsufficiency and tricuspid insufficiency. Triple valve replacements were performed under the deep hypothermia and pharmacologic cardiac arrest with aortic cross clamping for 80 minutes to 159 minutes. Total extracorporeal circulation time were 197 and 176 minutes respectively. The postoperative courses were uneventful.

Bilateral Coronary Artery-Pulmonary Artery Fistula - Reports of a Case- (양측 관상동맥-폐동맥간 동맥루 치험 1례)

  • Bilateral coronary artery-pulmonary artery fistula is very uncommon congenital heart disease which occupy small percentage of all coronary arterio-venous fistulas. We experienced a case who was 52 years old female with bilateral coronary artery-pulmonary artery fistula. She complained exertional dyspnea k angina[coronary steal syndrome]. On physical examination, any cardiac murmur was not audible. There was no 0y step-up in right heart catheterization. But selective coronary angiography revealed tortuous aberrant vessels which originated from the canal branch of the right coronary artery k the left anterior descending coronary artery. Both aberrant vessels traversed the right ventricular outflow tract, and conjoined just proximal the pulmonic annulus and drained into the main pulmonary artery. The operation was performed under the extracorporeal circulation with beating heart. The procedures were suture-ligation of the draining orifice in main pulmonary artery & the feeding vessels on the right ventricular outflow tract. Postoperatively her complaints were completely disappeared and the selective coronary angiography revealed no left-to-right shunt.

Emergency Quadrido-Bentall Procedure for Aortic Rupture in a Patient with Behcet's Disease

  • Cardiovascular involvement in cases of Behcet's disease is a rare but life-threatening condition, and prosthetic valve detachment is a frequent and serious complication attributable to Behcet's disease following the surgical repair of aortic regurgitation. We report the case of a patient with Behcet's disease presenting with contained aortic rupture around the aortic root. The patient had previously undergone aortic valve surgery three times due to recurrent prosthetic valve detachment. An emergency operation was performed, consisting of aortic root replacement (ARR) using a composite valved conduit and the replacement of the hemiarch. ARR may be an appropriate surgical option for patients with Behcet's disease in order to prevent recurrence of the disease.

Dissecting Aortic Aneurysm - Case Report - (해리성 대동맥류 1례 보)

  • Dissecting aneurysm has long been recognized as an ominous and highly lethal form of aortic disease. Aortic dissection are characterized by longitudinal separation of aortic media and extension proximally, distally or both from the site of intimal tear. DeBakey and associates defined three types based on where the process originates and how far extends. In type I, intimal tear is located in the ascending aorta and extend beyond the descending aorta. We experienced a case of dissecting aneurysm, Type I of DeBakey`s classification which dissection extend to the left iliac artery. The patient was 61 years old woman and suffered from excruciating pain on admission. Excision of aneurysm and ascending aorta reconstruction using to Dacron Vascular Prosthesis were performed under extracorporeal circulation. The post-operative course was uneventful and follow up is continued.

  • A personal computer, IBM compatible 386, was utilized to analyse total series of 1867 cases of cardiovascular operations including 1060 cases of open cardiac surgeries performed at the Cardiac Center of Paik Hospital from 1986 to 1992.All data were first assembled from the operation files and the operation records from the patient charts. Code numbers were placed to the diagnosis, the operations and the diseases.The analysis revealed the distributions of 1867 cases of operations;819 cases[77%] of the congenital heart diseases and 207 cases[20%] of the acquired heart diseases among 1060 open heart operations. The operative mortalities were 3.7% for the congenital heart diseases and 6.3% for the acquired heart diseases.The vascular operations showed the operative mortalities of 11.8% for the aortic surgery under extracorporeal circulation and 0.7% for the other vascular surgery.

  • The clinical experience with the activated clotting time[A.C.T.] for the control of heparin and protamine therapy during cardiopulmonary bypass in 40 patients between April, 1987 and September, 1987 is reviewed retrospectively. All of patients used with cold blood potassium cardioplegia for myocardial protection under standard cardiopulmonary bypass, priming and perfusate techniques respectively. This study was divided into 2 groups of patients followed by cardiopulmonary bypass time. Twenty patients, within 60 minutes of cardiopulmonary bypass time[group A] were compared with twenty patients, from 60 to 120 minutes of cardiopulmonary bypass time[group B]. Using blood cardioplegia for myocardial protection, Author observed wide variation of A.C.T. in individual response to initial heparinization[2mg /kg] and no requirement of additional heparin during cardiopulmonary bypass until 120 minutes. Total heparin amount during cardiopulmonary bypass was not related to body weight and body surface area in the both groups. After cardiopulmonary bypass, amounts of protamine for neutralization of heparin were more required in group B.

Surgical Treatment of Mitral Valvular Disease (승모판막질환의 외과적 요법)

  • The results of the clinical observations on the 76 cases of the mitral valvular heart disease treated in the Department of Thoracic and Cardiovascular Surgery, Chung-nam National University Hospital during the period of 3 years from June, 1983 to July, 1986, are as follows: l. Of 76 cases, 43 were male and 33 were female with sex ratio of 1.3:1. 2. The age of the patient varied widely from 12 years of the youngest to 60 years of the oldest. 3. The main clinical symptoms on admission were dyspnea on exertion[100%], palpitation[42%], generalized weakness[29%], indigestion[18%], hemoptysis[16%]. 4. The preoperative functional levels according to NYHA classification were class II, III, IV in 9%, 63%, 28% respectively. 5. All 76 patients were operated on under direct vision using extracorporeal circulation, open mitral commissurotomy was done in 15 cases, mitral valve replacement in 37 cases, mitral valve replacement and aortic valve replacement in 11 cases, mitral valve replacement and tricuspid annuloplasty in 8 cases. 6. The operative mortality was 11.8% and results of the operation were good and excellent in 65 cases of survivors.

  • Total anomalous pulmonary venous connection is relatively rare cyanotic congenital heart diaease, which represents 1-4% of all congenital cardiac defects. Generally in the majority cases, severe heart failure and cyanosis develops in the early infancy. Because of high mortality in the untreated infants and surgical risk, there are still many things to be improved. Two patients with total anomalous pulmonary venous connection are presented, which we recently experienced. The one was 10 year old female with supracardiac type drained through left innominate vein, and survived the operation and continuous to do well for 1 year. The other 5 year old female with mixed type (right pulmonary vein drained via coronary sinus and left pulmonary vein through left innominate vein) was operated successfully under hypothermia and extracorporeal circulation, and followed up for 6 months without problem. It was very rare case in the literature.

  • The clinical observations on the 63 cases of the mitral valvular heart disease treated in the department of Thoracic and Cardiovascular Surgery, Kosin medical college, during the period of 5 years from January, 1986 to December, 1989, are as follows. 1. Total 63 cases, 23 were male and 40 were female with sex ratio of 1: 1.7. 2. The age distribution was widely varied from 7 years of the youngest to 65 years oldest, average age was 35.3 year old. 3. The main clinical symptom and sign were D.O.E, general weakness, fatigability, palpitation, chest pain, hepatomegaly, pitting edema and both neck vein distension. 4. The preoperative N.Y.H.A functional classification were class II, III, IV in 28%, 54%, 16%. 5. The preoperative diagnosis were MS - 6.3%, MR - 53.9%, MSR - 36.5%, MVP - 3.1%. 6. All 63 patients were operated on the under direct vision using extracorporeal circulation, MVR in 18 cases, MVR and Tricuspid annuloplasty in 24 cases, DVR in 3 cases, DVR and Tricuspid annuloplasty in 5 cases, TVR in 1 case, Mitral annuloplasty in 4 cases, mitral valvuloplasty in 7 cases and mitral commissurotomy in 2 cases 7. The operative mortality was 7.9%, and 5 years actuarial survival rate was 89%.
