Background: Patients with chronic stroke often shows decreased trunk muscle activity and trunk performance. To resolve these problems, many trunk stabilizing techniques including the abdominal drawing-in maneuver (ADIM) and the diaphragmatic breathing maneuver (DBM) are used to improve trunk muscle strength. Objects: To compare the effects of the ADIM and the DBM on abdominal muscle thickness, trunk control, and balance in patients with chronic stroke. Methods: This was a randomized controlled trial. Nineteen patients were randomly allocated to the ADIM ($n_1=10$) and DBM ($n_2=9$) groups. The ADIM and DBM techniques were performed three times per week for 4 weeks. The thicknesses of the transversus abdominis (TrA), internal oblique muscle, and external oblique muscles on the paretic and non-paretic sides, Trunk Impairment Scale (TIS) score, and Berg Balance Scale (BBS) score were used to assess changes in motor development after 4 weeks of training. Results: After the training periods, the TrA thickness on the paretic side, TIS score, and BBS score improved significantly in both groups compared to baseline (p<.05). TIS score was significantly greater in the DBM group than in the ADIM group (p<.05). Conclusion: This study demonstrated that ADIM and DBM are beneficial for improving TrA muscle thickness in the paretic side, trunk control, and balance ability. Intergroup comparison revealed that TIS score was significantly improved in the DBM group versus the ADIM group. Thus, DBM may be an effective treatment for low trunk muscle activity and performance in patients with chronic stroke.

Background: This study comparatively evaluated the effects of equipment-based pilates exercises (EPE) and lumbar stability exercises (LSE) in patients with chronic low back pain in terms of their Visual Analogue Scale(VAS), Oswestry Disability Index(ODI), and abdominal muscle thickness. Methods: A total of 30 participants were recruited and randomly assigned to either the EPE or the LSE. The VAS, ODI, and abdominal muscle thicknesses of the participants were measured before and after the intervention. Results: The EPE were more effective in terms of the duration of a sustained reduction in VAS scores. post hoc test revealed that EPE were more efficacious in terms of a sustained improvement in ODI scores. With respect to changes in abdominal muscle thickness, there was a significant difference in the thickness of internal oblique muscles(IO) and the external oblique muscles(EO) between the two groups. Conclusion: In this study, both types of exercise interventions resulted in improvements in the VAS, ODI scores, and abdominal muscle thickness in patients with chronic low back pain. However, EPE were found to be more effective than LPE in terms of longer sustained improvements in VAS and ODI scores. Also, with respect to abdominal muscle thickness, the thickness of EO and IO improved only in the EPE group.
The purpose of this study was to determine the effects of slump sitting postures on the masticatory, neck, shoulder, and trunk muscles associated with work-related musculoskeletal disorders (WRMD). Eleven healthy adults (age, $23.3{\pm}2.7$ yrs; height, $174.0{\pm}4.1$ cm; weight, $61.4{\pm}6.6$ kg) participated in this study. The participants were free of injury history and neurologic deficits in the masticatory, neck muscles and upper extremities at the time of participation. The subjects were asked to perform erect and slump sitting postures under the guidance of physical therapists. The surface electromyography (EMG) was recorded from the anterior temporalis, masseter, upper trapezius, serratus anterior, middle trapezius, L3 paraspinal, external abdominal oblique, gluteus maximus muscles of 11 adults as they performed visual terminal display work, which are known as the weakened and tightened muscles owing to WRMD. The recorded signals were averaged and normalized to the mean amplitude of the EMG signal obtained during submaximal reference voluntary contractions. The results of study were as follows: The masseter, upper trapezius, serratus anterior, middle trapezius, L3 paraspinal, external abdominal oblique muscles significantly differed in the slump sitting posture (p<.05). The muscle activities of the serratus anterior, middle trapezius muscle, and external abdominal oblique were significantly lower and that of the masseter, upper trapezius, L3 paraspinal muscles were significantly higher. Further research is needed to assess the motor control problems and the function of the deep muscles in posture stability of patients with WRMD.
Purpose: This study examined the effect of vibration exercise on the thickness of the oblique extrinsic, oblique abdominal, and biceps muscles, which are trunk muscles, targeting children with spastic cerebral palsy. Methods: The participants in this study were 20 children (8 male and 12 female) with cerebral palsy aged 5-10 years. They were classified into two groups using a randomized allocation method, and the trunk muscle thickness was measured using an ultrasound-imaging device before and six weeks after the experiment. A paired t-test was used for the within-group changes, and an independent t-test was used for the inter-group changes. The significance level was set to α=0.05. Results: There was a significant increase in the inter-group change in the experimental group and control group in the intra-group change in the external oblique muscle and internal oblique muscle. After six weeks, there was a significant increase in the experimental group compared to the control group. Conclusion: Vibration exercise had a positive effect on the trunk muscle thickness of children with cerebral palsy. Vibration exercise produced a significant difference in the changes in the trunk muscle thickness in children with cerebral palsy compared to no vibration exercise. These results may provide basic data for future research and as a training method for strengthening the trunk muscles in clinical trials.
Purpose: Leg-crossing sitting is very common for men and woman. No solid evidence exists for either a beneficial or a detrimental effect of this posture. This study investigated the change of activities of trunk muscles between the normal group and the low back pain group during various leg-crossing positions. Methods: The subjects were consisted of 10 subjects who don't have low back pain and 10 subjects who have low back pain. In this study, we used electromyography(EMG) to evaluate the activities of both the trunk muscles (rectus abdominis, external oblique, internal oblique, and multifidus) during various leg-crossing positions (up-right, leg-crossing, tailor-crossing, and ankle-crossing). We analyzed the data by using repeated one way ANOVA. Results: In normal group, there were increased in EMG activities of trunk muscles, but no significant differences during leg-crossing positions. In back pain group, there were increased in EMG activities of right external oblique, left. internal oblique, and both multifidus muscles in leg-crossing and tailor-crossing position, but no significant differences during leg-crossing positions. There was no significant difference of muscle activity of trunk muscles between the back pain group and the normal group. Conclusion: We suggest that low back pain people who have weak muscles of rectus abdominis, external and internal oblique are often experienced in leg-crossing posture than normal. To compensate this unstability of trunk, leg-crossing posture is substituted passive structure for activities of active muscle.

PURPOSE: The purpose of this study was to investigate the effects of supplementary verbal encouragement according to the gender on the activation of abdominal muscles during performance of crunch exercise in healthy subjects. METHODS: A Total of 32 healthy subjects (15 male and 17 female) were randomly allocated to two conditions, crunch exercise with Ki-hap and verbal encouragement with same gender (CKVS) and crunch exercise with Ki-hap and verbal encouragement with different gender (CKVD). The interventions were conducted over three trials in each condition, and measurements were performed on each subject by one examiner in three trials. The activation of rectus abdominis, external oblique, internal oblique, and transverse abdominis muscles were evaluated using electromyography (EMG) during performance of crunch exercise with Ki-hap and verbal encouragement with same gender or different gender, respectively. RESULTS: The results showed that there were no significantly difference in the activation of all abdominal muscles in those of the CKVS compared with the CKVD (p>.05). The results also showed that there were no significantly difference in the ratio of muscle activation at global muscle to that at local muscle between two conditions (p>.05). CONCLUSION: These findings demonstrated that application of the supplementary verbal encouragement by gender does not affect to activate the abdominal muscles of subject to perform a movement, at the same time, it would suggest positive evidence for improving activation of abdominal muscles.

Purpose : Kegel exercises reported that it is effective in managing stress-related or complex urinary incontinence through contraction and relaxation of the pelvic floor muscles. In many previous studies, it was confirmed that Kegel exercise is involved in respiration as well as urinary system diseases. However, there is a lack of research on the effect of pelvic setting when performing Kegel exercises. Therefore, this study was conducted to investigate the effect on maximum voluntary ventilation (MVV) and abdominal muscle thickness through Kegel exercise after lumbar-pelvic motor control using pressure biofeedback unit (PBU). Methods : The subjects of this study were 10 healthy female students in their 20s. Subjects measured MVV with a spirometer. In hooklying, external oblique, internal oblique, and transverse abdominis of the dominant hand were measured using ultrasound. The measured value was an average of three times. After one week of intervention, measurements were made in the same manner. Before Kegel exercise, pelvic setting training was performed using PBU. In hooklying, PBU was placed in the waist and set to 40 mmHg, and it was adjusted to 60 mmHg through pelvic muscle contraction. For Kegel exercise, the pelvis was first set using PBU, and then the pelvic floor muscles were contracted for 8 seconds and relaxed for 8 seconds, 10 times, 1 set, and 3 sets. Results : In MVV, a significant difference was confirmed after exercise than before exercise (p<.05). There was also a significant difference in abdominal muscle thickness before and after exercise (p<.05). Conclusion : Based on the results of this study, Kegel exercise using PBU had an effect on MVV and abdominal muscle thickness. However, since this study was conducted without a control group as a preliminary study, additional research should be conducted to supplement this.
Background: The serratus anterior (SA) is a muscle that performs protraction of the scapulothoracic joint and plays a role in stabilizing the scapula. Imbalances or weaknesses in SA activation are associated with a variety of shoulder dysfunctions, making selective SA strengthening important for rehabilitation. Objects: We aimed to compare the muscle activation of the pectoralis major (PM), SA, external oblique (EO), and internal oblique (IO) during the push-up plus (PUP) exercise with isometric hip adduction (HA) and abdominal drawing-in maneuver (ADIM). Methods: Nineteen healthy male participants performed three PUP exercises: standard PUP, PUP with ADIM, and PUP with HA. Surface electromyography was used to measure and analyze the muscle activity for PM, SA, EO, and IO. Results: PUP with HA showed the lowest PM activity and highest SA activity, and no significant difference was observed between PUP and PUP with ADIM. PUP with ADIM showed significantly the highest EO and IO activity, followed by PUP with HA and PUP. Additionally, PUP with HA showed the lowest PM/SA ratio, and no significant difference was noted between PUP and PUP with ADIM. Conclusion: PUP with HA was able to show high SA muscle activity while reducing PM muscle activity. In addition, PUP with HA can lead to higher EO and IO muscle activity than standard PUP. This exercise could be used as a practical exercise method to selectively strengthen SA and improve scapular muscle stability during early shoulder rehabilitation.
The purpose of this study was to compare the thickness of the abdominal muscles during single leg holding exercise (SLH) in a hooked lying position on stable surface and on a foam roller. Healthy twenty subjects who had no medical history of lower extremity or lower back pain were recruited for this study. Muscle thickness of transverse abdominis (TrA), internal oblique (IO), and external oblique (EO) was recorded using real-time ultrasonography during SLH. Paired t-test with Bonferroni adjustment was used to compare the muscles thickness during SLH on stable surface and on a foam roller. The result showed that TrA and IO muscle demonstrated greater thickness during SLH on foam roller than those on the stable surface. This finding suggests that SLH on an unstable foam roller is more effective to increase thicknesses of TrA and IO muscles than stable surface.
Purpose: Abdominal breathing exercises are recommended to activate the breathing muscles and the pelvic floor muscles, as well as to increase postural alignment during exhalation. The purpose of this study was to clarify the effect of improving abdominal muscle strength on menstrual pain in women in their twenties using abdominal breathing exercises. Method: In this study, 32 female university students were included as the subjects. The subjects were divided into two groups based on the area of menstrual pain: lumbar pain (n=16) and lower abdomen pain (n=16). The abdominal breathing positions were divided into two positions, which included a hook lying position and hip and knee flexions at $90^{\circ}$ in the supine position. Exercises were used to strengthen the abdominal muscles during exhalation. Four sets of the exercises were completed three times a week over the course of eight weeks. The degree of pain was measured using the Numeric Rating Scale (NRS). Muscle thickness was measured using an ultrasound. Result: The thickness of the transverse abdominis (TrA) and internal oblique (IO) increased in the lower abdomen pain group. However, thickness of the external oblique (EO) did not increased following abdominal breathing. No significant difference in posture was identified in the lower abdomen group. TrA thickness increased significantly in the lumbar pain group. However, thickness did not increase significantly in the lumbar pain group. In addition, the lumbar pain group experienced no significant effects on posture. IO thickness increased following hip and knee flexions at $90^{\circ}$ in the lumbar pain group. Menstrual pain decreased following intervention in both groups. There was no significant difference in the degree of pain reduction between both groups. Conclusion: As examples of alternative medicine, abdominal breathing exercises may be effective in decreasing menstrual pain.
