Development of an experimental model for radiation-induced inhibition of cranial bone regeneration

  • Background: Radiation therapy is widely employed in the treatment of head and neck cancer. Adverse effects of therapeutic irradiation include delayed bone healing after dental extraction or impaired bone regeneration at the irradiated bony defect. Development of a reliable experimental model may be beneficial to study tissue regeneration in the irradiated field. The current study aimed to develop a relevant animal model of post-radiation cranial bone defect. Methods: A lead shielding block was designed for selective external irradiation of the mouse calvaria. Critical-size calvarial defect was created 2 weeks after the irradiation. The defect was filled with a collagen scaffold, with or without incorporation of bone morphogenetic protein 2 (BMP-2) (1 ㎍/ml). The non-irradiated mice treated with or without BMP-2-included scaffold served as control. Four weeks after the surgery, the specimens were harvested and the degree of bone formation was evaluated by histological and radiographical examinations. Results: BMP-2-treated scaffold yielded significant bone regeneration in the mice calvarial defects. However, a single fraction of external irradiation was observed to eliminate the bone regeneration capacity of the BMP-2-incorporated scaffold without influencing the survival of the animals. Conclusion: The current study established an efficient model for post-radiation cranial bone regeneration and can be applied for evaluating the robust bone formation system using various chemokines or agents in unfavorable, demanding radiation-related bone defect models.

  • It is known that radiation therapy is a kind of treatment choices of the maxillofacial tumors. This study is designed to investigate the irradiation effects on rat's periodontal tissues as functional tissues which relate to tooth-support, hard tissue formation and destruction. 20 rats (Sprague-Dowley branch, male) were devided into control group of 4 and experimental groupe of 16. Experimental group was singly exposed to Co-60 irradiation with 10 Gy in the head and neck region. Animals were sacrificed on 2 days, 1 week, 2 weeks and 3 weeks after the irradiation. The specimens were observed by histopathological examination employing H-E stain, van-Gieson stain and PA-ACH fluorescent stain. The results were as follows: 1. Cementoblasts and osteoblasts were gradually lost and rearranged along the external surfaces of the cementum and alveolar bone, but osteoclasts were almost not affected. 2. The cell numbers of the periodontal ligament were decreased due to the cellular atrophy and degeneration, but recovered almost normally on the 3rd week after irradiation. 3. The collagen fibers within the periodontal ligament were irregularly oriented, became finer and decreased in number. 4. The vessels of the periodontal ligament were decreased at the initial stage but increased again on the 2nd week after irradiation, and the hemorrhagic appearances, occurred within the tissues, due to the arterial destruction, were lasted until 3 weeks after irradiation. 5. The glycogen within the periodontal ligament was gradually increased and stored in the matrices of the cemental side on the 1st week after irradiation, but recovered almost normally on the 3rd week after irradiation.

CHARACTERISTICS OF THE PNEUMATIC TRANSFER SYSTEM AND THE IRRADIATION HOLE AT THE HANARO RESEARCH REACTOR

  • This paper describes the results of an irradiation test and the specifications of the pneumatic transfer system (PTS) in the NAA #3 irradiation hole at the HANARO research reactor, which was reinstalled after some modifications of the operation mode at the end of 2004. The outer and inner diameters of the PE transfer tube are 34.1 and 27.5 mm, respectively. PE rabbit was used for sample irradiation. The $N_2$ gas pressure of the PTS lines was adjusted to 0.75 bar. The average sending time to the reactor was $8.5{\pm}0.3$ s and the average receiving time back to the receiver was $3.2{\pm}0.2$ s. The internal and external temperature of the irradiation tube was measured in a range of 50 to $80^{\circ}C$ for a 40 s to 80 s irradiation time, respectively. The optimum irradiation time was estimated to be less than 80 s. The thermal, epithermal and fast neutron flux at 30 MW thermal power were $1.42{\pm}0.01{\times}10^{14},\;1.51{\pm}0.04{\times}10^{13}$ and $9.48{\pm}0.69{\times}10^{11} n{\cdot}cm^{-2}{\codt}s^{1-}$, respectively. The cadmium ratio was approximately 9.40. The data obtained will be applied to supplement user information and for reactor management.

  • Photovoltaic (PV) energy is a renewable and harmless energy which offers many advantages. However, solar energy is an extreme intermittent and inconstant energy source. In order to improve the photovoltaic system efficiency and utilize the solar energy more fully, and the DC current and DC power vary with the irradiation and module temperature, it is necessary to study the characteristics of photovoltaic I-V and P-V according to the external factors. This paper presents the analysis of characteristics of photovoltaic I-V and P-V according to the irradiation and the module temperature. The results show that the DC current and the DC power of the photovoltaic system are increased along with the increasing values of irradiation and module temperature.

In order to improve the photovoltaic system efficiency and utilize the solar energy more fully, and the DC current and DC power vary with the irradiation and module temperature, it is necessary to study the characteristics of photovoltaic I-V and P-V according to the external factors. This paper presents the analysis of characteristics of photovoltaic I-V and P-V according to the irradiation and the module temperature. The results show that the DC current and the DC power of the photovoltaic system are increased along with the increasing values of irradiation.

  • Intraoperative Radiation therapy (IORT) is a cancer treatment modality in which resectable masses or organs are removed surgically and residual cancer calls are sterilized by irradiation with a single massive dose during while patient is still anesthetized. Because it is possible that the turner mass can be visualized directly at the time of surgical exploration, tumor volume can be determined more precisely and at the same tin e sensitive adjacent structures can be pulled aside from the irradiation. With these theoretical advantages as compare to conventional external irradiation, IORT can improve the therapeutic ratio of tumor control to normal tissue injury. Yonsei cancer center initiated a pilot study of multidisciplinary IORT program in february of 1986 for the fist attempt in Korea. IORT Was performed in 7 patients with stomach cancer by using existing NELAC-1018 Linear Accelerator treatment room as a surgical suite. IOTR team included department of surgery, Department of Anethesiology, Department of Clinical pathology, operating room nursing personal and Department of radiation oncology.

External Beau Radiation Therapy of Adenocarcinoma of the Prostate

  • The record of radiation therapy cases during a five year period at the University of Michigan Hospitals has been analyzed. Of a total of 73 adenocarcinoma of prostate, the majority belonged to Stage B and C which represented 49 and 20 cases, respectively. The mortality rate after irradiation was clearly related to the tumor stage. Local irradiation resulted in $88\%$ of the local control of well·differentiated adenocarcinomas. Stage C cases had $50\%$ mortality, whereas that of Stage B patients was $14\%$. Results of this study are in general agreement with previous data in terms of the local disease control after irradiation and provide a basis for conservative radiotherapy regimen as an approach in the treatment of localized prostatic carcinomas.

  • Purpose : This study was done to evaluate preliminarily the role of intraluminal brachytherapy in the radiation treatment of non-metastatic esophageal cancer, Materials and Methods: We analyzed follow-up result of 21 patients treated at the dept. of therapeutic radiology in Gyeongsang national university hospital between April, 1989 and August, 1992. All patients received neoadjuvant chemotherapy(5-FU, Cispl-atin). Fifteen Patients were treated with external beam alone, and in remaining 6 patients, the external beam radiotherapy followed by intraluminal brachytherapy was done. Results : Among 21 patients, 7 patients showed complete tumor regression after completion of radiotherapy. But 2 of these complete responder recurred at the site of primary disease, so ultimate local control rate was $23.8\%$(5/21). Local control rate according to radiation treatment modality was $6.7\%$(1/15) in patients treaed with external irradiation only, and $66.7\%$ in patients treated with combined external irradiation and intraluminal brachytherapy. The 2 year NED survival rate was $6.6.\%$ in the former and $66.7\%$ in the latter. Conclusion: Although there should be consideration about case selection for addition of intraluminal brachytherapy intraluminal brachytherapy may be considerded as one of the method to enhance the local control probability of esophageal cancer.

자궁경부암의 방사선치료성적 (Results of Radiotherapy for the Uterine Cervical Cancer)

  • One hundred fifty-four patients with the carcinoma of the uterine cervix were studied retrospectively to assess the result and impact of treatment at Department of Radiation Oncology, Korea University, Hae-Wha Hospital from Feb 1981 through Dec. 1986. Prior to radiotherapy, the patients were evaluated and staged by recommendation of FIGO including physical examination, pelvic examination, cystoscopy, rectosigmoidoscopy, chest X-ray, IVP. Ba enema. Also, an additional pelvic CT scan was obtained for some of the patients. The patients were treated by radiotherapy alone or adjuvant postoperative irradiation; in case of radiation therapy only, whole pelvic irradiation was given with Co-60 teletherapy unit via AP and PA parallel opposing fields or 4-oblique fields, 180 cGy per day, 5 days per week and intracavitary insertion was performed. In satges Ia, Ib, and IIa with small primary lesion, external irradiation was initially given to pelvis up to $2,000\~3,000\;cGy/2frac{1}{2}\;-3frac{1}{2}$ weeks and then intracavitary insertion was performed using Fletcher-Mini-Declos Applicator with cesium-137 cources and followed by external irradiation of $1,000\~2,000\;cGy/1frac{1}{2}\;-2frac{1}{2}$weeks via AP and PA parallel opposing fields with midline shield to spare of bladder and rectum. However, if the primary lesion is large, external irradiation was given without midline shield. More than stages IIb, the patients were treated by external beam irradiation up to 5,400cGy/30f for 6 weeks via 4-oblique portals and at the dose of 5,040cGy/28f the field was cut 5cm from the top margin for spare of small bowel, and followed by intracavitary irradiation, If there was residual tumor an additional dose of $900\~l,200cGy/5\~7f$ was given to parametrium and/or residual tumor area. Total dose of radiation to A and B-point were as follows; A-point; In early stages, Ia, Ib, IIa; $8,000\~9,000$ B-point $5,000\~6,000 cGy$ A-point; In advanced stages IIb, IIIa, IIIb; $9,000\~10,000$ B-point $60,000\~7,000cGy$ The results were obtained and as fellows; 1 The patients distribution according to FIGO staging system were stage Ia 6, Ib 27, IIa 28, IIb 54, IIIa 12, IIIb 18, and stage IVa 9. 2. Value of CT scan were demonstration of cervix tumor mass, parametrial and pelvic side wall tumor spread, pelvic and inguinal lymph nodes metastases, and hydronephrosis. Three dimensional quantitative demonstration of tumor volume is also important in planning radiation therapy. Another advantage of CT scan was detection of recurrent tumor after radiation or surgery. 3. Local control rate of tumor according to the size was $91.3\%$ for less than 5cm in size and $44.6\%$ in tumor over 5cm (p<0.0068). 4. Thirty out of 50 recurrent sites has locoregional failures and 17 cases has distant metastases. And the para-aortic lymph nodes were the most common site for distant metastases. 5. The most common complication was temporal rectal bleeding which was controlled most by conservative management. However, 4 patients required for endoscopic cauterization. 6. The 5-year survival rates showed; stage la and Ib $95\%,\;stage\;IIa\;81\%\;stage\;lIb\;67\%,\;stage\;IIIa\;37.7\%,\;stage\;IIIb\;23\%$ and 3-year survival rate of stage IVa showed $11.6%$, retrospectively.
