Spontaneous extensor pollicis longus tendon rupture is commonly caused by attrition of the tendon from trauma or inflammatory processes. We experienced a patient with extensor pollicis longus tendon rupture after steroid injection, in which the rupture may have been caused by the effects of steroid itself as well as direct damage from the needle. A 51-year-old woman complained of inability to extend her right thumb at the first metacarpophalangal & interphalangeal joint level. The patient had a history of local steroid injection into the dorsal & radial side of wrist on two occations, and had no history of trauma or rheumatologic disease. After a physical examination of the patient, we decided to explore the wrist. The patient agreed with operation. Intraoperatively, an incision was made into the wrist and the proximal and distal ends of the ruptured extensor pollicis longus tendon were identified. The defect between the proximal and the distal end was measured to approach 8cm, and a palmaris longus tendon graft was performed. After three months of rehabilitation, the first metacarpophalangal & interphalangeal joint recovered the normal range of motion. Steroid injection has been widely used in various musculoskeletal disorders such as rheumatoid arthritis and osteoarthritis. However, inadvertent steroid injection into the extra or intra articular spaces may lead to tendon rupture. Steroids reduce tensile strength by decreasing tenocyte activity and collagen synthesis. Also, the physical effect of direct needle-stick injury into the mesotenon and blood vessels around the tendon may cause damage. In addition, hematoma and edema may increase pressure around the tendon and compromise blood supply, leading to tendon degeneration and subsequent rupture. When injecting steroid into an articular area, all physicians should have a complete understanding of the surrounding anatomy and always keep in mind the hazards of such procedures.

Late rupture of the extensor pollicis longus (EPL) tendon is an occasional complication following a distal radius fracture. Early rupture of the EPL tendon as a complication of distal radius fracture is rare. We report one case of early rupture of the EPL tendon after distal radius fracture.

Chronic extensor hallucis longus (EHL) tendon rupture is relatively rare, but in such cases, surgical repair is necessary to prevent hallux dysfunction. To the best of our knowledge, reconstruction of chronic EHL rupture using a split tibialis anterior tendon autograft has not been previously reported. Here we present a case of spontaneous EHL tendon rupture with a 5 cm gap in a healthy 57-year-old woman. At the 1-year follow-up evaluation, hallux function was restored, and the patient was well satisfied with results.

Spontaneous rupture of the extensor tendon has been reported in association with predisposing inflammatory conditions including rheumatoid arthritis, diabetes, trauma, tophaceous gout, and steroid injection. The authors experienced a case of spontaneous rupture of the extensor digitorum longus tendons caused by an osteophyte of the tarsometatarsal joint in a patient with rheumatoid arthritis. The authors stress that aggressive treatment including surgery could be considered for prevention of spontaneous tendon rupture in a patient with predisposing conditions despite an asymptomatic spur.

Chronic extensor hallucis longus tendon ruptures are very rare, and may lead to hallux dysfunction. To the best of our knowledge, reconstruction of a chronic extensor hallucis longus rupture using interposed scar tissue has not been previously reported. Our results show that direct repair method using interposed scar tissue for chronic extensor hallucis longus rupture can successfully restore function of the hallux and provide good satisfaction in carefully selected patients.

Kienböck disease, a rare disease that can cause chronic pain and motor dysfunction, occurs due to avascular necrosis of the lunate bone, which leads to dislocation of the carpal bone. Among various other etiologies, Kienböck disease can cause closed tendon rupture of the finger. In this report, we introduce a case of total rupture of the second extensor digitorum communis and the extensor indicis proprius tendons caused by undiagnosed Kienböck disease in an elderly female patient.

Closed rupture of the extensor hallucis longus (EHL) tendon is uncommon and rarely reported. We present a Taekwondo player who had sustained a closed rupture of the EHL tendon after striking the other player's shin. He had practiced Taekwondo more than 6 hours a day for more than 6 years, including repetitive striking on the dorsum of the foot, which probably caused degeneration of the EHL tendon. The tendon ends could not be approximated directly, so reconstruction was performed with bisecting the distal tendon and combining the repair of the one end with an intercalary scar tissue and the other with tenodesis to the extensor hallucis brevis. He returned to the preoperative level of activity 6 months postoperatively with a satisfactory range of motion.

Spontaneous rupture of the extensor pollicis longus (EPL) tendon can occur in the 3rd extensor compartment after a distal radius fracture involving Lister's tubercle, steroid injections, or rheumatoid arthritis. We report a case of spontaneous rupture of the EPL tendon in a 26-year-old male patient who played a rhythm game, which requires repetitive wrist motions to play the drums. We also provide a comprehensive literature review along with the case report. From the authors' point of view, excessive and repetitive motion of the wrist, as shown in our case, can be a potential cause of spontaneous rupture of the EPL tendon.

The accessory tendon of the extensor hallucis longus is a common type of extensor hallucis longus variation. This is a case of a 38-year-old female patient who initially considered conservative treatment for a suspected partial rupture, but finally underwent surgery after being diagnosed with a complete rupture of the main tendon and accessory tendon medial to the main tendon on MRI scan.

Acute rupture of the tibialis anterior tendon is rare, but the diagnosis is often delayed when it occurs. Acute rupture of the tibialis anterior tendon is often caused by minor trauma or it occurs spontaneously. Therefore, the diagnosis is more likely to be delayed. Among ruptures of the tibialis anterior tendon, longitudinal ruptures are less common and difficult to diagnose. Thus far, there are no reports of ruptures of the tibialis anterior tendon caused by direct trauma in Korea. This paper reports a case of chronic longitudinal tear of the tibialis anterior tendon caused by trauma in a 50-year-old male patient with no specific history of the disease, along with a review of the relevant literature.
