Spontaneous Rupture of the Extensor Carpi Radialis Brevis and Radial Collateral Ligament of the Elbow in a Recreational Golfer: Surgical Experience of Repair of a Chronic Retracted Tendon and Ligament

  • Lateral epicondylitis with rupture of the radial collateral ligament of the elbow has not been reported in the literature. We report on a case of a recreational golfer who had not received steroid injection and had no trauma history. The patient was treated with open surgical repair. At 2 years follow-up, satisfactory clinical and radiological outcomes were observed with return to pre-injury level. The authors report this case and review the literature.

최대 수의적 등척성 수축을 하는 동안 장요측 수근 신근에서 최대 수의적 등척성 수축력과 근 구조와의 상관관계 (Correlation between MVIC and Muscle Architecture in the Extensor Carpi Radialis Longus Muscle during Maximum Voluntary Isometric Contraction)

  • Purpose: This study examined the correlation between the maximum voluntary isometric contraction (MVIC) and the muscle architecture in the extensor carpi radialis longus muscle during MVIC. Methods: The muscle area, volume were measured using a ultrasound imaging system to obtain the muscle architecture during the MVIC. For the mechanical muscle strength measurements, the MVIC was obtained using a dynamometer. Results: There was a significant correlation between the MVIC and the muscle area (r=0.498, p<0.01) and muscle volume (r=0.602, p<0.001). There was a significant correlation between the MVIC and density (r=-0.429, p<0.05). The area showed significant correlations with the muscle volume (r=0.699, p<0.001) and density (r=-0.429, p<0.05). In addition, there was a correlation between the volume and muscle density (r=0.555, p<0.01). Conclusion: There is close relationship between the MVIC and the muscle architecture in the extensor carpi radialis longus muscle during the MVIC.

  • Lateral epicondylitis is one of the most common causes of elbow pain and has been known to be caused by degeneration of the extensor carpi radialis brevis (ECRB). Nonoperative treatment should be tried first in all patients, because it has been deemed highly successful; however only few prospective studies suggest that symptoms frequently was completely resolved. Operative treatment is indicated for recalcitrant pain after failed conservative treatment, which involves excision of the pathologic portion of the ECRB and results in a high degree of subjective relief and functional restoration. We will review the pathology of the lateral epicondylitis and operative and nonoperative treatment of lateral epicondylitis.

치위생 작업 수행 시 치료 자세에 따른 근육활동과 움직임 (Muscle Activity and Range of Motion According to Operating Posture at Dental Hygiene Work)

  • Purpose : Recommended posture according to the location of operating teeth have been standardized in dental clinic to prevent musculoskeletal disorder. However, clinicians do not comply with this rule in many cases. This study investigated the effects of operating posture on cranio-cervical range of motion (CROM) and muscles activity of neck and upper extremity. Methods : Sixteen healthy dental hygiene students were participated. During operating posture (3 recommended and 3 experimental postures which were set front, side, back, respectively), CROM in the fronal and sagittal plane were measured by Cervical Range of Motion Instrument and muscle activities of Sternocleidomastoid, upper trapezius, middle deltoid, extensor carpi radialis, brachioradialis, and abductor pollicis brevis were measured by Pocket EMG system. Result : CROM were significantly decreased in recommended posture in comparison with experimental posture (p<.05). In addition, muscle activity of middle deltoid was significantly decreased in recommended front posture. Moreover, brachioradialis and extensor carpi radialis showed the same result in recommended back posture (p<.05). Conclusion : Recommended posture is close to neutral posture and to reduce muscle fatigue and overuse, which may considered as a preventing musculoskeletal disorder and partially explain its efficacy in dental clinic.

The Most Effective Number of Elastic Taping Applications on the Muscle Activity and Maximum Peak of the Wrist Extensor Muscle in Patients with Stroke

  • Objective: The aim of this study is to evaluate the differences in electromyographic activities of upper extremity muscle between repeated taping and to compare the effects of each taping method in stroke patients. Background: Taping studies for functional improvement and pain relief of the UE have been conducted using various methods. Despite being an important factor when you treatment to a patient in a clinical attachment numbers situation quantitative research is that there is not at all to the taping at the time of application. Method: Twenty patients volunteered in this study and were tested under four taping conditions as follows, in random order: (1) no taping, (2) taping applied once, (3) taping applied twice, and (4) taping applied thrice. The muscle activity and maximum peak of the extensor carpi radialis muscle in electromyographic activities were measured, respectively. Results: The muscle activity and maximum peak of the extensor carpi radialis muscle showed significant differences among the four conditions (p<.05). In the post hoc test, the extensor muscle showed significant differences in muscle activity and maximum peak in electromyographic activities, except between taping applied twice and thrice. Conclusion: These findings demonstrate that repeated taping up to two times may be useful in improving the muscle activity and maximum peak of the extensor muscle. Application: This study provides useful information to future researchers regarding the effects of repeated taping applications on muscle activity and maximum peak of the muscles of the human body.

탄성저항 및 필라테스 운동이 야구투수의 근전도에 미치는 영향 (The Effects of Elastic Resistance and Pilates Exercise on EMG in Baseball Pitcher)

  • The purpose of this study was to examine the pitching ability of baseball player by analysing the effect of elastic resistance exercise and pilates exercise on EMG. Five out of currently active highschool baseball player performed 5 times a week of elastic resistance exercise and pilates for 8 weeks, each for 100 minutes. They demonstrated significant increase in EMG after 8 weeks of exercise. Especially, in extensor carpi radialis brevis and flexor carpi radialis of fore arm, they demonstrated significant increase of inning figure after exercise. These results suggested that elastic resistance exercise and pilates exercise can improve pitching ability of baseball player according to inning.

노트북 거치대가 건강한 젊은 성인 손목관절근육의 생체역학적 특징에 미치는 영향 (Effects of a Notebook Computer Supporter on Biomechanical Characteristics in Wrist Joint Muscles of Healthy Young Adults)

  • Purpose: As laptop use increases throughout the COVID-19 pandemic and its use outside of traditional workstations proliferates, it is imperative to expand the limited research available regarding ergonomic exposure. This study aimed to identify the effects of a laptop supporter on biomechanical characteristics in the wrist joint muscles of healthy young adults. Methods: This was a cross-sectional observational study design with thirty-four healthy young adults as participants. They conducted a typing exercise performed under two different conditions, which were with and without a notebook computer supporter. This study measured the biomechanical characteristics of the muscles of the wrist joints including the flexor carpi ulnaris (FCU), the flexor carpi radialis (FCR), the extensor carpi radialis longus (ECRL), and the extensor carpi ulnaris (ECU). Measurements were taken three times: before typing, immediately after typing for 30 minutes with a supporter, and immediately after typing for 30 minutes without a supporter. The statistical method to compare the three different measurement conditions was the repeated measures ANOVA. Results: The participants showed significantly different levels of dynamic stiffness in both the FCU before typing and immediately after 30 minutes of typing with a supporter, and showed significantly different levels of dynamic stiffness in the FCR before typing and immediately after 30 minutes of typing with a supporter. The dynamic stiffness level immediately after 30 minutes of typing without a supporter was significantly different than that immediately after 30 minutes of typing with a supporter. However, the muscle tone was not significantly different among the three different conditions. Conclusion: The results of this study revealed that a laptop supporter used to correct the eye level of the electronic screen increases the dynamic stiffness of the wrist joint flexors, so it is necessary to consider the neutral position of the wrist joint during typing.

전완(前腕)에서 등장성수축(等長性收縮)과 등력성(等力性) 수축(收縮)의 근전도적(筋電圖的) 분석(分析) (Electromyographic Analysis of Isometric and Isotonic Contraction in the Forearm.)

  • Electromyographic analysis was made in the forearm to clarify the prime mover of the wrist joint in flexion and extension. Loads of 5 and 10 pounds were given to the hand during isometric and isotonic contraction. The results of this study were summarized as follows: 1) M. flexor carpi ulnaris and m. extensor carpi radialis acted as the prime mover during flexion and extension, respectively, of the wrist joint. 2) The flexor and the extensor of the forearm showed synergistic activities under isotonic contraction, but under isometric contraction the flexor only acted. 3) Muscular activity during the isotonic contraction slightly increased compared with the isometric contraction. 4) EMGs and integrated EMGs were somewhat enhanced as the load increased, but there was no significant difference between 5 and 10 pounds load.

팔꿉관절 부위에 적용한 다이아몬드 테이핑이 정상인의 파악력 수준별 손목 폄근의 근활성도에 미치는 영향 (Effect of Diamond Taping Applied to the Elbow Joint on Wrist Extensor Muscle Activity in Healthy Subjects)

  • Background: Lateral epicondylitis (LE) is the most common chronic musculoskeletal pain condition of the upper extremities. LE is often related to forceful grip activities that require isometric contraction of the wrist extensors. A previous study evaluated the effect of the diamond taping technique on grip strength and pain; however, there has been no report on the change in the electromyography (EMG) findings of wrist extensors. Objects: The aim of this study was to investigate the effect of diamond taping technique, using a rigid tape, on the EMG activities of the extensor carpi radialis (ECR) during grip activities. Methods: Twenty-four healthy subjects (mean age = 21.50 ± 2.76 years) volunteered to participate in this study. The subjects were instructed to perform forceful grip activities with and without diamond-type taping on the origin area of the ECR. Grip strength tests were performed at 100%, 75%, 50%, and 25% for maximal isometric contraction force. EMG data were collected from the ECR. Repeated measure analysis of variance was used to analyze the effect of grip force and taping (with and without). Statistical significance levels were set at α = 0.05. Comparison of the results with and without taping at different grip force were analyzed using independent t-test. Statistical significance levels were set at α = 0.01. Results: Statistically significant association was observed between the taping application and forceful grip activity as revealed by the EMG data of the ECR (p < 0.05). EMG of the ECR significantly reduced for all muscle strength levels (p < 0.01) after taping. Conclusion: This study shows an impressive effect of the diamond taping technique, using rigid tape, on wrist extensors during grip activities. Decreasing muscle activity via this taping approach could be utilized to enhance pain-free grip force and reduce pain in patients with LE. Our study suggested that this taping technique could be considered as an effective management strategy of LE.
