SPECTRAL CHARACTERISTICS OF RESONANCE DISORDERS IN SUBMUCOSAL TYPE CLEFT PALATE PATIENTS (점막하 구개열 환자 공명장애의 스펙트럼 특성 연구)

  • Submucosal type cleft palate is subdivision of the cleft palate. It is very difficult to find submucosal cleft, because when we exam submucosal type cleft palate patients, it seems to be normal. But in fact, there are abnormal union of palatal muscles of submucosal type cleft palate patients. Because of late detection, the treatment - for example, the operation or the speech therapy - for the submucosal type cleft palate patient usually becomes late. Some patients visited our hospital due to speech disorder nevertheless normal intraoral appearance. After precise intraoral examination, we found out submucosal cleft palate. We evaluated the speech before and after surgery of these patients. In this study, we want to find the objective characteristics of submucosal type cleft palate patients, comparing with the normal and the complete cleft palate patients. Experimental groups were 10 submucosal type cleft palate patients and 10 complete cleft palate patients who got the operation in our hospital. And, the controls were 10 normal person. The sentence patterns using in this study were simple 5 vowels. Using CSL program we evaluated the Formant, Bandwidth. We analized the spectral characteristics of speech signals of 3 groups, before and after the operation. In most cases, the formant scores were higher in experimental groups (complete cleft palate group and submucosal type cleft palate group) than controls. There were small differences when speeching /a/, /i/, /e/ between experimental groups and control groups, large differences when speeching /o/, /u/. After surgery the formant scores were decreased in experimental groups (complete cleft palate group and submucosal type cleft palate group). In bandwidth scores, there were no significant differences between experimental groups and controls.

The Effects of Semi-Fowler's Position on Post-Operative Recovery in Recovery Room for Patients with Laparoscopic Abdominal Surgery (복강경 수술 후의 반좌위가 수술직후 회복정도에 미치는 영향)

  • Purpose: To find the effects of semi-Fowler's position on the post-operative recovery for patients with laparoscopic abdominal surgery in recovery room. Method: The research was performed by nonequivalent control group non-synchronized quasi-experimental design. The subjects are forty patients who had laparoscopic abdominal surgery in a hospital from Aug. thru Nov. of 2003. Post-recovery scores and $O_2$saturation degree were measured. The experimental group was place in semi fowler's position while the control group was placed in supine position. The homogeneity between the control group and experimental group was analyzed using the Chi-square, and the hypothesis were tested using t-test. Result: 1. The patients in the experimental group placed in semi fowler's position showed significant higher post-recovery scores than those in the control group who were in a supine position. 2. The patients in the experimental group who were in semi Fowler's position showed no significant higher $O_2$ saturation degree than those in the control group who were in supine position. Conclusion: Based on the results described above, it is considered that the semi-Fowler's position might be effective in enhancing the post-operative recovery score of the patients with laparoscopic abdominal surgery in recovery room.

Effects of Surgery Information Service on One-Day Surgery Patients' Anxiety and Satisfaction with Nursing Care (간호정보 제공이 당일수술 환자의 불안과 간호만족도에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of surgery information service on one-day surgery patients' anxiety and satisfaction with nursing care. Methods: The study used the nonequivalent control group time difference design. Sampling and measurement of the control group participants (n=30) was completed first. Later, participants in the experimental group (n=30) were sampled, intervened, and measured. The experimental group participants received the surgery information services twice: before and after the surgery, 20~30 minutes for each of the sessions. The anxiety was measured with the State-Trait Anxiety Inventory developed by Spielberger (1975), and modified by Kim & Shin (1978). The patient satisfaction with nursing care was measured with the Patient Satisfaction with Nursing Care Scale developed by La Monica and colleagues (1986), and modified by Shin (1999). The data was collected between February 1 and May 30, 2006. Results: The results were as follows: 1) The level of anxiety in the experimental group was not significantly different from that in the control group. 2) The level of patient satisfaction with nursing care in the experimental group was significantly higher than in the control group (t=-4.53, p=.00). Conclusion: These findings suggested that the one-day vocal cord surgery information service could be a useful nursing intervention to improve patient satisfaction with nursing care, but not for controlling the anxiety of one-day surgery patients.

An Experimental and Clinical Study of Reimplantation of Frozen Autogenous Mandibular Bone (냉동처리한 자가골 재이식에 관한 연구)

  • This is an experimental and clinical study on the use of an autogenous frozen mandibular bone. Two aspects must be considered in the treatment of mandibular tumors: the eradication of the disease, and the maintenamce of the mandibular continuity for a good functional and esthetic results. The authors used the cryosurgical method in the dogs and the tumor of the mandible in human, which consists of partial mandibular resection; immersion of the bone in the liquid nitrogen at -196℃, followed by reimplantation of the mandibular bone in its bed. In experimental study, the mandibular segment showed favorable healing process without rejection during the cohole experimental periods. In clinical study, the frozen bone showed no antigenicity and excellent cosmetic results.

Effect of Aroma Therapy on the Anxiety, Blood Pressure and Pulse of Uterine Leiomyoma Patients before Surgery (아로마 향기요법이 자궁근종 여성의 수술전 불안, 혈압 및 맥박에 미치는 효과)

  • Purpose: The study was performed to evaluate effects of aroma therapy on the anxiety, blood pressure and pulse of uterine leiomyoma patients before surgery and to apply it as the actual nursing intervention. Method: From Aug1st to Nov. 30th, 2007, the experimental group (n=30) and the control group (n=30) were selected in 1 hospital, Busan. Lavender and Bergamot oil mixed together at a 3:2 ratio was applied to the experimental group two times at 8-9 pm the day before surgery and twice 30 minutes before the surgery. Anxiety, VAS, blood pressure, and pulse were measured to evaluate the effects of aroma therapy. The data analysis was done using the SPSS/PC 12.0 program using chi-square test and t-test. Results: Hypothesis 1 was supported. The VAS of the experimental group was lower than control group after aroma therapy. Hypothesis 2 was partly supported. The systolic blood pressure and pulse of experimental group was significantly decreased, but diastolic blood pressure wasn't difference. The VAS, systolic blood pressure and pulse of control group were increased. Conclusion: The study verified that the aroma therapy is a very effective nursing intervention that can reduce anxiety before surgery. It is recommended to apply aroma therapy actively to patients experiencing anxiety before surgery procedure.

  • To assure that cyclophosphamide has been considered something that must be effective to retard wound healing, the Author has studied clinically and histopathologically. All rabbits were incised on the mucogingival junction of anterior teeth in mandible about 1 Cm. in length with mucosa and periosteum layer. The 21 rabbits within 6 months old, weighing about 1.5-1.8 kg were divided into a experimental group and a control group. 1) Group 1 (Experimental group)-After 14 rabbits dieted as the control group were injected with Endoxan (=Cyclophosphamide) in dosage of 30mg per 1kg-body weight intramuscularly, the surgery was performed. 2) Group 2 (Control group)-the surgery in this group was performed without Endoxan. The rabbits were sacrificed on the 1st, 3rd, 5th, 7th, 10the, 14th day after surgery and microscopic slides were made with H-E stain. The results might be summarized as follows; 1) Cyclophosphamide effected to retard healing process on the incised wound of oral mucosa and connective tissue. 2) Control wounds were healed by the 7th day after surgery. 3) Comparison between control and experimental wound did not show significant differences 14th day after surgery.

Effect of early oral nutrition supplement using Encover in patients undergoing hepato-biliary-pancreatic surgery

  • Backgrounds/Aims: Early recovery after surgery has become a popular trend. The aim of this study was to evaluate effect of nutritional intervention using Encover, an oral nutritional supplement, in patients undergoing hepato-biliary-pancreatic surgery. Methods: This single center, prospective case-control study was conducted in Gangnam Severance Hospital from September 2018 to April 2019. Through randomization, patients were divided into an experimental group (30 patients) and a control group (30 patients). At postoperative seven days, the experimental group was instructed to take two packs of Encover (JW Pharmaceutical, Seoul, Korea) daily for seven days. Body cell mass index was measured at seven days after surgery and 14 days after discharge and Patient-Generated Subjective Global Assessment (PG-SGA) was performed at 14 days after discharge. Results: Body cell mass index during outpatient follow-up was significantly decreased compared to that at discharge in both groups. However, the amount of body cell mass index showed no significant difference between postoperative seven days and outpatient follow-up in either group. During outpatient follow-up, the experimental group had a higher mean value of PG-SGA score than the control group (11.32 ± 3.46 vs. 9.48 ± 3.97; p = 0.037). Conclusions: Short-term Encover doses after surgery may not produce significant results in weight gain or other body cell mass index. Encover did not significantly affect other dietary conditions based on PG-SGA.

Treatment of Diabetic Foot Ulcer Using Matriderm In Comparison with a Skin Graft

  • Background For patients with neuropathy, vasculopathy, and impairment of wound healing, treatment of a diabetic foot ulcer poses many challenges. A large number of dermal analogues have been invented in an effort to overcome these challenges. Matriderm, a dermal analogue, is made from bovine collagen and elastin. This study was conducted in order to evaluate the effectiveness of Matriderm for treatment of diabetic foot ulcers, in comparison with skin grafting. Methods Sixty patients with diabetic foot ulcer were included in this prospective study. The average age of the patients, who had type II diabetes mellitus, was 58 years old. The patients were allocated to an experimental or control group with their consents. The patients were selected with their consent for inclusion in an experimental group and a control group. Patients in the experimental group received a Matriderm appliance and a split-thickness skin graft, while those in the control group received only a split-thickness skin graft. Results A shorter hospitalization period (7.52 weeks) was observed in the experimental group than in the control group (9.22 weeks), and a shorter period of time (8.61 weeks) was required for complete healing, compared with the control group (12.94 weeks), with statistical significance (P<0.05). A higher elasticity ratio of the affected side to the non-affected side was observed in the experimental group, compared with the control group (P<0.01). Conclusions Matriderm enables effective healing and improves elasticity in treatment of patients with diabetic foot ulcer.

The Effect of Hand Massage on the Anxiety of the Hysterectomy Patients in Immediately prior to Surgery (손마사지가 자궁절제술 환자의 수술직전 불안에 미치는 효과)

  • The purpose of this study was to examine the effects of hand massage as a nursing intervention on the anxiety of the hysterectomy patients in immediately prior to surgery. The method of this study was Nonequivalent Control Group Non-Synchronized Design. The data were selected from at K university hospital in Pusan, and they consisted of Experimental group-25 patients, Control group -24 patients. The data were collected from Jan. 4 to Jan. 30 in 1999. The subjects′ self-reports of anxiety (measured by the Spielberger Trait-State anxiety Inventory and Visual Analogue Scale developed by Cline et al.) were recorded before and immediately after the intervention. The objective physiologic measures of blood pressure and pulse rate. The collected data were analysed by means of frequency, percentage, standard deviation, chi- square test, t-test, ANCOVA with SPSS program. The results of this study were as following; 1. Hypothesis 1: The 1st hypothesis that "There will be significant difference of the state anxiety level just before surgery in the experimental group and control group" was supported(P= .000). 2. Hypothesis 2: The 2nd hypothesis that "There will be significant difference of the visual analogue scale score just before surgery in the experimental group and control group"was supported(P= .000). 3. Hypothesis 3: The 3rd hypothesis that "There will be significant difference of the systolic and diastolic blood pressure level just before surgery in the experimental group and control group"was supported (P= .003, P= .041). 4. Hypothesis 4: The 4th hypothesis that "There will be significant difference of the pulse rate just before surgery in the experimental group and control group"was supported(P= .004). In conclusion, hand massage is a benefical nursing intervention that alleviates the psychological, physiological anxiety of the hysterectomy patients in immediately prior to surgery. therefore it is recomended to use the hand massage as a nursing intervention for patients undergoing anxiety. The results of this study appear promising, additional research is recomended to further the appropriate uses of hand massage in nursing practice for this and other patient population.

An Experimental Animal Model of Anomalous Pancreaticobiliary Duct Union (췌담관 합류이상의 실험동물 모델)

  • The anomalous pancreaticobiliary duct union (APBDU) might cause the formation of choledochal cyst and malignancies of hepatopancreaticobiliary system. The purpose of this study is to make an experimental animal model of APBDU similar to that of human. One to two-month-old Mongrel dogs (n=12) were divided into two groups; the control group (n=2) had a sham operation performed, and in the experimental group (n=10) the end of distal ' common bile duct (CBD) was anastomosed to the side of the dorsal pancreatic duct making APBDD. Serum was obtained for chemical analysis on the 10th postoperative day. The dogs were sacrificed at the 5th week (n=3), the 6th week (n=3), the 7th week (n=2), the 8th week (n=2) and the 6th month (n=2) after the experimental surgery. With sacrifice, operative cholangiogram was taken, and bile juice was obtained for chemistry and bacterial culture. The en-bloc specimens of the hepatopancreaticobiliary system were removed for microscopic examination. Serum and bile juice amylase levels were elevated in the experimental group(n=10), but not in the control group(n=2). Operative cholangiograms of control group revealed no evidence of bile duct dilatation.. On the other hand, the bile duct in the experimental group was markedly dilated without any evidence of stenosis at the anastomosis site (n=10). Histologic examination of the hepatopancreaticobiliary system in the experimental group resembled the findings of choledochal cyst in human. The APBDU of this animal model can produce bile duct dilatation by pancreaticobiliary reflux. We think that this animal model can be potentially promising for the research about the APBDU associated hepatopancreaticobiliary diseases.
