Impact of Energy and Access Methods on Extrahepatic Tumor Spreading and the Ablation Zone: An Ex vivo Experiment Using a Subcapsular Tumor Model

  • Objective: To evaluate the impact of energy and access methods on extrahepatic tumor spreading and the ablation zone in an ex vivo subcapsular tumor mimic model with a risk of extrahepatic tumor spreading. Materials and Methods: Forty-two tumor-mimics were created in bovine liver blocks by injecting a mixture of iodine contrast material just below the liver capsule. Radiofrequency (RF) ablations were performed using an electrode placed parallel or perpendicular to hepatic surface through the tumor mimic with low- and high-power protocols (groups 1 and 2, respectively). Computed tomography (CT) scans were performed before and after ablation. The presence of contrast leak on the hepatic surface on CT, size of ablation zone, and timing of the first roll-off and popping sound were compared between the groups. Results: With parallel access, one contrast leak in group 1 (1/10, 10%) and nine in group 2 (9/10, 90%) (p < 0.001) were identified on post-ablation CT. With perpendicular access, six contrast leaks were identified in each group (6/11, 54.5%). The first roll-off and popping sound were significantly delayed in group 1 irrespective of the access method (p = 0.002). No statistical difference in the size of the ablation zone of the liver specimen was observed between the two groups (p = 0.247). Conclusion: Low-power RF ablation with parallel access is proposed to be effective and safe from extrahepatic tumor spreading in RF ablation of a solid hepatic tumor in the subcapsular location. Perpendicular placement of an electrode to the capsule is associated with a risk of extrahepatic tumor spreading regardless of the power applied.

Altered Cell to Cell Communication, Autophagy and Mitochondrial Dysfunction in a Model of Hepatocellular Carcinoma: Potential Protective Effects of Curcumin and Stem Cell Therapy

  • Background: Hepato-carcinogenesis is multifaceted in its molecular aspects. Among the interplaying agents are altered gap junctions, the proteasome/autophagy system, and mitochondria. The present experimental study was designed to outline the roles of these players and to investigate the tumor suppressive effects of curcumin with or without mesenchymal stem cells (MSCs) in hepatocellular carcinoma (HCC). Materials and Methods: Adult female albino rats were divided into normal controls and animals with HCC induced by diethyl-nitrosamine (DENA) and $CCl_4$. Additional groups treated after HCC induction were: Cur/HCC which received curcumin; MSCs/HCC which received MSCs; and Cur+MSCs/HCC which received both curcumin and MSCs. For all groups there were histopathological examination and assessment of gene expression of connexin43 (Cx43), ubiquitin ligase-E3 (UCP-3), the autophagy marker LC3 and coenzyme-Q10 (Mito.Q10) mRNA by real time, reverse transcription-polymerase chain reaction, along with measurement of LC3II/LC3I ratio for estimation of autophagosome formation in the rat liver tissue. In addition, the serum levels of ALT, AST and alpha fetoprotein (AFP), together with the proinflammatory cytokines $TNF{\alpha}$ and IL-6, were determined in all groups. Results: Histopathological examination of liver tissue from animals which received DENA-$CCl_4$ only revealed the presence of anaplastic carcinoma cells and macro-regenerative nodules. Administration of curcumin, MSCs; each alone or combined into rats after induction of HCC improved the histopathological picture. This was accompanied by significant reduction in ${\alpha}$-fetoprotein together with proinflammatory cytokines and significant decrease of various liver enzymes, in addition to upregulation of Cx43, UCP-3, LC3 and Mito.Q10 mRNA. Conclusions: Improvement of Cx43 expression, nonapoptotic cell death and mitochondrial function can repress tumor growth in HCC. Administration of curcumin and/or MSCs have tumor suppressive effects as they can target these mechanisms. However, further research is still needed to verify their effectiveness.

진인화철탕가와송(眞人化鐵湯加瓦松)이 N-Nitrosodiethylamine으로 유발된 흰쥐의 간암에 미치는 영향 (The Effect of Jininwhachul-tang-ga-wasong on the Hepatic Cancer in Mouse by N-Nitrosodiethylamine)

  • Objective : The purpose of our study was to investigate the effect of JinlnWhaChul-tang-ga-wasong (JIN) on NDEA-induced liver tumorigenesis. Materials and Methods : We investigated the possible protective effects of Jininwhachul-tang-ga-wasong (JIN) as an anticancer against NDEA-induced liver injury in mice. Experimental mice were classified into 3 groups; normal, saline administered group (control group), and JIN extract (0.15g/kg/every other day) administered group (JIN group) after being injected with NDEA over 12 weeks. We examined the state of differentiation of these tumors and the effects of JIN after 6 weeks. To confirm the induction of apoptosis, the cells were analyzed by terminal deorynucleotidyl transferase-mediated dUTP nick-end labeling (TUNEL) assay, acridine orange staining and flow cytometric analysis. To investigate inhibitory effect on the expression of COX-2 by JIN, we performed COX-2 immunohistochemistry and reverse transcription polymerase chain reaction analysis. Results : Body weights significantly decreased in the control and JIN groups compared with the normal group. The levels of cholesterol, hemoglobin and testosterone decreased in the control compared with the normal group. The level of estradiol significantly increased in the control compared with the normal group. The control group reacted with TUNEL assay more than the normal and JIN groups. Upon naked eye, light and electron microscopic examination, JIN improved the morphological and histopathological changes of the liver caused by NDEA-induced hepatic neoplasm. COX-2 immunoreactivity decreased in the JIN group compared with the control group, mRNA expression of the control group was greater than the normal and JIN groups. Conclusion : these results suggest the possibility that JIN may exert an anti-tumor effect on NDEA-induced liver tumorigenesis.

Development of multifocal nodular lesions of a liver mimicking hepatic metastasis, following resection of an insulinoma in a child

  • Insulinoma, which arises from insulin-producing pancreatic beta cells, is a rare tumor in children. Only 5%-10% of insulinomas are malignant and undergo metastasis. We report a case of an 11-year-old girl who experienced hypoglycemia-related seizures induced by an insulinoma; after resection of the primary tumor, she developed hepatic focal nodular hyperplasia (FNH). Laboratory test results indicated marked hypoglycemia with hyperinsulinemia. Abdominal ultrasonography (US) and computed tomography results were normal; however, magnetic resonance imaging (MRI) showed a solid mass in the pancreatic tail. Therefore, laparoscopic distal pancreatectomy was performed. Two months after the surgery, an abdominal MRI revealed multiple nodular lesions in the liver. An US-guided liver biopsy was then performed, and histological examination revealed FNH without necrosis or mitotic activity. The patient has been free of hypoglycemia for 2 years, and recent MRI studies showed a decrease in the size of FNH lesions, without any evidence of metastasis. Even though no metastatic lesions are noted on imaging, close observation and follow-up imaging studies are required in a child with insulinoma that has malignant potential on histopathologic findings.

인진삼물탕(茵蔯三物湯)이 DENA로 유발된 흰쥐의 간암에 미치는 영향 (The Effect of Injinsammul-tang(IJS) on the Hepatocellular Carcinogenesis Induced by Diethylnitrosamine(DENA) in Rats)

  • Objectives : This study was designed to investigate the effect of Injinsammul-tang (IJS) on hepatocellular carcinogenesis in rats. Methods : Sprague Dawley (SD) rats of control and treatment groups received intraperitoneal injection of 50 mg/kg/day diethylnitrosamine (DENA) weekly for 8 weeks. Experimental rats were classified into 3 groups; normal group (Nor), hepatic cancer induced control group (Con), and IJS extract 250 mg/kg administered group (IST) after being injected with DENA. Thereafter the changes of body weight, liver weight and weight of liver/100g body weight, the activities of aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), lactate dehydrogenase (LDH), and superoxide dismutase (SOD) were measured. Gross anatomy and optical microscopy were also observed. Results : The body weight decreased in Con and IST compared with the Nor. The weight of liver and the weight of liver/100g body weight increased significantly in Con and IST compared with the Nor. The activities of AST, ALT, ALP, LDH increased in the Con compared with Nor, but decreased in IST compared with Con. The activities of SOD increased in the Con and IST compared with Nor. Upon naked eye and light microscopic examination, IJS improved the morphological and histopathological changes of liver caused by DENA-induced hepatic neoplasm. The number of hepatic p53 positive cells decreased in the IST compared with Con. Conclusions : Most of the results did not show a significant effect, but some of the results showed a significant effect. It can be estimated that IJS has some effects on hepatocellular carcinogenesis induced by DENA in rats, and further studies will be needed.

폐암 세포주를 사용한 신, 비장 및 간 피막하 분식법의 비교 (Human Lung Cancer Cell Xenografts Implanted under the Capsule of Kidney, Spleen and Liver)

  • 배경: 폐암은 근치적 절제술이 가장 좋은 치료법이나 수술 후에 재발한 경우나 수술 시기가 지난 경우에는 항암 화학요법의 중요성이 강조된다. 항암 치료 전에 감수성 있는 항암 화학 치료제가 선별되어진다면 치료 효과를 극대화할 수 있다. 신피막하 분석법은 흉, 복부 종양의 생체 내 검사법으로 중요성이 인정되고 있고 항암 감수성 검사로서도 짧은 기간 내에 판별이 가능한 이점이 있다. 신장은 각종 암세포의 이식 장소로 잘 알려져 있으나 비장이나 간에서의 이식 성적을 비교한 연구는 없는 실정이다. 인체 암세포 이식의 실험방법 중에 비장과 간장에 암 세포주를 이식하였을 경우와 신장에 시행되는 신피막하 분석법에 의한 성장의 차이점 유무를 평가하였다. 대상 및 방법: 인체 폐암 세포주(SW-900 G IV)를 RPMI 1640 (Leibovitz L-15 medium)배지에서 배양하여 fibrin clot으로 만들어 $10^{8}$ 개의 암세포가 포함되도록 한 후 3${\times}$${\times}$3mm의 크기로 Spague Dawely (S.D.) 암컷 쥐의 신, 비장 및 간 피막하에 이식하였다. 이식 후 1일부터 6일간 면역억제를 위하여 cyclosporin-A (80 mg/kg)를 피하투여하였다. 이식 전후 실험동물의 체중 변화, 종양의 성장 여부 및 종양의 크기를 계측하고 병리 조직 검사와 혈청 내 암 표지자 검사를 실시하여 비교하였다. 걸과: 실험 5.D. 쥐의 체중 변화는 대조군이나 실험 군 모두에서 체중 증가가 있었다. 혈청 내 암 표지자의 정량 검사 결과 Cyfra 21-1은 검출되지 않았고, CEA및 NSE는 의미 있는 변화가 없었으며, SCC-Ag이 실험 군에서 유의한 증가가 있었다. 비장에 이식한 폐암 세포주의 성장이 신장 및 간장에 비해 더 증가된 결과를 보였다. 표면적, 두께와 용적 모두가 비장에서 유의한 증가를 나타내었다. 이식 성공률은 신장이 80%, 비장이 76.7%, 간장이 43.3%이었다. 병리학적 검사 결과 신장에는 비교적 둥글게 성장하며 크기는 제일 작았으며 성장 방향이 일정한 모양을 보이는 특징이 있으며 비장은 성장이 잘되어 제일 큰 종양을 형성하나 불규칙한 성장과 위성 종양(satellite tumor)이 빈번히 발견되었으며 현미경적으로는 혈관 신생과 함께 종양 혈전이 발견되었다. 간장은 간 내부로 침투 성장하여 이식 성공률이 가장 저조하며 현미경적 소견은 응고성 괴사와 점액양 섬유성 병변을 가지는 특징을 보였다. 걸론: 이식 성공률은 신장과 비장이 높으나, 성장이 일정하고 계측이 용이한 것은 신장이었다. 혈청 암 표지자는 SCC-Ag이 가장 조기에 반응하였으며, Cyfra 21-1은 조기에 검출되지 않았다. 이상에서 감수성 검사를 위한 종양이식 실험에 이용하기에 가장 적합한 장기는 신장을 이용한 신피막하 이식법으로 생각되며, 조기에 유용한 암표지자 검사는 SCC-Ag 정량법이라고 판단된다.

Excellent treatment outcomes in children younger than 18 months with stage 4 MYCN nonamplified neuroblastoma

  • Purpose: Although the prognosis is generally good in patients with intermediate-risk neuroblastoma, no consensus has been reached on the ideal treatment regimen. This study analyzed treatment outcomes and toxicities in patients younger than 18 months with stage 4 MYCN nonamplified neuroblastoma. Methods: We retrospectively analyzed 20 patients younger than 18 months newly diagnosed with stage 4 MYCN nonamplified neuroblastoma between January 2009 and December 2015. Patients received 9 cycles of chemotherapy and surgery, with or without local radiotherapy, followed by 12 cycles of differentiation therapy with 13-cis-retinoic acid. Chemotherapy consisted of alternating cycles of cisplatin, etoposide, doxorubicin, and cyclophosphamide (CEDC) and ifosfamide, carboplatin, and etoposide (ICE) regimens. Results: The most common primary tumor site was the abdomen (85%), and the most common metastatic sites were the lymph nodes (65%), followed by the bones (60%), liver (55%), skin (45%), and bone marrow (25%). At the end of induction therapy, 14 patients (70%) achieved complete response, with 1 achieving very good partial response, 4 achieving partial response, and 1 showing mixed response. Nine patients (45%) received local radiotherapy. At a median follow-up of 47 months (range, 17-91 months), none of these patients experienced relapse, progression, or secondary malignancy, or died. Three years after chemotherapy completion, none of the patients had experienced grade ${\geq}3$ late adverse effects. Conclusion: Patients younger than 18 months with stage 4 MYCN nonamplified neuroblastoma showed excellent outcomes, without significant late adverse effects, when treated with alternating cycles of CEDC and ICE, followed by surgery and differentiation therapy.
