View on Treatment of Exogenous Disease in Yeoksimanpil in the Late Joseon Dynasty - from the Viewpoint of the Theory on Epidemic Febrile Disease - (『역시만필』에 기록된 조선 후기 외감병 치료에 대한 소고 - 온병학적 관점에서 본 -)

  • Objectives : This paper summarizes results from review of 36 cases and medical records extracted from Yeoksimanpil, involving treatment of exogenous disease in the last Joseon dynasty, and considers significance of treatment protocols. Methods : About 130 medical records from Yeoksimanpil related to exogenous disease were reviewed, and 36 cases were extracted for closer investigation. The author examined the characteristics of exogenous diseases treated in Yeoksimanpil during the Joseon dynasty, employing Yi Suki's theory about exogenous febrile disease, and analyzed the theory from the viewpoint of epidemic febrile disease (溫病學). Results : The majority of exogenous diseases recorded in Yeoksimanpil include Sanghan (傷寒); Seo (暑); Ryeohwan (厲患); and Hongjin (紅疹). Sanghan was found to be prevalent among the exogenous diseases, but diseases caused by warmth and epidemic are recognized separately from Sanghan. Yi Suki's theory about exogenous febrile disease is similar to the basic theory of modern epidemic febrile disease, although his theory was developed independently during the last Joseon dynasty. Conclusions : Although some exogenous diseases in Yeoksimanpil were recognized separately, Yi Suki did not specify each feature nor did he focus on each symptom. Instead, the author's research suggests that Yi Suki was able to identify and treat a variety of various exogenous diseases due to his accumulation of medical knowledge and skill.

  • It is well known that Treatise on Exogenous Febrile Disease is one of the oldest and most authoritative books in Oriental Medicine, suggesting the concept of Exogenous Febrile and Six channels as a theoretical basis of clinical experience and prescription. But, since Thang Thongjing had written the book, the numberous medical practitioners and theorists asserted their various and different views on the concept of Exogenous Febrile and its Six channels. 3UM-3YANG of Treatise on Exogenous Febrile Disease is basically the thing of specialization UM-YANG, eventually UM-YANG are two functional characteristics in human body. It is specialized to 3UM-3YANG by spatial and time criteria Therefore, it is important to apprehend the concept correctly that was written on Treatise on Exogenous Febrile Disease. I'd like to look into a bodily state by answering the question that is easy to access and based on Six Channels.

  • Until the middle of the 20th century, exogenous febrile disease was the most common disease that threatened the human health. For a long time, oriental medicine doctors developed many ways to cure this disease by studying pathogenic factors. The phthogenic factors and treatments of exogenous febrile disease with time are as followings. "Naegyeong(內經)" : Cold pathogen. Diaphoretic therapy, purgation therapy. Hwata : Cold pathogen. Oguktong(吳鞠通) : Six pathogenic factors. Eight principal therapeutic methods including diaphoretic therapy with pungent and cool properties.

  • "Sanghanjeonsaengjip(傷寒全生集)" is a classic medical work, written by the outstanding physician Dohwa(陶華) in the Ming Dynasty. The characteristic of "SangHanJeonSaengJip" is that this book succeeded to the spirit of pattern identification and treatment of Treatise on Cold Damage Diseases, newly changed a table of contents by symptoms, and together with this indicated the prescriptions in accordance with diswase-pattern at "YujeunghwalInseo(類證活人書)", "Hwajegookbang(和劑局方)" etc. Also because this kept the existing ephedra decoction, cinnamom twig decoction, minor decoction of bupleurum, decoction for reinforcing middle-energizer and replenishing qi etc.'s name on and unlikely indicated the medicine composition, it caused confusion, but at the later ages "Euhakipmun(醫學入門)" the so-called 'Doci(陶氏)' was added to the prescription name, so we are able to distinguish. Together with this, this book dose not indicate the dosage of medicine and indicates the first, the second, and the third classes[上中下] below medicine. As this dose not mean the three grades of quality"good, fair, and poor[上中下] of "Shennong's Classic of Materia Medica" but expresses the sovereign medicinal as the first class[上], minister medicinal as the second class[中] and assistant and courier medicinal as the third class[下], doctors can voluntarily decide the dosage of medicine in accordance with the degree of disease. At this thesis, I single out ten chapters in contents of 2nd volume named Hyeong(亨) corresponding to the details, among "Sanghanjeonsaengjip(傷寒全生集)". I discussed superficial fever types of exogenous febrile disease in chapter 1, aversion to cold types of exogenous febrile disease in chapter 2, syndrome caused un-sufficient sweating in chapter 3, organic fever types of exogenous febrile disease in chapter 4, aversion to wind types of exogenous febrile disease in chapter 5, Tidal fever types of exogenous febrile disease in chapter 6, Alternative attacts of chills and fever in chapter 7, Dysphoria with smothery sensation in chapter 8, Fidgetiness of exogenous febrile disease in chapter 9, and Headache of exogenous febrile disease in chapter 10, and together with this I discussed, in detail, which influence the prescriptions which are listed on each chapter have caused on future generations In accordance with this, I think that the above-mentioned symptoms and prescriptions are important when I research cold damage and warm disease study. So I orderly research revision, annotation, rendering and an investigation.

  • I studied some important medical liter atures in order to examine the cause of Aphasia and found out some facts as follows : 1. Hwang Je Nae Kgong(黃帝內經) is more referred exogenous causes of disease of Aphasia than endogenous causes of disease. 2. Among many causes of Aphasia, exogenous causes of disease was cinsidered to be most common in Soh Shi Je Byong Won Hu chong Rhon(巢氏諸病源喉總論), which has had a wide influence down the ages. 3. Chon Keum Yo Bang(千金要方), Chi Tae Bhi Yo(外臺秘要), Tae Pyong Song Hye Bang(太平聖惠方), Song Je Chong Rok(聖濟總綠). Bu In Yang Bang(婦人良方) and so forth had been quoted from Hwang Je Nae Kyong and Soh Shi Je Byong Won Hu Chong Ron(巢氏諸病源喉總論). 4. In the ming dynasty had been quoted non-exo-endogenous causes of disease and the Kinds of Aphasia and prescription. 5. In the Choeun dynasty and the Ching dynasty, exogenous causes of disease was confined to the quotation of the books referred to above as well, but endogenous causes of disease was developed. 6. Today is refered kidney and Aphasia. The cause of Aphasia obtained was as follows. The exogenous cause of disease is wind, wind and cold, wind and heat. The endogenous cause of disease is pregnancy, heart block of postpartum, weakness of heart and kidney, impairment of seven modes of emotions, stagnation of phlegm and phlegm-fire, hemorrhage. The non exo-endogenous causes of disease is sing ballads and call out.

  • Objectives : We search the contents about Bi disease in Huang Di Nei Jing(黃帝內經), to analyze the significance of Bi disease, etiopathogenisis and pathogenesis of Bi disease, treatment of Bi disease. Methods : We find that the key feature of Bi disease is joint pain induced by impatency of Qi and blood. Exterior and interior etiological factors are involved in, such as exogenous evil of cold and dampness, emotional disorders, intemperance of taking food, dysfunction of yingqi and weiqi, strong or weak constitution, etc. Results : The important pathogenesis are invaded by exogenous evil because of deficiency, disharmony of yingqi and weiqi and disharmony of five viscera. The key points of treatments are the individual concerned therapy and climate concerned therapy, selecting the acupoint according to the differentiation of symptoms and signs. Conclusions : The combined therapy should be used such as acupuncture and moxibustion, hot application of medicine, massage, Daoyin, outside apply, etc. These supply the theory foundation for etiological factor, pathogenesis, syndrome and treatment, and to direct the diagnosis and treatment of Bi disease later generations.

  • In recent years, there were frequent exogenous contagious diseases in Eastasia like SARS(severe acute respiratory syndrome), Avian influenza, Swine influenza, MERS etc. But there are various interpretations about their pathological differentiations and lead to controversy to diagnosis and medicinal use. So there needs universal and consistent understanding methods. Several conclusions are obtained from the research on differentiation theories of various epidemic diseases. Essential elements of differential diagnostic system are pathogen, characters and matters of disease and loci, especially three yin and three yang has close affinity with constitutional features or body shape. Binding these 3 categories, an integrated differentiation 3 dimensional coordinates are made. Out of these, each elements of 3 pathogen-axial lines are related with names of exogenous disease, and those of 3 feature-axial lines are related with 8 principal patterns. And those of 3 locus-axial lines implicating therapeutic method are related with steps and location of exterior and interior, 3 yin 3 yang, Defense, Qi, Nutrient and Blood, five viscera and six bowels and tissues. Additionally, 3 lines of each axis consist of factors which have their own affinity each other, so classification of pathogen, feature, locus of disease has layered interconnectedness. This classification system is included in constitutional features of individual patient. Afterwards, these cognitive structure can be used as a general theory guiding method of therapy, prevention and aftercure healthcare.

  • The main current in the pathology of oriental medicine is composed of etiologic factor, pathogenesis and clinical manifestation. The access of a disease is consist of etiologic factor, location of the lesion, nature of the disease and patient's condition. The nature of disease and the property of a drug are inseparably related to each other. The nature of disease is composed of six exogenous factors, cold and heat, deficiendy and excess, Yinyang and pain. Cold nature is divided into cold symptom due to excess and asthenia cold, fever nature is divided into sthenic fever and asthenic fever. According to the location, cold and heat can be subdivided into heat in the upper and cold in the lower, cold in the upper and heat in the lower, exterior cold and interior heat, exterior heat and interior cold. Yin syndrome characterized by hypofunction of the viscera is generated from insufficiency of yang-qi, excess of yin-coldness, deficiency of both qi and the blood.

  • Three planks of the Triangular position theory was first introduced by Sanghallon(傷寒論) authored by Jang Gi. It is the regulation for the matter of diagnosis of three different clinical symptoms in the causes of exogenous disease. It brought two questions to the scientists. First question is what are the Wind stroke(中風), Affection by cold(傷寒) and the complex form of Wind stroke(中風) and Affection by cold(傷寒) mentioned in Sanghallon. It is related to the theoretical analysis of the causes for the outbreaks of exogenous disease. Second question is what are the characteristics of symptoms to use Gyejitang, Mahwangtang, Daecheongnyongtang according to Sanghallon? It is the matter of clinical skills for dispensing a prescription. Through the theoretically and empirically deepening processes for last two millenniums, those two questions brought us two more problems further. That is, how and by what processes the Six vital substances for humal life outbreaks disease as a energy transformation? And, how the individual peculiarities of human body as a disease container should be treated? In conclusion, it is also a matter of the existence of dialectic medical science.

  • Spinal epidural lipomatosis, which causes symptomatic compression of neural elements, is a relatively uncommon disease. Although it has been reported frequently in association with the administration of exogenous steroids, a few cases of epidural lipomatosis with no association to exogenous steroids, have been reported. Idiopathic spinal epidural lipomatosis may be a separate disease from that induced by steroid. Here, the authors present two cases of symptomatic epidural lipomatosis with no history of steroid-dependent diseases and review the relevant literature.
