  • The purpose of this thesis is to investigate whether taurine supplementation in combination with fructose improves both energy metabolism and exercise capacity. Eight collegiate female subjects were recruited for the study. Each subject went through threecross-over designs: control(fluid), fructose, and taurine plus fructose supplementation trials. Subjects received taurine supplementation 100 mg/kg a day for two weeks. After the supplementation, all subjects take 10% fructose at 15 min prior to exercise, immediately before exercise, and every 15 min during exercise. Subjects received 150 ml fluid as placebo during the same procedure. The subjects performed submaximal exercise at the exercise intensity of 60% for 45 min and then 80% of maximal oxygen uptake (VO2max) until exhaustion time. A 10ml blood sample was taken for measuring the level of glucose, ammonia, lactate, free fatty acids, and insulin every 15 min during exercise at 60% of VO2max. The blood glucose levels was significantly higher at 45 min and 50 min exercise after supplementation of fructose, and immediately before exercise and 50 min exercise after taurine plus fructose compared to the placebo trial. However, the values tended to be lower in taurine plus fructose supplementation compared to the fructose trial. The levels of both lactate and ammonia were significantly lower compared to the placebo, while the exhaustion time was significantly increased. The level of free-fatty acids was significantly lower at 30, 45, and 50 min after fructoseand fructose plus taurine supplementation compared to the placebo trial. The level of glucagon was significantly lower at 15, 30, 45, and 50 min after fructose and fructose plus taurine supplementation compared to the placebo trial. There was no differences in insulin concentration among three treatments. This thesis concludes that combined taurine and fructose supplementation prior to exercise may improve exercise tolerance time and energy metabolism, lowering the muscle fatigue factors such as lactate and ammonia.

  • Although cardiac rehabilitation (CR) has been shown to improve exercise tolerance and prognosis in patients with cardiovascular diseases, there remains low participation in outpatient CR. This may be attributed to the patients' busy schedules and difficulty in visiting the hospital due to distance, cost, avoidance of exercise, and severity of coronary disease. To overcome these challenges, many countries are exploring the possibility of remote CR. Specifically, there is increasing attention on the development of remote CR devices, which allow transmission of vital information to the hospital via a remote CR application linked to a wearable device for telemonitoring by dedicated hospital staff. In addition, remote CR programs can support return to work after hospitalization. Previous studies have demonstrated the effects of remote CR on exercise tolerance. However, the preventive effects of remote CR on cardiac events and mortality remain controversial. Thus, safe and effective remote CR requires exercise risk stratification for each patient, telenursing by skilled staff, and multidisciplinary interventions. Therefore, quality assurance of telenursing and multi-disciplinary interventions will be essential for remote CR. Remote CR may become an important part of cardiac management in the future. However, issues such as cost-effectiveness and insurance coverage still persist.

  • Purpose: Metabolic syndrome (also known as insulin resistance syndrome) represents a constellation of hypertriglyceridemia, hypertension, impaired glucose tolerance, and obesity. Presently, the influence of various factors on metabolic syndrome was assessed in patients of a university hospital comprehensive medical examination center. Methods: Age, sex, blood pressure, height, weight, triglyceride level, high-density lipoprotein cholesterol, and glucose levels were measured in 67 people (37 males and 30 females). These factors were correlated with tobacco use, alcohol consumption, and exercise habits. Metabolic syndrome and abdominal obesity were assessed according to NCEP-ATP III criteria and the Asia-Pacific guidelines (male obesity defined as a waist circumference exceeding 90 cm), respectively. Data was analyzed using t-test, 2-test, and logistic regression. Results: Respective percentages were: tobacco use (14.9% of the 67 people), no tobacco use (85.1%), alcohol consumption (62.7%), no alcohol consumption (37.3%), regular exercise (25.4%), no regular exercise (74.6%). Logistic regression analysis revealed a gender-related odds ratio of 2.3 for metabolic syndrome and no exercise. Conclusions: Weight reduction and physical exercise may decrease the prevalence of metabolic syndrome. Early identification of metabolic syndrome and risk factor modification is prudent in cases of obesity, diabetes, hyperlipidemia, and hypertension.

  • Dysfunction of the anterior and dorsal muscles of the trunk have been studied in relation to low back pain of many years. Many muscles of the trunk are capable of contributing to the stabilization and protection of the lumbar spine, recent evidence has suggested that transversus abdominis may be critically involved and has been the focus of rehabilitation. The delay in onset of contraction of trunk muscles associated with movement of the upper or lower limb in patients with low back pain indicates a significant deficit in the automatic motor command for control of disturbance to the spine. The function of transversus abdominis has been largely ignored in the evaluation of spinal stabilization and protection. The most essential stabilizing muscles for the lumbar column are the transversus abdominis and the multifidus. Sling exercise therapy(SET) concept consists of a system of diagnosis and treatment. The system of diagnosis involves testing the muscle's tolerance through progressive loading in open and close kinetic chains. The SET system contains elements such as relaxation, increasing the range of movement, traction, training the stabilizing musculature, sensorimotor exercises, training in open and close kinetic chains, dynamic training of the mobilizing musculature, cardiovasc+ular exercises, group exercise, personal exercise at home. Sensorimotor training is an essential element of the SET concept. The emphasis is on closed kinetic chain exercises on an unstable surface, thereby achieving optimum stimulation of the sensorimotor apparatus.

  • Ginseng has been used as a key constituent in traditional medicine prescriptions for centuries. Other than its well-known anti-stress and adaptogenic properties, ginseng has also been shown to be very effective in treating age-related deterioration in metabolic and memory functions. Although it is generally believed that the saponin (GS) fraction of the ginseng root accounts for the bioactivity of ginseng, a direct demonstration on which ginsenoside does what is still generally lacking. In the past decade, our laboratory has endeavored to identify the active GS components involved in energy metabolism, memory, and anti-neurotoxicity. To examine the ergogenic effects of GS in enhancing aerobic capacity, rats were subjected to either severe cold ($40^{\circ}C$ under helium-oxygen, two hours) or exercise workload $(70\%\;VO_{2}max,$ to exhaustion). Acute systemic injection (i.p.) of ginseng GS (5-20 mg/kg) significantly elevated both the total and maximum heat production in rats and improved their cold tolerance. However, pretreating the animal with the optimal dose (10 mg/kg) of GS devoid of $Rg_1\;and\;Rb_1$ failed to elicit any beneficial effects in improving cold tolerance. This indicates that either $Rb_1\;and/or\;Rg_1$ may be essential in exemplifying the thermogenic effect of GS. Further studies showed that only pretreating the animals with $Rb_1(2.5-5\;mg/kg),\;but\;not\;Rg_l,$ resulted in an increase in thermogenesis and cold tolerance. In contrast to the acute effect of GS on cold tolerance, enhancement of exercise performance in rats was only observed after chronic treatment (4 days). Further, we were able to demonstrate that both $Rb_1\;and\;Rg_1$ are effective in enhancing aerobic endurance by exercise. To illustrate the beneficial effects of GS in learning and memory, a passive avoidance paradigm (shock prod) was used. Our results indicated that the scopolamineinduced amnesia can be significantly reversed by chronically treating (4 days) the rats with either $Rb_1\;or\;Rg_1$ (1.25 - 2.5 mg/kg). To further examine its underlying mechanisms, the effects of various GS on ${\beta}-amyloid-modulated$ acetylcholine (ACh) release from the hippocampal slices were examined. It was found that inclusion of $Rb_1$ (0.1 ${\mu}M$), but not $Rg_1$, can attenuate ${\beta}-amyloid-suppressed$ ACh release from the hippocampal slices. Our results demonstrated that $Rb_1\;and\;Rg_1$ are the key components involved in various beneficial effects of GS but they may elicit their effects through different mechanisms.

  • Cervical pain is a rapid increase that is owing to a flexion-extension whiplash injury, unappropriated posture, chronical repetition injury from abdominal position of head and neck, excessive repeating work, chronical deficiency of excercise. Because of that is bring about muscle unbalance, tightness of cervical extensor muscle, weakness of cervical deep flexor muscles, instability of cervical region and reduction of proprioceptive sensor. Recent the role of muscle is more emphasized for preservation of sine stabilization. And cognition of integrated muscular system, importance for the operation and relation is increased to maintain stability of the motor system and pertinent function. Therefore we are going to introduce the sling exercise and stabilization exercise method for advanced efficient of cervical and upper limb and for the muscle strengthening to importance cervical stabilization through neurological program as control the reaction of cervical stabilization. • Oxygen is the final acceptor of electron transport from fat and carbohydrate oxidation, which is the rate-limiting factor for cellular ATP production. Under altitude hypoxia condition, energy reliance on anaerobic glycolysis increases to compensate for the shortfall caused by reduced fatty acid oxidation [1]. Therefore, training at altitude is expected to strongly influence the human metabolic system, and has the potential to be designed as a non-pharmacological or recreational intervention regimen for correcting diabetes or related metabolic problems. However, most people cannot accommodate high altitude exposure above 4500 M due to acute mountain sickness (AMS) and insulin resistance corresponding to a increased levels of the stress hormones cortisol and catecholamine [2]. Thus, less stringent conditions were evaluated to determine whether glucose tolerance and insulin sensitivity could be improved by moderate altitude exposure (below 4000 M). In 2003, we and another group in Austria reported that short-term moderate altitude exposure plus endurance-related physical activity significantly improves glucose tolerance (not fasting glucose) in humans [3,4], which is associated with the improvement in the whole-body insulin sensitivity [5]. With daily hiking at an altitude of approximately 4000 M, glucose tolerance can still be improved but fasting glucose was slightly elevated. Individuals vary widely in their response to altitude challenge. In particular, the improvement in glucose tolerance and insulin sensitivity by prolonged altitude hiking activity is not apparent in those individuals with low baseline DHEA-S concentration [6]. In addition, hematopoietic adaptation against altitude hypoxia can also be impaired in individuals with low DHEA-S. In short-lived mammals like rodents, the DHEA-S level is barely detectable since their adrenal cortex does not appear to produce this steroid [7]. In this model, exercise training recovery under prolonged hypoxia exposure (14-15% oxygen, 8 h per day for 6 weeks) can still improve insulin sensitivity, secondary to an effective suppression of adiposity [8]. Genetically obese rats exhibit hyperinsulinemia (sign of insulin resistance) with up-regulated baseline levels of AMP-activated protein kinase and AS160 phosphorylation in skeletal muscle compared to lean rats. After prolonged hypoxia training, this abnormality can be reversed concomitant with an approximately 50% increase in GLUT4 protein expression. Additionally, prolonged moderate hypoxia training results in decreased diffusion distance of muscle fiber (reduced cross-sectional area) without affecting muscle weight. In humans, moderate hypoxia increases postprandial blood distribution towards skeletal muscle during a training recovery. This physiological response plays a role in the redistribution of fuel storage among important energy storage sites and may explain its potent effect on changing body composition. Conclusion: Prolonged moderate altitude hypoxia (rangingfrom 1700 to 2400 M), but not acute high attitude hypoxia (above 4000 M), can effectively improve insulin sensitivity and glucose tolerance for humans and antagonizes the obese phenotype in animals with a genetic defect. In humans, the magnitude of the improvementvaries widely and correlates with baseline plasma DHEA-S levels. Compared to training at sea-level, training at altitude effectively decreases fat mass in parallel with increased muscle mass. This change may be associated with increased perfusion of insulin and fuel towards skeletal muscle that favors muscle competing postprandial fuel in circulation against adipose tissues.

  • It is known that a pulmonary rehabilitation program improves dyspnea and exercise tolerence in patients with chronic obstructive pulmonary disease. However, it is also known that although it does not improve pulmonary function. This study was performed to evaluate the effect of a 4 week pulmonary rehabilitation on pulmonary function, gas exchange, and exercise tolerance in patients with chronic obstructive pulmonary disease. The pulmonary rehabilitation programs included breathing exercises, such as pursed-lip breathing and diaphragmatic breathing, upper-limb exercises, and inspiratory muscle training. These activities were performed for 4 weeks in twenty one patients with chronic obstructive pulmonary disease. Pre and post-rehabilitation pulmonary function and exercise capacities were compared after the 4 week period. Results are as follows: 1) Before the rehabilitation, the predicted value of FVC and FEV1 of the patients were 70.3$\pm$16.7% and 41.1$\pm$11.9% respectively. These pulmonary functions did not change after pulmonary rehabilitation. 2) Aloility of walking a 6 minute distance (325.29$\pm$122.24 vs 363.03$\pm$120.01 p=.01) and dyspnea (p=.00) were significantly improved after rehabilitation. Thus showing that pulmonary rehabilitation for 4 weeks can improve exercise performance and dyspnea in patients with chronic obstructive pulmonary disease.
