운동치료의 변화와 발전 (Change and Development of Therapeutic Exercise)

  • Therapeutic exercise had developed with human being. It depend upon development of physiology, anatomy, kinesiology and biomechanics. There are two categories in therapeutic exorcise. One is classical therapeutic exercise which is a activity of one muscle group, one axis and one plane motion. It is ROM, FRE. osteokinematics, arthrokinematics. Another is neurophysiological approach which is multi muscle group, multi axis and plane exercise. It is PNF and Bobath that start from 1940 to early 1950. Classical therapeutic exorcise develop from ROM to osteokinematics and arthrokinematics. It is foundation of the joint mobilization and orthopaedic manual physical therapy. Neurophysiological therapeutic approaching has more theory and skills than before. Bobath methods had changed from reflex-inhibiting posture to key points of control and added the theory of musculoskeletal, biomechanics, motor behaviour and cognition. We call it motor control.

원시반사통합운동이 신경발달장애 아동의 앞쪽머리자세, 균형능력, 주의집중력에 미치는 효과 : 예비연구 (Effects of Primitive Reflex Integration Exercises on Forward Head Posture, Balance, and Concentration in Children with Neurodevelopmental Disability : A pilot study)

  • Purpose : Therapeutic exercise should improve the health outcomes of rehabilitation in children with neurodevelopmental disability. The purpose of this study was to investigate the feasibility of primitive reflex integration exercises on forward head posture, balance ability, and concentration in children with neurodevelopmental disability. Methods : This study included 10 children with neurodevelopmental disability. Primitive reflex integration exercises were performed for 40 minutes, twice a week for 8 weeks (16 sessions). The reflective markers were placed at the center of the shoulders and on the ears. A caliper was used to measure the distance between the attachments of the reflective markers to assess the forward head posture. Pediatric balance scale was used to quantify balance ability. Their abilities in terms of changeless sitting, looking at the teacher, putting children's hands on their knees, and looking at immovable and movable objects, were assessed to quantify concentration. Results : There were significant improvements in forward head posture after the intervention (p=.005). Primitive reflex integration exercises significantly improved balance ability of children with neurodevelopmental disability (p=.027). There were also significant improvements in changeless sitting (p=.005), looking at the teacher (p=.004), putting children's hands on their knees (p=.005), and looking at the immovable (p=.004) and movable (p=.004) objects. Conclusion : This study showed that primitive reflex integration exercises were a useful intervention to improve forward head posture, balance, and concentration in children with neurodevelopmental disability. Therefore, primitive reflex integration exercises may also promote and improve their general development. Further studies with appropriate sample size and control group are needed to conclude the effectiveness of primitive reflex integration exercises on improving posture, motor function, and concentration in children with neurodevelopmental disability.

  • Purpose: This study purpose to comparison the effects of passive scapular alignment exercise and hold & relaxation technique on craniovertebral angle (C1~C7) with forward head posture due to using variety bag and using computer in healthy subjects. Design: Randomized Controlled Trial. Methods: 22 subjects were divide into two groups: PSAEx group and H/R group two groups. After exercise, the CVA were measured using C7 and tragus. Collected data were statistically analyzed by SPSS 20.2 Results: There were significant differences pre and post the exercise. Conclusion: PSAEx and H/R technique was effective to correct a abnormal CVA posture. The result of this study may helpful as basic data for orthopedic physical therapy.

시청각 매체를 활용한 목, 어깨 자가신장운동이 만성 목통증 여성의 통증, 자세정렬과 관절위치감각에 미치는 영향 (The Effect of Neck and Shoulder Self-Stretching Exercise Using Audiovisual Media on Neck Pain, Postural Alignment, and Joint Position Error in Women with Chronic Neck Pain)

  • Background: The purpose of this study is to investigate the effect of neck and shoulder self-stretching exercise using audiovisual media on neck pain, postural alignment, and joint position error in women with chronic neck pain. Methods: The subjects included 20 women that gave consent to participate in the study voluntarily. They performed the self-stretching exercises using audiovisual media was carried out 20 minutes 5 times a week during 3 weeks. Neck disability index (NDI) and visual analogue scale (VAS) were used to measure the functional disability and pain, A pressure pain threshold was measured using an algometer, and a cervical range of motion (CROM) measurement tool was used to measure the range of motion and error of proprioceptive position sense of the cervical spine. To assess posture alignment, forward head angle (FHA), forward shoulder angle (FSA) were measured using image J software. Results: The neck pain intensity was statistically significantly within group (p<.05). Neck and shoulder functional disability were a statistically significant difference within group (p<.05). Splenius capitis and upper trapezius pressure pain threshold were statistically significant difference in within group (p<.05). The postural alignment was statistically significantly within group (p<.05). The cervical range of motion in neck extension, right and left lateral flexion were statistically significantly within group (p<.05). The joint position error in neck flexion, extension, right and left lateral flexion decreased statistically significantly within group (p<.05). Conclusion: Self-stretching exercise using audiovisual media increased the mobility of the neck, decreased neck pain and joint position error, and improved posture alignment. As a result, there was a positive effect by applying the self-stretching exercise using audiovisual media to people with neck pain. Based on this, it is thought that it can be used as the basis for research related to home training programs for healthy self-management.

Effects of 3D Stabilization Exercise on the Muscle Activity and Static Balance of Patients with Lumbar Instability

  • Purpose: The paper presents an intervention for clinical applications in the future by examining the effects of 3D stabilization exercise on patients with lumbar instability, which causes problems in the muscles and balance, and analyzing the effects of balanced lumbar muscles on the static balance. Methods: After collecting samples randomly from thirty patients with lumbar instability, fifteen patients selected for 3D stabilization exercise were placed in the stability group and fifteen patients selected for Swiss ball exercise were placed in the ball exercise group. The intervention program was applied for thirty minutes a session, once a day, three days a week for four weeks. Before the intervention, the lumbar muscle activity and static balance were measured. After four weeks, they were re-measured in the same way and the data were analyzed. Results: In relation to the within-group changes in muscle activity, all groups except for the LEO and REO groups showed significant differences. Regarding the between-group changes in muscle activity depending on the left and right difference, ES, RA, and TrA but not EO showed significant differences. In addition, there were significant differences in the between-group change in static balance. Conclusion: 3D stabilization exercise improves the muscle activity by promoting a balanced posture of lumbar muscles and changing senses, such as a proprioceptor but this had a positive influence on the static balance by controlling the balance of muscles.

Effects of Yoga on Pain, Function, and Depression in Individuals with Nonspecific-Low Back Pain

  • Objective: Nonspecific low back pain (NSLBP) is experienced worldwide by many age groups. Yoga is recommended as an exercise to reduce back pain and stress because it is a breathing exercise, posture, and meditation as key elements. The aim of this study is to compare the effects of yoga and stabilization exercise on pain intensity, function, and depression. Design: An open-label, parallel arm, randomized controlled trial Methods: Twenty-four participants were allocated to the experimental and the control group in a ratio of 1:1. Yoga (experimental group) and stabilization exercise (control group) were received twice a week for 6 weeks Participants were assessed at baseline and post-intervention for pain intensity (numeric pain rating scale), function (Aberdeen low back pain scale, flexibility,and strength), and depression (Beck depression inventory). Results: When the experimental group (Yoga) and control group (stabilization exercise) were performed twice a week for 6 weeks, numeric pain rating scale, Aberdeen low back pain scale, and flexibility in post-intervention showed significant improvement in both groups (P<0.05), However, in all variables, the experimental group showed a positive benefit compared to the control group (P<0.05). Conclusions: The results of this study show that yoga has more positive benefits compared to stabilization exercise in pain intensity, function, and depression in individuals with NSLBP.

기립자세 증진 운동이 파킨슨병 환자의 제자리 돌기 동작에 미치는 영향 (The Effects of Standing Posture Improvement Exercise to the Turning Round Movement in Patients with Parkinson Disease)

  • Purpose: The aims of this study was to evaluate the effects of shoulder lateral rotation and trunk extension exercises on standing posture improvement exercises using turning round in the patients with Parkinson's disease. Methods: For twenty-one patients with Parkinson's disease, shoulder lateral rotation and trunk extension exercises were performed for a total of three sets, ten times a set, three days a week for twelve weeks. To measure turning round, ink foot-print method was used. The number of steps and time were measured every week for before and after study (twelve weeks). Results: The results are as follows: Changes in steps and time on each week during twelve weeks showed significant differences, compared with before test(p<0.05). Conclusion: Standing posture improvement exercises can help perform turning round in the patients with Parkinson's disease.

PNF 목 패턴이 전방머리자세를 가진 성인의 목 움직임 및 목 장애 지수에 미치는 영향 (Effect of PNF Neck Patterns on Neck Movement and NDI in Adults with Forward Head Posture)

  • Purpose: The purpose of this study was to investigate the effects of neck patterns in proprioceptive neuromuscular facilitation (PNF) for neck movement and the neck disability index (NDI) among adults with forward head posture. Methods: Thirty-nine subjects were randomly assigned into two groups. Subjects in the proprioceptive neuromuscular facilitation exercise group (PNFG, n = 20) received 20 minutes of PNF neck pattern (flexion-Rt. lateral flexion-Rt. rotation followed by extension-Lt. lateral flexion-Lt. rotation) 3 times weekly for 4 weeks. Outcomes were measured using absolute rotation angle (ARA), anterior weight bearing (AWB), range of flexion and extension motions (RFEM), and neck disability index (NDI) methods before and after the 4-week intervention period. Results: There were significant effects for the PNFG, pre- and post-intervention, in ARA, AWB, RFEM, and NDI. There were significant differences in ARA, AWB, RFEM, and NDI compared with CG. Conclusion: The results of this study suggest the PNF neck pattern could be beneficial for adults with forward head posture.

Effect of Cervical Corrective Exercises on Pain, Neck Posture, and Intersegmental Motion of Cervical Spine in a Patient With Cervical Radiculopathy: A Case Report

  • This case report describes the effectiveness of cervical corrective exercises in a patient with cervical radiculopathy (CR) who experienced radicular pain, upper limb paresis, and limited functional activity. A 39-year-old male with cervical radiculopathy performed the cervical corrective exercises for reducing pain. Pain intensity, cervical posture, and active range of motion of cervical intersegmental spine motion were measured baseline, after 4 weeks, and after 8 weeks with self-reported questionnaire and radiographs. After 8 weeks of intervention, the patient demonstrated alleviated radicular symptoms, improved neck posture and active range of flexion and extension of the cervical intersegmental spine. Especially in the angle between the cervical vertebra 6 and 7, the angle was changed from $-4.69^{\circ}$ to $3.30^{\circ}$ during resting position after intervention. The present case indicates that the cervical corrective exercises might be a possible treatment to effectively reduce radicular symptoms, improve neck posture, and active cervical intersegmental motion for patient with CR.

Multifidus Thickness Comparison between the Effectiveness of Abdominal Bracing and Maximum Expiration Maneuvers in Lumbo-Pelvic Upright Sitting Posture

  • Background: Trunk flexor-extensor muscles' co-activation and upright posture are important for spinal stability. Abdominal bracing and maximal expiration are being used as exercises to excel torso co-contraction. However, no study has on comparison of the effect of this exercise on multifidus in the upright sitting posture. Objectives: This study aims to verify the effectiveness of abdominal bracing and expiration maneuvers in lumbo-pelvic upright sitting. Design: Cross-sectional study. Methods: Eighteen healthy women were recruited for this study. The multifidus muscle thickness of all subjects was measured in three sitting conditions (lumbo-pelvic upright sitting, lumbo-pelvic upright sitting with abdominal bracing, and lumbo-pelvic upright sitting with maximum expiration) using ultrasound. One-way repeated measure analysis of variance was used for the evaluation. Results: Compared to lumbo-pelvic upright sitting, lumbo-pelvic upright sitting with abdominal bracing and lumbo-pelvic upright sitting with maximum expiration were associated with significantly increment of muscle thickness. There was no significant difference in muscle thickness between lumbo-pelvic upright sitting with abdominal bracing and lumbo-pelvic upright sitting with maximum expiration. Conclusion: Abdominal bracing and maximum expiration could be beneficial to increasing lumbar multifidus thickness in lumbo-pelvic upright sitting.
