Purpose : The aim of this study was to evaluate myocardial injury in children treated with adriamycin by echocardiography, which is non-invasive and safe measurement for children. Methods : Left ventricular dimensions, wall stress, and contractile function were determined by echocardiographic methods in 17 patient recepients with adriamycin chemotherapy at rest(group 1) and during stress(group 2). Twenty age-matched normal subjects were established as control group. Results : End-diastolic dimension was decreased in both groups(group 1; $92{\pm}7%$ of normal, group 2; $87{\pm}8%$ of normal, P<0.05). Left ventricular end diastolic volume and wall mass were also decreased in both groups(group 1; $96{\pm}12mL/m^2$ and $145{\pm}18g/m^2$, group 2; $87{\pm}8mL/m^2$ and $137{\pm}16g/m^2$, respectively, P<0.05 and P<0.05) and group 2 showed lower values than group 1. Meridional end systolic stress(ESSm) was increased in both groups but there was no significant difference between the two groups(group 1; $52.6{\pm}6.2g/cm^2$, group 2; $63.5{\pm}8.5g/cm^2$, P<0.05, normal value $45.7{\pm}3.5g/cm^2$). The load-independent relation of rate-corrected circumferential fiber shortening velocity(Vcfc) to ESSm has a significant abnormal change in 7 out of 17(41%) in group 1 and 12 out of 17(71%) in group 2. Conclusion : The load-dependent systolic index, such as fractional shortening, may fail to show abnormality because of the compensatory changes in preload and afterload which can mask the impaired contractility. Therefore, systolic performance also should be monitored by a load-indepedent contractility index such as slope value of the end-systolic pressure-dimension relation and the position of the left ventricular stress-fiber shortening velocity after exercise.
The purpose of this study was to determine the effect of three different plates($0^{\circ}$, $10^{\circ$}, $20^{\circ}$)under heels on the lower limb muscles and erector spinae during squat exercise. Ten high school korean traditional wrestling players(age: $18.5{\pm}0.7$, weight: $1972.2{\pm}128.5N$, height: $177.8{\pm}6.0cm$, weight of barbell: $1004.5{\pm}132.4N$) performed squat exercise using three different tilting plates under heels at a cadence of 40beats/sec with 80% one repetition maximum load. Surface electrodes were placed on the participants' left and right erector spinae, and rectus femoris, vastus medialis, vastus lateralis, tibialis anterior, biceps femoris, medial gastrocnemius, and lateral gastrocnemius in the right lower extremity. One S-VHS camcorder(Panasonic AG456, 60fields/s) was placed 10m to the side of the participant. To synchronize the video and EMG data, a synchronization unit was used for this study. Average and Peak IEMG values were determined for each participant. For each variable, a one-way analysis of variance was used to determine whether there were significant differences among three different tilting plates under heels. When a significant difference was found in plates type, post hoc analyses were performed using the Tukey procedure. A confidence level of p<.05 was used to determine statistical significance. As a result of this study, maximum nEMG values of the tibialis anterior in $0^{\circ}$ plates was significantly higher than the corresponding values for the other plates during the knee extension. This increased activation in the tibialis anterior muscle indicates an increase in displacement of center of gravity of body. It is very likely that additional muscle activation are needed to stop the forward and backward movement. The results also showed that muscular activities of quadriceps femoris and erector spinae were decreased with increasing angle of plates. This suggests that increasing angle of plate may help to sustain the balance and posture of squat exercise. It is considered that very few significant differences were found among three different plates($0^{\circ}$, $10^{\circ}$, $20^{\circ}$) since elite players with much experience in squat exercise, were chosen as a participant of this study. In order to obtain meaningful results regarding the tilting angle of heel plates in squat exercise, kinetic and 3D kinematic analysis will be needed in the future study.

The purpose of this study was to investigate the influence of gender and load carriage difference on the lower extremity kinetics during stair descent. Ten healthy males and 10 healthy females were recruited (n=20). In the Maximum resultant velocity, it showed more velocity was decreased with difference by the change of gender (males>females) and load carriage (30%, 20%, 10%> 10%). And, resultant velocity showed interaction between gender and a load (load>gender). Main effect by gender during stair descent showed leg length was decreased in females than that of males at initial contact phase. Also, main effect by gender during stair descent showed more hip, knee flexed and plantar flexion of ankle joint in females than that of males. In the kinetics variables, main effect by gender during stair descent showed more higher reaction force of medial-lateral direction, and leg stiffness in males than that of females. We found that females successfully accommodated a load during stair descent by decreasing the range of motion of the hip joint angle and resultant velocity of movement. Males, on the other hand, require greater medial-lateral, vertical reaction force, and leg stiffness to accommodate a load, and control of momentum.
Purpose: This study aimed to analyze the visual and spatial elements of the gait of a stroke patient who had diverse ankle weight loads applied, according to weight changes. Methods: The subject was a 57-year-old stroke patient diagnosed and hospitalized with a left intracerebral hemorrhage. A weight equivalent to 0%, 1%, and 2% of his body weight was applied to the area 5cm upward from the ankle using a Velcro strap. He was then trained on a treadmill, receiving a six-minute walk test to evaluate his gait ability. A gait analyzer was used to collect visual and spatial elements, such as gait distance, gait velocity, cadence, step length, stride length, and swing phase, according to a weight load equivalent to 0%, 1%, and 2% of his body weight. Results: According to the results of applying 0%, 1%, and 2% of his body weight on the ankle, except for gait velocity, his gait distance, cadence, step length, stride length, and swing phase were higher when 1% of his body weight was applied compared to 0% or 2% of his body weight. Conclusion: Applying a weight equivalent to 1% of the body weight to the ankle positively affected the visual and spatial element of the gait and heightened the efficiency of exercise during treadmill training, a gait-training tool generally used for stroke patients. However, the result is difficult to generalize because the number of subjects was small with only one subject.

In order to provide a safe free-weight-training environment to people without workout trainers, we suggest a smart bench press machine with an automatic weight control system sensitive to user tiredness. Physical weight plates on the machine are replaced with a hydraulic cylinder as a press load and the cylinder knob is coupled with a step motor to change its tensile force automatically in-between lifting exercises. Three subjects participated to verify the usability of the smart bench press machine. They were asked to lift a 6-RM press load 10 times with 3 different lifting conditions: 1) no assistance, 2) a human assistance, and 3) the automatic weight control. All subjects were not able to complete the 10 sets without assistance due to tiredness, but they finished the full sets under the two assistive conditions. Average lifting speeds under the automatic weight control condition showed the most consistent level. Normalized quasi-tension data based on surface electromyogram signals of both Pectoralis Majors revealed that the subjects maintained the target muscle activation level above 50% but not more than 80% throughout the 10 sets. Therefore, the smart bench press machine is expected to both keep pace with the lifting exercise and reduce risk of injuries due to excessive muscle tensions.
Background : Patients with locally advanced non-small cell lung cancer are often treated with radiation alone or in combination with chemotherapy. Both modalities have a potentially damaging effect on pulmonary function. In order to examine changes in the cardiopulmonary exercise function of patients with locally advanced non-small cell lung cancer before and after conventional radiotherapy, we conducted a prospective study involving patients with such cancer, that had received radiation therapy. Method : Resting pulmonary function test, thoracic radiographic finding and cardiopulmonary exercise test(CPET) were assessed prior to and 4 weeks following radiation therapy in 11 male patients with locally advanced non-small cell lung cancer. Patient with endobronchial mass were excluded. Results : The forces vital capacity (FVC), forced expiratory volume in 1 second ($FEV_1$ and maximal voluntary ventilation (MVV) did not decreased between before and 4 weeks after radiation but the diffusing capacity (DLCO) had decreased by 11% 4 weeks after radiation, which was not statistically significant. No changes in maximal oxygen consumption ($VO_2$max), carbon dioxide production ($VCO_2$), exercise time and work load were attributed to radiation therapy. Follow up cardiopulmonary exercise testing revealed unchanged cardiovascular function, ventilatory function and gas exchange. No difference in cardiopulmonary exercise test performance was observed between pre- and post-radiation. Conclusion : Cardiopulmonary exercise function did not decrease within the short-term after the radiation of patients with locally advanced non-small cell lung cancer.

Objective: This study aimed, first, to develop core strength training equipment with elderly-friendly, easy-to-use features and, second, to investigate the effect of core strength training using the equipment on the performance and stability of the elderly in activities of daily living. Method: In this study, we developed training equipment with a stability ball that can be used for performing core strength exercises in the elderly. Twenty-three elderly subjects (age: $77.87{\pm}6.95years$, height: $149.78{\pm}6.95cm$, and weight: $60.57{\pm}7.21kg$) participated in this study. The subjects performed the core strength training exercise with 16 repetitions for 8 weeks (2 repetitions per week). Performance in activities of daily living was assessed by using the Short Physical Performance Battery (SPPB), a test of going up and down 4 stairs, and one-leg static balance test. Stability was quantified as changes in the center of pressure (COP) and C90 area. Results: With the core strength equipment, trunk core strength exercise could be performed by pulling or pushing a rope with 2 hands on the stability ball. During the task, the tension in the rope was manipulated by a motor connected to the rope and the COP of the subject was measured by 4 load cells mounted in the equipment. Our results showed that the SPPB score was significantly higher (p < .05), the time to complete the "going up and down 4 stairs" test was significantly shorter (p < .05), and one-leg static balance statistically improved under an eyes-open condition (p < .05) after as compared with before the core strength training. The changes in the COP in the anteroposterior and mediolateral directions, and C90 area were significantly lower in the posttest (p <. 05) than in the pretest. Conclusion: The core strength training exercise using the equipment developed in the present study improved the performance and stability of the elderly in activities of daily living.
The effects of Physical exercise, gymnastics and sports on the cardiopulmonary function were studied in the middle and high school toys. The subjects were divided into 4 groups; non-training group and training group in both middle school and high school boys. In the above groups, pulmonary function studies were performed, and blood pressure and the heart rate were also checked to evaluate physical fitness during and immediately after running exercise on the tread-mill, with the speed of 5 MPH and elevation of 9% and 11.25%. The types of sports in the training group were base ball, body building, Taekwondo (Korean style boxing) and hand ball. The results obtained were as followings: 1) In the training group, cardiopulmonary function showed some tendency of the increase comparing to the non-training group. 2) The increase in cardiopulmonary function was observed according to the age became older, but the clear changes on cardiopulmonary function was not observed as the difference of the group between the training and the non-training. 3) The expiratory volume was decreased as the increase of age except 17 years of age for the value of the per kg body weight. 4) In the non-training group, the mean value of oxygen consumption under maximum work load was increased, while those in the training group was decreased. But it may be noted that oxygen consumption for the expiratory volume was increased in the training group, and that the oxygen cost in the training group was .higher than that of the non-training group. 5) The pulse pressure of the high school group during and immediately after running exercise was observed in the higher value comparing with that of the middle school group It was suggested that the changes of the pulse pressure was owing to the method of determination and that to the decrease of diastolic pressure caused by the decrease of peripheral vascular resistance up to critical closing pressure. 6) Any differences of the changes in the heart rate between the training group and non-training group was not observed during and immediately after running exercise. 7) The relative value of the expiratory volume to the heart rate was decreased in the elder age group.

[Purpose] The purpose of this study was to (i) assess hydration levels in elite male football players during a national team training camp before and during qualifying matches, (ii) evaluate the effect of coaching strategies for hydration based on feedback from hydration monitoring, and (iii) assess possible relationships between hydration status and training load or wellness markers. [Methods] Thirty-one male players (age 27±4 yrs; height 185±6 cm; weight 82.9±6.7 kg; body fat 10.4±2.3%) representing a national team from the Union of European Football Associations (UEFA) participated. The players were studied during three different national team training camps related to the UEFA Nations League tournament. Urine specific gravity (USG) was measured to assess hydration status. During all camps, the players were actively coached on improving strategies for hydration and given individual feedback on their test results. The training load was measured using GPS technology, and wellness questionnaires were completed. [Results] USG decreased progressively and significantly (p<0.005) during camp 1 and hydration status improved over the three camps, with fewer dehydrated and more well-hydrated players identified during the last part of camp 3. Significantly (p<0.05) higher USG values were observed 2 days prior to a match (MD-2) than on match day (MD); consequently, 52% of the players were dehydrated on MD-2 and only 6% on MD. No correlations were observed between hydration status and training load or wellness markers. [Conclusion] Dehydration is a challenge in elite male football, but continuous monitoring of hydration status and coaching on hydration strategies can lead to major improvements and reduce the degree of dehydration.
Purpose: This study set out to investigate the effects of prone hip extension with knee flexion (PHEKF) exercises according to external load on the thickness of the gluteus maximus (Gmax) and biceps femoris (BF). Methods: Twenty-three healthy men participated in this study. All subjects randomly practiced PHEKF under the burden of external loads created by sandbags (0 kg, 1 kg, and 2 kg). Rehabilitative ultrasound imaging (RUSI) was used to measure the thickness of the Gmax and BF during the PHEKF with different external loads. Results: The thickness of the Gmax was highest during the 2 kg resistance exercise and lowest at 0 kg. The thickness of the Gmax was significantly increased at 1 kg and 2 kg compared to 0 kg and significantly increased at from 1 kg to 2 kg (p < 0.05). The thickness of the biceps femoris was highest at 0 kg and lowest at 2 kg. The thickness of the biceps femoris muscle was significantly reduced at 1 kg and 2 kg compared to 0 kg (p < 0.05), but there was no significant difference between 1 kg and 2 kg (p > 0.05). Conclusion: The thickness of the Gmax was increased by applying PHEKF with a higher external load, whereas the muscle thickness of the biceps femoris decreased. These results suggest that the application of external loads during PHEKF exercises may be an effective method for selective strengthening of the Gmax.
