DNA 염기손상 치유유전자의 변이와 두경부암 발생 위험성 (THE EFFECT OF GENETIC VARIATION IN THE DNA BASE REPAIR GENES ON THE RISK OF HEAD AND NECK CANCER)

  • DNA 손상 치유 유전자 연구를 기초로 한 임상적 접근이 새로운 치료방법으로 떠오르고 있다. 많은 연구들이 중요한 DNA 수복유전자의 다형성을 찾아내어 각각의 단백질의 활동성에 대한 영향을 알아내고 특정한 치료법을 찾아내고 임상적 적용을 시도하고 결과를 평가하였다. 그 결과 암 치료에서 정상 세포와 암세포에서 DNA 수복 유전자의 발현 분석은 화학요법이나 방사선 치료에서 개인맞춤형 치료법을 가능하게 하고 있다. 예를 들어, NER이 결핍된 종양은 cisplatin 치료에 민감성을 나타내고, MMR 결핍세포는 알킬화 화학요법 약제에 높은 내성을 나타낸다. 선천성 비폴립성 결장암과 같은 MMR 결손종양 또한 알킬화 화학요법 약제에 의한 치료에 내성을 가진다. 신경교종(glioma)에서 MGMT 유전자 프로모터가 흔히 메틸화되는데 이것은 유전자 발현이 억제되고 알킬화 화학요법제에 대한 반응성을 증가시킨다. 향후 구강악안면외과 영역에서도 구강암의 발생의 위험성을 증가시킬 수 있는 더 많은 DNA 수복 유전자의 다형성을 발굴하고 임상적으로 개인맞춤형 치료법을 개발하고 적용할 수 있는 많은 연구가 필요할 것으로 사료된다.

악성 연부조직 종양에 대한 무계획적 절제술 후 잔여 종양의 영상학적 진단의 정확성과 임상적 위험인자 (Diagnostic Accuracy of Imaging Study and the Impact of Clinical Risk Factors on the Presence of Residual Tumor Following Unplanned Excision of Soft Tissue Sarcomas)

  • 목적: 악성 연부조직 종양에 대한 무계획적 절제술 후 잔여 종양의 영상학적 진단의 정확성과 임상적 위험인자를 파악하고자 하였다. 대상 및 방법: 2008년부터 2014년까지 무계획적 종양절제술을 시행 후 재절제술을 받은 98명을 대상으로 하여 분석하였다. 재수술 전 모든 환자의 환부를 조영 증강 자기공명영상(magnetic resonance imaging, MRI)을 이용해 영상의학적으로 평가하였으며 54명의 환자는 전신 양전자 방출 단층촬영(positron emission tomography [PET]/computed tomography)을 시행하였다. 모든 환자는 광범위 절제술 후 병리학적 검사를 시행하였다. 각 변수는 일변량 로지스틱 회귀와 다변량 로지스틱 회귀를 이용하여 분석하였다. 결과: 종양이 근막하에 위치한 경우 잔여 종양의 발생률이 높았다(odds ratio: 3.21, p=0.02, 95% confidence interval: 1.25-8.30). MRI는 잔여종양을 발견하는 데 높은 민감도를 보였다(sensitivity 0.79). 결론: 종양이 근막하에 위치한 경우에는 근막 상부에 위치한 경우보다 잔존암이 남을 가능성이 유의하게 높음을 알 수 있었고, MRI 및 PET 검사의 음성예측도가 매우 낮으므로 음성 판정이 나오더라도 이를 근거로 재수술을 시행하지 않는 것은 정당화될 수 없음을 알 수 있었다.

  • $\cdot$ Arthroscopic management is the effective method with acceptable results for coronoid impingement of stiff elbow contributing to the functional improvement and pain relief. $\cdot$ The functional improvement and pain relief seem to be affected by the severity of a degenerative change of the elbow joint. $\cdot$ Excision of coronoid process is required in a marked limitation of further flexion in addition to deeping of the coronoid fossa and anterior capsular release. $\cdot$ Excision of olecranon tip or posterior capsular release are effective method in severe flexion contracture.

  • Ossifying fibroma is a rare benign tumor of the maxilla and the mandible. And its nosology and diagnosis have had many problems. It occurs more common in young adult, especially in female, and there is a predilection for the mandible. The lesion is generally asymptomatic and recurrence is rare after conservative excision. Among ossifying fibroma, juvenile active ossifying fibroma grows rapidly, produces many symptoms, and frequently recurs after conservative excision. Authers experienced a case of juvenile active ossifying fibroma which recurred after two times of conservative surgery, and report is with review of the literatures.

  • Angioleiomyomas are relatively uncommon benign tumors originating from smooth cells of a blood vessel. Although curative by surgical excision, they are rarely diagnosed definitely before surgery. We report on a case of calcified angioleiomyoma occurring on the sole, which was treated by surgical excision without recurrence and a review of literature is presented.

  • A case of nonfunctioning paraganglioma of the anterior mediastinum in a 64-year-old woman is presented. The tumor was adherent to the pericardium and parietal pleura but not to the phrenic nerve. Complete excision of the tumor is the treatment of choice, but partial excision may provide long-term survival. A new classification and terminology suggested by Glenner and Grimley is emphasized. This aorticopulmonary paraganglioma is the first case in the Korea.

  • Squamous cell carcinoma of the skin was diagnosed in an 11-year-old, Australian shepherd dog with a hard mass on the right rump area. On histopathological examination of the tumor showed laminated keratin "pearls" surrounded by proliferated squamous cells, and mitotic figures. The dog was treated by surgical excision and chemotherapy with vinblastine sulfate, cyclophosphamide and prednisolone for 4 weeks. The tumor was effectively treated with a combination of surgery and chemotherapy.motherapy.

  • We have experienced two cases of video thoracosopic excision of mediastinal tumors at the department of thoracic and cardiovascular surgery, Yonsei University, College of medicine. Histologically the mediastinal tumors were cystic thymoma in one & bronchogenic cyst in another. The operative times were rather short and the post-operative courses were not eventful. These patients were discharged with less chest discomforts in post-operative 5 days & have been in good conditions to now.

Repair of UV-induced Cyclobutane Pyrimidine Dimers in Human Mitochonrial DNA-less Cells

  • UV-induced DNA damage causes cell killing and mutations leading to carcinogenesis. In normal human cells, UV damage such as cyclobutane pyrimidine dimers (CPDs) and primidine-prymidone (6-4) photoproducts are mainly repaired by nucleotide excision repair mechanism. The molecular processes have been well characterized recently. To know the influence of mitochondrial genome on the nucleotide excision repair mechanism against CPDs, we comparatively examined the production of CPDs by UVC irradiation and their repair kinetics in human cells completely lacking mitochondrial DNA (mtDNA) and the parental HeLa S cells. Whole DNA extracted from the cells exposed to UVC was treated with T4-endonuclease V to break the phosphodiester bond adjacent to CPDs. The DNA was electrophoresed in a denaturing agarose gel, which was visualized by ethidium bromide staining. The relative amount of CPDs was determined by image analysis using NIH Image software. MtDNA- less (rho-O) cells were apparently more sensitive to UVC than HeLa S cells, while the level of induction of CPDs in rho-O and HeLa cells was comparable. The repair of CPDs was less efficient in rho-O cells compared with HeLa cells. The residual amount of CPDs after 24-h repair was larger in rho-O cells than in HeLa cells where more than 90 % of CPDs were repaired by then. The non-repaired CPDs would lead to apoptosis in rho-O cells. These results suggest that mitochondrial genome may contribute to some ATP-dependent steps in nucletide excision repair by supplying sufficient ATP which is generated through a respiratory chain in mitochondria.

소아의 제 2 새궁 기형의 임상적 고찰 (Clinical Analysis of Second Branchial Cleft Anomalies in Children)

  • Branchial cleft anomalies are the second most common head and neck congenital lesions seen in children. Amongst the branchial cleft malformations, second cleft lesions account for 95 % of the branchial anomalies. This article analyzes all the cases of second branchial cleft anomalies operated on at Seoul National University Hospital from September 1995 to February 2011. We analyzed sex, age, symptom and sign, accompanied anomaly, diagnosis, treatment, pathologic report and outcome via retrospective review of medical records. In this series, we had 61 patients (27 female and 34 male). The mean age at the time of operation was 38 months. 31 lesions were on the right, 20 were on the left and 10 were bilateral. The most frequent chief complaints at presentation were non-tender mass and cervical opening without any discharge. According to anatomic type, 29 patients had branchial cleft sinuses, 14 had cysts, 14 had fistulas and 4 had skin tags. Complete excision was attempted if possible and antibiotics challenged when infection was suspected. Complete excision was achieved in 96.7 % of cases. Incision and drainage was done in 2 cases due to severe inflammation, and both recurred. Postoperative complications included wound infection in 2 cases. Microscopic examonation revealed squamous epithelium in 90.2 % and squamous metaplasia in one case in the branchial cleft cyst wall. In summary, second branchial anomaly is found more frequently on right side of neck. Fistulas are diagnosed earlier than cystic forms. Most cases could be diagnosed by physical examination. The definitive treatment is complete excision and sufficient antibiotics coverage for cases with inflammation. After drainage of infected lesions, follow up excision after 1 year might be beneficial for preventing recurrence.
