재발성 거대 세포종의 치료 (Treatment of the Recurrent Giant Cell Thmor)

  • Between Sep. 1986 and Mar. 1996, twenty four patients with recurrent giant cell tumor of their long bones were treated and followed up for more than two years. We applied three different modalities for treatment: curettage with bone graft, curettage and packing with polymethylmethacrylate, wide excision with or without reconstruction. During the average 51.5 months after operation(24-97 months), two of three patients who underwent curettage with bone graft showed recurrence. Patients who underwent wide excision showed no recurrence. Five of fifteen patients who underwent curettage and packing with polymethylmethacrylate showed recurrence. Four of seven who showed recurrence were treated with reapplication of curettage and packing with polymethylmethacrylate. All four patients showed no recurrence, and two of them have been followed up for more than two years. We suggest that the curettage and packing with polymethylmethacrylate is an effective treatment modality of the recurrent giant cell tumor.

Improved Patient Outcomes with Electrocauterization Following Wedge Resection and Curettage for Ingrown Toenails: A Prospective Comparative Study

  • Background Ingrown toenail is a common condition that results in chronic pain, recurrent infections, and difficulty in performing daily activities. Our aim is to compare two surgical methods for the treatment of ingrown toenails: wedge resection with curetting versus wedge resection curetting followed by electrocauterization of the nail bed. Methods A prospective, comparative study that included 130 patients with ingrown toenails. All patients had stage II or III disease. We divided the participants into two groups according to the type of surgery and all patients were followed up for 6 months. The outcomes measured were the incidence of postoperative bleeding and infection, recovery time, patient satisfaction, and recurrence rate 6 months after surgery. Results Of the 130 patients included, 59 (45.4%) underwent excision and curetting of the nail matrix (group 1) and 71 (54.6%) underwent excision, curetting, and electrocauterization of the nail matrix (group 2). The postoperative infection rates were 20.3 and 4.2% in the first and second groups, respectively (p = 0.004). Patient satisfaction was 76.3% among the first group, while 91.5% of patients in the second group were satisfied with the results of surgery. Six months postoperatively, recurrence rates were 25.4 and 4.2% in the first and second groups, respectively (p = 0.001). Conclusion Wedge excision and curettage, followed by electrocauterization of the ingrown toenail is a safe treatment modality with a high success rate, that is evident by a lower recurrence rate, and greater patient satisfaction, with no effect on postoperative pain score or recovery time.

거골 박리성 골연골염의 치료 (Treatment of Osteochondritis Dissecans of the Talus)

  • Purpose: We present our experience about the clinical aspect and treatment modalities of ostechondritis dissecans of the talus. Material and Method: This study included 35 patients, 38 cases diagnosed as ostechondritis dissecans of the talus that were proved by clinical and radiologic finding. The methods of treatment were conservative (2 cases), multiple drilling (1 case), excision (1 case), excision, curettage & multiple drilling (23 cases), curettage & bone peg fixation (2 cases), curettage & bone graft (2 cases), and autogenous osteochondral transfer(Mosaicplasty) (7 cases). Results: Of the 38 cases, 30 cases were associated with a history of trauma (17 cases of medial lesion, 13 cases of lateral lesion), and 9 cases of which had major trauma history and 21 cases, minor repetitive trauma history. According to O'Farrel grading system, 21 cases (55%) had good results, 11 cases (29%) had fair results, and 6 cases (16%) had poor results. Of the medial lesion 16 cases had good results, 6 cases, fair results, and 2 cases, poor results. Of the lateral lesion 5 cases had good results, 5 cases, fair results, and 4 cases, poor results. Conclusion: There were fair or good results in 84% of patients with surgical treatment. Curettage and multiple drilling reveals a good result in medial lesion in grade I, II, III and lateral lesion in grade I, II. Autogenous osteochondral transfer(Mosaicplasty) is considered for the cases in grade III, IV(medial and lateral), lesion exceeds 1Cm of size and poor result with another treatment modality.

척추 측만증을 동반한 흉추에 발생한 동맥류성 골낭종 - 증례 보고 - (Aneurysmal Bone Cyst of the Thoracic Spine with Scoliosis - Case Report -)

  • Aneurysmal bone cysts are uncommon bony lesions of the spine. Approximately 3-20% of the aneurysmal bone cysts occur in the spine, predominantly in the lumbar region, but they may occur at the any level of the spine. These lesions commonly arise from the neural arch and occasionally invade the pedicle and the vertebral body. The clinical diagnosis of a spinal lesion can be very difficult in the early stages of the disease because specific symptoms and signs are usually absent or only amount to back pain. However, depending on the level of involvement and the extent of neurological compression, a wide variety of neurological symptoms and signs may appear, ranging from mild radicular symptoms to complete paraplegia or tetraplegia. Available treatment options include complete excision or curettage of the lesion with bone graft, but where excision cannot be achieved, low dose radiation or arterial embolization may be used. We report a case of aneurysmal bone cyst in the pedicle of the T10 spine with nonstructural scoliosis of $40^{\circ}$ Cobb's angle which was treated successfully with only curettage of the lesion.

  • Amelobalstic fibroma is a rare true mixed tumor of odontogenic origin with both mesenchymal and ectodermal components. It usually appears in the mandible and in the posterior segments of young patients without gender predilection, and sometimes is associated with an impacted tooth. It is a benign slow growing tumor that is less infiltrative than an ameloblastoma but tends to expand bone. Surgical treatment with excision followed by curettage seems to be the most appropriate therapeutic option. The objective of this presentation is to report a case of ameloblastic fibroma of the mandible body in a 14-year-old woman, to make a brief review of the literature about its differential diagnosis and its clinical and histologic features and the treatment.

종골 골내 결절종 - 증례 보고 - (Intraosseous Ganglion of the Calcaneus - A Case Report -)

  • Intraosseus ganglion cysts are uncommon, benign, juxta-articular and usually found within long bones. A 35-year-old male presented with acute right ankle pain. He denied any traumatic event. Tenderness was localized on the subtalar joint. Radiologic studies demonstrated a cystic lesion in the juxta-articular portion of the posterior facet of the calcaneus. The patient underwent excision and curettage. Postoperative recovery was uneventful. A histological examination confirmed a typical intraosseus ganglion cyst. A case of an intraosseus ganglion cyst of the calcaneus causing acute subtalar joint pain is described.

BCG림프선염의 치료 (Management of BCG Lymphadenitis)

  • Bacillus Calmette-Guerin(BCG) lymphadenitis is a complication of post-BCG vaccination and its treatment is still controversial. Eighty patients who had been operated for BCG lymphadenitis from 1987 to 1996 were reviewed. Thirty-one of them were treated with antituberculous drugs(mean duration: 3 months) preoperatively and 49 patients were not given preoperative antituberculous medication. No one was treated with antituberculous drugs postoperatively. Operation methods were excision(72) and partial excision with curettage(8). There was no statistical difference in recurrence rate between groups In two patients(2.5 %) treated with preoperative antituberculous drugs, the lesions recurred after operation. The results suggest that preoperative antituberculous medication does not play any role in the treatment of BCG lymphadenitis and in preventing recurrence in surgically excised cases.

경부 결핵성 림프절염에서 외과적 절제수술후 항결핵제 요법시의 치료 성적 (The Treatment Result of Antituberculous Chemotherapy Followed by Surgical Excisions in Tuberculous Cervical Lymphadenitis)

  • Objectives: Tuberculous cervical lymphadenitis is a frequently recurring disease when treated with chemotherapy alone without enough surgical removal of the tuberculous lesions. Authors reviewed retrospectively the treatment result of antituberculous chemotherapy following almost complete surgical removal of tuberculous foci in the neck. Materials and Methods: A retrospective clinical review and analysis was made in 127 cases of tuberculous cervical lymphadenitis patients treated during the past 10 years from 1989 to 1998 at the Department of General Surgery, Inje University Paik Hospital, Pusan. Results: 1) The peak age incidence was the 2nd decade(37.8%), and female was predominated over male by 2.3:1. 2) The time interval from the onset of symptoms to the first visit was less than 3 months in 60.6% of the patient. 3) The location of lymphadenitis was the right neck in 60%, the left neck 34%, and bilateral in 6% of the patient. 4) Signs on the first visit showed solitary masses(60%), abscess(25%) and both mixed(15%). 5) 25 patients(19%) had present or past history of tuberculosis; pulmonary tuberculosis 12 patients, tuberculous lymphadenitis 10 patients, and others 3 patients. 6) Locations of tuberculous lymphadenitis were posterior cervical triangle 70, supraclavicular 51, submandibular 19, anterior triangle 16 and others 4 cases. 7) The principle of treatment of cervical lymphadenitis was surgical management followed by chemotherapy. Surgical procedures were excision(s), curettage and drainage of abscess, combination of both, and biopsy in 60%, 22%, 12% and 6% respectively. Mean duration of antituberculous medication was 9 months after surgery. 8) The rate of recurrent and persistent tuberculous lymphadenitis was 9% in 4 years follow up. Conclusion: Tuberculous cervical lymphadenitis is a frequently recurring disease in young adult when only antituberculous chemotherapy was employed without almost complete removal of the lesions. It is considered that antituberculous medications for 6-9 months after removing the foci at a maximal extent by surgical excision and curettage will reduce the recurrence rate or persistence of tuberculous lymphadenitis.

거대 세포종에서 골 소파술 및 시멘트 충전술 후의 재발 분석 (Recurrence Analysis of Giant Cell Tumor after Curettage and Cementation)

  • 목적: 최근 체험한 59례의 거대 세포종 중 골 소파술 및 시멘트 충전술을 시행한 37례의 재발의 특성과 이를 예방할 수 있는 방법에 대해 알아 보고자 한다. 대상 및 방법: 1995년 1월부터 2000년 3월까지 본원 정형외과에 입원하여 진단적 생검 또는 수술적 치료에 의하여 병리학적으로 거대 세포종으로 확진되고, 그 치료로 골 소파술과 Burring 후 시멘트 충전술을 시행했던 3 7례의 재발의 위치, 특성, 시기에 관하여 분석하였다. 결과: 총 37례 환자 중 일차 치료(Curettage & Cementation) 후 13례에서 재발하여 35%의 재발율을 보였다. 일차 치료 후 재발까지의 기간은 평균 16 개월로 분석되었으며, 최단 5개월에서 최장 3년 7개월이었다. 2례를 제외하고는 모두 2년 내에 재발하였다. 재발 부위는 수술적 접근 후 창(Window)을 만든 부위의 인접한 피질골에서 대부분 관찰되었으며(11례), 2례는 수술적 접근을 한 골 시멘트의 심부(골수강측)에 발생하였다. 골 소파 술 후의 시멘트 충전술은 즉각적 안정성과 빠른 재활, 재발의 조기 발견의 유용성이 있다. 또한 이론적으로 주변의 남아있는 종양세포를 사멸시킬 수 있는 장점을 가지고 있다. 수술을 시행한 실제 임상 례에서 재발의 부위가 수술시 충분한 소파가 안된 부위(Window를 작게 내어 불충분한 소파가 된 부위, 주위 해부학적 구조상 건, 인대의 기시부나 종지부로 소파가 안된 부위)에서 대부분(85%)이 재발하였다. 결론: 거대 세포종의 치료에 있어서 골 소파술과 Burring 후 시멘트 충전술이 많이 사용되고 있다. 또한 소파술 후 보조적인 치료로서 페놀, $H_2O_2$를 이용한 화학적 소작, 냉동요법, 무수 알코올 등이 소개 되었지만, 아직 그 효과가 입증된 것이 없다. 많은 경우 주위의 인대나 건의 기시부나 종지부 또는 다른 해부학적 구조로 불충분한 창(Window)으로 인해, 수술적 접근을 한 부위의 골 소파술 및 Burring 이 불완전하게 되는 경향이 발생할 수 있다. 본 연구에서 재발의 부위는 대부분(85%)에서 불충분한 개창술이 된 부위에서 발생하였다. 즉, 중요한 해부학적 구조가 있더라도 충분한 개창술과 광범위한 소파와 Burring이 재발을 방지하는 가장 중요한 요소가 되리라 사료된다. 본 연구에서 거대세포종의 재발은 대부분 2년 이내에 발생하였다. 특히 이 기간에는 철저한 추적검사를 통하여 재발에 유의해야 한다.
